This project aimed to improve stool charting quality on Ward 23 by increasing complete chart documentation from 10.19% to 50%. The team worked on educational interventions, increasing awareness, checking patient charts and moving charts to the front of patient notes.
Dr Haris Nasrullah, Dr Rahim Abbas
Dr Hera Batool, Dr Sunbal Usman
As a result, well-maintained charts rose dramatically to 83.61%. This demonstrates that focused education and diligence can drive significant improvements. Ongoing monitoring will ensure charts remain complete. Click here to view QI Poster on the hub!

I’d like to take a moment to express just how much we truly value our incredible Community MSWs. We honestly couldn’t do what we do without them — they go above and beyond every single day, making a real and lasting difference across all our community teams.

Your professionalism, compassion, and teamwork do notgo unnoticed, and I am truly grateful for all that you do. Thankyou for your continued commitment and for making a positive impact on the lives of our patients.
I just wanted to say how invaluable the support workers are, from keeping the unit in order to keeping the staff updates to changes in environment and patient concerns. You are the ones in contact with the woman and families the most and pick up on subtle changes that might otherwise get missed.
Toni Parkin, Palliative Care Nurse, has recently been nominated for a Cavell Star Award for her exceptional compassion, professionalism, and dedication. The Orthopaedic Physiotherapy team has introduced new processes, reviewed patient safety systems and strengthened a multidisciplinary approach to enhance recovery for hip fracture patients. Changes have included a restructure, increased training, and better support for junior staff. Nursing teams assist with early mobilisation, and cardiac chair positioning is used post-surgery.
Thanks to these efforts, more patients are mobilised on the first day after surgery, reflecting the dedication and leadership of the Orthopaedic Physiotherapy team in driving quality improvements. Over 18 months, mobilisation rates rose to 94.5%, reflecting strong leadership and team commitment.

Jake was brilliant because he listened carefully to my concerns, showed genuine kindness, and worked hard to resolve a difficult roombooking issue. His perseverance meant our three days of Clinical Coding training could go ahead with only minor compromises, supporting the Trust’s strategic goal of maintaining accurate coding and protecting income.
He demonstrated the Trust’s values through respect, teamwork, and recognising diverse needs. Jake’s attentive approach and collaborative problemsolving truly made a difference, and I am very grateful for his outstanding support.

On Sunday 7 September, despite significant operational pressures, the PSAs on duty Abhilash Nandikattu, Rachel Mansell and Paula Walker went above and beyond to support the silver, bronze and site matron team. Their timely, accurate information and skilled decision-making enabled us to maintain strong ED performance and ensure safe, high-quality patient care.
Our achievements were truly a ‘One Team’ effort, made possible by the PSAs’ dedication. The on-call and interim site matron team wish to recognise and thank them for their outstanding support.
