Patient Rights and Responsibilities As a patient at Hofstra University Student Health Services, you have the right to: • Receive care without regard to age, race, color, sexual orientation, religion, marital status, sex, gender identity, national origin, or other protected characteristics. • Be treated with consideration, respect, and dignity, including privacy during check-in and in evaluation and treatment areas. • Receive care and information about services and treatment in a language most comfortable for you. • Be informed of the services available at Student Health Services and the provisions for after-hours care. • Receive an itemized copy of your account statement, upon request. • Receive a price list of all services, upon request. • Obtain from your healthcare practitioner, or the healthcare practitioner’s delegate, complete and current information concerning your diagnosis, treatment, and prognosis in terms you can be reasonably expected to understand. • Receive from your healthcare provider information necessary to give informed consent prior to the start of any nonemergency procedure or treatment. • Participate in decisions surrounding your health care to the extent permitted by law, including refusing treatment and being fully informed of the medical consequences of your decision. • Request a change in nurse practitioner, if other qualified nurse practitioners are available. • Refuse to participate in experimental research. • Voice grievances and recommend changes in policies and services to the Student Health Services staff, Hofstra University, and the New York State Department of Health without fear of reprisal. • Express complaints about the care and services provided and to have Student Health Services investigate such complaints. • Request credentials of the healthcare provider or other staff members involved in your care. • Privacy and confidentiality of all information and records pertaining to your treatment. • Approve or refuse the release or disclosure of the contents of your medical record to any healthcare practitioner and/or healthcare facility except as required by law or third-party payment contract. • Access to your medical record per Section 18 of the Public Health Law, and Subpart 50-3. • Receive and review a printed and/or posted copy of the Patients’ Bill of Rights. As a patient at Hofstra University Student Health Services, your responsibilities include: • Be truthful and forthcoming with our team and strive to express your concerns clearly. • Provide as complete a medical history as you can, including providing information about past illnesses, hospitalizations, family history of illness, medications, allergies, and other matters relating to present health. • Cooperate with agreed-on treatment plans. Since adhering to treatment is often essential to public and individual safety, you should disclose whether you have or have not followed the agreed-on plan and indicate when you would like to reconsider the plan. • Recognize that a healthy lifestyle can often prevent or mitigate illness and take responsibility to adopt health-enhancing behaviors. • Be aware of, and refrain from, behavior that unreasonably places the health of others at risk. • Behave respectfully toward all staff members, patients, and visitors.
