guage. You may feel like you have to get a “crash course” in medical jargon and navigate a bewildering and frustrating system while your life is on the line. So much of the narrative in our culture is about “being strong” and “fighting” or “beating” cancer valiantly that it may not leave room for feeling scared, overwhelmed, or angry. It’s also a setup for failure. If my cancer spreads or doesn’t respond to treatment, is it my fault? Have I done something wrong? Did I choose the wrong doctor? Should I have found this earlier? The Folic Acid Debate

Impact Of Synthetic B9 On Long-Term Cognition Submitted By Head Researcher Eradizyme.com

Consumers are unknowingly participating in an unintentional public health experiment regarding synthetic folic acid fortification introduced in 1998, prompting biochemists to study its long-term effects on human brain receptors and cognitive health. While marketing words like “fortified” or “enriched” trick consumers into thinking these processed foods are superior, the metabolic bottleneck in the liver, and genetic variations like MTHFR polymorphisms drastically affect how the body processes synthetic additives compared to natural folate. The Hepatic Bottleneck: Synthetic Vs. Natural Folate The core discussion centers on how the body processes vitamin B9.

Natural folate, found in foods like spinach, asparagus, romaine lettuce, and farm eggs, is easily absorbed by the gut. Conversely, synthetic folic acid must pass through the liver to be transformed by a slow-moving enzyme called DHFR. Clinical data in the PNAS Journal Archive shows human liver enzyme activity is remarkably slow. According to data in the MDPI Nutrients Database, the liver’s conversion ability can easily get backed up at intakes around 200 to 400 micrograms per day. Frenectomies and Periodontal Health When a Small Fold Of Tissue Creates a Bigger Problem

A small band of tissue inside the mouth can sometimes have a surprisingly significant effect on periodontal health. Known as a frenum, this fold of connective tissue helps attach the lips and cheeks to the gums. Most people never have a problem with their frena, but when one is unusually prominent, tight, or positioned too close to the teeth, it can contribute to problems involving the gums and surrounding tissues. A frenectomy is a relatively mi-

nor surgical procedure that releases or removes problematic frenum tissue. In periodontics, the procedure may be recommended when the attachment places excessive tension on the gingiva or interferes with maintaining healthy periodontal tissues. When the Frenum Affects the Gums One concern is a frenum that attaches too close to the margin of the gums. Movement of the lip can repeatedly pull on this tissue, potentially contributing to gum recession or making an existing area of recession more difficult to stabilize. An abnormal frenum attachment can also make effective brushing more challenging or uncomfortable in the affected area. When plaque is not adequately removed, inflammation may develop and further compromise periodontal health. In certain patients, frenectomy may be performed in conjunction with gum grafting. Releasing the tension created by the frenum can help

create a more favorable environment for the grafted tissue and improve long-term stability. The Orthodontic Connection Frena can also play a role in orthodontic treatment. A prominent frenum between the upper front teeth may be associated with a persistent space, known as a diastema. When appropriate, a periodontist and orthodontist may coordinate treatment to determine whether frenectomy should be incorporated into the overall treatment plan. Timing is important. A visible frenum or gap between the teeth does not automatically mean surgery is necessary. The position of the teeth, periodontal tissues, age of the patient, and orthodontic treatment plan must all be considered. A Small Procedure With a Specific Purpose Frenectomy is not routinely necessary simply because a frenum is prominent. Treatment should

be based on whether the tissue is contributing to a functional or periodontal problem. A comprehensive periodontal evaluation can determine whether abnormal tissue tension is affecting gum health and whether treatment is appropriate. When properly indicated, releasing a problematic frenum can help protect vulnerable gum tissue, complement other periodontal procedures, and create healthier conditions for maintaining the teeth over the long term.

Managing Gout: A Guide To Relief and Prevention By Lubrina Bryant, DPM District Podiatry, PLLC

Gout is a common yet painful form of arthritis that occurs when excess uric acid in the blood forms sharp crystals in the joints, typically affecting the big toe. These sudden flare-ups can cause intense pain, swelling, and redness, making even the lightest pressure unbearable. Managing gout effectively involves a combination of lifestyle changes, dietary adjustments, and medical treatments to prevent flare-ups and reduce long-term complications. Understanding the Causes of Gout Uric acid is a byproduct of the breakdown of purines, which are substances found in certain foods and beverages, including red meat, shellfish, and alcohol. Normally, uric acid is dissolved in the blood and eliminated through the kidneys. However, when the body produces too much or fails to excrete enough, uric acid accumulates and forms crystals in the joints, leading to gout attacks. Lifestyle Modifications One of the most effective ways to manage gout is by making lifestyle changes. Weight management is crucial, as excess weight increases uric acid production. Regular physical activity, particularly lowimpact exercises such as swimming and cycling, can improve joint mobility and reduce the risk of gout attacks. Staying hydrated is another important strategy. Drinking plenty of water helps flush out excess uric acid, preventing it from crystallizing in the joints. Aim for at least 8 glasses of water a day, and avoid sugary drinks, which can increase uric acid levels. Dietary Adjustments Diet plays a significant role in managing gout. meats, and certain types of seafood. Alcohol, particularly beer, can also increase uric acid levels and should be consumed in moderation or avoided altogether. Instead, focus on a diet rich in vegetables, fruits, and whole grains. Cherries, in particular, have been shown to reduce uric acid levels and decrease the frequency of gout attacks. Low-fat dairy prod-

ucts can also help lower uric acid levels, making them a good alternative protein source. Medical Treatments For those who experience frequent or severe gout attacks, medications may be necessary. Nonsteroidal anti-inflammatory drugs (NSAIDs), corticosteroids, and colchicine are commonly prescribed to

reduce pain and inflammation during flare-ups. Long-term medications such as allopurinol or febuxostat can help lower uric acid levels and prevent future attacks. By adopting these strategies, you can effectively manage gout and lead a healthier, more comfortable life. Consult with a healthcare provider for personalized advice and treatment.

The prostate can cause several issues as men age; however, with proactive medical care and diligent screening, men can improve their quality of life and avoid unnecessary delays in diagnosing curable prostate diseases. What Is the Prostate? The prostate is a walnut-sized gland that sits at the base of the bladder and contributes components of se-

cancer in men worldwide. About 1 in 8 men will be diagnosed with prostate cancer during their lifetime, and an estimated 36,000 men will die from the disease in 2026. Urologists specialize in identifying and treating prostate cancer. Screening and early detection have been proven to save lives. How Do Urologists Detect Prostate Cancer? Prostate cancer is most commonly found in men age 65 and older and is rare in men under 40. The American Urologic Association recommends screening beginning between ages 45-50 with a simple PSA blood test. Men at higher risk, including those with a family history of prostate cancer and Black men in the US and Caribbean, may begin screening as young as age 40. Ask your primary care physician or urologist about your individual risk and when screening should begin. What Treatments Are Available For Prostate Cancer? Treatment depends on the cancer’s grading and the patient’s goals. Observation or Active Surveillance may be appropriate for low-risk prostate cancer, while higher-risk disease may require surgery to remove the prostate or radiation therapy. Cancer that has spread to other parts of the body is generally treated with medications. Treatment decisions ultimately involve shared decision making between the patient and urologist. What Other Problems Can the Prostate Cause? Many men in their 50’s and 60’s develop urinary problems, including a weak stream, frequent urination, nighttime urination, leakage and urgency. Prostate obstruction is often the cause. Medications and minimally-invasive procedures can improve urinary flow and bladder emptying. What Can I Do For My Prostate Health? Along with an active lifestyle and heart-healthy diet, routine prostate cancer screening with a PSA blood test should be discussed with your primary care doctor or urologist. Limit red meat, processed foods, and sugary drinks that have been linked to higher cancer risk. Hydration is another powerhouse tool. Your body needs water to flush out toxins and support cellular health. Aim for half your body weight in ounces every day—and more if you’re active. Ditch sugary sodas and opt for green tea or lemon water when you need something flavorful. Now let’s talk movement. You don’t need to run marathons. But you do need to move. Physical activity helps regulate hormones, reduce inflammation, and boost immune function. Just 30 minutes of walking, five days a week, can lower your risk for several cancers. Equally important? Managing your stress. Chronic stress weakens the immune system and contributes to disease. Take 10 minutes each day to breathe. Meditate. Journal. Stretch. Pray. Whatever brings you peace – do more of that. And don’t ignore the value of routine screenings. Early detection saves lives. Whether it’s a mammo-

gram, colonoscopy, or skin check, make these appointments non-negotiable. Listen to your body. If something feels “off,” don’t wait. This isn’t about fear. It’s about empowerment. You don’t need to overhaul your life overnight. You just need to commit to showing up for yourself – one meal, one walk, one mindful breath at a time.

Invisalign Vs. Traditional Braces What’s the Difference?

By Michael Rogers, DDS Fairlington Dental Two Paths To a Straighter Smile If you are considering orthodontic treatment, one of the first decisions may be whether to choose clear aligners or traditional braces. Both can move teeth into healthier positions, improve alignment, and correct certain bite concerns, but they accomplish these goals in different ways. Traditional braces use brackets and wires that remain attached to the teeth throughout treatment. Clear aligner systems use a series of custom-made, removable trays that gradually shift the teeth. Understanding the differences can help patients discuss which approach best fits their orthodontic needs, lifestyle, and preferences. How Do Clear Aligners Work?

Clear aligner treatment uses transparent trays designed specifically for the patient’s teeth. Treatment typically begins with a digital scan or impression that allows the orthodontic provider to develop a customized plan for gradually moving the teeth. Patients progress through a series of aligners, often changing to a new set every one to two weeks. For treatment to progress as planned, aligners generally must be worn approximately 20 to 22 hours each day. Clear aligners may be used to address crowded or crooked teeth, gaps between teeth, and certain overbites, underbites, crossbites, and other alignment concerns. Whether they are appropriate depends on the complexity of the individual case. How Are Traditional Braces Different? Traditional braces use metal or ceramic brackets bonded directly to the teeth and connected with orthodontic wires. Adjustments apply controlled forces that gradually guide the teeth toward their desired positions.

Because braces remain attached to the teeth, patients do not have to remember to put them back in after meals or brushing. Braces can also give orthodontic providers precise control over tooth movement, which can make them particularly useful for certain complex orthodontic cases. The tradeoff is that braces are more noticeable and may require some changes to eating and oral hygiene routines. Comfort and Appearance Appearance is one of the primary reasons many patients consider clear aligners. The transparent trays are relatively discreet, making them popular among adults and teenagers who prefer less noticeable orthodontic treatment. Because aligners are smooth and do not contain brackets or wires, they may also cause less irritation to the cheeks and lips. Patients can still experience temporary pressure or tenderness when beginning a new set of aligners. Traditional braces may initially cause irritation or discomfort, particu-

larly following adjustments, although many patients adapt as treatment progresses. Eating and Oral Hygiene One practical advantage of removable aligners is that they can be taken out while eating. This means patients generally do not have the same food restrictions associated with traditional braces. After eating, patients should clean their teeth before replacing the aligners. With braces, hard, sticky, or chewy foods may damage brackets and wires and often need to be limited or avoided. Removable aligners can also make routine brushing and flossing easier. Patients with braces must carefully clean around brackets and wires to remove plaque and food particles and protect their teeth and gums. Which Treatment Is Better? way back to the body. Yoga does not cure cancer, but appropriately adapted movement, breathing, meditation, and relaxation may support overall well-being during treatment and recovery. The practice does not have to be vigorous. Chair yoga, gentle stretching, restorative poses, and mindful breathing can help us reconnect with what the body can do today. The breath is particularly powerful. A slow, comfortable breath can encourage relaxation and give the mind something peaceful on which to focus. Even a few minutes of conscious breathing can become a daily reminder: I am here. I am breathing. I am living this moment. Self-hypnosis can also be used as a relaxation and mind-body practice. It is not a treatment for cancer and should never replace medical care. Instead, it can help a person

enter a deeply relaxed state and practice supportive thoughts and imagery. A simple practice might begin by sitting or lying comfortably, closing your eyes if comfortable, and taking several slow breaths. Imagine a peaceful place – a garden, beach, forest, or favorite room. With each breath, silently repeat: “I am safe in this moment. I allow my body to rest. I give myself permission to receive support. I choose peace, one breath at a time.” Living with cancer does not mean that cancer must become the only definition of a person’s life. There can still be laughter, relationships, creativity, spirituality, movement, nature, learning, and purpose. Some days may be strong days. Other days may simply be days to rest. Both are valuable.

The goal is not to force the body to be something it is not. The goal is to develop compassion for the body that is carrying us through this experience. Yoga can teach us to listen. Breathing can teach us to pause. Self-hypnosis can teach us to quiet the mind. And living with intention can remind us that there is still life to be lived, love to be shared, and moments to be cherished. This material is for relaxation, wellness, and emotional support. Meet Rami Makhoul

Meet Matthew Skancke

After completing his fellowship in Colon and Rectal Surgery at Robert Wood Johnson University Hospital-Rutgers University, Rami Makhoul, MD, joined Holy Cross Health Hospital in 2014 to serve patients of the DMV area.

Dr. Matthew Skancke is a colon and rectal surgeon serving Maryland, Virginia and Washington, DC. Dr. Skancke grew up in Great Falls, VA earning his undergraduate degree in electrical and biomedical engineering at the University of Virginia’s engineering school. After engineering school he spent three years working for a Swiss based GPS company U-blox America while going to night school to finish his premedical degree. He then attended medical school at the University of North Carolina at Chapel Hill where he did basic science research of mesenchymal stem cells to progress the field of regenerative medicine. Dr. Skancke continued his General Surgery training at the George Washington University Hospital and subsequently did his Colon and Rectal Surgery Fellowship at the Cleveland Clinic.

Upon graduating from medical school, Dr. Makhoul first completed his internship in General Surgery at the University of Massachusetts Medical School. He, then, completed his residency training in General Surgery at the George Washington University Hospital in Washington, DC. During his residency training at GW, he spent a year in clinical research where he published some of his work on colon and rectal cancer, and the use of Robotic Surgery in various abdominal procedures performed in the Colon and Rectal Surgery field. He continued with his passion for research and published peer-reviewed articles during his fellowship training. Several of his work, was presented at national conferences including the American Society of Colon and Rectal Surgery, and the Society of American and Gastrointestinal and Endoscopic Surgeons annual conferences.

Dr. Makhoul diagnoses and treats various diseases of the colon, rectum, anus, and other parts of the GI tract using national standards and guidelines. He has a special interest in treating colon and rectal cancer using advanced surgical minimally invasive techniques including robotic, laparoscopic, and transanal minimally invasive surgery. Dr. Makhoul is Board Certified in both General Surgery and Colon and Rectal Surgery and is an active member of the American Society of Colon and Rectal Surgeons education committee. As part of his commitment to surgical training and education, Dr. Makhoul serves as a Clinical Instructor of Surgery at the George Washington University. Dr. Makhoul is currently the chairman of the Department of Surgery at Holy Cross Hospital in Silver Spring. He also holds the position of Cancer Liaison Physician for Holy Cross Health, where he manages and overlooks clinically related cancer activities in collaboration with the Commission on Cancer and American Cancer Society.

During his training, Dr. Skancke has published over 24 peer reviewed publications, authored three book chapters, and presented over 45 times at national conferences in medicine and surgery. He has participated in clinical trials of bench top and mechanical sciences and is a member of the American College of Surgeons and the American Society of Colon and Rectal Surgeons. Dr. Skancke is Board Certified in General Surgery and Colon and Rectal Surgery and is an active member of the American Society of Colon and Rectal Surgeons Quality and Safety National committee.

When he is away from the office, Dr. Makhoul may be found spending time with his family. Dr. Makhoul enjoys skiing, cycling, scuba diving, tennis, and traveling.

Outside of work, Dr. Skancke has found a new love for French cooking and North Carolina barbeque. To offset these vices, Dr. Skancke also enjoys running and powerlifting in his free time.

It is intended to help patients that are struggling with their weight and suffering from health problems related to their weight. It is typically offered to individuals who are 100 pounds or more over their ideal weight. How Is It Performed? Surgery is performed using lapa-

roscopy. Several small incisions are made on the abdomen and surgery is performed with long instruments under the guidance of a camera. A large incision is avoided and recovery is faster than traditional operations. What Happens During Surgery? The stomach is cut into two parts with a stapling device. A small stomach pouch becomes the new stomach and the larger part of the stomach is left in place. The small intestine is cut, one part is attached to the new small stomach and the other part is reconstructed. The result is that the new stomach can only hold small portions of food, typically about 6-8 ounces per serving. The food is then diverted, bypassing the larger part of the stomach and parts of the small intestines. Digestion and absorption of food is delayed due to the re-routing of the small intestine creating a phenomenon of malabsorption. This change in the anatomy of the intestines also leads to changes in the gut hormones. Please see “Gastric Bypass,” page 31
