When patients hear the term “crown lengthening,” they often assume it is a purely cosmetic procedure. While crown lengthening can certainly improve the appearance of a smile, it also serves important functional and restorative purposes. In many cases, it is an essential step in preserving a tooth and ensuring the long-term success of dental treatment.
Crown lengthening is a peri-
odontal procedure that involves reshaping the gum tissue and, when necessary, the underlying bone around a tooth. The goal is to expose more of the natural tooth structure. Depending on the patient’s needs, the procedure may be performed on a single tooth or multiple teeth.
One of the most common functional reasons for crown lengthening is to prepare a tooth for a restoration such as a crown or filling. When a tooth is fractured, decayed, or damaged below the gumline, there may not be enough healthy tooth structure available for a dentist to properly restore it. Crown lengthening can expose additional tooth structure, making it possible to place a restoration while maintaining the health of the surrounding tissues. Removing this hard and soft tissue also creates enough “retention form” so that

a properly made crown will stay cemented and withstand chewing forces.
Another important benefit relates to what dentists refer to as the biologic width (supracrestal attachment), the natural space needed between the gum tissue and the underlying bone. When restorations encroach upon this space, chronic inflammation, discomfort, and bone loss can occur. Crown lengthening helps establish the proper relationship between the tooth, gum tissue, and bone, promoting long-term periodontal health.
From an aesthetic perspective, crown lengthening is often used to correct what is commonly known as a “gummy smile.” In some individuals, excessive gum tissue covers a significant portion of the teeth, making them appear short or
uneven. By carefully reshaping the gumline, crown lengthening can reveal more of the natural tooth surface and create a more balanced, symmetrical smile.
As with many periodontal procedures, treatment planning is critical. Factors such as gum health, bone levels, tooth position, mobility, adjacent structures, cosmetics, and overall oral health must all be considered before determining whether crown lengthening is appropriate.

Many common foot problems do not happen overnight. Bunions, heel pain, arch discomfort, and toe deformities often develop gradually after years of wearing shoes that prioritize appearance over support. While stylish footwear may look appealing, poorly designed shoes can place constant stress on the feet and slowly alter their natural structure.
One simple way to evaluate a shoe is with the “twist test.” Hold the shoe in both hands and twist it as though wringing out a towel. If it folds easily or collapses in the middle, it likely offers minimal support. A quality everyday shoe should resist twisting while still allowing flexibility near the toes where the foot naturally bends during walking.
Age also changes the needs of the feet. As the body ages, ligaments loosen, arches may flatten, and the natural cushioning under the heel and ball of the foot gradually thins. Shoes that felt comfortable years ago may no longer provide adequate support or stability. Foot size can even change over time, making regular shoe fittings increasingly important.
Comfort should never be judged by softness alone. Some extremely soft shoes may actually allow too much movement and fail to properly
support the foot. A supportive shoe should feel stable, especially around the heel and arch, while still allowing natural motion near the front of the foot.
Choosing healthier footwear does not mean giving up style entirely. Many modern shoe designs successfully combine appearance with proper support and stability. The key is learning to look beyond aesthetics and focus on how a shoe functions.
When shopping for shoes, pay attention to how they bend, twist, and
fit throughout the day. Feet should feel supported rather than cramped or fatigued after hours of walking or standing. Small changes in footwear choices today may help prevent chronic pain and deformities in the future.

Summer means sandals, swimming pools, beach vacations, water parks, and long days outdoors. While most people focus on sunscreen and hydration, foot health is often overlooked. Unfortunately, summer activi-
ties can expose your feet to bacteria, fungi, injuries, and painful skin conditions that can quickly ruin the season.
From hot sand and public showers to flip-flops and wet shoes, your feet work hard during the summer months. Taking a few preventive steps can help keep them healthy, comfortable, and ready for every adventure.
One of the most common summer foot problems is fungal infec-
tion, especially athlete’s foot. Warm, damp environments such as swimming pool decks, locker rooms, public showers, and water parks are ideal places for fungus to spread. Walking barefoot in these areas increases the risk of developing itchy, peeling, or burning skin between the toes and on the soles of the feet.

Another common summer issue is cracked heels and dry skin. Sand, saltwater, chlorine, and sun exposure can dry out the feet quickly. Cracked skin may become painful and can allow bacteria to enter and cause infection. To help keep feet healthy during summer:
• Wash and dry feet thoroughly every day
• Moisturize heels and soles regularly
• Change out of wet shoes and socks promptly
• Use antifungal powder if feet sweat heavily
• Avoid sharing towels or footwear
• Check feet regularly for cuts, redness, or irritation
People with diabetes or circulation problems should be especially cautious. Even small cuts, blisters, or burns from hot pavement can become serious if not treated quickly. Protective footwear is essential during all summer activities.
Children are also more vulnerable to summer foot injuries. Running barefoot around pools, playgrounds, and beaches increases the risk of burns, splinters, cuts, and infections. Lightweight water shoes can provide important protection while still allowing kids to enjoy outdoor fun. Healthy feet make summer activities safer, more comfortable, and far more enjoyable. There are several beaches in our area that we can go to for a short vacation get away. Our local beaches are not like the Caribbean’s, but that’s okay. Our beaches are enjoyable as well. There’s the boardwalk in Ocean City that has lots of shops, boutiques, and restaurants. Shopping on the boardwalk gives you the feeling of summer, fun and vacationing. The temporomandibular joints (TMJs) play an important role in both oral health and overall wellness. Located just in front of each ear, these small joints connect the lower jaw to the skull and allow essential movements such as chewing, speaking, and breathing. Although they are small, problems with the TMJs can affect many areas of the body.
What Are the TMJs?
The TMJs function like hinges, allowing the jaw to move smoothly. Inside each joint is an articular disc, a cushionlike structure that helps keep the joint properly aligned and prevents the bones from rubbing together during movement. When the joints and discs are functioning properly, jaw movement feels effortless.
However, when the disc becomes displaced or the joint becomes inflamed, the mechanics of the jaw can be



disrupted. Because the TMJs are closely connected to the muscles, nerves, and bones of the head, neck, and shoulders, dysfunction can contribute to a variety of symptoms beyond the jaw itself.
TMJ disorders may develop due to disc displacement, inflammation, injury, or wear and tear. Common symptoms include:

• Jaw pain or tenderness
• Headaches or migraines
• Clicking, popping, or grinding sounds in the jaw
• Difficulty chewing or discomfort while eating
• Neck pain and stiffness
• Dizziness or balance problems
Many people do not immediately associate these symptoms with their jaw joints, which can delay diagnosis and treatment. Over time, untreated TMJ disorders may worsen. Chronic inflammation can contribute to degeneration of the jaw joint and surrounding structures, leading to increased pain and dysfunction.

response to allergens such as pollen, dust, and animal dander. Non-allergic rhinitis can be caused by irritants such as smoke, pollution, and strong odors.
Asthma is a chronic respiratory condition characterized by inflammation and narrowing of the airways, which makes it difficult to breathe. Everyone can come together and the time spent can offer an opportunity to catch up on the activities of each family member. Get the children involved in shopping for groceries and preparing meals. You can teach them about different foods and how to read food labels. In addition, they are more likely to eat what they helped prepared. Be aware of portion sizes, you can obtain this information on MyPlate.gov. Never insist that your child clean their plate, and never use food as a reward or bribe.
Make healthy snacks available. Keep plenty of fruit, vegetables, and healthy beverages (water, milk, and limited amounts of pure fruit juice) on hand so the children can avoid unhealthy snacks like soda, chips, and cookies.

After completing his fellowship in Colon and Rectal Surgery at the prestigious Robert Wood Johnson University Hospital-Rutgers University, Rami Makhoul, MD, joined Holy Cross Hospital in 2014 to serve patients in the DC Metropolitan area. Dr. Makhoul diagnoses and treats various diseases of the colon, rectum, anus, and other parts of the GI tract using national standards and guidelines. He has a special interest in treating colon and rectal cancer using advanced surgical minimally invasive techniques including robotics, laparoscopy, transanal total mesorectal excision and transanal endoscopic microsurgery.

Dr. Shin is the founder and president of BCC Pediatric Dentistry. She is a Board Certified Pediatric Dentist with more than 20 years of practice experience. Dr. Janet V. Johnson was born in Brooklyn, NY. She received her undergraduate degree in Biology at Hunter College in Manhattan, NY. She pursued a career in research medicine as a research assistant at Downstate Medical School in the Department of Gastroenterology. Obtaining her masters

Dr. Mary Alexander, periodontist, obtained a Bachelor of Science degree in Biology from the University of South Carolina and continued her education in dentistry at the University of Maryland School of Dental Surgery where she received her DDS degree. Prior to and during her dental studies, Dr. Alexander received funding and support from the National Institutes of Dental Research for her involvement in the periodontal research.
After obtaining her DDS degree Dr. Alexander spent one year at the Veteran’s Administration Hospital, in general practice residency focusing her training in the area of anxiety patient management. She then attended Harvard University and earned her clinical specialty in periodontics and a doctorate in medical science. Her doctoral research was focused in the study of bone function and metabolic disease of the bone.
Dr. Alexander received extensive training in dental implantology, periodontal plastic surgery, bone regenerative procedures and sedation dentistry. All aspects of Dr. Alexander’s training are incorporated in her periodontal practice assuring patient’s thorough and comprehensive periodontal treatment.


Dr. Horowitz has been practicing in the Bowie area for over 25 years. He graduated from the Illinois College of Podiatric Medicine with a Doctorate of Podiatric Medicine and did his postgraduate training in podiatric medicine and surgery in association with the Podiatry Group of Greater New York and the New York College of Podiatric Medicine. Dr. Jeffrey Brown grew up in the state of Maine and went to Bowdoin College, where he graduated Magna Cum Laude in 1982. He accepted early decision into the Georgetown School of Dentistry, and graduated from there in 1986. While at Georgetown, Dr. Brown worked in the Neurology Department as an assistant on a project that involved studying the regeneration of nerve tissue in the spinal column after major trauma.
Upon graduation from Georgetown Dental School, Dr. Brown worked for a short time as an associate in a practice in Falls Church, VA. From there, he moved on and built his own practice in the Fairlington neighborhood in Arlington, VA, creating one of the largest practices ever seen on the East Coast.

Dr. Brown took a short sabbatical to help raise his four young children and to continue his education. Rachelle is a compassionate Certified Registered Nurse Practitioner with a strong background in patient-centered care.
Originally from Haiti, she moved to the U.S. at 15 and began her nursing journey at Medgar Evers College, earning her Associate Degree in 2014 and BSN in 2018. She gained experience in long-term care and later at Upper Chesapeake Medical Center, where she spent six years in acute and hospital-based care. Rachelle earned her MSN from Walden University and is Board Certified as a Nurse Practitioner.
Fluent in English and Haitian Creole, she is passionate about preventive medicine, patient education, family, and travel.

Born and raised in Kenya, I moved to the U.S. in 2000 and earned my Bachelor of Science in Nursing from Prairie View A&M University in 2008. During my Internship at MD Anderson Cancer Center, I discovered my passion for oncology and began my nursing career there. Over the next 15 years, I gained experience in Med-Surg, oncology, liver transplant, and Neurotrauma ICU.
In 2021, I completed my master’s degree and became a nurse practitioner. I’ve spent the past three years in urology, creating personalized, collaborative care plans. Now living in Maryland, I enjoy tennis, museums, traveling, and time with loved ones.


Dr. Erin Hays grew up in Bethesda, MD. She received her undergraduate degree from Vanderbilt University before attending medical school at University of Maryland where she graduated with honors. She then moved down the road to DC and completed her Urologic Residency at Georgetown University Hospital. Dr. Hays has experience treating a broad range of urologic conditions in both men and women. She holds specific interest in the management of benign prostatic hyperplasia, kidney stones, erectile dysfunction, and urinary incontinence.
In her free time Dr. Hays enjoys biking, playing tennis, and needlepointing.
Dr. Hays joined the AA Urology team in September 2025.

Caroline’s deep compassion for caring for others inspired her nursing career, which began in 2003 as a Nursing and Medication Assistant and LPN through the University of the District of Columbia. She later earned her RN from Hocking College and completed her BSN (2020), MSN-FNP (2022), and DNP (2024) at Walden University.
With over 22 years of bedside nursing experience in Telemetry, Medical-Surgical, and Pediatric Primary Care, she is valued for her clinical expertise and compassionate approach.
Multilingual in English, French, Pidgin English, and Oroko, Caroline also cherishes her family life, faith, gardening, music, and travel.


Belkis was born in the Dominican Republic and raised in Bayonne, NJ, where she developed a deep appreciation for diverse cultures. Fluent in English and Spanish, she connects easily with patients and families from all backgrounds.
She earned her Nursing degree from Bloomfield College and built her career in Anne Arundel County, gaining extensive experience in medical-surgical and general surgical units at Johns Hopkins Bayview and Anne Arundel Medical Center.
Passionate about comprehensive care, she later earned her Master’s as a Family Nurse Practitioner from Chamberlain University.
Outside work, Belkis enjoys family, church, weightlifting, baking, travel, and her German Shepherds.

One of the most common questions I hear from parents is:
“Dr. Shin, my child grinds their teeth at night. Are they damaging their teeth?”
I understand why parents are concerned. Teeth grinding can be surprisingly loud. Some parents tell me they can hear it from down the hall. Others worry that their child is slowly wearing away their teeth while they sleep.
The good news is that, in most
cases, teeth grinding is far more alarming to parents than it is to pediatric dentists.
After nearly 30 years of caring for children, I can tell you that teeth grinding, also known as bruxism, is extremely common. I have seen children who grind occasionally and others who grind so aggressively that their parents are convinced permanent damage is inevitable.
Fortunately, that is rarely the case. How Common Is Childhood Teeth Grinding? Very common.
Studies have reported that anywhere from 15% to 40% of children grind their teeth at some point during childhood, depending on how bruxism is measured and the age group being studied.
While researchers have been studying childhood bruxism for decades, one surprising fact remains: we still do not know exactly why children grind their teeth.
Why Do Children Grind?
If you search online, you will find a long list of possible explanations.
Stress. Anxiety. Airway problems. Allergies. Sleep disorders. Bite problems. Genetics.
The truth is that none of these theories fully explains every child.
Some children who grind have enlarged tonsils, allergies, or disrupted sleep. Others have none of those issues. Some children who grind are anxious. Many are not.
Current evidence suggests that childhood bruxism is likely multifactorial, meaning that several factors may contribute, but there is no single explanation that applies to every child.
One of the most consistent findings in the literature is that grinding often occurs during brief arousals from sleep. Researchers increasingly view childhood bruxism as a sleep-related movement behavior rather than simply a dental problem.
Although the exact cause remains
unclear, there is one pattern I have observed repeatedly throughout my career.
Many children who grind heavily during the primary dentition years gradually reduce or stop grinding around age seven to eight.
Interestingly, this is also the period when the first permanent molars become established in the bite.
Has research proven that the eruption of the first permanent molars causes grinding to stop? No.
At this point, that relationship remains an observation rather than an established scientific fact.
However, after seeing this pattern in thousands of children, I believe it is worth considering whether at least some childhood grinding may be related to the maturation of the developing dentition and occlusion.
Future research may help answer that question more clearly.

It is critical to understand that dietary adjustments have absolutely nothing to do with removing the physical nest; instead, one must consistently apply the enzyme spray and actively rid the environment of these migrating mites on a daily basis. As

What About Worn Teeth?
This is often what worries parents the most.
Some children who grind develop noticeable wear on their baby teeth. Sometimes the wear is mild. Sometimes it can appear quite dramatic, especially on the front teeth.
What I have found reassuring over the years is that significant wear in the primary dentition rarely seems to predict significant wear in the permanent dentition.
Many children who wear down their baby teeth go on to develop healthy permanent teeth with little or no evidence that they were ever grinders.
That observation is one reason I am often more conservative than parents expect when discussing treatment.
Of course, wear should never be ignored. Your pediatric dentist should continue to monitor changes over time, looking for excessive wear, fractures, sensitivity, or other signs of pathology.
But visible wear on baby teeth alone is not usually a reason to panic.
Does Grinding Mean My Child Has an Airway Problem?
Not necessarily.

meal, they are safe to be placed in a vacuum-sealed container and put in the freezer, they can often keep for several weeks.
• Make Time for Sit-Down Meals. Try to have at least three (preferably four) family sit-down meals a week. It helps strengthen the family bond. There is nothing that brings people together like food! It also helps show children the importance of taking time to be with people they love.
• Fiber. Not an essential nutrient, performs several vital functions: Natural laxative and may lower cholesterol concentration in the blood. A high-fiber intake also supplied greater amounts of vitamins A, B6, B12, C, niacin, thiamin, riboflavin and folate, as well as the minerals magnesium, iron, zinc, calcium and phosphorus.
There is growing research exploring associations between bruxism and sleep-disordered breathing, enlarged tonsils, chronic nasal congestion, allergies, and mouth breathing.
These associations are important and deserve attention.
However, association does not mean causation.
Many children who grind have no airway concerns at all, and many children with airway issues never grind their teeth.
That said, if your child grinds and also snores loudly, breathes through their mouth, sleeps restlessly, pauses breathing during sleep, or seems unusually tired during the day, further evaluation may be appropriate.
Should My Child Wear a Night Guard?
This is another common question.
Parents are often surprised when
I tell them that I rarely recommend night guards for young children simply because they grind.
Unlike adults, children are constantly changing. Their jaws are growing, baby teeth are exfoliating, permanent teeth are erupting, and their bite is developing.
For that reason, we must be careful about placing appliances on growing dentitions.
A night guard may protect teeth in certain situations, but it does not address the underlying cause of grinding. More importantly, many children who grind never develop significant dental problems and eventually outgrow the habit on their own.
There are situations where an appliance may be appropriate, such as severe tooth wear, sensitivity, pain, muscle discomfort, fractures, or specific developmental and medical circumstances.
However, these cases are the exception rather than the rule.
In pediatric dentistry, treatment should be based on clinical findings, not simply on the sound of grinding.
Sometimes the most appropriate treatment is careful observation.
The Bottom Line
Childhood teeth grinding is one of the most common concerns parents bring to a pediatric dentist.
Although the sound can be alarming, the scientific evidence and clinical experience both suggest that most children outgrow the habit, most do not require treatment, and most do not experience significant long-term consequences.
As a pediatric dentist, one of the most reassuring observations I can share is this: many children who grind heavily during childhood grow into teenagers and adults with healthy, functional permanent dentitions and little evidence that they ever ground their teeth at all.
So if you hear that familiar grinding sound coming from your child’s room tonight, mention it to your pediatric dentist, have it monitored, and try not to lose sleep over it. Your child probably won’t.
