Pain patients often feel that just as they begin to see some progress with their painful condition, they feel that progress start to slip away. This can be demoralizing as persistent chronic pain affects all aspects of one’s life: personal relationships, career advancement, self-image, leisure activities, and even religious beliefs. Fortunately, there are many therapies that can help patients live happier and more fulfilling lives. Occasionally, however, a patient’s pain might continue even after trying all of the therapies available. For these patients, we look

An exciting development in the field of pain medicine is a new technology called a Dorsal Root Ganglion (DRG) Stimulator. The DRG is a part of the nervous system that acts like a gatekeeper for pain. When a person stubs their toe, that painful sensation travels along a nerve, up their leg, to the DRG in their back. As the gatekeeper, the DRG can either stop the pain in its tracks or it can allow the pain to travel through the gate and travel up the spinal cord to the brain. When the pain gets to the brain, the person knows that they have stubbed their toe and it is painful. If the pain signal is not allowed through, the person will never feel the pain. An interesting aspect of the DRG, however, is that it not only can allow or prevent pain from passing to the brain,

but it can also decrease or increase the level of pain. In fact, in many patients with chronic pain, the DRG is actually malfunctioning by constantly increasing the pain dramatically before it gets to the brain, turning a whisper of pain into a shout. So, sensations that shouldn’t be painful become painful; pain that should go away naturally over time continues. Sometimes a non-painful sensation – like a bed

sheet lightly touching your skin – can be instantly changed into excruciating pain by this malfunctioning nervous system. Fortunately, there is now the DRG stimulator, designed to stop this type of malfunctioning. The DRG stimulator is a very thin wire that is placed over the DRG during a minimally-invasive outpatient procedure, which takes only about 45 minutes. It stimulates the DRG using a very small current, which is not felt by the patient. This stimulation can stop a malfunctioning DRG from increasing the pain before it gets to the brain, stopping the pain in its tracks. Before deciding whether this stimulator is the right fit, a patient can actually try it for 5-7 days without surgery or a hospital stay. Many patients who previously had untreatable pain can now find some relief without dangerous opioids or other medications. Patients who have been diagnosed with Chronic Regional Pain Syndrome (CRPS) or have suffered a nerve injury can often see significant improvement. Other conditions that can be treated include knee pain after knee replacement, hip pain after hip replacement, groin pain, pain after hernia repairs, stump pain, and phantom limb pain. Periodontal disease is a chronic inflammatory condition caused by bacterial infection of

the supporting structures of the teeth. Left untreated, it leads to progressive bone loss, tooth mobility, and eventual tooth loss. More importantly, the inflammatory burden created by periodontal disease does not remain confined to the mouth. Research has consistently demonstrated associations between periodontal inflammation and systemic conditions, including cardiovascular disease, diabetes, respiratory disease, and adverse pregnancy outcomes. In patients with diabetes, periodontal disease can impair glycemic control; in patients with heart disease, chronic oral inflammation may contribute to vascular changes. The mouth serves as a gateway to the bloodstream, allowing inflammatory mediators and pathogenic bacteria to

circulate beyond the oral cavity. Missing teeth also have broader implications than many patients realize. Tooth loss accelerates jawbone resorption, alters bite mechanics, and can negatively affect nutrition and speech. Over time, these changes may impact facial structure and overall quality of life. Dental implants, when placed in healthy, stable bone, can help preserve bone volume and restore proper function—but early evaluation is critical before advanced bone loss occurs. The winter season shouldn’t have to signal a pause in your exercise routine, but it does require some extra caution, especially when it comes to foot health. As a podiatrist, practice owner, and lifelong advocate for outdoor fitness, I’ve seen firsthand the damage that cold, wet, or poorly protected feet suffer from during the colder months. Whether you’re a runner, hiker, or power walker, braving the colder months requires taking proper precautions to safeguard your feet. Motivation to exercise is always a concern. Reducing the hurdles in colder months will help with compliance. So, how do we make winter workouts comfortable instead of a chore? Let’s dive into the simple, yet profound, changes that can pay long-term dividends in comfort, performance, and, most importantly, injury prevention. Why Foot Protection Matters In Winter Your feet are your foundation, and in winter they face a unique set of challenges. Without proper preparation, this can compromise comfort and health. Cold temperatures reduce circulation, leading to numbness, tingling, and even frostbite. Snow, slush, and sweat can dampen socks, increasing the risk of blisters, fungal infections, and skin breakdown. Slippery ice and packed snow make falls more likely, particularly without proper footwear grip. Lastly, many winter shoes use stiffer materials that compress the toes, worsening conditions such as bunions or neuromas. Proactive Strategies For Safe Winter Workouts When exercising in winter, start with footwear designed for cold and wet conditions. The best winter fitness shoes combine waterproofing, such as Gore-Tex or other breathable materials, with insulation that retains heat, but without reducing flexibility. www.yourhealthmagazine.net

Essential Strategies For Health, Comfort, and Performance

Deep soles provide better traction on icy or snowy surfaces, and the fit should leave enough room for warm socks without cramping your toes. Avoid shoes that feel tight once layered to maintain comfort and proper circulation throughout your workout. Never underestimate the importance of a good sock when exercising in cold weather. Choosing the right materials and fabrics make a significant difference. Look for materials like merino wool, Coolmax, or polypropylene blends that wick moisture away from the skin and stay warm even when damp. Whatever you do, ditch the cotton! You know that instantly chilling, clammy feeling when your cotton socks get wet? That’s what you’re setting yourself up for. Cotton traps moisture and will quickly turn your workout from invigorating to miserable. For extra blister protection, consider doublelayer socks, but be sure your shoes have enough room to accommodate them without restricting circulation. Keep your feet warm but avoid excessive insulation, as overheating can lead to sweating, friction blisters, and fungal problems. Layer your socks and footwear sensibly, ensuring warmth without trapping too much heat. If your feet become damp during a workout, change into dry socks right away to maintain comfort and prevent irritation. When exercising or walking on snowy or icy ground, prepare your footwear for the terrain. Clip-on traction devices such as Yaktrax or microspikes can greatly reduce the risk of slipping and falling. Since winter surfaces are often uneven, choose shoes with good ankle support, such as a firm heel counter or a slightly higher collar, to improve stability and protect against ankle injuries. Cold muscles, including those in your feet, are more susceptible to injury, so it’s important to warm up properly before winter exercise. Start

indoors or in a sheltered area with light dynamic movements such as toe flexes, point-and-flex drills, calf raises, ankle circles, walking lunges, or a gentle march in place. These activities prepare your muscles and joints for outdoor activity while improving balance and flexibility. Pay close attention to any warning signs of cold-related injury during winter activity. Early frostbite may cause numbness, pale or grayish skin, and pain as the area warms. Chilblains can appear as tender red bumps with itching or burning sensations, while blisters may form as swelling or fluid-filled pockets, especially between the toes. If you notice any of these symptoms, stop immediately, warm up, and avoid further exposure. Persistent numbness or pain should be evaluated by a medical professional without delay. After any outdoor activity in cold weather, make daily foot care part of your routine. Wash your feet thoroughly with warm water and mild soap, then dry them carefully, especially between the toes. Apply a moisturizing or barrier lotion to prevent dryness and cracking, and take a moment to inspect for redness, blisters, or skin breakdown that could signal irritation or injury. Regular attention helps keep your feet healthy, warm, and ready for your next winter workout. Here’s a small podiatry secret that makes a huge difference! Always trim your toenails straight across. This is vital year-round, but it becomes critical in winter. Tight shoes will press against long nails, almost guaranteeing an ingrown nail. Regular trimming not only prevents discomfort but also helps maintain proper foot alignment and reduces irritation during winter activities. Even in cold weather, it’s important to stay hydrated, as dry indoor heating and low humidity can dehydrate you more quickly than you might expect. Dehydration affects

not only overall performance but also skin health and wound healing, making it essential to drink water before, during, and after your workouts to keep your body and feet in top condition. Special Considerations For People With Diabetes Or Circulatory Conditions Anyone with diabetes, neuropathy, or vascular disease must take extra care. Poor circulation and decreased sensation heighten the risk of frostbite or injury. Always use insulated, well-fitted shoes and socks, inspect feet before and after activity, and report any unusual symptoms to your podiatrist. Red Flags That You Might Require Professional Medical Assistance Seek immediate medical attention if you experience persistent numbness or tingling in your toes, notice sores, cracks, or ulcers that fail to heal, or observe color changes such as redness, bluish tones, or black discoloration. Signs of infection, including pus, spreading redness, or fever, also warrant evaluation. Winter doesn’t have to limit your activity, but it does call for extra awareness. By combining proper footwear, smart habits, and regular foot checks, you can stay active and comfortable while keeping your feet safe all season long. Final Thoughts Exercise is important for many aspects of life. Health, both physical and mental, are improved with regular exercise. Finding a form of activity that you enjoy and are willing to do is hard enough without having to worry about weather. Using the correct shoe gear, taking precautions, and knowing your limitations will make this both enjoyable, and productive. Does Fluoride Lower the IQ? A pediatric dentist’s evidence-based look at what new studies are showing and how families can stay both cavity-free and safe.

By Elizabeth Shin, DDS Bethesda Chevy Chase Pediatric Dentistry What People Are Really Asking When people hear about fluoride and IQ, most are not thinking about research papers or chemical names. They are thinking about the fluoride their children use every day, brushing their teeth, rinsing, or getting fluoride treatments at the dental office. That makes sense. But the studies that raised this question are not about fluoride that touches the teeth and gets spit out. They focus on fluoride that is swallowed, the kind that goes through the body in drinking water, baby formula mixed with tap water, and foods or beverages made with fluoridated water. To understand the science, it helps to separate topical fluoride, which works on the surface of the tooth, from systemic fluoride, which travels through the bloodstream. Both play a role in oral health, but only the systemic kind is being studied for possible effects on the brain and body. The Study That Reignited the Discussion In early 2025, JAMA Pediatrics published a review combining 74 studies on fluoride exposure and children’s IQ. The researchers found that, overall, children with higher fluoride levels in their bodies had slightly lower IQ scores. They did not claim fluoride directly causes lower IQ, but the link appeared stronger in studies that measured fluoride inside the body, such as urinary levels, rather than simply using city water data. Their message was straightforward. The pattern is strong enough to warrant a fresh look at how much fluoride children are receiving from all sources combined. Differing Scientific Opinions Some academics have questioned the review, noting that many of the studies it included came from countries with higher natural fluoride levels or from communities with co-exposures such as arsenic or poor nutrition. Others have argued that the review’s methods may have combined studies of unequal quality. However, even among critics, there is general agreement that the findings raise valid questions and that more well-controlled research at U.S. exposure levels is urgently needed. Time Scientists Saw This The JAMA study confirmed what earlier research had already noticed. In Canada (2019) pregnant women with higher urinary fluoride had children with slightly lower IQ scores, especially boys. In Mexico (2017–2018) two separate studies linked prenatal fluoride exposure with lower cognitive and attention scores. In Canada (2020) babies fed formula mixed with fluoridated tap water scored lower on non-verbal IQ tests than breastfed babies. Earlier reviews (2012–2018) also showed an inverse relationship between fluoride exposure and IQ, though quality varied. In New Zealand (2015) one study found no difference, but it used residence rather than biomarkers to estimate exposure. Taken together, this growing body of evidence suggests a consistent pattern that deserves attention, especially for developing brains. Beyond IQ: Other Health Clues Even if we set aside brain development, fluoride that is swallowed leaves unmistakable signs inside the body. The most visible sign is dental fluorosis, faint white lines, cloudy patches, or brownish streaks that appear on teeth as they form. Many professionals call it a cosmetic issue, but that view misses the deeper biological meaning. Fluorosis as a Warning Sign Fluorosis is not merely a surface change; it is a biological warning sign of overexposure. It tells us that, during tooth development, the body absorbed more fluoride than it could safely manage. At that point, the safe threshold for cellular balance has already been surpassed. Because fluoride is strongly attracted to calcium, it tends to settle in tissues rich in calcium, including teeth, bones, the pineal gland, and even developing regions of the brain. The enamel changes we can see are simply the visible tip of a deeper metabolic shift. Systemic Effects Beyond the Teeth The same chemical interactions that create white streaks in enamel may also be occurring silently in the thyroid, kidneys, and nervous system, where we cannot see them with the naked eye. Fluorosis, therefore, should not be dismissed as cosmetic. It is the body’s way of signaling that fluoride exposure has reached a level capable of altering normal cellular activity. The teeth are the dashboard light, and once they show a warning, the rest of the system deserves inspection. Topical Fluoride:

Still a Trusted Ally Fluoride that stays on the tooth surface remains one of dentistry’s most effective tools. Toothpaste, varnish, and sealants all work topically to harden enamel and prevent decay. The question today is not whether fluoride works, but how much and how it is delivered. In the 1950s, when toothpaste was rare, water fluoridation helped communities reduce cavities. Now, with widespread fluoride toothpaste, professional varnish, and better nutrition, mass ingestion may no longer add much benefit and could increase total exposure unnecessarily. Modern prevention should be targeted rather than universal ingestion. Practical Questions People Are Asking About Fluoride 1. If fluoride helps teeth, why are people suddenly worried about it? Because scientists are now examining how swallowed fluoride affects other organs besides teeth. Fluoride is beneficial in small amounts, but it can accumulate in bones and tissues. Researchers are exploring its possible influence on the brain, thyroid, and kidneys. The goal is balance, not elimination. 2. How can parents protect their children without overdoing fluoride? Use toothpaste properly: rice-sized for toddlers, pea-sized for preschoolers, and teach them to spit, not swallow. Professional fluoride varnish and sealants are safe and stay on the teeth. When preparing baby formula, alternate between fluoridated and low-fluoride bottled or filtered water. Good brushing and diet remain the best cavity prevention. 3. Is fluoridated water safe during pregnancy? Pregnancy is a sensitive time for brain and organ development. Expectant mothers can use reverseosmosis or carbon filters for drinking water, mix bottled and filtered water to reduce total intake, and maintain gum health since inflammation during pregnancy can affect the baby. Lowering ingestion is a reasonable precaution, not an alarm. 4. What about adults with kidney or thyroid conditions? People with reduced kidney function clear fluoride more slowly, so it can accumulate. Those with thyroid disorders may also be more sensitive. Choose filtered or non-fluoridated water when possible. Continue using fluoride toothpaste since it acts topically. Inform your doctor about local water fluoridation to help interpret labs. Fluoride is not automatically harmful, but those with chronic illness should manage intake carefully.

5. Are fluoride-removing filters expensive? Not really. Reverse-osmosis systems cost about $150–300 with annual filter changes, and smaller countertop models start near $50. For families with infants or chronic illness, it is a practical safeguard. 6. How can I find out if my water has fluoride? Check your city’s annual water report or the CDC’s “My Water’s Fluoride” website. If you use a private well, have it tested, since natural levels vary widely. 7. Does bottled water contain fluoride? Some brands do and others do not. Always read the label. For baby formula, most pediatricians suggest non-fluoridated bottled or filtered water. 8. What is the safest overall approach? Keep topical fluoride such as toothpaste, varnish, and sealants. Reevaluate systemic exposure during pregnancy, infancy, or chronic illness. Support new U.S.-based studies that measure real fluoride levels instead of relying on decades-old data. Fluoride’s value remains, but dose and delivery are what truly determine safety. So, Does Fluoride Lower IQ? The fair answer is that higher systemic fluoride exposure is associated with slightly lower IQ in many studies. At typical U.S. fluoridation levels, the data are still limited. Topical fluoride remains safe and effective. In short, keep fluoride on the teeth, not in the bloodstream. A Final Word Community water fluoridation was one of the great health achievements of the twentieth century. Yet children today already receive fluoride from toothpaste, varnish, and food. We can preserve its benefits while reducing unnecessary ingestion. Re-examining systemic fluoride is not anti-science; it is good science. Our responsibility is to use fluoride wisely, to protect both smiles and developing minds. During his Residency training at GW, Dr. Makhoul spent a year in clinical research where he published much of his work in colon and rectal cancer, and the use of Robotic Surgery in various abdominal procedures. He continued with his passion for research and published peer-reviewed articles during his Fellowship training. Dr. Jeffrey Brown grew up in the state of Maine and went to Bowdoin College, where he graduated Magna Cum Laude in 1982. He accepted early decision into the Georgetown School of Dentistry, and graduated from there in 1986. While at Georgetown, Dr. Brown worked in the Neurology Department as an assistant on a project that involved studying the regeneration of nerve tissue in the spinal column after major trauma. Upon graduation from Georgetown Dental School, Dr. Brown worked for a short time as an associate in a practice in Falls Church, VA. From there, he moved on and built his own practice in the Fairlington neighborhood in Arlington, VA, creating one of the largest practices ever seen on the East Coast. Dr. Brown took a short sabbatical to help raise his four young children and to continue his education. It was then that he began the process of understanding sleep apnea and how it correlates to TMJ treatment. His knowledge in the combined fields of TMJ, sleep and orthodontics has given him a perspective unlike most other practitioners who perform basic dentistry. In addition, Dr. Brown has also learned the advanced techniques involved in expanding an airway so that both children and adults can breathe better. His training continued over time at the famous Sleep Medicine Center near the Emory University Hospital. He is a certified Fellow of the American Academy of Craniofacial Pain, awarded the Level of Legacy with the ALF Interface Academy and holds memberships with many academies. In 2013, Dr. Brown met Dr. Brendan Stack and was immediately impressed with the success Dr. Stack had in treating TMJ cases and being able to repair so many damaged lives. Caroline’s deep compassion for caring for others inspired her nursing career, which began in 2003 as a Nursing and Medication Assistant and LPN through the University of the District of Columbia. She later earned her RN from Hocking College and completed her BSN (2020), MSN-FNP (2022), and DNP (2024) at Walden University. In 1988, I delved into veterinary acupuncture, earning certification from the International Veterinary Acupuncture Society. This led me into holistic medicine, where I gained certification in homeopathy and underwent training with Dr. Richard Pitcairn. In 1993, I sold my practice in Maryland and pursued spirituality in Sedona, AZ, becoming a Tibetan Buddhist nun until 2023. Embracing the belief in the equality of all life, I dedicated myself to alleviating suffering. Returning to Maryland in 2006, I co-founded Holistic Veterinary Healing in 2013. Recognizing the interconnectedness of human and animal health, I pursued a Master's degree in Human Acupuncture in 2010. Since 2016, I've focused solely on animal patients, certified in Medical Ozone Therapy and as a Fear Free Practitioner. Children emulate the adults in their lives in every way. Be they parents, grandparents, aunts, uncles, siblings, or childcare providers. Make sure you use “thank you” and “please” when you talk to them. Insist that they use their words too. Remember, good

manners and gratitude overlap. Here are several ways to help with teaching gratitude to your children: Work gratitude into your daily conversation. Weave appreciation for mundane things into your everyday talk. Pick an “I am thankful for…” part of the day to turn up the gratitude in your home. This can be done at dinnertime or at nighttime just before bed as part of a nightly routine. Have kids help. By participating in simple household chores like feeding the dog or stacking dirty dishes on the counter, kids realize that all these things take effort. There is a better appreciation for the person who does the chore on a more regular basis. Provide your family opportunities to serve. Figure out some way your child can actively participate in helping someone else. Even if it is to help cook a meal, bake a cake, or walk a pet for a sick person.
