People love foot creams, and I’m all for them – but they only work when internal hydration and nutrition are addressed. Venous insufficiency and
lymphatic issues cause fluid to pool in the lower legs and feet. Ankles swell, skin becomes shiny and fragile, and small injuries can turn into chronic wounds because the tissue is waterlogged and poorly nourished.
Diet plays a quiet but powerful role here. High sodium intake leads to more fluid retention, swelling, and pain. Adequate water intake helps the kidneys flush excess sodium and keeps blood volume stable. Potassium-rich foods like bananas, leafy greens, and beans, along with magnesium from

dense food. Dehydration makes fascia stiff and irritable, while poor nutrition leaves skin thin, dry, and prone to cracking. Regular water intake, waterrich produce, adequate protein, vitamin C, zinc, and healthy fats support collagen production and skin repair. You can’t out-cream a bad diet. Your feet contain 26 bones and dozens of joints. When these weaken, everything downstream becomes harder. Standing at work, walking for exercise, even balancing in the shower depend on foot strength and stability. Osteopenia and osteoporosis often show up as stress fractures, midfoot collapse, and painful deformities. These aren’t just “bad luck” –they’re often nutritional and lifestyle stories playing out in bone.
Calcium from dairy, leafy greens, and fortified foods builds bone. Vitamin D helps absorb that calcium, while protein and trace minerals like magnesium, zinc, and manganese keep bones resilient.
In my practice, I see a familiar pattern: a busy postmenopausal woman, minimal dairy, lots of coffee, little sun, long hours on hard floors. She comes in with sudden forefoot pain. X-rays reveal a stress fracture. We use a boot – but we also talk about breakfast, vitamin D levels, and realistic dietary changes.
Arthritis in the feet thrives on inflammation and excess load. Food won’t reverse severe arthritis, but it absolutely influences pain and progression. Anti-inflammatory eating patterns – fatty fish, olive oil, nuts, seeds, leafy greens, colorful vegetables, berries, turmeric, and ginger – can reduce systemic inflammation. Limiting sugar, refined carbohydrates, and excess alcohol helps too. Even modest weight loss can significantly reduce joint stress.
When patients ask, “Okay, Doc – what should I actually do?” I keep it simple: build meals around vegetables, lean protein, and healthy fats; choose low-glycemic carbs; cut back on sodium and ultra-processed foods; drink water throughout the day; and move daily – even 10–15 minutes helps circulation and lymph flow.
Finally, don’t ignore the basics. Check your feet daily, especially if you have diabetes or circulation issues. Address blisters, calluses, swelling, or pain early. Wear supportive footwear that fits properly. Healthy feet keep you moving – and movement keeps you living.

When patients think about improving their smile, they often focus on teeth – whitening, straightening, or replacing them. However, from a clinical standpoint, the health and architecture of the supporting periodontal tissues play an equally critical role in determining aesthetic outcomes.
A “gorgeous smile” is not defined by teeth alone. It is the result of a balanced relationship between teeth, gingival contours, bone sup-
port, and overall oral symmetry. Even the most advanced cosmetic or restorative procedures can fall short if the underlying periodontal foundation is compromised.
Periodontal disease disrupts this foundation through chronic inflammation, leading to gingival recession, loss of papillae (the triangular gum between teeth), and resorption of supporting bone. These changes alter tooth proportions, create visible root surfaces, and produce uneven gum lines – factors that significantly affect smile aesthetics. In advanced cases, the loss of interdental papilla results in open spaces, often referred to as “black triangles,” which are particularly difficult to correct once established.
Inflammation also affects tissue quality. Healthy gingiva exhibits firm, well-contoured margins that frame the teeth symmetrically. In contrast, inflamed tissue appears swollen, irregular, and prone to

bleeding. This not only impacts appearance but also compromises the predictability of cosmetic procedures such as veneers, crowns, and implant restorations.
From a treatment perspective, periodontal stability must be established before aesthetic enhancement. This includes controlling bacterial infection, reducing inflammation, and ensuring that bone levels are stable. Once a healthy foundation is achieved, periodontal procedures can be used to refine aesthetics. These may include crown lengthening to correct uneven gum lines, soft tissue grafting to address recession, and regenerative techniques to rebuild lost support.
Dental implants further illustrate the importance of periodontal principles in aesthetics. Successful implant outcomes depend not only on osseointegration but also on the preservation and management of
surrounding bone and soft tissue. Without adequate tissue volume and contour, even well-integrated implants may appear unnatural. It is also important to recognize that aesthetic concerns are often early indicators of underlying disease. Patients who notice gum recession, shifting teeth, or changes in spacing may be observing the visible effects of periodontal breakdown. Addressing these concerns early allows for more conservative treatment and better long-term results.

Urologic health is a vital component of overall well-being, yet it is often overlooked – especially when it comes to women. Women’s urologic health involves the care and maintenance of the urinary tract and reproductive organs, and addressing the unique challenges that women face in this area is essential
for long-term health and quality of life.
1. Urinary Tract Infections (UTIs): UTIs are among the most common urologic issues affecting women. Due to the shorter length of the female urethra, bacteria can more easily reach the bladder, leading to infections. Symptoms include a burning sensation during urination, frequent urges to urinate, and cloudy or strongsmelling urine. Prompt treatment with antibiotics and good hygiene practices can help prevent recurrent infections.
2. Overactive Bladder (OAB): OAB
is characterized by a sudden and frequent need to urinate, sometimes accompanied by leakage. It can be caused by factors like weakened pelvic muscles, nerve damage, or hormonal changes. Treatment options include lifestyle modifications, pelvic floor exercises, medications, and in some cases, surgical interventions.
3. Urinary Incontinence: Incontinence, or the involuntary loss of urine, affects women of all ages but becomes more common after childbirth and menopause. Stress incontinence (leakage during physical activity) and urge incontinence (a sudden,


intense need to urinate) are the most common types. Treatments range from physical therapy and bladder training to medications and surgical procedures.
4. Interstitial Cystitis (IC): Also known as painful bladder syndrome, IC is a chronic condition causing bladder pressure, pain, and frequent urination. Though its exact cause is unclear, treatments focus on symptom relief through dietary changes, physical therapy, medications, and bladder instillations.
5. Pelvic Organ Prolapse: This condition occurs when the muscles and tissues supporting the pelvic organs weaken, causing organs like the bladder, uterus, or rectum to drop into the vaginal canal. Symptoms include pressure, discomfort, and difficulty with urination or bowel movements. Treatment may include pelvic floor exercises, pessaries, or surgery.
Prevention and Care
Maintaining urologic health involves a combination of lifestyle choices, preventive care, and regular medical check-ups. Here are some key strategies:
• Stay Hydrated: Drinking enough water helps flush bacteria from the urinary tract and supports overall kidney function.
• Practice Good Hygiene: Wiping from front to back, wearing breathable cotton underwear, and avoiding harsh soaps can reduce the risk of infections.
• Strengthen Pelvic Floor Muscles: Kegel exercises can help prevent incontinence and support bladder control.
• Maintain a Healthy Diet: Limiting bladder irritants like caffeine, spicy foods, and acidic beverages can help manage symptoms of overactive bladder and IC.
• Regular Medical Checkups: Annual visits to a gynecologist or urologist can help catch potential issues early and ensure comprehensive care. Orthodontic care is often associated with braces during the teenage years. However, many orthodontic concerns begin much earlier as a child’s jaws and facial structures develop. Interceptive orthodontics focuses on identifying and addressing these issues during childhood, while the bones are still growing and more responsive to guidance.
Rather than waiting for permanent teeth to come in and correcting problems later, interceptive treatment
aims to guide development early. This proactive approach can improve how the teeth, jaws, and airway develop, often reducing the complexity of treatment in the future.
Children’s mouths are constantly changing. As baby teeth fall out and permanent teeth emerge, the jaws expand and facial structures mature. During this period of growth, small adjustments can have a significant impact on long-term alignment and function.
Interceptive orthodontics typically begins between the ages of six and ten, when early signs of crowding, bite misalignment, or jaw development issues may become visible. Addressing these concerns during active growth allows providers to work with the body’s natural development rather than trying to correct fully formed structures later.
Early orthodontic evaluation can reveal several developmental patterns that benefit from early guidance. These may include crowding caused by narrow
dental arches, crossbites where upper and lower teeth do not align properly, or significant overbites and underbites.
Interceptive care may also address habits that affect development, such as prolonged thumb sucking or mouth breathing. These patterns can influence how the jaw grows and how teeth align over time. Identifying them early helps prevent more complex issues from developing.

Every April, healthcare professionals and wellness advocates recognize Foot Health Month, a time dedicated to raising awareness about the importance of caring for our feet. Although feet are often overlooked in daily health routines, they play a crucial role in mobility, balance, and overall well-being. With each step we take, our feet support the entire weight of our bodies, making proper foot care essential for maintaining an active and healthy lifestyle.
The human foot is a complex structure made up of 26 bones, 33 joints, and more than 100 muscles, tendons, and ligaments. This intricate design allows the feet to absorb shock, maintain balance, and help us move efficiently. Because they endure constant pressure from walking, running, and standing, feet are vulnerable to a variety of problems if they are not properly cared for.
One of the most important aspects of foot health is wearing the right footwear. Shoes that are too tight, too loose, or lack proper support can lead to common conditions such as blisters, corns, bunions, and plantar fasciitis. Choosing shoes that fit well and provide adequate cushioning and arch support can help prevent many of these issues. For individuals who spend long hours on their feet, supportive footwear can significantly reduce strain and discomfort.
Daily foot hygiene is another key part of maintaining healthy feet. Washing feet with warm water and mild soap, drying them thoroughly –especially between the toes – and applying moisturizer to prevent dryness can help keep skin healthy. Regularly trimming toenails straight across and keeping them at a moderate length also helps prevent painful problems like ingrown toenails.
Foot health is particularly important for individuals with certain medical conditions. People living with diabetes, for example, must pay special attention to their feet because nerve damage and reduced circula-
tion can make it harder to notice injuries. Even small cuts or blisters can develop into serious infections if left untreated. Regular foot checks and visits to healthcare providers are essential for preventing complications.
Exercise and stretching can also benefit foot health. Strengthening the muscles of the feet and ankles improves stability and reduces the risk of injury. Simple activities such
as toe stretches, ankle rotations, and picking up small objects with the toes can help maintain flexibility and strength.

To diagnose TMD, the first step is obtaining a high-quality MRI of the jaw joints. However, this alone isn’t

enough. The real challenge is having the scans interpreted by a radiologist with the necessary expertise. Unlike MRIs of the knee or shoulder, TMJ imaging requires advanced skills and significant experience to analyze properly. Time and again, we’ve encountered patients whose MRI reports don’t align with their symptoms. They report persistent clicking, popping, or pain, yet their reports claim no displacement. Once we redo the imaging at a trusted facility of mine, we uncover the true issues – displaced discs and degeneration – matching their discomfort. This stark contrast highlights the importance of relying on both advanced imaging technology and skilled radiologists for effective TMD diagnosis.

Hyperpigmentation is a common skin concern where patches of skin become darker than surrounding areas due to excess melanin production. Melanin, produced by cells called melanocytes, protects skin from damage but can overproduce in response to various triggers. This results in dark spots like sun damage, melasma, or post-inflammatory marks. It affects all skin tones but is often more noticeable and persistent in medium to darker complexions.
Two categories address some of
the possible causes: external (environmental exposures) and internal (systemic or bodily factors). Many involve oxidative stress, cellular “rust” from free radicals that amplifies melanin production.
External Causes: Environmental Triggers
Ultraviolet (UV) Radiation from Sunlight: UV rays (especially UVA) remain the primary driver of hyperpigmentation. They stimulate melanocytes to produce more melanin as a defense mechanism, leading to solar lentigines (age spots) on sun-exposed areas like the face, hands, and chest. Cumulative exposure over years explains why spots often worsen with age.
High Energy Visible Light (HEV) , also known as blue-violet light, is everywhere around us from screens and LEDs. Light from phones, computers, and indoor lighting all induces oxidative stress and directly activates melanogenesis pathways. Studies confirm constant exposure contributes to hyperpigmentation, particularly in skin of color, by gen-
erating reactive oxygen species that boost melanin. This “digital aging” effect adds to UV damage in our screen-heavy lives.
Pollution, irritants, and chemicals in face and hair products, laundry detergent, harsh ingredients in creams, lotions, or unregulated lightening products all cause inflammation, triggering post-inflammatory hyperpigmentation (PIH). Unregulated bleaching creams often contain mercury or high-potency steroids, risking permanent discoloration, ochronosis, or systemic toxicity (e.g., kidney effects). Evidence warns against nonprescription “fast-whitening” products; stick to regulated, dermatologistrecommended options.
These external factors are largely controllable through protection and mindful product choices.
Internal Causes:
The Body’s Silent Contributors
Not all pigmentation stems from the environment; internal signals play an equally powerful role. Hormonal fluctuations top the list, most famously in melasma (chloasma or “mask of pregnancy”). Estrogen and progesterone spiking during pregnancy, with oral contraceptives, hormone replacement therapy, or even puberty stimulate melanocytes, particularly on the forehead, cheeks, and upper lip. Melasma patches are larger and more symmetrical than sun spots and notoriously resistant to treatment without addressing the hormonal root.
Genetics set the stage for vulnerability. Families prone to freckles or those with darker complexions inherit more active melanocytes. Maturational hyperpigmentation, a gradual darkening on the cheeks in middle age, often ties to genetic predisposition combined with chronic sun exposure. Medications can induce widespread or patchy darkening. Oral antibiotics (especially tetracyclines), antimalarials, tricyclic antidepressants, chemotherapy agents, and even nonsteroidal anti-inflammatories increase photosensitivity or directly stimulate

After completing his fellowship in Colon and Rectal Surgery at Robert Wood Johnson University Hospital-Rutgers University, Rami Makhoul, MD, joined Holy Cross Health Hospital in 2014 to serve patients of the DMV area.
Upon graduating from medical school, Dr. Makhoul first completed his internship in General Surgery at the University of Massachusetts Medical School. He, then, completed his residency training in General Surgery at the George Washington University Hospital in Washington, DC.
During his residency training at GW, he spent a year in clinical research where he published some of his work on colon and rectal cancer, and the use of Robotic Surgery in various abdominal procedures performed in the Colon and Rectal Surgery field. He continued with his passion for research and published peer-reviewed articles during his fellowship training. Several of his work, was presented at national conferences including the American Society of Colon and Rectal Surgery, and the Society of American and Gastrointestinal and Endoscopic Surgeons annual conferences.
Dr. Makhoul diagnoses and treats various diseases of the colon, rectum, anus, and other parts of the GI tract using national standards and guidelines. He has a special interest in treating colon and rectal cancer using advanced surgical minimally invasive techniques including robotic, laparoscopic, and transanal minimally invasive surgery. Dr. Makhoul is Board Certified in both General Surgery and Colon and Rectal Surgery and is an active member of the American Society of Colon and Rectal Surgeons education committee.
As part of his commitment to surgical training and education, Dr. Makhoul serves as a Clinical Instructor of Surgery at the George Washington University. Dr. Makhoul is currently the chairman of the Department of Surgery at Holy Cross Hospital in Silver Spring. He also holds the position of Cancer Liaison Physician for Holy Cross Health, where he manages and overlooks clinically related cancer activities in collaboration with the Commission on Cancer and American Cancer Society. When he is away from the office, Dr. Makhoul may be found spending time with his family. Dr. Makhoul enjoys skiing, cycling, scuba diving, tennis, and traveling.


Dr. Matthew Skancke is a colon and rectal surgeon serving Maryland, Virginia and Washington, DC. Dr. Skancke grew up in Great Falls, VA earning his undergraduate degree in electrical and biomedical engineering at the University of Virginia’s engineering school. After engineering school he spent three years working for a Swiss based GPS company U-blox America while going to night school to finish his premedical degree. He then attended medical school at the University of North Carolina at Chapel Hill where he did basic science research of mesenchymal stem cells to progress the field of regenerative medicine. Dr. Skancke continued his General Surgery training at the George Washington University Hospital and subsequently did his Colon and Rectal Surgery Fellowship at the Cleveland Clinic.
During his training, Dr. Skancke has published over 24 peer reviewed publications, authored three book chapters, and presented over 45 times at national conferences in medicine and surgery. He has participated in clinical trials of bench top and mechanical sciences and is a member of the American College of Surgeons and the American Society of Colon and Rectal Surgeons. Dr. Skancke is Coard Certified in General Surgery and Colon and Rectal Surgery and is an active member of the American Society of Colon and Rectal Surgeons Quality and Safety National committee.

A growing number of individuals are seeking healthcare solutions from Chinese medicine and other Complementary and Alternative Medicine (CAM) practices. According to the National Center for Complemen-
tary and Alternative Medicine (NCCAM), approximately 38% of all adults, 44% of those aged 50-59, and 12% of children reported using CAM in 2007. Notably, 24-44% of epilepsy patients are already incorporating CAM into their treatment regimens.
The ancient Chinese text, Huang Di Nei Jing, dating back over 2,000 years, mentions epilepsy. Chinese herbal medicine and acupuncture are the primary modalities within TCM. Recent studies suggest promising results for epilepsy treatment using Chinese medicine therapies, including herbal medicines and acupuncture.

Tooth decay is one of the most common chronic conditions of childhood in the United States. Untreated tooth decay can cause pain and infections that may lead to problems with eating, speaking, playing, and learning. Even though your baby doesn’t have teeth, you should still clean his or her mouth. It makes for a good idea to get in the habit of cleaning your baby’s gums soon after birth. Although there will be a little fussing at first, your infant will get used to having the mouth cleaned like other parts of their body. Many children grow to enjoy brushing their teeth as part of their daily routine.

From early adulthood through midlife and beyond, a woman’s health is shaped by changing hormones, metabolism, stress levels, lifestyle, and life demands. These systems are interconnected. When one area becomes imbalanced, others often follow. Yet many women spend years treating symptoms without understanding the underlying cause.
True women’s health care begins with listening – to both the patient and the body. Subtle changeså in energy, focus,
appetite, or mood may be early signs of hormonal shifts, metabolic changes, thyroid imbalance, or chronic stress. These changes don’t always show up clearly on routine tests, which is why individualized evaluation and context matter so much.

It’s completely normal and healthy for a baby or young child to suck on a thumb, finger or pacifier. It is not something you need to be alarmed about or discourage.
Children usually give up sucking habits on their own by the time they are 3-4 years old. If they stop the habit at this age, the shape of the jaw is usually not affected. The teeth should grow in normally. However, some children find the sucking habit hard to break. Children who are still sucking on a pacifier, finger or thumb

when their permanent (adult) teeth start to come in are more likely to have bite problems.
To help an older child break a sucking habit, it is important to explain clearly why the habit can be harmful. Be supportive and encouraging, and praise the child’s efforts to end the habit.
If the child sucks a finger or pacifier because of a stressful situation, it can be very helpful to address the source of the stress.
