Cataract Implants Seeing In Three Dimensions

Approximately 20.5 million Americans age 40 and older have cataracts. The opportunity for freedom from reading glasses and bifocals was not available in intraocular lens implants. There are currently two of these advanced lenses that have become popular. Clareon PanOptix & Vivity by Alcon. They are known as “multifocal lenses” meaning they allow quality vision at near distance and intermediate zones through a series of focusing rings. Our lives have become more complex and our vision is oftentimes tested throughout an average day of work. We have to read materials at close range, look at a computer in the intermediate range and perform tasks such as driving, requiring good distance vision. During the course of a day we have to constantly refocus our eyes in order to see multiple distances. Those of us who are over 40 have noticed that our reading vision has changed for the worse. Many of us require reading glasses because we no longer have the functional ability to focus on near objects like we did when we were younger. The ability to change focus rapidly and focus on near objects is called accommodation and this process declines with age. In addition to this, the natural lens within our eyes starts to become cloudy with age. What Is a Cataract? Cataracts are, in fact, the clouding of the eye’s clear lens, like a window that is “fogged” with steam. When the lens becomes cloudy, light rays can not pass through it easily and vision becomes blurry. Cataracts are not a growth or a film over the eye.

Cataracts start out small and have little effect on vision at first, but they continue to grow slowly. As the cataract grows, so does the impact on vision. The most common signs of cataract development are: • Painless blurring of vision • Sensitivity to light and glare • Double vision in one eye • Poor night vision • Fading or yellowing of colors • Frequent changes in glasses or contact lens prescriptions Although cataracts usually develop as part of the aging process (more than half of all Americans develop cataracts by age 80), they can also result from: • Eye injuries • Certain diseases, such as diabetes • Genetic inheritance • Certain medications • Frequent, unprotected exposure to UV-A and UV-B rays What Is the Lens? The lens is the part of the eye that helps focus light on the retina. The retina is the eye’s light-sensitive layer that sends visual signals to the brain. In a normal eye, light passes through the lens and gets focused on the retina. To help produce a sharp image, the lens must remain clear.
How Is a Cataract Treated? For an early cataract, vision may improve by using different eyeglasses, magnifying lenses, or stronger lighting. If these measures don’t help, surgery is the only effective treatment. This treatment involves removing the cloudy lens and replacing it with a substitute lens, also known as an implant. A cataract needs to be removed only when vision loss interferes with your everyday activities, such as driving, reading, or watching TV. You and your eye surgeons can make that decision together. How Is a Cataract Removed? Cataracts are removed with surgery, performed in an outpatient surgical center. In most cataract surgeries your native lens is replaced by an intraocular lens (IOL). The traditional method of replacing your native lens is with an IOL, aimed for good distance vision only. These are known as “monofocal lenses”. You would oftentimes still need reading glasses with monofocal lenses. One way to decrease your need for reading glasses would be to have one eye corrected for distance and one for near. This is called monovision. Depth perception can sometimes be affected by this option. That is why the new IOL technology is so exciting. Each of the approved IOLs has its advantages and disadvantages. Your eye surgeon will review the risks, benefits and alternatives of multifocal implants with you in order to choose the best lens for you. Cataract surgery is covered by most insurance plans, however, the newer IOL implants are generally not covered and require additional outof-pocket expenses besides the usual co-pay, co-insurance, or deductibles you would normally be responsible for. While this is unfortunate financially, the IOLs themselves are certainly worth the extra expenditures in appropriate patients. Your surgeon will determine if you are a good candidate for these new implants.

Guiding Healthy Growth, Function, and Development In Children and Young Adults By Maribel M. Vann, DDS Childhood is a remarkable period of growth. While parents often pay attention to their child’s height, weight, and academic progress, one very important aspect of development is frequently overlooked – the growth of the jaws, airway, tongue posture and facial bones. These structures influence much more than a beautiful smile. They affect airway (breathing), sleep quality, posture, speech, chewing, swallowing and overall health. An orthopedic/orthodontic treatment in guiding the jaws to grow correctly is the Advance Lightwire Functional (ALF) Appliances. Rather than simply straightening teeth, the ALF appliances work with the body’s natural growth processes, encouraging proper development and position of the jaws and supporting normal oral function during the years when the child is growing. Nasal breathing and proper tongue posture during swallowing has a direct effect on the development of the jaws and the posture.
TMJ remains flat in its development, resulting in temporomandibular dysfunction (TMD). My examination revealed that this child was already exhibiting symptoms of TMD. To the left and below are diagrams showing the sequence of events that occurs when a child’s breathing is compromised. Anterior Tongue Thrust: tongue is thrusted forward during swallowing instead of resting on the roof of the mouth. This habit creates a narrow upper jaw.

If the child does not swallow properly, the tongue cannot assume its correct position preventing the upper jaw to develop properly leading to a very narrow palate (roof of the mouth) as the child grows. It also prevents the proper development of the temporomandibular joints (TMJ). If the swallowing pattern remains in infantile swallow (anterior tongue thrust), the 8 | Your HEALTH Magazine
The tip of the tongue is not able to touch the roof of the mouth due to a tongue-tie. This contributes to the improper swallowing preventing the upper jaw to grow horizontally as the child is growing.
Originally from the Philippines, Dr. Vann completed her undergraduate studies and earned her first dental degree at Centro Escolar University, College of Dentistry in the Philippines in 1978. She practiced general dentistry for six years in the Philippines. She attended Georgetown University School of Dentistry and graduated in 1988. She has been practicing in Northern Virginia ever since. What Happens During a Preventive Dental Visit? General dentists focus on the overall health of the teeth, gums, tongue, and mouth, while orthodontists specialize in diagnosing and treating misaligned teeth, bites, and jaws. Understanding the difference can help you determine which professional is right for your needs. How Do You Become a Dentist? Becoming a dentist requires extensive education and training. After completing a four-year undergraduate 12 | Your HEALTH Magazine
degree, students attend four years of dental school to earn a Doctor of Dental Surgery (DDS) or Doctor of Dental Medicine (DMD) degree. Dentists must then meet licensing requirements before practicing. General dentists diagnose and treat a wide variety of oral health conditions. Services may include examinations and X-rays, fillings, tooth extractions, repairing cracked teeth, treating gum disease, crowns, veneers, teeth whitening, and other restorative or cosmetic procedures. Dentists also emphasize preventive care, helping patients develop good oral hygiene habits and maintain healthy teeth and gums. How Is an Orthodontist Different? An orthodontist first completes the same education and training required of a general dentist. After dental school, however, an orthodontist completes an additional two to three years of specialized residency training focused specifically on tooth movement, facial development, bites, and jaw alignment. Some orthodontists take their
training a step further by pursuing board certification through the American Board of Orthodontics, which involves additional examination and evaluation. What Does an Orthodontist Do? Orthodontists specialize in correcting improperly positioned teeth and jaws. They diagnose conditions such as crowded or crooked teeth, gaps between teeth, and malocclusion— commonly known as an improper bite. Treatment may include traditional braces, clear aligners such as Invisalign, retainers, palatal expanders, and other specialized orthodontic appliances. Orthodontists also monitor the development of children’s teeth, jaws, and facial structures, allowing them to identify potential problems early and determine whether treatment may be beneficial. Orthodontic treatment isn’t only for children and teenagers. Adults who have crooked teeth, spacing, crowding, or bite problems can also benefit from an orthodontic evaluation and today’s discreet treatment options.
Who Should You See? For routine dental needs such as toothaches, cavities, cleanings, gum problems, crowns, or general oral health concerns, your dentist is usually the appropriate place to start. If your dentist discovers an alignment or bite problem, you may be referred to an orthodontist. Patients who already know they are concerned about crooked teeth, crowding, spacing, or jaw alignment can schedule directly with an orthodontist. The American Association of Orthodontists recommends that children receive their first orthodontic evaluation no later than age seven. While medications and other treatments may help slow hair loss or encourage regrowth in some individuals, hair transplantation offers a surgical option for restoring hair in areas where follicles are no longer producing healthy growth. A hair transplant works by relocating healthy hair follicles from an area of the scalp where hair continues to grow—commonly the back or sides of the head—to areas experiencing thinning or baldness. Because the transplanted follicles come from the patient’s own scalp, the new hair typically grows naturally and can often be cut, washed, and styled like surrounding hair. There are two primary techniques used in hair transplantation. Follicular Unit Extraction (FUE) involves removing individual hair follicles one at a time and transplanting them into the treatment area. This method generally leaves very small scars that are difficult to detect. Follicular Unit Transplantation (FUT), sometimes called the strip method, removes a narrow strip of scalp from the donor area before separating it into individual follicular units for transplantation. FUT may leave a linear scar but can be appropriate for some patients requiring a larger number of grafts. Candidates for hair transplantation typically have stable hair loss and sufficient donor hair available for transplantation. A thorough medical evaluation is important to determine whether hair loss is caused by hereditary pattern baldness or another condition that should be treated before surgery. It is important to understand that transplanted hair does not grow immediately. Newly transplanted hairs often shed within the first several weeks, which is a normal part of the process. What is Temporomandibular Disorder (TMD)?

So what does it mean when a person says “I have TMJ”? Well, technically, it means that you have a temporomandibular joint. And technically, that’s all it means- you have a jaw joint. In reality, most people are actu-
ally saying “I have TMD”- which more literally means they have TMJ disorder. TMD refers to the displacement or actual slipping of the articular disc off of the condyle. In English, this means that the little cushioning disc on top of your jaw bone has slipped off the bone and is now rubbing around in that little socket just in front of
your ear. That slipped disc can and will pinch nerves and blood vessels and this can wreak havoc with your entire body. The symptoms of TMD include head and neck pain, migraines, jaw clicking/popping/pain, facial tics or tremors, and various other body issues. There are many patients over

Jeffrey L. Brown, DDS the years who have presented with body tics ( and this is sometimes a Tourette’s situation) and the appliances used can help mitigate their symptoms. There was actually a study done in Osaka at the university that confirmed symptoms of Tourette’s were actually reduced greatly by wearing a TMD appliance! The understanding is that the nerve in the jaw joint- most likely the auriculotemporal nerve- is being pinched and this can cause the tics of Tourette’s. By decompressing the TMJ joint disc, this can alleviate those tics and Osaka university has confirmed the efficacy of the dental appliance used for Tourette’s. The bottom line here is that when the little disc is slipped off the condyle bone in the jaw socket, it can really hurt the body badly. Our office has become quite efficient at identifying the damaged disc through the use of a detailed MRI that looks directly at the discs and the radiologist we use is able to produce a very detailed report that can be used to diagnose the problem. From there, a treatment plan that includes wearing various devices can be developed. You do not need to wear the device(s) forever. The good news is that after a year of treating the damaged joints, usually the patient feels so much better and we can then use orthodontic techniques to raise the teeth up to the right height to support the TM joints. It is quite a process and took years to develop, but nowadays we can use clear aligners in almost all cases and this makes the work so much easier for everyone. It is rare, but sometimes we need to have surgery done for the discs when they are beyond repair. Years ago this was a difficult procedure, but today it has become much easier for both the patient and the doctor. We even have a technique called ‘arthrocentisis’ in which the joints are flushed out and cleaned to remove debris and bony pieces that can cause pain. This procedure has shown good promise. There are other techniques in treating TMD and those can be reviewed in the next article. 301-805-6805

Strengthen Your Child’s Teeth Through a Healthy Diet By April Toyer, DDS, FAAPD Lifetime Dental Care Children with healthy teeth are more likely to grow into adults with healthy smiles. Your child’s diet not only affects their overall health, but
their oral health as well. Foods and drinks that contain sugars of any type can contribute to tooth decay. Most items found in your local grocery store will contain nutrition facts and sugar contents on the label. Many of your child’s favorite items may have a low sugar alternative. Healthy Choices For Your Child
Introducing yogurt, cheese or other milk products with meals or as a snack can be a great alternative to carbohydrates. Food and drink items containing dairy can help increase pH levels in the oral cavity, which can help decrease the acids produced by cavity causing bacteria. The Calcium and vitamin D available in dairy products can also aid in enamel protection.
Fruits and vegetables, such as strawberries and apples, that contain vitamin C can strengthen gum tissue and can act as a natural plaque removing abrasive when extensive chewing is required. Tips To Prevent Decay If possible, regulate the amount of sticky sugary foods and candies your child consumes such as taffies, caramels, raisins and gummies. These items get stuck in the grooves of your teeth and may sit there until brushed out. Sugary food items are better served with meals than as a snack in between meals. This allows for the increased saliva needed for digestion to help rinse these sugars away. The action of chewing gum containing xylitol can also increase salivary flow and can be a natural abrasive to naturally rinse away food particles. Juices with high sugar contents should also be limited for four to six ounces per day. Try diluting your child’s juice to decrease the sugar content. Although sodas and juices that are high in sugar should be limited, introducing water in the oral cavity can help to neutralize the pH and decrease the harmful effects. Water is also the liquid of choice for babies at bedtime instead of milk, formula, juice, or soda. It is important for infants and children to start an oral care routine twice a day, as well as attend their regular six-month dental checkups. These habits, along with a healthy diet, can help lead your child to a lifetime of good oral health.

Sports Injuries: Prevention, Recovery, and a Safe Return To Activity

Sports injuries can affect anyone who is physically active, from competitive athletes to weekend runners, recreational golfers, and people beginning a new exercise program. While some injuries happen suddenly, many develop gradually as muscles, tendons, and joints are repeatedly stressed without enough time to recover. Understanding common causes of injury and responding appropriately can help protect long-term mobility and keep physical activity a healthy part of life. Why Sports Injuries Happen Acute injuries occur suddenly and may include sprains, strains, fractures, dislocations, or muscle tears. Overuse injuries develop over time from repetitive stress and can affect tendons, muscles, bones, and joints. Tendinopathy, shin pain, and certain types of knee, shoulder, or elbow pain are common examples. Risk can increase when activity levels rise too quickly, particularly when someone suddenly adds more mileage, weight, repetitions, or training days. Poor technique, inadequate conditioning, muscle weakness, limited mobility, fatigue, and insufficient recovery can also contribute. Prevention Starts Before an Injury A well-designed exercise routine should gradually prepare the body for increasing demands. Strength training can improve the ability of muscles and connective tissues to tolerate stress, while mobility and flexibility exercises can help maintain functional movement. Warm-ups should also reflect the activity ahead. Rather than relying solely on static stretching, a warm-up can include light aerobic activity and controlled movements that gradually increase in intensity. Rest is another important part of training. Persistent soreness, declining performance, unusual fatigue, or pain that worsens with activity may indicate that the body needs additional recovery. Recovering and Returning To Activity After an injury, the goal is not simply to eliminate pain. A safe return to activity also requires restoring appropriate strength, mobility, balance, coordination, and confidence. Returning too quickly can expose tissues to www.yourhealthmagazine.net
demands they are not yet prepared to handle. Recovery should generally progress from basic movement to increasingly challenging, activity-specific tasks. For example, an athlete recovering from a lower-body injury may progress from controlled strengthening
and balance exercises to running, jumping, changing direction, and eventually full participation. Pain that is severe, persistent, associated with significant swelling or instability, or prevents normal movement should be professionally evaluated. Recurrent injuries also deserve atten-
tion, as they may signal an underlying movement, strength, or training issue. Ultimately, staying active is important for health at every age. Thoughtful training, adequate recovery, and a gradual return after injury can help people continue participating in the sports and activities they enjoy.

Your gums protect the tissues and bone that support your teeth, and inflammation in the gums can affect much more than your smile. Gum health deserves the same attention as the rest of your oral health. Keeping your gums healthy can help protect your teeth while also helping
control inflammation. What Happens When Gums Become Unhealthy? Gum disease often starts quietly. Plaque and bacteria collect along the gumline, causing inflammation known as gingivitis. You may notice that your gums bleed when you brush or floss or that they look red or swollen. Without treatment, inflammation can move deeper beneath the gumline. This can lead to periodontitis, which can damage the bone and tissues that hold your teeth in place. The challenge is that gum disease does not always hurt. Some people have significant inflammation without realizing anything is wrong. Signs Your Gums Need Attention Changes in your gums are worth paying attention to. This may lead to cardiovascular diseases such as hypertension, stroke, aortic valve sclerosis and coronary problems. • There may be an increase in gastric and duodenal ulcers, pancreatic tumors, upper gastrointestinal tract tumors and kidney problems due to the lack of antioxidants that control the inflammatory cascade. • There is an increased risk of insulin resistance and type-2 diabetes, due to the increases fat and long chain carbohydrate consumption. • People may decrease daily physical and social activity. • People may develop sleep disorders, such as sleep apnea. Diet appears to play a decisive role in the relationship between edentulism and general health. There are studies that report a significant decrease in blood concentrations in vitamins A, C, folate and beta carotene in edentulous versus those with teeth. It is abundantly clear that a compromised or absent dentition has consequences, such as the restriction on the choice of foods, choking on food, poor digestion, alteration of taste, change in chewing efficiency, discomfort and premature facial aging. The selfimage issues associated with edentulism adversely impact on ones socialization. Studies show that 39% of the elderly edentulous patients cannot eat the food they want, 29% report a decrease in satisfaction from food and 14% avoid eating with other people. simplicity that distinguishes today’s orthodontics. Although many patients are referred by their family dentist, you can still make an appointment without a referral from your dentist. During this first consultation appointment the doctor will conduct a thorough examination of teeth, bite, TMJ and facial soft tissue. This information is used to outline the problems that are present. The doctor also presents you with a tentative treatment plan, an estimate of the length of treatment and any alternative treatment options such as Invisalign. If you have any further questions or concerns they may also be discussed at this appointment. When all your questions have been answered a treatment coordinator will cover payment plans, insurance matters, and appointment scheduling. Some patients (especially kids) are not ready for treatment when examined. In those cases, the patients are placed on a recall program (observation) and the family is sent a reminder in the future to come back for an orthodontic
re-evaluation. If treatment is recommended and you would like to get started, the next step is to schedule an appointment for full diagnostic records. Diagnostic records comprise photographs, radiographs, bite registration and models of your teeth. These records will give the doctor all the information needed to develop a detailed custom treatment plan that will give an optimal bite and smile for each patient. Diagnostic records are also helpful in tracking the progress of treatment as the teeth move into their corrected positions. Mostly, records may be taken during the initial consultation appointment, if not you may choose to schedule a second appointment. Records appointments take about 2030 minutes. After the final diagnosis and treatment plan are developed, you will be ready to begin your orthodontic treatment. How Shockwave Therapy Restores Range of Motion Submitted By BioMechanic Physical Therapy This article explores a new technologies in PT, Shockwave. In a previous article I mentioned it speeds healing; post injury, post-surgery, longstanding problem joints, scars, chronic pain, tendinitis and arthritis. What I didn’t mention is how powerful Shockwave is at restoring range of motion (ROM) in joints. After injuries, some people are never able to get enough motion back. Maybe it’s your shoulder, ankle or hip. Shockwave sends pulses into the joint and, believe it or not, immediately increases the ROM. Patients have gained 20° in shoulder ROM after a single treatment! or eliminate long-standing shoulder pain. The diagnoses I’m referencing are osteoarthritis, rotator cuff injuries, and shoulder replacements. For knees, results increase ROM 10° or 15° with the first treatment. As many of you may know, having your therapist haul on your knee is not fun. The diagnoses this has worked on include osteoarthritis, old knee replacements, and stubborn postsurgical knees. Many ankle injury patients who have been in a boot have difficulty regaining ankle ROM. Commonly in fractures and severe injuries. Prior to Shockwave, I would use heat, ultrasound, deep massage and aggressive manual mobilization, yielding slow results. CEREC combines a camera, computer, and milling instrument into one machine allowing tooth restorations to be created in the dentist office, all within a single visit. In 1985 Serona launched CEREC, which marked the beginning of digitalization in dentistry. CEREC crowns are more efficient and technologically advanced and have become the preferred option. While it can take weeks to receive a traditional dental crown, a CEREC crown uses computer-aided design (CAD) and computer-aided manufacturing (CAM) to create crowns in a single appointment. Not only that, but they allow for a better bite and more natural teeth for patients. CEREC crowns fit perfectly. They look and feel like a real tooth thanks to computer-aided technology.
