When the Emergency Room Isn’t Enough By Cristalle Madray, MPH, PA-C Virginia Family Medicine
I’ve learned that “urgent” isn’t always the same thing as “helpful.” Sometimes a patient walks out of the emergency room with pain under control – but without real answers, without next steps, and without a plan they can afford. That’s when I try to go the extra mile. A single mother of four came in with an issue that brought her to the ER for care. She was uninsured, and like many people in that position, she had a limit on what she could realistically handle financially. The emergency department did what it could in the moment: it gave her pain medicine. But medicine alone doesn’t address the fear of what might be going on underneath. I wasn’t the first stop in her story, but through a personal connection, I became involved when it mattered most – after the ER had run out of options. I called around, asking questions and pushing for something simple but crucial: a mammogram appointment scheduled quickly enough to keep her from waiting in uncertainty for weeks or months. Because she couldn’t pay for screening on her own, I also secured funding through a nonprofit organization that provides medical support for people who can’t afford care. Getting that support in place wasn’t just paperwork – it was the bridge between a scary symptom and the diagnosis she deserved. When the funding was approved, she was able to complete the mammogram. And that’s when we found a mass in her breast. I won’t pretend those moments aren’t heavy. Finding something new is frightening by definition. But the news that followed was a relief: the mass was benign. www.yourhealthmagazine.net
Still, I know that a benign result doesn’t mean a patient is finished with care. It means follow-up matters. I made sure she had a breast specialist she could contact for next steps, so she wouldn’t have to start from scratch or wonder where to go
when questions came up again.
I took this case personally because I’ve seen what happens when access is delayed – how uncertainty grows, how people fall through cracks that shouldn’t exist. The ER can calm symp-
toms, but follow-up is where the reassurance becomes real. When I’m able to connect patients to the right immediate and followup care – appointments, funding, specialists – that helps ensure they aren’t left carrying everything alone.

Your Mouth May Be Warning You Seven Signs It’s Time To See Your Dentist

A little sensitivity, occasional bleeding when brushing, or a small crack in a tooth may seem easy to ignore—especially when there is no severe pain. But your mouth often gives you warning signs before a dental problem becomes more complicated. Recognizing these signs and having them

evaluated early may help protect your teeth, gums, comfort, and overall oral health. Here are seven signs you shouldn’t ignore. 1. Bleeding Or Swollen Gums Healthy gums generally should not bleed regularly when you brush or floss. Persistent bleeding, redness, swelling, tenderness, or gum recession may indicate gingivitis or periodontal disease. Because gum disease can progress with relatively few symptoms, regular dental and periodontal evaluations are important. 2. Tooth Pain That Comes and Goes A toothache doesn’t have to be severe to deserve attention. Pain when biting, spontaneous aching, or discomfort that disappears and returns can be associated with decay, a cracked tooth, an inflamed dental pulp, infection, or other conditions.
Pain is your body’s way of telling you that something may need attention. Don’t wait for it to become an emergency. 3. Increasing Tooth Sensitivity A brief reaction to something cold isn’t always a serious problem. However, new, persistent, or worsening sensitivity to hot, cold, sweets, or pressure should be evaluated. Sensitivity may be related to gum recession, enamel wear, decay, a leaking restoration, a cracked tooth, or inflammation inside the tooth. 4. A Cracked, Chipped, Or Broken Tooth Even a small crack can sometimes become a larger problem. A damaged tooth may weaken over time and can potentially expose deeper tooth structure to bacteria. Depending on the extent of damage, treatment may range from a restoration or crown to root canal therapy or other treatment.
Early evaluation may provide more options for saving the natural tooth. 5. Persistent Bad Breath Occasional bad breath is common, but persistent bad breath despite regular brushing and flossing deserves attention. It can sometimes be associated with plaque accumulation, gum disease, cavities, dry mouth, infection, or other oral and medical conditions. Your dentist can help determine whether the source may be dental. 6. A Loose Tooth Or Changes in Your Bite Adult teeth should not normally become loose. A cataract is a clouding of the lens inside the eye, causing vision loss that cannot be corrected with glasses, contact lenses or corneal refractive surgery like LASIK.
• Creating peripheral corneal incisions to reduce astigmatism (when needed)
In cataract surgery, the lens inside your eye that has become cloudy is removed and replaced with an artificial lens (called an intraocular lens, or IOL) to restore clear vision. The procedure typically is performed on an outpatient basis and does not require an overnight stay in a hospital or other care facility. Most modern cataract procedures involve the use of a high-frequency ultrasound device that breaks up the cloudy lens into small pieces, which are then gently removed from the eye with suction.
A femtosecond laser can also be used to perform some of the steps of surgery to remove a clear lens or refractive lens exchange (RLE), and to make arcuate incisions in the cornea (AK) to reduce astigmatism. There are benefits and risks associated with the use of the laser, and there may be additional costs.

The possible benefits of the laser include the ability to make more precise and consistent incisions in the cornea, a more circular and centered capsulotomy, and to pre-soften the cataract so less ultrasound energy is necessary with the phacoemulsification device. How Is the Laser Used To Treat Astigmatism?
What Is the Femtosecond Laser? Recently, a number of femtosecond lasers – similar to the lasers used for LASIK – have been approved by the FDA for use in cataract surgery to do the following steps:

Restorative Dentistry and Cancer Rebuilding Oral Health During and After Treatment
By Ali Ghatri, DDS Let’s Smile Dental Cancer treatment can be lifesaving, but chemotherapy, radiation therapy, surgery, and certain medications may have significant effects on oral health. For some patients, restorative dentistry becomes an important part of recovering comfort, function, and confidence during survivorship. Restorative dentistry focuses on repairing or replacing damaged and missing teeth. Treatments may include fillings, crowns, bridges, dentures, and dental implants. For cancer patients and survivors, however, restorative treatment often requires additional planning and close coordination with the oncology team. How Cancer Treatment Can Affect the Mouth Chemotherapy may temporarily affect the rapidly dividing cells lining the mouth, contributing to mouth sores, inflammation, infection, and changes in taste. Some patients also experience dry mouth, which increases the risk of tooth decay because saliva normally helps protect teeth and neutralize acids. Radiation involving the head and neck can have longer-lasting effects. Depending on the treatment area and dose, patients may experience reduced saliva production, increased cavity risk, changes in oral tissues, jaw stiffness, or effects on the jawbone. Cancer surgery can also alter the teeth, gums, jaw, or facial structures, potentially affecting a person’s ability to chew, speak, or smile comfortably. Restoring Function and Appearance Once a patient’s medical condition allows, a dentist can develop an individualized restorative plan. Damaged teeth may be repaired with fillings or crowns, while missing teeth may sometimes be replaced with bridges, dentures, or dental implants. The appropriate treatment depends on many factors, including the patient’s cancer history, medications, radiation exposure, bone health, saliva production, immune status, and overall health. www.yourhealthmagazine.net
Dental implants require particular consideration for patients who have received radiation to the jaws or certain medications that affect bone metabolism. These patients may have an increased risk of complications, so consultation between the dentist, oral surgeon, and oncology team may be necessary before invasive procedures. Prevention Is Part Of Restoration Restorative dentistry isn’t only about repairing existing damage. Protecting remaining teeth is equally
important. Regular dental examinations, professional cleanings, fluoride treatments when appropriate, meticulous brushing and flossing, and strategies for managing dry mouth can help reduce future problems. Ideally, patients beginning chemotherapy or head-and-neck radiation should receive a dental evaluation before cancer treatment starts. Addressing infections and unstable teeth beforehand may help prevent complications later.
For cancer survivors, restoring oral health can mean much more than improving a smile. Being able to eat comfortably, speak clearly, and smile confidently can contribute to nutrition, social interaction, and overall quality of life. With thoughtful planning and communication between dental and oncology professionals, restorative dentistry can play an important role in helping patients move forward after cancer treatment.

TMJ Disorder and Bruxism

Figure 1 is the Panorex of a 23-year-old female. She presented with numerous complaints of TMJ symptoms but also an extensive list of postural problems. Her difficulties started around the age of 12 years and had worsened steadily since then despite frequent attempts by various dentists and other professionals to assist her. Over a span of 11 years, these included equilibration of the dentition, repeated efforts with both maxillary or mandibular orthotics, various forms of therapy including physical therapy, chiropractic treatment and a variety of medications both over-the-counter and prescription. As can be seen, there has been extensive loss of tooth material, enough to require root canal treatment on three

molars and extensive restorations on most of the other posterior teeth. In her teens, veneers were placed on the six maxillary anterior teeth to compensate for excessive incisal wear, but she then developed an open bite from second molar to second molar. The usual suspects of severe caries or erosion from reflux acid were not the cause. The evidence suggested some powerful force was involved. The problem was to identify why such severe bruxism was being triggered. Figure 2A gives the key to the puzzle. In osteopathic language, there is a right torsion. In a torsion, both the ocular plane and the lateral occlusal plane run up to one side, roughly parallel to each other. In this case, the planes were tipped up to the right. Many individuals seem to cope with this anomaly all their lives without serious harm. For many others, there can be a host of unfavourable reactions including balance difficulties, possible strabismus and a wide variety of head, neck and spinal difficulties. The head of an adult female weighs about 6-7 kilos. If the head is not balanced, the rest of the body adapts to achieve as efficient a distribution of weight as it can, sometimes at the expense of tissues a long way from the origin of the problem such as the pelvis.
Treatment for a torsion has been described previously. As the maxillary dentition and the ocular plane became more level the postural problems diminished steadily as did the bruxing. It was decided to restore the posterior dentition on the right quadrants as there had been so much loss of tooth material and the treatment had created

spacing on that side. Gradual widening of the palate was also done to help provide adequate space for the tongue to be contained within the closed dentition. The anterior open bite closed spontaneously partly because of extraction of the mandibular third molars. A photo supplied by the patient of her face at age six years showed that the condition had been present at least since then and was most probably as a result of birth trauma.

The Common Cold and Your Oral Health

cold and flu season. Children in the United States miss about 22 million days of school every year due to the common cold. The average child alone gets between six to twelve colds every year and is two to three times more likely to come down with the flu. Although children are most susceptible to the cold and flu they easily spread their aliments to their caregivers and schoolmates. When you are
feeling down and out the last thing many people want to do is worry about their teeth. However, a few key tips during those down and sick days can help lead you and your family to a speedy recovery without devastation to your oral cavity. • Stay Hydrated – Being made mostly of water, it is nature’s cure for a variety of conditions. A stuffy and congested nose may cause
you to breath through your mouth. Excessive mouth breathing can slow saliva production and make our mouths susceptible to harmful bacteria. Drinking water during a cold also thins mucus and prevents dehydration. If your child refuses to drink try soups, sugar-free popcicles or Jello. • Use Sugar-free Cough Drops – Many cough syrups and cough drops contain excessive amounts of sugar to make them taste great. If you are using sugary cough medicines make sure you rinse the residue off your teeth with water and brush before going to bed. There are xylitol candies and nasal sprays that help fight cavities as well as alleviate sore throat and post nasal drip. • Minimize Sugar Intake – Refined sugars such as fruit juices and white flour can depress your immune system, which weakens your body’s ability to fight infection on its own. • Don’t Forget to Brush – When you are sick you tend to take several naps throughout the day. Although you are often more tired do not forget to brush your teeth before lying down. Saliva production decreases when you are sleeping, decreasing the bodies ability to cleanse itself. • Replace your Toothbrush – Bacteria and viruses can live on your toothbrush for hours or even days on hard surfaces. The last thing you want to do is re-infect yourself after just getting over an ailment. Make sure you do not store your toothbrush next to your family members. This could lead to cross-contamination of the new toothbrush and further spread the illness.

Thermography, also known as digital infrared thermal imaging (DITI), is a totally non-invasive clinical imaging procedure that detects thermal differences within the body, which may indicate the presence of disease or physical injury. It can be used in conjunction with other diagnostic tools, for diagnosis and prognosis, as well as monitoring therapy progress. DITI is used for conditions and injuries, including back injuries, arthritis, headache, nerve damage, unexplained pain, fibromyalgia, RSD (CRPS), dental problems and TMJ, artery inflammation, heart disease, vascular disease, breast

Teeth begin developing before birth. The foundations of both primary and permanent teeth form beneath the gums during pregnancy. Baby teeth, also called primary or deciduous teeth, help children chew, develop speech, maintain space for permanent teeth, and support healthy jaw development. Birth To Age 3: Baby Teeth Arrive Most babies get their first tooth around six months, although timing varies. Lower central incisors
typically appear around 6–10 months, followed by upper central incisors around 8–12 months. Lateral incisors usually emerge between 9–16 months, first molars around 13– 19 months, canines around 16–23 months, and second molars around 23–33 months. By age three, most children have all 20 primary teeth. Although these teeth eventually fall out, keeping them healthy is important for comfort, nutrition, and developing permanent teeth.

Ages 4 To 6: Building Healthy Habits During the preschool years, parents should reinforce good oral hygiene. Children should brush twice daily with fluoride toothpaste, floss when teeth begin touching, limit sugary foods and drinks, visit the dentist regularly, and wear mouthguards during appropriate sports. Ages 6 To 12: Permanent Teeth Emerge Around age six, baby teeth begin falling out and permanent teeth start emerging. During this “mixed dentition” stage, children have both primary and permanent teeth. First permanent molars and lower central incisors generally erupt around ages 6–7. Upper central incisors and lower lateral incisors follow around ages 7–8, with upper lateral incisors around ages 8–9. Canines and premolars typically appear between ages 9–12, with additional permanent teeth continuing to erupt through approximately age 13. Loose teeth, temporary gaps, and changes in alignment are common during this transition. Regular dental visits help monitor development and identify potential concerns. Ages 13 To 17: The Teenage Years By the early teenage years, most adolescents have 28 permanent teeth. Consistent brushing, flossing, healthy food choices, and routine dental care remain essential because these teeth are intended to last a lifetime. Wisdom teeth typically develop during the teenage years and may erupt during the late teens or early twenties. Dental examinations and X-rays can monitor their position and development. By Michael Rogers, DDS Fairlington Dental Two Paths To a Straighter Smile If you are considering orthodontic treatment, one of the first decisions may be whether to choose clear aligners or traditional braces. Both can move teeth into healthier positions, improve alignment, and correct certain bite concerns, but they accomplish these goals in different ways. Traditional braces use brackets and
wires that remain attached to the teeth throughout treatment. Clear aligner systems use a series of custom-made, removable trays that gradually shift the teeth. Understanding the differences can help patients discuss which approach best fits their orthodontic needs, lifestyle, and preferences. How Do Clear Aligners Work? Clear aligner treatment uses transparent trays designed specifically for the patient’s teeth. Treatment typically begins with a digital scan or impression that allows the orthodontic provider to develop a customized plan for gradually moving the teeth. Patients progress through a series of aligners, often changing to a new set every one to two weeks. Meet Rami Makhoul
Meet Matthew Skancke
After completing his fellowship in Colon and Rectal Surgery at Robert Wood Johnson University Hospital-Rutgers University, Rami Makhoul, MD, joined Holy Cross Health Hospital in 2014 to serve patients of the DMV area.
Dr. Matthew Skancke is a colon and rectal surgeon serving Maryland, Virginia and Washington, DC. Dr. Skancke grew up in Great Falls, VA earning his undergraduate degree in electrical and biomedical engineering at the University of Virginia’s engineering school. After engineering school he spent three years working for a Swiss based GPS company U-blox America while going to night school to finish his premedical degree. He then attended medical school at the University of North Carolina at Chapel Hill where he did basic science research of mesenchymal stem cells to progress the field of regenerative medicine. Dr. Skancke continued his General Surgery training at the George Washington University Hospital and subsequently did his Colon and Rectal Surgery Fellowship at the Cleveland Clinic.
Upon graduating from medical school, Dr. Makhoul first completed his internship in General Surgery at the University of Massachusetts Medical School. He, then, completed his residency training in General Surgery at the George Washington University Hospital in Washington, DC. During his residency training at GW, he spent a year in clinical research where he published some of his work on colon and rectal cancer, and the use of Robotic Surgery in various abdominal procedures performed in the Colon and Rectal Surgery field. He continued with his passion for research and published peer-reviewed articles during his fellowship training. Several of his work, was presented at national conferences including the American Society of Colon and Rectal Surgery, and the Society of American and Gastrointestinal and Endoscopic Surgeons annual conferences.
Dr. Makhoul diagnoses and treats various diseases of the colon, rectum, anus, and other parts of the GI tract using national standards and guidelines. He has a special interest in treating colon and rectal cancer using advanced surgical minimally invasive techniques including robotic, laparoscopic, and transanal minimally invasive surgery. Dr. Makhoul is Board Certified in both General Surgery and Colon and Rectal Surgery and is an active member of the American Society of Colon and Rectal Surgeons education committee. As part of his commitment to surgical training and education, Dr. Makhoul serves as a Clinical Instructor of Surgery at the George Washington University. Dr. Makhoul is currently the chairman of the Department of Surgery at Holy Cross Hospital in Silver Spring. He also holds the position of Cancer Liaison Physician for Holy Cross Health, where he manages and overlooks clinically related cancer activities in collaboration with the Commission on Cancer and American Cancer Society.
During his training, Dr. Skancke has published over 24 peer reviewed publications, authored three book chapters, and presented over 45 times at national conferences in medicine and surgery. He has participated in clinical trials of bench top and mechanical sciences and is a member of the American College of Surgeons and the American Society of Colon and Rectal Surgeons. Dr. Skancke is Board Certified in General Surgery and Colon and Rectal Surgery and is an active member of the American Society of Colon and Rectal Surgeons Quality and Safety National committee.
When he is away from the office, Dr. Makhoul may be found spending time with his family. Dr. Makhoul enjoys skiing, cycling, scuba diving, tennis, and traveling.
Outside of work, Dr. Skancke has found a new love for French cooking and North Carolina barbeque. To offset these vices, Dr. Skancke also enjoys running and powerlifting in his free time.

When Should My Child See an Orthodontist?

As orthodontic treatment not only involves the teeth but also the surrounding boney structures, it is highly recommended to start treatment before the face and jaws have finished growing. By age seven, the six-year molars (adult first molars) and incisors have usually erupted. After the first molars erupt the space available for the front teeth does not increase as a child grows. To the contrary, in most people after the permanent molars erupt, the space available for the front teeth decreases with age. Early treatment can also regulate the width of the dental arches, gain space for permanent teeth, avoid the need for permanent tooth extractions, reduce the likelihood of impacted permanent teeth or the need for surgical procedures to realign the jaw.
A majority of children will not require orthodontic procedures until their teenage years, however an early exam allows the orthodontist to monitor the child’s growth and development and if indicated begin treatment at the most appropriate time. If your child is nearing his or her seventh birthday, schedule an orthodontic consultation appointment. You don’t need to have a referral to see an orthodontist. Many pro-actively schedule a consultation appointment either as a new patient or to seek a second opinion. However, you should see your family dentist for professional cleanings and check-ups every six months, or more often if recommended. Early treatment may prevent more serious problems from developing and may make treatment at a later age less complicated.

Shockwave Technology Submitted By BioMechanic Physical Therapy This article explores a new technology in Physical Therapy, Shockwave. There are two new Regenerative treatments in physical therapy; MLS Laser and Shockwave. Regenerative Therapies actually heal. They speed healing and reduce pain for arthritis, tendonitis, bursitis, back pain, acute injuries, post surgery, scars, nerve damage and more. When I first purchased the shockwave several years ago I didn’t realize the power it has in restoring motion (ROM) because Shockwave is known for speeding healing and decreasing pain. Some patients don’t just have pain, they also struggle with getting their motion back. In only a few visits, not only does that joint finally move, it moves pain free! Although the results are immediate, medical research says 6 treatments are needed for permanent change. How powerful is it? Patients have gained 20° in shoulder ROM after a single treatment! Subsequent
treatments further increase ROM and reduce pain. The diagnoses I’m referencing are osteoarthritis, rotator cuff injuries, and shoulder replacements. For knees, results increase ROM 10° or 15° with the first treatment. As many of you may know, having your therapist haul on your knee is not fun. The diagnoses this has worked on include osteoarthritis, old knee replacements, and stubborn postsurgical knees. Many ankle injury patients who have been in a boot have difficulty regaining ankle ROM. Prior to Shockwave, I would use heat, ultrasound, deep massage and aggressive manual mobilization, all yielding slow results. With Shockwave, I get results in a single visit that previously would have taken a month. Both the hip and sacroiliac joint (where your hip bone meets your tailbone) can suffer arthritis and cause pain, limping and limit one’s willingness to walk. Used alongside conventional oncology care, they may help ease symptoms associated with cancer and treatments such as chemotherapy, radiation, and hormonal therapy. through the meridians (jing-luo), balance between yin and yang, and harmony among the zang-fu organs. Cancer treatment may consume qi and yin, create internal heat, or contribute to blood stasis (yu xue). Acupuncture uses fine needles at selected points to regulate qi, nourish yin, and calm the shen (spirit). Cupping (baguanfa) is used to move qi and blood, loosen the sinews, and address stagnation. Pain TCM holds that “where there is obstruction, there is pain.” Cancer and treatment-related pain may be viewed as patterns of qi stagnation and blood stasis. Maintaining relationships, favorite routines, hobbies, independence, and personal goals can provide a sense of normalcy and purpose. Every older adult’s cancer journey is different. Open communication among the patient, family, oncologist, primary-care physician, and other specialists can help create a plan that considers not only the cancer, but the whole person.

Breathlessness Shortness of breath may involve lung qi deficiency or constrained lung qi. Treatment may focus on spreading and descending lung qi while calming the shen. Responses vary among individuals. Acupuncture and cupping should complement—not replace—conventional cancer treatment. There is no single orthodontic treatment that is best for everyone. Clear aligners may be an excellent option for patients who value discretion, convenience, and the ability to remove their orthodontic appliance. However, they require commitment. If aligners are not worn consistently for the recommended amount of time, treatment may not progress as expected. Traditional braces may be preferable for certain complex tooth movements or for patients who may have difficulty consistently wearing removable aligners. Choosing the Right Treatment For Your Smile Both clear aligners and traditional braces can help patients achieve healthier, straighter smiles. The best choice depends on factors such as the position of the teeth, bite alignment, complexity of treatment, age, lifestyle, and ability to follow treatment recommendations.
