Cataract ImplantsSeeing In Three Dimensions

Approximately 20.5 million Americans age 40 and older have cataracts. The opportunity for freedom from reading glasses and bifocals was not available in intraocular lens implants. There are currently two of these advanced lenses that have become popular. Clareon PanOptix & Vivity by Alcon. They are known as “multifocal lenses” meaning they allow quality vision at near distance and intermediate zones through a series of focusing rings.
Our lives have become more complex and our vision is oftentimes tested throughout an average day of work. We have to read materials at close range, look at a computer in the intermediate range and perform tasks such as driving, requiring good distance vision. During the course of a day we have to constantly refocus our eyes in order to see multiple distances. Those of us who are over 40 have noticed that our reading vision has changed for the worse. Many of us require reading glasses because we no longer have the functional ability to focus on near objects like we did when we were younger.
The ability to change focus rapidly and focus on near objects is called accommodation and this process declines with age. In addition to this, the natural lens within our eyes starts to become cloudy with age.
What Is a Cataract?
Cataracts are, in fact, the clouding of the eye’s clear lens, like a window that is “fogged” with steam. When the lens becomes cloudy, light rays can not pass through it easily and vision becomes blurry. Cataracts are not a growth or a film over the eye.
Cataracts start out small and have little effect on vision at first, but they continue to grow slowly. As the cataract grows, so does the impact on vision. The most common signs of cataract development are:
• Painless blurring of vision
• Sensitivity to light and glare
• Double vision in one eye
• Poor night vision
• Fading or yellowing of colors
• Frequent changes in glasses or contact lens prescriptions
Although cataracts usually develop as part of the aging process (more than half of all Americans develop cataracts by age 80), they can also result from:
• Eye injuries
• Certain diseases, such as diabetes
• Genetic inheritance
• Certain medications
• Frequent, unprotected exposure to UV-A and UV-B rays
What Is the Lens?
The lens is the part of the eye that helps focus light on the retina. The retina is the eye’s light-sensitive layer that sends visual signals to the brain. In a normal eye, light passes through the lens and gets focused on the retina. To help produce a sharp image, the lens must remain clear.
How Is a Cataract Treated?
For an early cataract, vision may improve by using different eyeglasses, magnifying lenses, or stronger lighting. If these measures don’t help, surgery is the only effective treatment. This treatment involves removing the cloudy lens and replacing it with a substitute lens, also known as an implant.
A cataract needs to be removed only when vision loss interferes with your everyday activities, such as driving, reading, or watching TV. You and your eye surgeons can make that decision together.
How Is a Cataract Removed?
Cataracts are removed with surgery, performed in an outpatient surgical center. In most cataract surgeries your native lens is replaced by an intraocular lens (IOL). The traditional method of replacing your native lens is with an IOL, aimed for good distance vision only. These are known as “monofocal lenses”. You would oftentimes still need reading glasses with monofocal lenses. One way to decrease your need for reading glasses would be to have one eye corrected for distance and one for near. This is called monovision. Depth perception can sometimes be affected by this option.
That is why the new IOL technology is so exciting. Each of the approved IOLs has its advantages and disadvantages. Your eye surgeon will review the risks, benefits and alternatives of multifocal implants with you in order to choose the best lens for you. Cataract surgery is covered by most insurance plans, however, the newer IOL implants are generally not covered and require additional outof-pocket expenses besides the usual co-pay, co-insurance, or deductibles you would normally be responsible for. While this is unfortunate financially, the IOLs themselves are certainly worth the extra expenditures in appropriate patients. Your surgeon will determine if you are a good candidate for these new implants.

Hormones and Oral Health
Fluctuations in estrogen and progesterone influence blood flow to the gums and can make gum tissue more sensitive to plaque and bacteria. During puberty, menstruation, pregnancy, and menopause, women may notice increased gum inflammation, tenderness, or bleeding.
Because of these hormonal changes, women are at a greater risk for gingivitis and periodontal disease. Gum disease has also been linked to systemic conditions such as heart disease and diabetes, making oral health an important part of overall wellness.

Unlocking the Jaw How MRI Helps Diagnose TMJ Disorders

Jaw pain, clicking, or difficulty opening and closing the mouth can be signs of a temporomandibular joint (TMJ) disorder. These issues affect the hinge that connects your jawbone to your skull and can lead to chronic discomfort if left untreated. Because the joint involves both bone and soft tissue, getting an accurate diagnosis is essential. This is where MRI technology plays an important role.
Understanding TMJ Disorders
TMJ disorders can have many underlying causes. Sometimes it stems from injury or arthritis. Other times it may be related to teeth grinding, muscle tension, or a misaligned bite. Common symptoms include jaw pain, headaches, earaches, and clicking or popping sounds when the jaw moves. In more advanced cases, it can become difficult to eat or speak comfortably.
Pinpointing the source of the problem requires more than a visual exam or basic imaging. That is because many TMJ conditions involve soft tissues such as the disc that cushions the joint and the surrounding muscles and ligaments. These cannot be clearly seen with standard x-rays.
How MRI Provides a Clearer View
MRI, or magnetic resonance imaging, offers one of the most detailed ways to evaluate soft tissues.
While x-rays and CT scans focus on the bones, MRI uses magnetic fields to create high-resolution images of muscles, discs, and connective tissue. This makes it possible to assess whether the disc inside the TMJ is in the correct position, whether there is inflammation, or if there are signs of damage or deterioration.
MRI is especially valuable when diagnosing internal derangements, which occur when the disc shifts out of place. This is a common cause of jaw locking, clicking, or chronic discomfort during everyday jaw movement.

Shouldn’t Be an Either-Or

As the U.S. population ages, the national conversation about senior care has grown louder but not clearer. Too often, cognitive aging is framed as a binary choice: older adults either live independently or need full memory care. What Is Temporomandibular Disorder (TMD)?

So, what does it mean when a person says “I have TMJ”? Well, technically, it means that you have a temporomandibular joint. And technically, that’s all it means – you have a jaw joint. In reality, most people are actually saying “I have TMD”– which
more literally means they have TMJ disorder.
TMD refers to the displacement or actual slipping of the articular disc off of the condyle. In English, this means that the little cushioning disc on top of your jawbone has slipped off the bone and is now rubbing around in that little socket just in front of your
ear. That slipped disc can and will pinch nerves and blood vessels and this can wreak havoc with your entire body.
We Are NOT Your Typical Dental Office
The symptoms of TMD include head and neck pain, migraines, jaw clicking/popping/pain, facial tics or tremors, and various other body issues. There are many patients over the years who have presented with body tics (and this is sometimes a Tourette’s situation) and the appliances used can help mitigate their symptoms. There was actually a study done in Osaka at the university that confirmed symptoms of Tourette’s were actually reduced greatly by wearing a TMD appliance! The understanding is that the nerve in the jaw joint- most likely the auriculotemporal nerve- is being pinched and this can cause the tics of Tourette’s. By decompressing the TMJ joint disc, this can alleviate those tics and Osaka University has confirmed the efficacy of the dental appliance used for Tourette’s.

Our office has become quite efficient at identifying the damaged disc through the use of a detailed MRI that looks directly at the discs and the radiologist we use is able to produce a very detailed report that can be used to diagnose the problem. From there, a treatment plan that includes wearing various devices can be developed.
You do not need to wear the device(s) forever. The good news is that after a year of treating the damaged joints, usually the patient feels so much better and we can then use orthodontic techniques to raise the teeth up to the right height to support the TM joints. It is quite a process and took years to develop, but nowadays we can use clear aligners in almost all cases and this makes the work so much easier for everyone.
It is rare, but sometimes we need to have surgery done for the discs when they are beyond repair. Years ago, this was a difficult procedure, but today it has become much easier for both the patient and the doctor. We even have a technique called ‘arthrocentesis’ in which the joints are flushed out and cleaned to remove debris and bony pieces that can cause pain. This procedure has shown good promise.

Does My Child Really Need Dental X-rays?

It might be shocking to realize that children often need dental X-rays more often than adults. Your child is constantly growing and changing, and they are often more susceptible to tooth decay than adults. The enamel on childrens’ teeth is generally thinner allowing decay to spread more rapidly than on adult teeth. The American Academy of Pediatric Dentistry recommends that X-rays are preformed every six months to a year or more depending on their risk for tooth decay.
What Is the Purpose Of X-rays On Children?
Many parents are frustrated that their dentist reports that their child now has cavities, but at their previous check up they were cavity free. Cavities can form rapidly on children, however the absence of X-rays at a previous visit due to age or behavior may be the cause for the initial under diagnosis. Most parents are shocked to find that their child has cavities because they regularly brush twice a day and limit high consumption of sugars in their child’s diet.
Many parents admit that they have not yet begun flossing due to either lack of child cooperation or failure to realize the importance of flossing at such a young age. X-rays can detect cavities that form in the areas between your child’s teeth that may not be seen on visible inspection. Once cavities are visually seen on areas between the teeth, these areas may be quite large, and actually approach the nerve of the tooth requiring more extensive treatment.
Eruption patterns of teeth can also be diagnosed with X-rays. Many children have delayed or advanced eruption patterns, or may even have permanent teeth that are missing or developing in an incorrect location. Early detection of these occurrences may help determine if early intervention or orthodontic treatment may be necessary. Many of these problems are more difficult to treat after the growth process is complete.
Children are known to being highly active and unfortunately prone to injury. Most commonly the upper front teeth take the brunt of the abuse and can suffer from root fracture, tooth displacement, color change, and mobility. Your child’s dentist
may elect to take regular X-rays of this area to determine if any damage has occurred to the baby teeth or the permanent teeth underneath. Are X-rays Safe?
Radiation is all around us, from microwaves, to airplanes, and even from the sun. With the amount of radiation we are exposed to with daily activities, the radiation from dental films is very negligible. Different
dental offices have different forms of X-ray technology and therefore different levels of radiation from their equipment.
Digital X-rays offer the lowest form of radiation to patients, in some cases up to 80% lower than the traditional films most adults had as children. All dental offices are required to have routine inspections of their X-rays equipment by their state.
It is also required that a lead apron be placed on your child for additional protection.

From a diagnostic aspect, an evaluation of the relationships and functioning of the joints and associated structures, including the maxilla (upper jaw), temporal bones, and occiput (back of the head), becomes more important in understanding what is happening. From a treatment aspect, the focus shifts from the details of temporomandibular joint (TMJ) malfunction to creating an environment within which the mandible (lower jaw) is in balance with the maxilla and surrounding structures.
That environment consists of postural, cranial, facial, and dental structures that comprise the stomatognathic system. The stomatognathic system is a highly coordinated neuromuscular system in which the teeth, jaws, temporomandibular
joints, muscles of the head and neck, tongue, lips, and associated nerves and tissues work together to enable chewing, swallowing, speaking, breathing, and maintaining oral and facial function.
Because these structures are interdependent, dysfunction in one component can influence the health and function of the entire system. A problem in one area can affect other areas. Disorientation of the temporal bones affects the angulation of the glenoid fossae (socket-like depressions in the temporal bones of the skull that receive the heads of the condyles to form the TMJs) and, in due course, results in TMJ dysfunction.
If the temporal bones are not level relative to each other, there may be a corresponding imbalance of the pelvis. This re-
lationship between the temporal bones and the pelvis should be recognized.
Below is an example of a postural assessment of a TMD patient who came to me because of his locked jaw. The orientation of the temporal bones affects the alignment of the TMJ, pelvis, and spine. As you can see, his head is off-center to the left, the right shoulder and shoulder blade are much lower than the left, there is scoliosis of the spine, and the right pelvis is higher.

A Broader View of the Jaw
The MRI image below reveals craniofacial asymmetry, which impacts the three-dimensional position of the TMJs, potentially affecting jaw function and the overall balance of the stomatognathic system.
To the right are the ALF appliance designs for this patient. I design these appliances according to the patient’s jaw misalignment. They are not one-design-fits-all.
TMJ disorder is a symptom and not the main problem. Success in treatment is finding the root cause of why the patient is experiencing TMJ issues.
In my years of experience treating temporomandibular joint disorders, it is common to observe spontaneous improvement or resolution of symptoms in many non-dental areas, such as a reduction in neck and shoulder discomfort, improvement in hearing, resolution of a sense of nausea, etc.
When a broader view of the patient is taken, which includes postural and cranial factors, the origins of many apparently unrelated symptoms can be understood. Even more importantly,
with a well-planned, comprehensive treatment approach, it is often possible to resolve these symptoms and improve overall function instead of just focusing on the TMJ symptoms.

Anatomical differences related to sex can also contribute to lower back pain. For instance, the pelvis of a person assigned female at birth is generally wider than that of a person assigned male at birth, altering the alignment and stress distribution on the lumbar spine.

Prior to entering private practice, he served a general practice residency at Sepulveda Veteran’s Hospital. Family dentistry has always been a part of my life, inspired by my mother, Dr. Lynda Dean-Duru, a pediatric dentist. I began my journey working as a pediatric dental assistant at Ashburn Children’s Dentistry, where I spent summers learning from her. Watching her compassionate care and understanding the importance of preventive dental health for children led me to specialize in pediatric dentistry.
Today, I’m honored to join my mother at Ashburn Children’s Dentistry, where I help children and their families experience dentistry in a positive and transformative way. As an integrative laser pediatric dental surgeon, I stay committed to ongoing learning—drawing from the latest research to improve outcomes and enhance children’s quality of life. While eating, trays can be removed and you need not worry about food being stuck between the front teeth. These trays can also be removed for brushing, flossing and special occasions.
We live in a society that places great emphasis on first impressions. A smile filled with beautifully aligned teeth will make a better impression than one that has crooked teeth. Recently, a lot of people have used Invisalign as a means to achieve this goal. Invisalign is manufactured and marketed by Align Technology, Inc, which was founded in 1997 and re-
The treatment consists of a series of clear, removable aligners or trays. These aligners need to be worn 20-22 hours per day. The success of Invisalign treatment is largely dependent on patient commitment. Like braces, these aligners put gentle pressure on the teeth to move them into a new position.
Each set is worn for 2-4 weeks before proceeding to the next set. Besides the obvious cosmetic advantage, they are more comfortable than traditional braces and there are no wires or metal to rub against your cheeks.
Can Your Dentist Spot the First Signs of Osteoporosis?

A woman in her late fifties comes in for a routine cleaning. No pain, no complaints. During the visit, her dentist notices the bone supporting her lower molars looks thinner than it did on previous X-rays, and one tooth has started to feel slightly loose. She suggests the patient ask her physician about a bone density scan. Six weeks later, that patient has an osteoporosis diagnosis she had no idea was coming.
Dentists can’t diagnose osteoporosis from an X-ray. But changes in the jaw can be an early prompt to look harder, and for some women that prompt arrives in a dental chair before it arrives anywhere else.
The reason is straightforward: your jaw is bone. The mandible and maxilla go through the same remodeling process as your hip and spine — a
balance between osteoclasts breaking old bone down and osteoblasts building new bone back up. When estrogen falls at menopause, that balance can tip. The alveolar bone that holds your teeth in their sockets is especially responsive, because it is thin, metabolically active, and under load every time you chew. Observational studies consistently link lower jawbone density to tooth loss and receding gum support — though these are associations, not proof of cause, and smoking and gum disease muddy the picture considerably.
There is also a two-way street here, and it is the part most women haven’t heard. Some osteoporosis medications — bisphosphonates and denosumab — carry a small risk of medication-related osteonecrosis of the jaw, especially after extractions or implant surgery. At the doses used for osteoporosis, the risk sits well under one percent; large cohort data put it in the range of 0.17 percent, and lower still for oral bisphosphonates. Small, but it’s entirely manageable if the right people know in advance.
Which points to three things worth doing. Tell your dentist if you’ve been diagnosed with osteopenia or osteoporosis, and hand over the full medication list — it changes how surgical work
