Kidney stones are a common and painful condition that affect millions of people worldwide. They occur when certain substances in urine— such as calcium, oxalate, and uric acid—crystallize and stick together, forming hard deposits. These stones can range in size from a grain of sand to a golf ball, and depending on their size, they may pass on their own or require medical or surgical treatment.
What Are Kidney Stones?
Kidney stones are solid pieces of material that form in the kidneys when the concentration of certain minerals in your urine becomes too high. The stones may stay in the kidney or travel through the urinary tract, which includes the ureters (the tubes that carry urine from the kidneys to the bladder), the bladder, and the urethra (the tube that carries urine out of the body).
Types Of Kidney Stones
There are several types of kidney stones, classified based on their
composition:
1. Calcium Stones: The most common type, typically made of calcium oxalate or calcium phosphate. Oxalate is a naturally occurring substance found in some foods.
2. Uric Acid Stones: More common in people who don’t drink enough fluids, have a high-protein diet, or have gout.
3. Struvite Stones : These can form after a urinary tract infection and grow quickly.
4. Cystine Stones: Rare, occurring in

Causes Of Kidney Stones
Kidney stones form when your urine contains more crystal-forming substances than the fluid in your urine can dilute. Risk factors for developing kidney stones include:

• Diet: A diet high in salt, protein, and sugar can increase your risk. Foods high in oxalates, such as spinach, nuts, and chocolate, can also contribute.
• Family Or Personal History : If someone in your family has had kidney stones, you’re more likely to develop them.
• Medical Conditions : Conditions like gout, hyperparathyroidism, and urinary tract infections can increase your risk.
• Medications: Certain medications, such as diuretics, antacids, or medications for seizures, may increase the likelihood of stone formation.
Symptoms Of Kidney Stones
Symptoms of kidney stones may not be noticeable until the stone begins to move within the kidney or into the ureter. When this happens, symptoms can include:
• Severe Pain: Often in the back or side, below the ribs. The pain may spread to the lower abdomen and groin.
• Painful Urination: You may feel discomfort or a burning sensation when urinating.
• Blood In the Urine: Your urine may appear pink, red, or brown.
• Cloudy or Foul-Smelling Urine: This could indicate an infection.
• Nausea and Vomiting : These symptoms often accompany the pain.
• Frequent Urination: You may feel an urgent need to urinate more often than usual.
• Fevers & Infection : In some cases, stones can be associated with bladder, kidney or even systemic infections such as sepsis.

What is Temporomandibular Disorder (TMD)?

So what does it mean when a person says “I have TMJ”? Well, technically, it means that you have a temporomandibular joint. And technically, that’s all it means- you have a jaw joint. In reality, most people are
actually saying “I have TMD”- which more literally means they have TMJ disorder.
TMD refers to the displacement or actual slipping of the articular disc off of the condyle. In English, this means that the little cushioning disc on top of your jaw bone has slipped off the bone and is now rubbing around
in that little socket just in front of your ear. That slipped disc can and will pinch nerves and blood vessels and this can wreak havoc with your entire body.
The symptoms of TMD include head and neck pain, migraines, jaw clicking/popping/pain, facial tics or tremors, and various other body issues. There are many patients over the years

who have presented with body tics ( and this is sometimes a Tourette’s situation) and the appliances used can help mitigate their symptoms. There was actually a study done in Osaka at the university that confirmed symptoms of Tourette’s were actually reduced greatly by wearing a TMD appliance! The understanding is that the nerve in the jaw joint- most likely the auriculotemporal nerve- is being pinched and this can cause the tics of Tourette’s. By decompressing the TMJ joint disc, this can alleviate those tics and Osaka university has confirmed the efficacy of the dental appliance used for Tourette’s.

The bottom line here is that when the little disc is slipped off the condyle bone in the jaw socket, it can really hurt the body badly. Our office has become quite efficient at identifying the damaged disc through the use of a detailed MRI that looks directly at the discs and the radiologist we use is able to produce a very detailed report that can be used to diagnose the problem. From there, a treatment plan that includes wearing various devices can be developed.
You do not need to wear the device(s) forever. The good news is that after a year of treating the damaged joints, usually the patient feels so much better and we can then use orthodontic techniques to raise the teeth up to the right height to support the TM joints. It is quite a process and took years to develop, but nowadays we can use clear aligners in almost all cases and this makes the work so much easier for everyone.
It is rare, but sometimes we need to have surgery done for the discs when they are beyond repair. Years ago this was a difficult procedure, but today it has become much easier for both the patient and the doctor. We even have a technique called ‘arthrocentisis’ in which the joints are flushed out and cleaned to remove debris and bony pieces that can cause pain. This procedure has shown good promise. There are other techniques in treating TMD and those can be reviewed in the next article.

Broken bones, dislocations, sprains, contusions, infections, and other serious injuries can occur at any time. Early attention is vitally important. Whenever you sustain a foot or ankle injury, you should seek immediate treatment from a podiatric physician.
There are lots of myths about foot and ankle injuries. Some of them follow:
Myths
• “It can’t be broken, because I can move it.” False; this widespread
idea has kept many fractures from receiving proper treatment. The truth is that often you can walk with certain kinds of fractures.
• “If you break a toe, immediate care isn’t necessary.” False; a toe fracture needs prompt attention. If X-rays reveal it to be a simple, displaced fracture, care by your podiatric physician usually can produce rapid relief. However, X-rays might identify a displaced or angulated break. In such cases, prompt realignment of the fracture by your podiatric physician will help prevent improper or incomplete healing. Often, fractures do not show up in the initial X-ray. It may be necessary to X-ray the foot a second time, seven to ten days later.
• “If you have a foot or ankle injury, soak it in hot water immediately.” False; don’t use heat or hot water on an area suspect for fracture, sprain, or dislocation. Heat promotes blood flow, causing greater swelling. An ice bag

Aric Adlam, DC
Dr. Aric Adlam discovered the benefits of chiropractic care early in life, beginning treatment at just two years old. After a serious football injury at age 16 threatened his athletic future, he chose chiropractic care over spinal surgery. Recovering in only half a season instead of the expected two years, he returned to the field and found his calling to become a chiropractor.

Dr. Mara Holton grew up in Maryland and has practiced with AA Urology for more than a decade since completing her residency.
Dr. Holton earned her undergraduate degree from The University of Vermont before attending medical school at Temple University in Philadelphia. She then completed her general surgery internship and urologic surgery residency at University of Maryland Medical System. While in residency, Dr. Holton practiced the full breadth of urologic training and surgery. Washington-Plaskett completed her medical school training at Meharry Medical College in Nashville, TN. She completed her Internal Medicine training at Morehouse School of Medicine in Atlanta, GA. She went on to pursue her Cardiology training at Boston Medical Center in Boston, MA where she also decided to stay and complete an additional year of training in Interventional Cardiology. She is a native of Fort Washington, MD and was eager to come back to familiar roots to start her career as a Cardiologist.
Dr. Washington’s interest in medicine started early as both her parents were in the medical field. Her inspiration is derived from the sacrifice of her parents as well as countless other health care workers that dedicate their careers in service to others.

Does Acupuncture Alleviate Shoulder Pain?
Research from June 2009, conducted at Johns Hopkins Bayview Medical Center, examined the efficacy of acupuncture in managing shoulder pain. In a single-blind randomized trial, adults experiencing shoulder pain for at least eight weeks due to osteoarthritis or rotator cuff tendonitis were assessed. Thirty-one participants were divided into three treatment groups: individualized acupuncture based on TCM principles, standardized acupuncture targeting typical shoulder pain points, and sham acupuncture without needle penetration.
Following six weeks of treatment, improvements in the total Shoulder Pain and Disability Index (SPADI)
scores were observed across all groups. However, clinically significant changes were only evident in the first two groups. Consequently, acupuncture emerges as a potentially effective therapy for chronic shoulder pain.
Why Does Acupuncture Sometimes Fail To Yield Results?
The practice of traditional Chinese medicine, encompassing acupuncture and herbal remedies, is intricate and requires extensive training. Optimal treatment outcomes hinge on identifying the correct acupuncture point formula and precise point locations. Without this precision, acupuncture may not deliver desired results.
Is Acupuncture Safe?
Overall, acupuncture is considered a safe treatment option. Potential side effects include dizziness, localized bleeding, dermatitis, nerve injury, or increased pain, particularly in cases where practitioners lack adequate training. Therefore, the expertise of a welltrained and experienced acupuncturist is crucial for both efficacy and safety.

“First visit by first birthday.” This is the view of the American Academy of Pediatric Dentistry. Pediatricians agree. The American Academy of Pediatrics suggests that children who are at risk of early childhood cavities visit a pediatric dentist by age one.
National studies have shown that preschool-aged children are getting more cavities. More than one in four children in the United States has had at least one cavity by the age of one. Many kids get cavities as early as age two.
To prevent early childhood cavities, parents first have to find out their child’s risk of developing cavities. They also need to learn how to manage diet, hygiene and fluoride to prevent problems.
But cavities aren’t all that parents need to learn about their child’s dental health. The age one dental visit lets parents discuss the following important facts.
How to care for an infant’s or toddler’s mouth

What Is Gout?

Gout is a kind of arthritis that occurs when uric acid builds up in blood and causes joint inflammation. An estimated 5.1 million people in the United States live with gout.
Gout is the most common form of inflammatory arthritis that affects approximately three times as many men as women. The number of newly diagnosed cases has increased over the past 40 years.
• Acute gout is a painful condition that typically affects one joint.
• Chronic gout is repeated episodes of pain and inflammation, which may involve more than one joint.
Causes, Incidence and Risk Factors
Gout is caused by having higher-than-normal levels of uric acid in your body. This may occur if:
• Your body makes too much uric acid
• Your body has a hard time getting rid of uric acid
If too much uric acid builds up in the fluid around the joints (synovial fluid/connective tissue), uric acid crystals form. These crystals are mainly formed as a result of a break down of purines found in the human tissue and many foods that we eat. These crystals cause the joint to swell up and become inflamed.
The exact cause is unknown. Gout may run in families. It is more common in men, in women after menopause, and those who drink alcohol. People who take certain medicines, such as hydrochlorothiazide and other water pills, may have higher levels of uric acid in the blood. Most uric acid is carried through the bloodstream to the kidneys, which eliminate it from the body in the urine.

Depression is a major health concern in our country and discriminates against no particular group. The disease affects all races, ages and has no regard for income. The focus of this article, however, is the expanding number of older folks who may experience depression.
Although adults 60 years and older compose 13% of the U.S. population, the use of mental health services is below the expected limit of use. Older Americans are disproportionately likely to commit suicide.
Individuals over 65 account for 20% of all suicide deaths, with white males being particularly susceptible.
Up to 5% of those persons over 65 suffer from some form of depression. This does not include the normal experiences of sadness, loss, or grief that all people feel as part of living. Major depression is persistent and can interrupt people’s ability to function. Depression should not be considered a normal phenomenon of growing older.
Both health care providers and the patient may have difficulty recognizing the signs of depression.
The National Institute of Mental Health has listed a few questions to consider when discussing general health concerns and the possibility of depression.
If some of these feelings or symptoms are persistent, lasting more than several weeks, and occur more often than not, talk about them with your healthcare provider. This may provide the best way to properly diagnose the problem and have a positive result ––feeling better.
