The Robots Are Here to Stay

How Robotic Technology Is Improving Spine Care By Fraser C. Henderson, Jr. MD, FAANS and Amin Amini, MS, MD, FAANS Most people picture neurosurgeons working constantly through a microscope to remove brain tumors or treat dangerous aneurysms. In reality, modern neurosurgeons often spend far more time operating on the spine than on the brain. As a result, advances in spine surgery technology have had a major impact on both surgeons and patients. One of the most important recent advances is robot-assisted spine surgery. Spinal fusion often uses titanium screws to stabilize the spine while bones heal together. Fusion may be necessary after certain fractures, because of severe wear and tear, or when relieving pressure on nerves requires removing enough bone that the spine would otherwise become unstable.

Traditionally, surgeons placed screws using anatomical landmarks and X-rays. In experienced hands, this technique is highly successful, but it can expose patients and surgical teams to radiation. Occasionally, a screw is later found to be in a less-than-ideal position and must be corrected. The Center for Brain & Spine began using the pioneering Mazor robotic system as early as 2017. Published studies report that approximately 98– 99% of robotically placed screws are in clinically acceptable positions, with revision rates below 1%. Our experience has matched or exceeded these results while also reducing blood loss, shortening hospital stays, and improving operating-room efficiency. What Does the Robot Actually Do? How New Technology Is Advancing Diabetic Foot and Wound Care

Health technology is advancing quickly, particularly in the treatment of diabetic foot wounds, infections, circulation problems, and other conditions that can threaten a limb. New products and devices are introduced regularly, but new does not always mean better. Emerging technologies must be carefully studied, compared with established treatments, and used for the right patient at the right time. This is especially important for people with diabetes and neuropathy. A blister, cut, or small ulcer may become serious quickly because reduced sensation can prevent a person from noticing an injury. Wounds may also have difficulty healing when pressure is not adequately relieved, blood flow is limited, infection is present, swelling is uncontrolled, or other medical conditions interfere with the healing process. Modern wound care now extends far beyond gauze and antibiotic ointment. Depending on the type and severity of a wound, treatment may include debridement to remove damaged tissue, advanced wound dressings, compression therapy, specialized offloading devices, and cellular or tissue-based products. Diagnostic and monitoring technologies may also help clinicians evaluate circulation, infection risk, wound depth, and healing progress. Clinical research plays an important role in determining which emerging technologies provide meaningful benefits. Clinical trials are structured studies designed to evaluate safety and effectiveness under defined conditions. before providing informed consent. Treatment protocols, follow-up visits, measurements, and monitoring help researchers determine whether a new approach performs better than, or offers advantages over, established care. This research can be particularly important in limb preservation. A nonhealing diabetic foot ulcer can progress to serious infection, bone infection, hospitalization, and, in severe cases, amputation. Earlier identification of healing problems

and more targeted treatment may provide clinicians with additional opportunities to intervene before complications become severe. Technology, however, cannot replace basic prevention. People with diabetes should regularly inspect their feet, wear properly fitting footwear, manage blood sugar and other medical conditions, and seek medical attention when they notice an unexplained blister, wound, swelling, redness, drainage, discoloration, or other con-

cerning change. The most valuable healthcare technology is not necessarily the newest device or treatment. It is technology supported by appropriate evidence and used as part of a comprehensive treatment plan. Continued research may provide better tools for wound healing and limb preservation while helping clinicians distinguish promising advances from treatments that do not ultimately provide meaningful benefits.

What Is the Prostate? The prostate is a walnut-sized gland that sits at the base of the bladder and contributes components of se-

cancer in men worldwide. About 1 in 8 men will be diagnosed with prostate cancer during their lifetime, and an estimated 36,000 men will die from the disease in 2026. Urologists specialize in identifying and treating prostate cancer. Screening and early detection have been proven to save lives. How Do Urologists Detect Prostate Cancer? Prostate cancer is most commonly found in men age 65 and older and is rare in men under 40. The American Urologic Association recommends screening beginning between ages 45-50 with a simple PSA blood test. Men at higher risk, including those with a family history of prostate cancer and Black men in the US and Caribbean, may begin screening as young as age 40. Ask your primary care physician or urologist about your individual risk and when screening should begin. What Treatments Are Available For Prostate Cancer? Treatment depends on the cancer’s grading and the patient’s goals. Observation or Active Surveillance may be appropriate for low-risk prostate cancer, while higher-risk disease may require surgery to remove the prostate or radiation therapy. Cancer that has spread to other parts of the body is generally treated with medications. Treatment decisions ultimately involve shared decision making between the patient and urologist. What Other Problems Can the Prostate Cause? Many men in their 50’s and 60’s develop urinary problems, including a weak stream, frequent urination, nighttime urination, leakage and urgency. Prostate obstruction is often the cause. Medications and minimally-invasive procedures can improve urinary flow and bladder emptying. What Can I Do For My Prostate Health? Along with an active lifestyle and heart-healthy diet, routine prostate cancer screening with a PSA blood test should be discussed with your primary care doctor or urologist. along the way, the question in the room quietly shifted. It’s no longer, “What’s the next treatment?” but, “How do we make the time that’s left as comfortable as possible?” The doctor mentions hospice, and suddenly you’re exhausted and unsure. Here’s a step-by-step look behind the scenes, from someone who walks families through it every day.

Step 1: The Consult Once a hospital care team believes hospice may be appropriate, they place a consult. A hospice liaison reaches out to meet with the patient and family, often within hours. Step 2: Goals Of Care That first meeting is a conversation, not paperwork. We talk about what

matters most right now and assess the appropriate level of care. Some families have already decided hospice is right; others simply want to understand their options. Conditions can change quickly, so it’s best to gather information early. Step 3: Level Of Care Some patients are stable enough to go home. Others need inpatient hospice, in the hospital or at an inpatient care center for intensive symptom management. This can shift day to day, so we continue to reevaluate. Step 4: Confer With Specialists We thoroughly review hospital records and communicate with any specialists involved. The goal is to make sure the plan reflects the patient’s wishes while prioritizing comfort and quality of life. Step 5: Preparing For Discharge We coordinate with the hospital’s discharge team to arrange transportation, order equipment like a hospital bed or oxygen, and send comfort medications home in case symptoms arise before the pharmacy delivers the rest the next day. Meanwhile, families are rearranging furniture and thinking through home safety. This can take some time, and that’s normal. Step 6: Discharge Day We aim for the hospice admission nurse to arrive the same day you’re discharged, so there’s no gap in care. Step 7: Becoming the Caregiver The biggest shift is moving from round-the-clock hospital staff to becoming the primary caregiver, with hospice supporting and guiding you rather than being there every hour. Help is always just a phone call away, including nights, weekends, and holidays. Any question is legitimate. It doesn’t have to be an emergency. It’s Okay To Ask the Same Questions Choosing hospice does not mean giving up. It means focusing on the person in front of you, surrounded by the people and place they know, rather than noisy equipment in an unfamiliar environment. Families often find relief once pain and other symptoms are eased. If stress makes it hard to absorb everything, please ask again. We’re happy to review the plan as many times as you need. What Is a Hammertoe?

By Howard Horowitz, DPM Bowie Foot & Ankle What Is a Hammertoe? A hammertoe is a contracture – or bending – of the toe at the first joint of the digit, called the proximal interphalangeal joint. This bending causes the toe to appear like an upside-down V when looked at from the side. Any toe can be involved, but the condition usually affects the second through fifth toes, known as the lesser digits. There are two different types: Flexible Hammertoes

These are less serious because they can be diagnosed and treated while still in the developmental stage. They are called flexible hammertoes because they are still moveable at the joint. Rigid Hammertoes This variety is more developed and more serious than the flexible condition. Rigid hammertoes can be seen in patients with severe arthritis, for example, or in patients who wait too long to seek professional treatment. The tendons in a rigid hammertoe have become tight, and the joint misaligned and immobile, making surgery the usual course of treatment. Symptoms • Pain upon pressure at the top of the bent toe from footwear. • The formation of corns on the top of the joint. • Redness and swelling at the joint contracture. • Restricted or painful motion of the toe joint. Meet Rami Makhoul

Meet Matthew Skancke

After completing his fellowship in Colon and Rectal Surgery at Robert Wood Johnson University Hospital-Rutgers University, Rami Makhoul, MD, joined Holy Cross Health Hospital in 2014 to serve patients of the DMV area.

Dr. Matthew Skancke is a colon and rectal surgeon serving Maryland, Virginia and Washington, DC. Dr. Skancke grew up in Great Falls, VA earning his undergraduate degree in electrical and biomedical engineering at the University of Virginia’s engineering school. After engineering school he spent three years working for a Swiss based GPS company U-blox America while going to night school to finish his premedical degree. He then attended medical school at the University of North Carolina at Chapel Hill where he did basic science research of mesenchymal stem cells to progress the field of regenerative medicine. Dr. Skancke continued his General Surgery training at the George Washington University Hospital and subsequently did his Colon and Rectal Surgery Fellowship at the Cleveland Clinic.

Upon graduating from medical school, Dr. Makhoul first completed his internship in General Surgery at the University of Massachusetts Medical School. He, then, completed his residency training in General Surgery at the George Washington University Hospital in Washington, DC. During his residency training at GW, he spent a year in clinical research where he published some of his work on colon and rectal cancer, and the use of Robotic Surgery in various abdominal procedures performed in the Colon and Rectal Surgery field. He continued with his passion for research and published peer-reviewed articles during his fellowship training. Several of his work, was presented at national conferences including the American Society of Colon and Rectal Surgery, and the Society of American and Gastrointestinal and Endoscopic Surgeons annual conferences.

Dr. Makhoul diagnoses and treats various diseases of the colon, rectum, anus, and other parts of the GI tract using national standards and guidelines. He has a special interest in treating colon and rectal cancer using advanced surgical minimally invasive techniques including robotic, laparoscopic, and transanal minimally invasive surgery. Dr. Makhoul is Board Certified in both General Surgery and Colon and Rectal Surgery and is an active member of the American Society of Colon and Rectal Surgeons education committee. As part of his commitment to surgical training and education, Dr. Makhoul serves as a Clinical Instructor of Surgery at the George Washington University. Dr. Makhoul is currently the chairman of the Department of Surgery at Holy Cross Hospital in Silver Spring. He also holds the position of Cancer Liaison Physician for Holy Cross Health, where he manages and overlooks clinically related cancer activities in collaboration with the Commission on Cancer and American Cancer Society.

During his training, Dr. Skancke has published over 24 peer reviewed publications, authored three book chapters, and presented over 45 times at national conferences in medicine and surgery. He has participated in clinical trials of bench top and mechanical sciences and is a member of the American College of Surgeons and the American Society of Colon and Rectal Surgeons. Dr. Skancke is Board Certified in General Surgery and Colon and Rectal Surgery and is an active member of the American Society of Colon and Rectal Surgeons Quality and Safety National committee.

When he is away from the office, Dr. Makhoul may be found spending time with his family. Dr. Makhoul enjoys skiing, cycling, scuba diving, tennis, and traveling.

The vaccines are available as an intradermal, intramuscular shot and as a nasal spray. They begin to arrive in doctors’ offices, pharmacies and clinics by mid-September. Every year, scientists design the vaccine to protect against the types of flu viruses that are expected to be the

most prevalent in the coming year. There are two basic types of viruses that can cause the flu – strain A and B. Both types can be potentially serious. “Trivalent” vaccines protect against two A strains and one B strain of flu, while newer quadrivalent vaccines protect against two A and two B strains. Everyday Preventive Actions The flu virus can spread by direct contact, such as sharing drinks, or through indirect contact, such as when an infected person sneezes on their hands and touches an object like the lunchroom microwave door. The influenza virus can live for 2-8 hours on surfaces. Sleep and Cancer Prevention

Getting enough sleep is essential to feeling alert and functioning well — but its benefits go far deeper. There are two different types of cupping methods commonly used. The first type is “retained” or “stationary cupping”; this is when the cups stay in one location for the duration of the treatment. This is mostly true of some bodily skin cancers, but this does not hold true for those that arise on the skin of the feet. Skin cancers of the feet are more related to exposure to chemicals, chronic inflammation or irritation, inherited traits and viruses. Regular foot check-ups for abnormalities is very important, but unfortunately the skin of the feet is often overlooked

during routine medical examinations. Types Of Skin Cancers Of the Legs and Feet Basal cell carcinoma frequently appears on sun-exposed skin surfaces. This is less with the feet. This is one of the less aggressive cancers in the body. It can cause local damage but rarely spreads beyond the skin. On the legs and feet, basal cell cancers often resemble non-cancerous skin tumors or benign ulcers. Squamous cell carcinoma is the most common form of cancer of the skin of the feet. This form is mostly confined to the skin during the early stages but can spread during the advanced stages. It usually begins as a small scaly bump or plaque, which might possibly appear inflamed. Sometimes there will be a history of recurrent cracking or bleeding. Squamous cell cancer can possibly resemble a plantar wart, a fungal infection, eczema, an ulcer, or any other common dermatological condition of the foot. They are not passive recipients of services. They experience themselves as recovering a new sense of self and purpose which goes beyond the limits of their disability. Rehabilitation refers to the services made available so persons with disabilities may learn to adapt to their world. Recovery refers to real life experiences as people accept and overcome the challenge of the disabil-

ity. It is the kindling of hope followed by action. Recovery is a process, not an end product. It is marked by a growing acceptance of personal limitations from which grow unique possibilities. For many it becomes an attitude, a way of life, a way to approach daily challenges. If possible, those of us working in rehabilitation would love to manufacture the spirit of recovery and hand it to those who attend our programs as we greet them at the door. We cannot. However, all of us can work to create an environment which will nurture recovery. To recover, people with a psychiatric disability must be willing to try and fail, and try again. A program is fail proof when a person feels he can always come back, pick up where he left off and try again. The person is welcomed, valued, and wanted.
