Thin Does Not Always Mean Healthy – Especially For Your Feet

Part 1 Being thin and being healthy are not the same thing. As a podiatrist, I see the consequences of that myth in people’s feet every single week. When most people picture a “healthy” person, they imagine someone thin – but that image is often misleading and, in some cases, harmful. In my practice, I regularly see thin patients dealing with poor circulation, numb toes, weak muscles, and slow-healing wounds. These are not issues most people associate with someone who “looks healthy,” yet they are more common than you might expect. At the same time, I see heavier patients who walk comfortably, maintain good blood sugar control, and have strong, stable feet. Their bodies may not fit the typical image of health, but functionally, they are doing well. There is even a medical term for people who appear to be a normal weight but still carry too much unhealthy fat and not enough muscle: normal-weight obesity. This condition increases the risk of diabetes, heart disease, and other serious health problems – even when the scale suggests everything is fine. It is a clear reminder that weight alone does not tell the full story of health. From a foot health standpoint, weight is only one piece of a much bigger picture. What matters more is how your body functions overall. Muscle strength, circulation, nerve health, and how your body distributes pressure with each step all play critical roles in keeping your feet healthy. While it is true that extra weight increases load – just www.YourHEALTHMagazine.net

one additional pound can translate into several pounds of added pressure through your hips, knees, and ankles during movement – low muscle mass and poor conditioning can be just as damaging. I often treat very thin individuals with stress fractures, tendon injuries, and chronic foot pain because their bodies are simply not strong enough to handle normal daily activity. Being underweight or undernourished can lead to weak bones, low energy, and a higher

risk of injury. In podiatry, this frequently shows up as stress fractures in the metatarsals – the long bones in the front of the foot. It can also appear as persistent tendon pain or plantar fasciitis that does not improve because the body lacks the protein and nutrients needed for proper healing and tissue repair. One patient stands out – a woman in her early 40s who walked five miles a day and took pride in staying thin. However, her diet consisted mostly of coffee, salad, and snack

bars. She came in with burning pain in both feet, and imaging revealed stress fractures. Her bones and muscles were not getting the support they needed. The solution was not more weight loss. It was better nutrition, strength training, supportive footwear, and time to heal. The takeaway is simple: thinness does not guarantee strength, resilience, or good health. The prostate, a male organ located below the bladder and wrapping 360 degrees around the urethra, can enlarge with age, restricting urinary flow. This condition, known as

benign prostatic hyperplasia (BPH), often leads to symptoms such as frequent urination, weak urinary stream, and urinary tract infections. Treatment focuses on reducing prostate size and improving urinary flow. Symptoms Of Enlarged Prostate Symptoms range from mild to severe, often developing gradually over years but potentially worsening after surgery or other triggers. It is almost impossible to maintain a nice smile when you are hurting, and especially when that pain comes from TMD (Temporomandibular Disorder). When the discs in the jaw joints are out of place, they can and will pinch nerves and blood vessels in the jaw joints. This is a concept that few doctors understand as it was

never taught in school. Such training often requires extensive post-graduate education, and oftentimes that can be very hard to find! As a patient, you should be aware that there are many ways to treat the TMJ disorder. To figure out which one(s) are right for you, the first step is diagnosing. A proper diagnosis is where it all starts and that means a very good MRI (soft tissue) and CBCT x-ray of the joints. From there, your

course of treatment might include orthotic (non-surgical dental oral appliance treatment), physical therapy, osteopathy, dry needling, acupuncture, massage, and even laser therapy. At our office, we most frequently use appliance therapy to make space for the displaced discs to return into the socket. Along with your appliances, we oftentimes do massage or laser therapy. When a patient wears an orthotic appliance on their lower teeth, this allows the displaced discs the chance to get back into proper position within the socket. This takes time. Most cases the displacements occurred over the course of years and it often takes a year for those discs to go back into place, if they will at all. In the event that they do not go back where they belong, a minor surgery can be done to put them back into the joints. This is called plication.

The other appliance that is used is the ALF. This little wire has the ability to open up the palate which makes it easier to breathe. It can also lower a high palatal vault which allows better nasal breathing. When a person breathes through their nose more, this allows for better production of nitric oxide which helps the body to heal better. It also helps level the cranial bones to some extent and thus reduce asymmetry of the skull. This little device really is amazing. Admittedly, it takes years of training to get a handle on it, but it is well worth it. Ultimately, when a TMD case has healed or improved enough to not need surgery, it is time to finish up so you do not need to wear appliances any longer. This can be accomplished either by orthodontics or prosthodontics. The purpose is to support your jaw joints and the discs in the correct anatomical position. That’s why the Maryland Accessible Telecommunications (MAT) program offers a wide variety of solutions and tools, provided by the State, to support day-to-day communications. The MAT program is administered by Telecommunications Access of Maryland, a State agency located within the Maryland Department of Disabilities. The MAT program is available to any Maryland resident who: • has difficulty using a standard telephone; • is 3 years of age or older;

• is certified as being Deaf, hard of hearing, DeafBlind, blind, low vision or having cognitive, mobility or speech-related difficulties; • is on a fixed income, or is a recipient of at least one state or federal financial benefit; • equipped with home phone service and high speed/WiFi If you meet the above criteria, you may apply for the MAT program. Applications are available online at mdrelay. com. Once your application is approved, you will be notified and given instructions to schedule a free evaluation at a MAT evaluation center near you. During this evaluation, you will have the opportunity to practice dialing, answering and talking on several different pieces of equipment until the evaluator finds the solution that works best for you. Your evaluator will order your equipment and it will be shipped to your home within 4-6 weeks. What Is a Bunion?

Why Dementia Caregivers Should Call Hospice Now, Before a Crisis By Steven Watson, LCSW-C APHSW-C, Clinical Social Worker Chesapeake Supportive Care When people imagine losing someone they love, they often picture a sudden event like a heart attack, an accident or a serious illness that unfolds quickly. Dementia is different. Instead, families witness small, gradual changes that can take place over many years.

Memory fades. Communication becomes harder. Personality and shared history may slowly disappear. Long before death occurs, loved ones are already experiencing grief. Meanwhile, the demands on the caregiver increase. Often there is one primary caregiver, usually a spouse or adult child, carrying most of the responsibility. A Sense Of Duty

Many people assume the hardest part of caring for someone with dementia is the physical work. But in my experience, the biggest challenge is often the constant stress. Caregivers live with a sense of uncertainty: When will something change? When will there be a fall, an infection or a new behavior to manage? That ongoing pressure can take a toll. Caregivers may struggle with

Steven Watson sleep, anxiety or depression. They may begin neglecting their own health. They will miss their own medical appointments, forget to take their own medications or withdraw from their friends and family. Many feel a deep sense of duty. They believe this is what they are supposed to do. Because of that, it can be very difficult to admit when they need help, never mind ask for help from a hospice organization. More Help, More Time One of the biggest misconceptions about hospice is that it is only for the last few days of life. In reality, hospice is designed to support patients and families for months. I often describe hospice as providing three things: people, equipment and medications. The hospice team may include nurses, nursing assistants, social workers, spiritual care providers and physicians who oversee the care plan. Hospice also provides medical equipment, such as hospital beds, walkers or oxygen. The team also manages the patient’s medications. Most of the time, medications are delivered to their home, along with personal supplies such as incontinence briefs, bandages, lotions and more. These services are usually covered by Medicare and most insurance plans. Hospice does not mean stopping medical care. Instead, it shifts the focus from curing disease to prioritizing comfort, quality of life and support for both the patient and the caregiver. Why Calling Earlier Helps Families often wait until a medical crisis to call hospice, but earlier support can make a significant difference. The challenges of caregiving and medical decline are happening whether hospice is involved or not. The question is whether families want additional guidance and support during that time. Cell phones stimulate brain chemistry in ways similar to addictive substances. A neurotransmitter called dopamine is released in the brain as a reward. Dopamine is released when we experience something pleasurable or exciting, such as eating enjoyable food, winning a game, gambling, socializing, and – most notably – checking notifications on our phones. Dopamine itself is not harmful. In fact, it is essential for motivation, learning, and pleasure. The problem arises when a behavior is repeated frequently, causing short, rapid bursts of dopamine. Social media platforms such as Instagram, TikTok, and Facebook are intentionally designed to keep us engaged in a constant state of stimulation. This dopamine-driven response keeps us using these platforms for as long as possible by employing psychological techniques that trigger repeated dopamine release. Every time we refresh our screen and see something that captures our attention, dopamine is released. It’s

similar to the feeling experienced with gambling or other repetitive habits. Likes and comments act as small bursts of dopamine that signal approval. This creates a sense that someone is paying attention to you, which feels rewarding. Scrolling also releases this same chemical. When we spend hours on an app, the pleasure reinforces the behavior through continued dopamine release. Researchers suggest that smartphone use can create a cycle driven by dopamine. When you feel bored or stressed, you check your phone. Each time you do, dopamine is released, rewarding the brain and encouraging the behavior. Over time, the brain begins to associate the device with relief and pleasure, making the habit increasingly automatic. The problem occurs when you feel anxious without your phone, constantly check notifications, and lose track of time while scrolling. This can lead to difficulty focusing and reduced sleep due to late-night phone use.

Estrogen also promotes fat breakdown, contributing to energy expenditure. Finally, estrogen modulates thyroid hormone metabolism, influencing overall metabolic rate. Testosterone is the muscle-making hormone. Since muscle burns more calories than fat tissue does, muscle mass is a key factor in metabolism. However, testosterone decreases as much as 50% in the years from our mid-20s to our mid-40s (yes, it’s found in females). This decreasing testosterone diminishes muscle mass making it harder to maintain, let alone build, muscle as we age. And maintaining

Does Fluoride Lower the IQ? A pediatric dentist’s evidence-based look at what new studies are showing and how families can stay both cavity-free and safe.

By Elizabeth Shin, DDS Bethesda Chevy Chase Pediatric Dentistry What People Are Really Asking When people hear about fluoride and IQ, most are not thinking about research papers or chemical names. They are thinking about the fluoride their children use every day, brushing their teeth, rinsing, or getting fluoride treatments at the dental office. That makes sense. But the studies that raised this question are not about fluoride that touches the teeth and gets spit out. They focus on fluoride that is swallowed, the kind that goes through the body in drinking water, baby formula mixed with tap water, and foods or beverages made with fluoridated water. To understand the science, it helps to separate topical fluoride, which works on the surface of the tooth, from systemic fluoride, which travels through the bloodstream. Both play a role in oral health, but only the systemic kind is being studied for possible effects on the brain and body. The Study That Reignited the Discussion In early 2025, JAMA Pediatrics published a review combining 74 studies on fluoride exposure and children’s IQ. The researchers found that, overall, children with higher fluoride levels in their bodies had slightly lower IQ scores. They did not claim fluoride directly causes lower IQ, but the link appeared stronger in studies that measured fluoride inside the body, such as urinary levels, rather than simply using city water data. Their message was straightforward. The pattern is strong enough to warrant a fresh look at how much fluoride children are receiving from all sources combined. Differing Scientific Opinions Some academics have questioned the review, noting that many of the studies it included came from countries with higher natural fluoride levels or from communities with co-exposures such as arsenic or poor nutrition. Others have argued that the review’s methods may have combined studies of unequal quality. However, even among critics, there is general agreement that the findings raise valid questions and that more well-controlled research at U.S. exposure levels is urgently needed. page 26 | Your HEALTH Magazine

This Was Not the First Time Scientists Saw This The JAMA study confirmed what earlier research had already noticed. In Canada (2019) pregnant women with higher urinary fluoride had children with slightly lower IQ scores, especially boys. In Mexico (2017–2018) two separate studies linked prenatal fluoride exposure with lower cognitive and attention scores. In Canada (2020) babies fed formula mixed with fluoridated tap water scored lower on non-verbal IQ tests than breastfed babies. Earlier reviews (2012–2018) also showed an inverse relationship between fluoride exposure and IQ, though quality varied. In New Zealand (2015) one study found no difference, but it used residence rather than biomarkers to estimate exposure. Taken together, this growing body of evidence suggests a consistent pattern that deserves attention, especially for developing brains. Beyond IQ: Other Health Clues Even if we set aside brain development, fluoride that is swallowed leaves unmistakable signs inside the body. The most visible sign is dental fluorosis, faint white lines, cloudy patches, or brownish streaks that appear on teeth as they form. Many professionals call it a cosmetic issue, but that view misses the deeper biological meaning. Fluorosis as a Warning Sign Fluorosis is not merely a surface change; it is a biological warning sign of overexposure. It tells us that, during tooth development, the body absorbed more fluoride than it could safely manage. At that point, the safe threshold for cellular balance has already been surpassed. Because fluoride is strongly attracted to calcium, it tends to settle in tissues rich in calcium, including teeth, bones, the pineal gland, and even developing regions of the brain. The enamel changes we can see are simply the visible tip of a deeper metabolic shift. Systemic Effects Beyond the Teeth The same chemical interactions that create white streaks in enamel may also be occurring silently in the thyroid, kidneys, and nervous system, where we cannot see them with the naked eye. Fluorosis, therefore, should not be dismissed as cosmetic. It is the body’s way of signaling that fluoride exposure has reached a level capable of altering normal cellular activity. The teeth are the dashboard light, and once they show a warning, the rest of the system deserves inspection.

Topical Fluoride: Still a Trusted Ally Fluoride that stays on the tooth surface remains one of dentistry’s most effective tools. Toothpaste, varnish, and sealants all work topically to harden enamel and prevent decay. The question today is not whether fluoride works, but how much and how it is delivered. In the 1950s, when toothpaste was rare, water fluoridation helped communities reduce cavities. Now, with widespread fluoride toothpaste, professional varnish, and better nutrition, mass ingestion may no longer add much benefit and could increase total exposure unnecessarily. Modern prevention should be targeted rather than universal ingestion. Practical Questions People Are Asking About Fluoride 1. If fluoride helps teeth, why are people suddenly worried about it? Because scientists are now examining how swallowed fluoride affects other organs besides teeth. Fluoride is beneficial in small amounts, but it can accumulate in bones and tissues. Researchers are exploring its possible influence on the brain, thyroid, and kidneys. The goal is balance, not elimination. 2. How can parents protect their children without overdoing fluoride? Use toothpaste properly: rice-sized for toddlers, pea-sized for preschoolers, and teach them to spit, not swallow. Professional fluoride varnish and sealants are safe and stay on the teeth. When preparing baby formula, alternate between fluoridated and low-fluoride bottled or filtered water. Good brushing and diet remain the best cavity prevention. 3. Is fluoridated water safe during pregnancy? Pregnancy is a sensitive time for brain and organ development. Expectant mothers can use reverseosmosis or carbon filters for drinking water, mix bottled and filtered water to reduce total intake, and maintain gum health since inflammation during pregnancy can affect the baby. Lowering ingestion is a reasonable precaution, not an alarm. 4. What about adults with kidney or thyroid conditions? People with reduced kidney function clear fluoride more slowly, so it can accumulate. Those with thyroid disorders may also be more sensitive. Choose filtered or non-fluoridated water when possible. Continue using fluoride toothpaste since it acts topically. Inform your doctor about local water fluoridation to help interpret labs. Fluoride is not automatically harmful, but those with chronic

illness should manage intake carefully. 5. Are fluoride-removing filters expensive? Not really. Reverse-osmosis systems cost about $150–300 with annual filter changes, and smaller countertop models start near $50. For families with infants or chronic illness, it is a practical safeguard. 6. How can I find out if my water has fluoride? Check your city’s annual water report or the CDC’s “My Water’s Fluoride” website. If you use a private well, have it tested, since natural levels vary widely. 7. Does bottled water contain fluoride? Some brands do and others do not. Always read the label. For baby formula, most pediatricians suggest non-fluoridated bottled or filtered water. 8. What is the safest overall approach? Keep topical fluoride such as toothpaste, varnish, and sealants. Reevaluate systemic exposure during pregnancy, infancy, or chronic illness. Support new U.S.-based studies that measure real fluoride levels instead of relying on decades-old data. Fluoride’s value remains, but dose and delivery are what truly determine safety. So, Does Fluoride Lower IQ? The fair answer is that higher systemic fluoride exposure is associated with slightly lower IQ in many studies. At typical U.S. fluoridation levels, the data are still limited. Topical fluoride remains safe and effective. In short, keep fluoride on the teeth, not in the bloodstream. A Final Word Community water fluoridation was one of the great health achievements of the twentieth century. Yet children today already receive fluoride from toothpaste, varnish, and food. We can preserve its benefits while reducing unnecessary ingestion. Re-examining systemic fluoride is not anti-science; it is good science. Our responsibility is to use fluoride wisely, to protect both smiles and developing minds. Understanding Hyperpigmentation

External and Internal Causes, Plus Proven Solutions

By Fana Asaafa, PhD, RN Fana SkinCare Hyperpigmentation is a common skin concern where patches of skin become darker than surrounding areas due to excess melanin production. Melanin, produced by cells called melanocytes, protects skin from damage but can overproduce in response to various triggers. This results in dark spots like sun damage, melasma, or post-inflammatory marks. It affects all skin tones but is often more noticeable and persistent in medium to darker complexions. Two categories address some of

the possible causes: external (environmental exposures) and internal (systemic or bodily factors). Many involve oxidative stress, cellular “rust” from free radicals that amplifies melanin production. External Causes: Environmental Triggers Ultraviolet (UV) Radiation from Sunlight: UV rays (especially UVA) remain the primary driver of hyperpigmentation. They stimulate melanocytes to produce more melanin as a defense mechanism, leading to solar lentigines (age spots) on sun-exposed areas like the face, hands, and chest. Cumulative exposure over years explains why spots often worsen with age. High Energy Visible Light (HEV), also known as blue-violet light, is everywhere around us from screens and LEDs. Light from phones, computers, and indoor lighting all induces oxidative stress and directly activates melanogenesis pathways. Studies confirm constant exposure contributes to hyperpigmentation, particularly in skin of color, by gen-

erating reactive oxygen species that boost melanin. This “digital aging” effect adds to UV damage in our screen-heavy lives. Pollution, irritants, and chemicals in face and hair products, laundry detergent, harsh ingredients in creams, lotions, or unregulated lightening products all cause inflammation, triggering post-inflammatory hyperpigmentation (PIH). Unregulated bleaching creams often contain mercury or high-potency steroids, risking permanent discoloration, ochronosis, or systemic toxicity (e.g., kidney effects). Evidence warns against non-prescription “fast-whitening” products; stick to regulated, dermatologist-recommended options. These external factors are largely controllable through protection and mindful product choices. Through SilverSneakers members are able to find classes designed for all abilities. Check the silversneakers.com website to find participating fitness facilities near you. An estimated 5.1 million people in the United States live with gout. Gout is the most common form of inflammatory arthritis that affects approximately three times as many men as women. The number of newly diagnosed cases has increased over the past 40 years. • Acute gout is a painful condition that typically affects one joint. • Chronic gout is repeated episodes of pain and inflammation, which may involve more than one joint.

Causes, Incidence and Risk Factors Gout is caused by having higher-than-normal levels of uric acid in your body. This may occur if: • Your body makes too much uric acid • Your body has a hard time getting rid of uric acid If too much uric acid builds up in the fluid around the joints (synovial fluid/connective tissue), uric acid crystals form. These crystals are mainly formed as a result of a break down of purines found in the human tissue and many foods that we eat. These crystals cause the joint to swell up and become inflamed. The exact cause is unknown. Gout may run in families. It is more common in men, in women after menopause, and those who drink alcohol. People who take certain medicines, such as hydrochlorothiazide and other water pills, may have higher levels of uric acid in the blood. In women, estrogen aids in the production of bone mass. At menopause, the amount of estrogen their bodies produce declines sharply. • Have your menstrual periods stopped? Women who have had a hysterectomy, women who are high-performance athletes, and women who diet excessively or who have an eating disorder may stop having periods. These women have had a hormonal change that can cause bone loss. • Are you taking medicines that increase the risk of osteoporosis? The most common culprits are steroid medications and high doses of thyroid hormone. Long-term use of these medicines can cause bone loss. Examine your lifestyle: • Smoking is linked to bone loss.
