Many common foot problems do not happen overnight. Bunions, heel pain, arch discomfort, and toe deformities often develop gradually after years of wearing shoes that prioritize appearance over support. While stylish footwear may look appealing, poorly designed shoes can place constant stress on the feet and slowly alter their natural structure.

One simple way to evaluate a shoe is with the “twist test.” Hold the shoe in both hands and twist it as though wringing out a towel. If it folds easily or collapses in the middle, it likely offers minimal support. A quality everyday shoe should resist twisting while still allowing flexibility near the toes where the foot naturally bends during walking.

Age also changes the needs of the feet. As the body ages, ligaments loosen, arches may flatten, and the natural cushioning under the heel and ball of the foot gradually thins. Shoes that felt comfortable years ago may no longer provide adequate support or stability. Foot size can even change over time, making regular shoe fittings increasingly important.

Comfort should never be judged by softness alone. Some extremely soft shoes may actually allow too much movement and fail to properly

support the foot. A supportive shoe should feel stable, especially around the heel and arch, while still allowing natural motion near the front of the foot.

Choosing healthier footwear does not mean giving up style entirely. Many modern shoe designs successfully combine appearance with proper support and stability. The key is learning to look beyond aesthetics and focus on how a shoe functions.

When shopping for shoes, pay attention to how they bend, twist, and

fit throughout the day. Feet should feel supported rather than cramped or fatigued after hours of walking or standing. Small changes in footwear choices today may help prevent chronic pain and deformities in the future.

Treatment Options For “Acid Reflux”

What Is “Acid Reflux”?

Gastroesophageal reflux disease (GERD), commonly referred to as “acid reflux”, represents the abnormal backflow of stomach acid and other contents into the esophagus and often into the mouth. The cause of GERD is the failure of a valve mechanism (sphincter) at the lower end of the esophagus that should prevent the backflow. The backflow of stomach acid results in heartburn and a bitter

taste in the mouth. The backflow of food into the mouth is commonly described as regurgitation.

Treatment

There are several treatment options for GERD. First-line treatments are aimed at reducing and managing the symptoms through lifestyle changes and medications. If first-line treatments are unsuccessful then operative treatment is required to enhance the valve mechanism at the lower end of the esophagus.

Lifestyle Adjustments

General measures include eating several small meals and avoiding large meals. Meals should also be avoided just prior to going to sleep. Some foods predispose to heartburn should be avoided. These include chocolate, spicy foods, and acidic foods such as tomato and citrus. Avoidance of alcohol and caffeine are also recommended. Why the TMJ Is Vital To Your Overall Health

The temporomandibular joints (TMJs) play an important role in both oral health and overall wellness. Located just in front of each ear, these small joints connect the lower jaw to the skull and allow essential movements such as chewing, speaking, and breathing. Although they are small, problems with the TMJs can affect many areas of the body.

What Are the TMJs?

The TMJs function like hinges, allowing the jaw to move smoothly. Inside each joint is an articular disc, a cushionlike structure that helps keep the joint properly aligned and prevents the bones from rubbing together during movement. When the joints and discs are functioning properly, jaw movement feels effortless.

However, when the disc becomes displaced or the joint becomes inflamed, the mechanics of the jaw can be

disrupted. Because the TMJs are closely connected to the muscles, nerves, and bones of the head, neck, and shoulders, dysfunction can contribute to a variety of symptoms beyond the jaw itself.

Common Symptoms Of TMJ Disorders

TMJ disorders may develop due to disc displacement, inflammation, injury, or wear and tear. Common symptoms include:

• Jaw pain or tenderness

• Headaches or migraines

• Clicking, popping, or grinding sounds in the jaw

• Difficulty chewing or discomfort while eating

• Neck pain and stiffness

• Dizziness or balance problems

Many people do not immediately associate these symptoms with their jaw joints, which can delay diagnosis and treatment. Over time, untreated TMJ disorders may worsen. Chronic inflammation can contribute to degeneration of the jaw joint and surrounding structures, leading to increased pain and dysfunction.

Recognizing early warning signs is one of the best ways to prevent long-term complications. Symptoms such as jaw clicking or popping are often dismissed as harmless, but they can indicate the beginning stages of joint dysfunction. When TMJ problems are identified early, treatment is generally less invasive and more effective. Depending on the cause and severity of the condition, treatment may include custom oral appliances to improve jaw position and reduce strain on the joints, physical therapy to improve jaw and neck function, and evaluation of contributing factors such as airway obstruction or bite imbalances.

Advanced imaging, including MRI and CBCT scans, can help clinicians accurately assess the condition of the joints and develop a personalized treatment plan.

When a loved one enters hospice care, families are often navigating

some of the most tender and frightening days of their lives. Decisions are weighty. Conversations are difficult. And when a nurse or physician mentions morphine, it is not unusual for the room to go quiet.

That concern is completely understandable. It comes from love. But it is also, I have found, something we can gently help families move through together. In hospice, morphine is not something to fear. It is one of our most

Morphine is one of the most common medications we use because it is one of the most effective. It treats two very distressing symptoms at end of life: pain and shortness of breath.

Most people understand its role in pain control. What surprises them is how helpful it is for breathing.

Patients often experience what we call “air hunger” – the frightening

sensation of not being able to catch your breath. Even when oxygen levels appear normal, the feeling can cause real distress. Morphine relaxes the body and makes breathing feel easier. We don’t use it to sedate. We use it to take the edge off so patients can be more present, more at ease.

Another common misconception is that morphine is only used in the final hours of life. That’s not true. Some patients may take it for months, others for days. It depends entirely on their symptoms. In fact, some people are already using morphine as prescribed by their primary care physician or specialist before hospice care even begins.

Dosing is far more cautious than people expect. We start low, especially for those who have never taken opioids. Then we adjust slowly. This is careful, individualized care. Our goal is always the smallest amount needed to provide relief. We monitor closely, making changes as needed.

When symptoms are properly managed, the difference can be profound. I’ve seen patients go from tense, restless and visibly struggling to calm, relaxed and at ease. Even when they can’t speak, their bodies tell the story. They have softer facial expressions, unclenched hands, a sense of peace. It’s not just the patient who benefits. Families feel it, too.

Watching someone you love be in pain or struggle to breathe is distressing. It can create anxiety, helplessness and even panic. When we can relieve that suffering, you can see the atmosphere in the room change. Families sit closer. They talk, hold hands, share memories.

I also hear concerns about addiction, which is understandable given today’s opioid crisis. But morphine is used under the close supervision of the hospice team, with the sole purpose of comfort.

If I could change one perception, it would be this: morphine is not something to fear.

At its core, hospice is about quality of life. Morphine is one of the tools that helps us achieve that. It is measured. It is monitored. It is compassionate.

At its best, it is a way to ease suffering so that what matters most can come forward for the patient and their family: Connection, presence and peace.

After completing his fellowship in Colon and Rectal Surgery at the prestigious Robert Wood Johnson University Hospital-Rutgers University, Rami Makhoul, MD, joined Holy Cross Hospital in 2014 to serve patients in the DC Metropolitan area. In January 2018, he joined Metro Colon and Rectal Surgery, PC.

Upon graduating from medical school, Dr. Makhoul first completed his Internship in General Surgery at the University of Massachusetts Medical School. He then completed his Residency training in General Surgery at the George Washington University Hospital in Washington, DC.

During his Residency training at GW, Dr. Makhoul spent a year in clinical research where he published much of his work in colon and rectal cancer, and the use of Robotic Surgery in various abdominal procedures. He continued with his passion for research and published peer-reviewed articles during his Fellowship training. Several of his works were presented at national conferences including the American Society of Colon and Rectal Surgery, and the Society of American and Gastrointestinal and Endoscopic Surgeons annual conferences.

Dr. Makhoul diagnoses and treats various diseases of the colon, rectum, anus, and other parts of the GI tract using national standards and guidelines. He has a special interest in treating colon and rectal cancer using advanced surgical minimally invasive techniques including robotics, laparoscopy, transanal total mesorectal excision and transanal endoscopic microsurgery.

As part of his commitment to surgical training and education, Dr. Makhoul serves as a Clinical Instructor of Surgery at the George Washington University. Dr. Makhoul holds the position of Cancer Liaison Physician for Holy Cross Health, where he manages and oversees clinically related cancer activities in collaboration with the Commission on Cancer and American Cancer Society.

When he is away from the office, Dr. Makhoul may be found spending time with his family. He also enjoys skiing, running, scuba diving, tennis, and traveling.

Dr. Matthew Skancke is a Colon and Rectal Surgeon serving Maryland, Virginia and Washington, DC. Dr. Skancke grew up in Great Falls, VA earning his undergraduate degree in electrical and biomedical engineering at the University ofVirginia’s engineering school. After engineering school he spent three years working for a Swiss based GPS company U-blox America while going to night school to finish his pre-medical degree. He then attended medical school at the University of North Carolina at Chapel Hill where he did basic science research

publications, authored three book chapters, and presented over 45 times at national conferences in medicine and surgery. He has participated in clinical trials of bench top and mechanical sciences and is a member of the American College of Surgeons and the American Society of Colon and Rectal Surgeons. Dr. Skancke is Board Certified in General Surgery by ABS and Colon and Rectal Surgery (ABCRS) and is an active member of the American Society of Colon and Rectal Surgeons Quality and Safety National committee.

Outside of work, Dr. Skancke has found a new love for French cooking and North Carolina barbeque.

Dr. Mary Alexander, periodontist, obtained a Bachelor of Science degree in Biology from the University of South Carolina and continued her education in dentistry at the University of Maryland School of Dental Surgery where she received her DDS degree. Prior to and during her dental studies, Dr. Alexander received funding and support from the National Institutes of Dental Research for her involvement in the periodontal research.

After obtaining her DDS degree Dr. Alexander spent one year at the Veteran’s Administration Hospital, in general practice residency focusing her training in the area of anxiety patient management. She then attended Harvard University and earned her clinical specialty in periodontics and a doctorate in medical science. Her doctoral research was focused in the study of bone function and metabolic disease of the bone.

Dr. Horowitz has been practicing in the Bowie area for over 25 years. He graduated from the Illinois College of Podiatric Medicine with a Doctorate of Podiatric Medicine and did his postgraduate training in podiatric medicine and surgery in association with the Podiatry Group of Greater New York and the New York College of Podiatric Medicine. Dr. Horowitz remained in New York for an additional two years to practice with several of his training program’s attending doctors in order to further hone his medical and surgical skills.

Dr. Jeffrey Brown grew up in the state of Maine and went to Bowdoin College, where he graduated Magna Cum Laude in 1982. He accepted early decision into the Georgetown School of Dentistry, and graduated from there in 1986. While at Georgetown, Dr. Brown worked in the Neurology Department as an assistant on a project that involved studying the regeneration of nerve tissue in the spinal column after major trauma.

Upon graduation from Georgetown Dental School, Dr. Brown worked for a short time as an associate in a practice in Falls Church, VA. From there, he moved on and built his own practice in the Fairlington neighborhood in Arlington, VA, creating one of the largest practices ever seen on the East Coast.

Dr. Brown took a short sabbatical to help raise his four young children and to continue his education. It was then that he began the process of understanding sleep apnea and how it correlates to TMJ treatment. A pivotal turning point came with her reading of “The Ministry of Healing,” a book that highlighted the therapeutic power of helping others. The Entourage Effect Why Different Cannabis Products Feel Different

One of the most common questions people ask about cannabis is why different strains and products can create completely different experiences. Some may feel calming and relaxing, while others feel energizing, uplifting, or mentally stimulating. The answer often comes down to a concept known as the entourage effect, an important idea in cannabis science that explains how the plant’s compounds work together inside the body.

Most people are familiar with THC and CBD, the two best-known cannabinoids found in cannabis. THC is primarily associated with psychoactive effects, while CBD is commonly linked to calming and wellness-related benefits. However, cannabis contains far more than just these two compounds. The plant naturally produces dozens of cannabinoids, along with aromatic compounds called terpenes and plant pigments known as flavonoids.

The entourage effect refers to the interaction between all of these compounds. Rather than acting independently, cannabinoids, terpenes, and flavonoids may work together synergistically to influence how cannabis affects the body and mind. In other words, the overall experience is shaped not just by one ingredient, but by the unique combination of compounds present in a particular product.

This concept helps explain why two cannabis products with similar THC levels may produce very different effects. One strain may feel relaxing and sedating, while another may feel uplifting or creative. Much of this difference comes from the balance of cannabinoids and terpenes working together.

Terpenes play a major role in this

process. These naturally occurring compounds are responsible for the aroma and flavor of cannabis, but they may also influence mood and effects. For example, linalool, which is also found in lavender, is often associated with relaxation. Limonene, commonly found in citrus fruits, is frequently linked to elevated mood and energy. Myrcene is another common terpene believed to contribute to calming or sedating effects.

THC is primarily associated with psychoactive effects, while CBD is commonly linked to calming and wellness-related benefits.

Researchers began exploring the entourage effect in the 1990s while studying the body’s endocannabinoid system, a network of receptors involved in regulating sleep, mood, pain, appetite, and immune function. Plant-based cannabinoids interact with this system in complex ways, and scientists continue to study how these combinations may affect the body.

Consumers often encounter terms like “full-spectrum,” “broad-spectrum,” and “isolate” when shopping for cannabis or hemp-derived products. Full-spectrum products contain a wider range of cannabinoids and terpenes, while isolates contain only a single cannabinoid, such as pure CBD. Many people prefer full-spectrum products because they preserve more of the plant’s natural compounds.

Because everyone responds differently to cannabis, finding the right product may require patience and experimentation. Paying attention to cannabinoid ratios, terpene profiles, dosage, and personal response can help individuals better understand which products work best for their needs and lifestyle goals.

Could Your Child Have Head Lice?

Head lice, these tiny insects, take up residence on people’s scalps and make their way into their hair. By biting the scalp, head lice feed on human blood. The issue of head lice has persisted for thousands of years, even evident in Egyptian mummies. Remarkably, head lice affect more school-aged children than all other contagious illnesses combined, excluding the common cold.

Initially, it may not be immediately apparent that your child has head lice. It takes about 4-6 weeks for the body’s immune system to develop sensitivity to the bites. During this period, a female louse can lay 6-10 nits per day. These nits take approximately two weeks to hatch, mature, and commence laying more eggs. By the time you detect an infestation, multiple generations of lice may already be residing in your child’s hair. You might suspect your child has head lice if they exhibit the following

indications:

• Expressing discomfort from a tickling sensation or sensing movement in the hair.

• Experiencing itching due to a reaction to the lice bites.

• Demonstrating irritability.

• Developing sores on the head as a result of scratching.

Scratch marks on the scalp, particularly behind the ears or at the nape of the neck, serve as telltale signs of a head lice infestation.

How Can You Eliminate Head Lice?

It is advisable to seek treatment if even a single nit is found. There are two effective methods for eliminating lice: manually removing the nits using a comb and employing a special shampoo. It’s unfortunate that individuals diagnosed with a mental illness often feel reluctant to discuss it due to feelings of embarrassment or shame, unlike how they would approach other medical conditions. The fear of potential avoidance or job loss further discourages them from seeking help, highlighting the pervasive stigma surrounding mental illness.

However, it’s crucial to recognize another important fact: stigma and myths are rooted in fear of the un -

known and a lack of understanding or information. This can be addressed by disseminating knowledge and utilizing it to gain a better understanding and manage the situation effectively.

Thanks to advancements in technology, scientists have discovered that many mental illnesses stem from imbalances in specific brain chemicals. Therefore, it is more appropriate to refer to these conditions as brain chemistry disorders, which should be treated like physical illnesses. Examples of brain chemistry disorders include depression, bipolar disorders, anxiety and panic disorders, schizophrenia, and others.

Similar to diabetes, restoring or achieving a chemical balance is a fundamental aspect of regaining one’s health. Ongoing progress in fields such as brain imaging, molecular biology, and genetic engineering allows scientists to expand their knowledge and develop more effective treatments.

Why Aren’t Your Teeth White?

White teeth add to anyone’s smile. A bright smile with white teeth makes one more attractive and confident. Dark, yellow, stained teeth detract from even the biggest smile. The visual media know this well, movie stars, magazine models, and TV personalities all show dazzling white teeth when they smile.

So, why aren’t your teeth as white as they used to be? It’s the result of heredity, environment and time. A few people are born with “soft teeth” or

defective enamel that absorbs staining foods and breaks down easily. For others, there are environmental factors of diet and certain drugs (tetracycline) that can discolor teeth. Even fluoride in excess can cause “brown stain” or “mottled” enamel. Coffee, tea, grape juice and dark-colored colas are common teeth stain producers. Tobacco use, smoking or chewing, is also a stain producer.

Teeth have a layer of enamel over an inner layer of dentin and, over time, the enamel wears away and lets the yellow inner dentin layer show through the thinner layer of translucent enamel. This is why teeth naturally yellow with age.

How can teeth be made whiter? Years ago, it was pull them and get full dentures. Then it was put porcelain crowns (caps) on them. Recently, porcelain veneers (thin porcelain layers bonded to the front of the teeth) was a less drastic way to restore teeth to whiteness without the excessive drilling that crowns require.

Snoring is one of the most common sleep-related concerns. While it may seem harmless, chronic or loud snoring can signal deeper issues, such as obstructive sleep apnea (OSA). Snoring results when airflow is partially blocked during sleep, causing vibration of the tissues in the throat. If the airway becomes fully blocked, breathing temporarily stops — a key feature of OSA.

Obstructive sleep apnea is marked by repeated pauses in breathing throughout the night. These interruptions disrupt sleep cycles and often leave individuals feeling exhausted during the day. OSA has been linked to high blood pressure, heart disease, and other serious health concerns.

Insomnia involves difficulty falling or staying asleep. It may be acute, triggered by temporary stress or lifestyle changes, or chronic, lasting months or years. Insomnia affects both mental and physical health, often contributing to irritability, memory issues, and weakened immunity.

Sleep paralysis occurs when a person wakes up and becomes consciously aware but cannot move or speak. These episodes are usually brief but can be distressing. In some cases, they are associated with other sleep disorders, such as narcolepsy.

My Child Grinds Their Teeth At Night Should I

Be Worried?

One of the most common questions I hear from parents is:

“Dr. Shin, my child grinds their teeth at night. Are they damaging their teeth?”

I understand why parents are concerned. Teeth grinding can be surprisingly loud. Some parents tell me they can hear it from down the hall. Others worry that their child is

slowly wearing away their teeth while they sleep.

The good news is that, in most cases, teeth grinding is far more alarming to parents than it is to pediatric dentists.

After nearly 30 years of caring for children, I can tell you that teeth grinding, also known as bruxism, is extremely common. I have seen children who grind occasionally and others who grind so aggressively that their parents are convinced permanent damage is inevitable.

Fortunately, that is rarely the case.

How Common Is Childhood Teeth Grinding?

Very common.

Studies have reported that anywhere from 15% to 40% of children grind their teeth at some point during childhood, depending on how bruxism

While researchers have been studying childhood bruxism for decades, one surprising fact remains: we still do not know exactly why children grind their teeth.

Why Do Children Grind?

If you search online, you will find a long list of possible explanations.

Stress. Anxiety. Airway problems. Allergies. Sleep disorders. Bite problems. Genetics.

The truth is that none of these theories fully explains every child.

Some children who grind have enlarged tonsils, allergies, or disrupted sleep. Others have none of those issues. Some children who grind are anxious. Many are not.

Current evidence suggests that childhood bruxism is likely multifactorial, meaning that several factors may contribute, but there is no single explanation that applies to every child.

One of the most consistent findings in the literature is that grinding often occurs during brief arousals from sleep. Researchers increasingly view childhood bruxism as a sleep-related movement behavior rather than simply a dental problem.

A Clinical Observation After 30 Years

Although the exact cause remains unclear, there is one pattern I have observed repeatedly throughout my career.

Many children who grind heavily during the primary dentition years gradually reduce or stop grinding around age seven to eight.

Interestingly, this is also the period when the first permanent molars become established in the bite.

Has research proven that the eruption of the first permanent molars causes grinding to stop?

No.

At this point, that relationship remains an observation rather than an established scientific fact.

However, after seeing this pattern in thousands of children, I believe it is worth considering whether at least some childhood grinding may be related to the maturation of the developing dentition and occlusion.

Future research may help answer that question more clearly.

What About Worn Teeth?

This is often what worries parents the most.

Some children who grind develop

noticeable wear on their baby teeth. Sometimes the wear is mild. Sometimes it can appear quite dramatic, especially on the front teeth.

What I have found reassuring over the years is that significant wear in the primary dentition rarely seems to predict significant wear in the permanent dentition.

Many children who wear down their baby teeth go on to develop healthy permanent teeth with little or no evidence that they were ever grinders.

That observation is one reason I am often more conservative than parents expect when discussing treatment.

Of course, wear should never be ignored. Your pediatric dentist should continue to monitor changes over time, looking for excessive wear, fractures, sensitivity, or other signs of pathology.

But visible wear on baby teeth alone is not usually a reason to panic. Does Grinding Mean My Child Has an Airway Problem?

Not necessarily.

There is growing research exploring associations between bruxism and sleep-disordered breathing, enlarged tonsils, chronic nasal congestion, allergies, and mouth breathing.

These associations are important and deserve attention.

However, association does not mean causation.

Many children who grind have no airway concerns at all, and many children with airway issues never grind their teeth.

That said, if your child grinds and also snores loudly, breathes through their mouth, sleeps restlessly, pauses breathing during sleep, or seems unusually tired during the day, further evaluation may be appropriate.

Should My Child Wear a Night Guard?

This is another common question. Parents are often surprised when I tell them that I rarely recommend night guards for young children simply because they grind.

Unlike adults, children are constantly changing. Their jaws are growing, baby teeth are exfoliating, permanent teeth are erupting, and their bite is developing.

For that reason, we must be careful about placing appliances on growing dentitions.

The under-eye area is one of the most delicate and expressive regions of the face. Not every patient is an ideal candidate, particularly those with significant skin laxity, festoons, or chronic fluid retention. Equally important is selecting the right filler. Softer, less hydrophilic products generally integrate more naturally and reduce the risk of swelling. Precise placement, conservative volumes, and a detailed understanding of facial anatomy are essential.

Aftercare also plays a vital role. Patients should sleep with their head elevated, stay well hydrated, limit salt and alcohol intake, and follow an antiinflammatory diet rich in leafy greens, berries, omega-3s, and turmeric.

Under-eye fillers remain one of the most rewarding non-surgical rejuvenation treatments when performed by experienced, qualified practitioners. When choosing a provider, look beyond the price tag. A child’s feet grow rapidly during the first year, reaching almost half their adult foot size. This is why foot specialists consider the first year to be the most important in the development of the feet.

Here are some suggestions to help you assure that this development proceeds normally:

• Look carefully at your baby’s feet. If you notice something that does not look normal to you, seek professional care immediately.

Narcolepsy is characterized by extreme daytime sleepiness and sudden episodes of falling asleep, sometimes without warning. People with narcolepsy may also experience sleep paralysis, vivid hallucinations during sleep transitions, and muscle weakness triggered by emotions (cataplexy).

Circadian rhythm disorders occur when the body’s internal clock is misaligned with the external environment. Common examples include jet lag or shift work sleep disorder. These disruptions can make it difficult to fall asleep or wake up at desired times.

Restless leg syndrome (RLS) causes uncomfortable sensations in the legs that create an overwhelming urge to move them, especially at night. This can interfere with falling asleep and lead to fragmented sleep.

Chronic fatigue syndrome (CFS) causes persistent, unexplained tiredness that does not improve with rest. While it may be linked to other conditions like sleep apnea, CFS can also occur independently.
