Orthotics are shoe inserts that are intended to correct an abnormal, or irregular, walking pattern. Orthotics are “arch supports.” They perform functions that make standing, walking, and running more comfortable and efficient, by altering slightly the angles at which the foot strikes a walking or running surface.
Podiatrists prescribe orthotics as a conservative approach to many foot problems or as a method of control after certain types of foot surgery. Orthot-
ics are highly successful and practical.
Orthotics take various forms and are constructed of various materials. All are concerned with improving foot function and minimizing stress forces that could ultimately cause foot deformity and pain.
Foot orthotics fall into three broad categories: those that primarily attempt to change foot function, those that are primarily protective in nature, and those that combine functional control and protection.
Rigid Orthotics
The so-called rigid orthotic device, designed to control function, may be made of a firm material such as plastic or carbon fiber, and is used primarily for walking or dress shoes. Rigid orthotics are chiefly designed to control motion in two major foot joints, which lie directly below the ankle joint. These devices are long lasting, do not change shape, and are usually difficult to break.

Many women are caregivers, and do not take care of themselves as much as they take care of their loved ones. Women are nurturers, loving and caring, however women need to take care of themselves too.
A lot of women work jobs, come home and prepare meals, do laundry, help kids with homework, dote on their husbands, and are involved in activities outside of the home, such as Bible study, choir practice or other types of meetings. This routine starts
over the next day and continues for weeks, months and years.
We may start to feel uncomfortable in our bodies. Headaches, backaches, cramps, insomnia, Irritable Bowel Syndrome, and other conditions may manifest. Our skin changes, becomes dry and may develop rashes; your hair may start to fall out; wrinkles show premature aging; and we may develop other symptoms.

What Is Temporomandibular Disorder (TMD)?

So, what does it mean when a person says “I have TMJ”? Well, technically, it means that you have a temporomandibular joint. And technically, that’s all it means- you have a jaw joint. In reality, most people are actually saying “I have TMD”- which
more literally means they have TMJ disorder.
TMD refers to the displacement or actual slipping of the articular disc off of the condyle. In English, this means that the little cushioning disc on top of your jawbone has slipped off the bone and is now rubbing around in that little socket just in front of your
ear. That slipped disc can and will pinch nerves and blood vessels and this can wreak havoc with your entire body.
We Are NOT Your Typical Dental Office
The symptoms of TMD include head and neck pain, migraines, jaw clicking/popping/pain, facial tics or tremors, and various other body issues. There are many patients over the years who have presented with body tics (and this is sometimes a Tourette’s situation) and the appliances used can help mitigate their symptoms. There was actually a study done in Osaka at the university that confirmed symptoms of Tourette’s were actually reduced greatly by wearing a TMD appliance! The understanding is that the nerve in the jaw joint- most likely the auriculotemporal nerve- is being pinched and this can cause the tics of Tourette’s. By decompressing the TMJ joint disc, this can alleviate those tics and Osaka University has confirmed the efficacy of the dental appliance used for Tourette’s.

The bottom line here is that when the little disc is slipped off the condyle bone in the jaw socket, it can really hurt the body badly. Our office has become quite efficient at identifying the damaged disc through the use of a detailed MRI that looks directly at the discs and the radiologist we use is able to produce a very detailed report that can be used to diagnose the problem. From there, a treatment plan that includes wearing various devices can be developed.
You do not need to wear the device(s) forever. The good news is that after a year of treating the damaged joints, usually the patient feels so much better and we can then use orthodontic techniques to raise the teeth up to the right height to support the TM joints. It is quite a process and took years to develop, but nowadays we can use clear aligners in almost all cases and this makes the work so much easier for everyone.
It is rare, but sometimes we need to have surgery done for the discs when they are beyond repair. Years ago, this was a difficult procedure, but today it has become much easier for both the patient and the doctor. We even have a technique called ‘arthrocentesis’ in which the joints are flushed out and cleaned to remove debris and bony pieces that can cause pain. This procedure has shown good promise.
There are other techniques in treating TMD and those can be reviewed in the next article.

Bariatric surgery (weight loss surgery) provides a tool for individuals struggling with weight management. It is reserved for individuals who are 100 pounds or more above their ideal weight and for those who are suffering the health consequences of obesity. Common procedures such as the gastric bypass and sleeve gastrectomy reduce the size of the stom-
ach and change the way nutrients are digested and absorbed.
Changes In Food Portions
Portion sizes are much smaller after bariatric surgery than they are for the average person. Meal sizes may start out as ¼-½ cup and gradually increase over time to approximately one cup as a serving size. Choosing Summer Shoes Without Sacrificing Foot Health

Summer often bring a shift toward sandals, slides, flip-flops, wedges, and lighter footwear. While warm-weather shoes may feel convenient and fashionable, many provide very little support for the feet. Over time, unsupportive footwear can contribute to pain, instability, and longterm structural problems.
High heels remain one of the most common causes of foot discomfort. Elevated heels shift body weight forward onto the ball of the foot while narrow toe boxes crowd the toes together. Repeated use may contribute to bunions, hammertoes, nerve irritation, and chronic forefoot pain. Even occasional wear during weddings, parties, or long workdays can leave the feet sore and fatigued.
Over time, unsupportive footwear can contribute to pain, instability, and long-term structural problems.
This does not mean dress shoes or heels must be avoided completely. Limiting the amount of time spent in them and alternating with more supportive shoes can help reduce strain. Gel cushions placed under the ball of the foot may also help redistribute pressure during occasional wear.
Flip-flops and flimsy sandals create a different type of problem. Many provide little arch support and force the toes to grip constantly just to keep the shoe in place. Over time, this extra muscle effort can contribute to arch pain, tendon irritation, fatigue, and instability.
A supportive sandal should offer more than just cushioning. Look for styles with a structured sole, moderate arch support, and preferably a back strap for stability. If a sandal folds completely in half or feels unstable while walking, it is likely not providing enough support for daily use.
Sneakers remain one of the healthiest options for prolonged standing and walking, but not all sneakers are equally supportive. A well-designed sneaker should resist collapsing when twisted and should remain stable through the arch and heel. The back portion of the shoe, known as the heel counter, should feel firm rather than soft or flimsy.
One common misconception is that softer always means better. Extremely soft shoes may actually allow excessive movement and fail
to properly support the foot. Proper footwear should provide stability while still allowing the foot to move naturally.
Summer activities can place additional stress on the feet, especially during vacations, outdoor events, amusement parks, and long days of walking. Wearing unsupportive shoes for extended periods often leads to heel pain, arch strain, blisters, and swelling.
The best approach is balance. Fashionable footwear can still be
enjoyed occasionally, but supportive shoes should make up the majority of daily wear. Paying attention to fit, support, and structure can significantly reduce foot stress while improving comfort and stability.
Healthy footwear choices affect far more than the feet alone. Proper support can improve posture, reduce joint strain, enhance balance, and help maintain an active lifestyle. Small adjustments in shoe selection today may help prevent pain and mobility problems later in life.

Dr. Mara Holton grew up in Maryland and has practiced with AA Urology for more than a decade since completing her residency.
Dr. Holton earned her undergraduate degree from The University of Vermont before attending medical school at Temple University in Philadelphia. She then completed her general surgery internship and urologic surgery residency at University of Maryland Medical System. While in residency, Dr. Holton practiced the full breadth of urologic training and surgery. Additionally, while there, she spent a year doing research dedicated to interstitial cystitis / pelvic pain alongside leaders in the field.
In clinical practice, Dr. Holton treats all urologic conditions but has a special interest in female incontinence and pelvic floor dysfunction along with the noninvasive treatment of kidney stones. I began my journey working as a pediatric dental assistant at Ashburn Children’s Dentistry, where I spent summers learning from her. Dysmenorrhea, a prevalent issue among women globally, is characterized by periodic pain during, before, or after
menstrual periods. This pain is caused by abnormal uterine contractions due to an excess of harmful prostaglandins. For some women, these pains can be intense enough to disrupt daily activities for several days each month.
Common symptoms of dysmenorrhea include cramping, lower abdominal pain, lower back pain, and a pulling sensation in the inner thighs. Women may

experience headaches, dizziness, vomiting, nausea, diarrhea, or constipation. There are two types of dysmenorrhea: primary and secondary. Primary dysmenorrhea occurs without any physical abnormalities and typically begins within three years of the onset of menstruation. Contributing factors include allergies, poor diet, high estrogen levels, being overweight, smoking, and stress. Many experts link the uterine muscle contractions to prostaglandins, which are hormone-like substances involved in pain and inflammation. Secondary dysmenorrhea results from gynecological conditions such as endometriosis, fibroids, and pelvic inflammatory disease (PID).
In Chinese medicine, pain is seen as a symptom of an underlying issue, which can stem from either an excess pattern or a deficiency pattern. Excess patterns involve blockages of Qi and blood or the retention of heat, dampness, or wind. Deficiency patterns arise from insufficient Qi and blood or an imbalance in the kidney and liver.
Practitioners of Chinese medicine use acupuncture and herbal medicine
to treat dysmenorrhea. Their strategies include expelling wind, dissipating cold, eliminating dampness, moving Qi, invigorating blood, nourishing the blood, clearing heat, and tonifying the kidneys.
A recent review examined the effectiveness and safety of acupuncture for treating primary dysmenorrhea. The review included ten trials with data from 944 participants, with six trials focusing on acupuncture and four on acupressure. The results showed that acupuncture provided better pain relief compared to a placebo.
In Chinese medicine, ginger has long been used to alleviate dysmenorrhea due to its anti-inflammatory, spasmolytic, and circulatory properties. Ginger inhibits cyclooxygenase and lipoxygenase enzymes, which contribute to menstrual cramps. Another common Chinese medicine ingredient for treating dysmenorrhea is peach kernel. By integrating traditional Chinese medicine practices such as acupuncture and herbal remedies, women can find relief from dysmenorrhea and improve their well-being during menstrual cycles.

Ensuring a new school year gets off to a good start can influence children’s attitude, confidence, and performance both socially and academically.
This transition from August to September can be difficult for both children and parents. Even those children who are eager to return to class must adjust to the greater levels of activity, structure, and for some, pressures associated with school life.
The level and degree of adjust-
ment depends on the child, but parents can help their children manage the increased pace of life by planning ahead, being realistic in expectations, and maintaining a positive attitude. Here are just a few suggestions to help with the transition and promote a successful school experience.
Before School Starts
Plan to schedule doctor and dental checkups early. Discuss any concerns you have over your child’s emotional or psychological development with your pediatrician. Your child will benefit if a potential issue is identified and addressed before school starts. Review the material sent by the school as soon as it arrives. Make note of important dates, especially back-to-school nights. Keeping a calendar can help you efficiently juggle obligations, especially if you have children in more than one school. Re-establish the bedtime and mealtime routines (especially breakfast) at least one week before school starts.

Throughout the menstrual cycle, changing levels of estrogen and progesterone can influence sleep quality. Many women report insomnia, restless sleep, or vivid dreams in the days leading up to menstruation. These symptoms may be more pronounced in those with premenstrual syndrome (PMS) or premenstrual dysphoric disorder (PMDD), which can disrupt both mood and rest.
During pregnancy, sleep often becomes more fragmented. In the first trimester, increased progesterone levels can lead to excessive daytime sleepiness. As pregnancy progresses, physical discomfort, frequent urination, leg cramps, and vivid dreams can make sustained sleep difficult. Some women also develop sleep apnea or restless legs syndrome (RLS) during pregnancy, even if they never had these issues before.

Everyone experiences anxiety or stress during their lives. Some anxiety is actually helpful. It moves us to action. It can make us study more for an exam or intensify practice prior to an important presentation.
If you have an anxiety disorder, this normally helpful emotion can do just the opposite. It can prevent coping and disrupt lives.
Anxiety disorders are not just a bunch of nerves. They are illnesses, often related to the biological makeup
and life experiences of the person. The disorder appears to run in families. There are several types of anxiety disorders each with its own features.
Generalized anxiety disorder, panic disorder, phobias related to anxiety, and obsessive compulsive disorder are the most prevalent anxiety disorders documented by the National Institute of Mental Health. It appears with recent research that the anxiety may occur due to the increased sensitivity in a part of the brain called the locus ceruleus.
Many people misunderstand these illnesses and think that they can be overcome by sheer willpower. Wishing away the symptoms would be great but does not work. There are treatments that can help. Generalized anxiety disorder (GAD) is much more than the normal day-to-day stress we usually experience. It is chronic, exaggerated tension with no obvious reason creating such an intense response.

Sleep disturbances also interfere with the production of ghrelin, a hor-
mone that acts in areas of the brain to stimulate a feeling of hunger and promote feeding. Sleep deprivation increases ghrelin levels, basically sending out a hunger message to the body. As a result, the person starts to eat though there is no real need to! Estrogen also influences the metabolism of sugar and fat so, as levels of estrogen drop, women’s bodies burn calories more slowly and bad cholesterol increases. Lower estrogen affects body tissue, contributing to less muscle response to strength training. Estrogen is also tied to tissue repair, so injured muscles heal more slowly. The resulting lower levels of muscle also lowers the rate of metabolism. There is some good news: A 2019 UCLA study showed that weight increases from shifting hormones often level out after two years from a woman’s final menstrual period. However, those lower postmenopausal hormones mean one must eat less and work out harder just to maintain body weight. Losing weight will take considerable effort, but is possible. And adding HRT could boost control of body fat.

The typical meal should take approximately 30 minutes to eat. Chewing becomes very important so that foods are mushy and can be digested easily. Drinking while eating should be avoided because the stomach is often too small to accommodate both at the same time. Ideally, liquids should be delayed by 30 minutes. Dietary adjustments are an important part of the lifestyle changes after bariatric surgery.

The First Week
Clear your own schedule. If possible, postpone business trips, volunteer meetings, and extra projects.
Make lunches the night before school, ensuring healthy choices are made. When setting alarm clocks, leave extra time. Make sure your child has plenty of time to get up, eat breakfast, and get to school.
Review your child’s schoolbooks and send a brief note to your child’s teacher. Let the teachers know that you are interested in getting regular feedback on how and what your child is doing in school.
Overcoming Anxiety
Let your children know you care. If your child is anxious about school, send personal notes in their lunch box or book bag, this will help to reinforce the ability to cope.
Also, model optimism and confidence for your child. Do not overreact to situations and remain calm and positive.
Extracurricular Activities
Look for activities that will give quality, not quantity.
Your child will benefit most from one or two activities that are fun, reinforce social development, and teach new skills. Too much scheduled time can be stressful. Also, consider your family schedule and personal energy level.
When Problems Arise
Most children can display a variety of behaviors. It is generally wise not to over interpret those behaviors. If faced with an issue, more often than not time and a few intervention strategies will remedy the problem. Most children are resilient, and with your support and encouragement, can thrive throughout their school experience.
