The IMC Care Team–Who Are We?

We are a team of healthcare professionals who will work closely with you and your familyin overseeing your care. Depending on your care needs, you may interact with more than one professionals listed.

A Primary Coordinating Doctor (PCD) will be assigned to you based on your medical condition.

Why IMC?

IMC is designed with yourneeds in mind. You may have:

1. Seen more than one doctor from KTPH SpecialistOutpatient Clinic (SOC) / Polyclinics

2. Visited our Emergency Department or warded for the management of your medical conditions

Our dedicated team focuses on holistic management of your medical condition as well as your well-being and confidence for self-care.

This programme is currentlyby invitation only.

How Would My Care Journey Be?

A pre-visit assessment will be done by answering a questionnaire to allow your IMC care team to understand your care needs.

You will meet your care team who will:

• Assess your condition and advise on treatment plans

• Share with you tips to manage your condition better

• Share with you how to recognise worsening symptoms and recommend next actions

• Support your lifestyle adjustment, physical and mental well-being

• Link you up with resources in your community

Do share your concerns with your care team and ask any questions you may have.

Your IMC care team will follow through your journey to ensure you are confident and able to manage your condition well at home. You will be able to seek advice and support if you experience any sudden changes to your condition while at home.

IMC is located within each SOC in KTPH.
