Caring Communities: Production & Participation by People.
A collection of 38 community projects facilitated by Yishun Health's Population Health and Community Transformation (PHCT) team, which have benefited around 10,000 participants since 2011. Filled with photographs and case studies, the book showcases PHCT's asset-based approach to building community resilience through these projects, underpinned by the belief that individuals not only have the capability to take charge of their health, but contribute to keeping other residents in the community healthy.
