Welcome to the Breast Center At Smilow Cancer Hospital
As part of a nationally accredited center, a multidisciplinary team is available to provide you with exemplary personalized care. One of our experienced breast nurse navigators will be assigned to support you throughout your care with us. The nurse navigator will contact you prior to your visit to answer any questions you may have and gather further information to ensure your first visit is successful.

Prior to your appointment:
• Please ensure both MyChart questionnaires are completed (Smilow Breast Center New Patient Intake and Medical, Surgical, and Family History)
• Please complete Pre-Check-In via MyChart and ensure your health insurance and demographic information is up to date. You can also update them by calling 888-639-9253 or 203-785-3808.
• If you have concerns about health insurance or medical expenses, please call Yale Hospital Financial Assistance Office at 475-287-6380 or via e-mail at LIST_AgencyCoordinators@YNHH.ORG.
Please note: In order to provide exceptional care, pathology and imaging reviews will be performed for any outside medical records. All health insurance policies vary, please check with your insurance carrier to confirm coverage.
