With the biennial legislative session and a hugely successful WCA spring convention in the rear view mirror, it would seem logical for WCA staff and board members to catch our collective breath and enjoy a bit of Wisconsin summer.
But not so fast.
The WCA staff is busy finalizing the details for the 2026 Fall Convention at the KI Center in Green Bay this September.
We will also be launching a new website and member platform through CE 21 that will make it easier for Wisconsin chiropractic staff to access the WCA's educational programming, services, and resources.
Finally, the WCA government affairs team notched another win for chiropractors who treat Medicaid patients with the addition of extraspinal manipulation as a payable service (see page 44).
This is yet another example of the value of having a strong state association who can effectively advocate for you!

You must still renew your license. NO CE hours needed this biennium

Î Renewing both CT and CRT licenses?
If you are both a CT and a CRT, you may satisfy the CE requirement for both licenses at the same time if the course you take is approved for both credentials.
Using a Combined Course
• A 12-hour CT/CRT-approved course will fully satisfy the renewal requirements for both licenses.
Mixing Online and In-Person Hours
• You may take up to 2 hours online.
• You still need 10 hours of in-person CE from a course approved for both CT & CRT.

Whether you are practicing in downtown Milwaukee or a quiet corner of the Northwoods, the Wisconsin chiropractic landscape is shifting.
Between fluctuating reimbursement rates and the administrative "headaches" of paperwork, many of us feel like we’re working for the insurance companies rather than our patients.
Know Your Numbers: The TCL Formula
Before making any changes, you must determine your Target Collection LevelTM (TCL). This is the minimum dollar amount you need to collect per visit to remain profitable. It’s not a goal; it’s your "minimum speed limit."
To calculate it, use this simple formula:
I want to be clear: it doesn't have to be all or nothing. You can have a thriving, low-stress, high-retention practice while still being selective about the contracts you keep.
In my own practice, I maintained an 80/20 split—where the 20% of insurance I accepted (like PI or specific profitable out-of-network providers) actually made sense for my bottom line. Here is how you can begin your journey toward greater financial independence.
1. Total your overhead for a specific period.
2. Divide that by the number of patient visits in that same period.
3. This gives you your cost per visit.
4. To ensure profit, a good rule of thumb is to double that cost.
If it costs you $30 to see a patient, your TCL is $60. If an insurance contract pays you $35, you aren't just "making less"—you are barely covering the lights.
The "4 F's" of Patient Retention
As chiropractors, we value long-term relationships. To transition away from insurance, your patient retention must be rock-solid. I use the 4 F’s to create a "safety net" of loyal patients: Feedback
Use intuitive tools to show patients their progress. If they understand why they need care beyond just "feeling better," they stay.
Forecast Care
Develop all-inclusive care plans that offer affordable payment options for both covered and non-covered services.
Frictionless Payments
Use automated systems to eliminate frontdesk friction. When payments are automated, the focus stays on clinical care, not "the bill."
Frequent Contact
Use automated education and email marketing to stay top-of-mind.
Evaluating Your Contracts
Once your retention systems are in place, evaluate your contracts against your TCL. If a provider is costing you money, don't just "flip a switch." Use a phased cancellation approach.
Start with the insurance company that has the least impact on your patient base.
Be transparent with your patients.
Explain the changing landscape and offer affordable cash-pay options or programs like ChiroHealthUSA to ensure they can still receive the care they need.
By mastering your TCL and focusing on the 4 F's, you can build a practice that thrives on your terms—providing the high-quality care Wisconsin families deserve while securing your own professional future.

Natural amino acids and peptides — the fundamental building blocks of proteins that form the human body — are emerging as powerful therapeutics in modern chiropractic medicine.
Once viewed mainly as nutritional supplements, they are now central to precision regenerative treatments. Their high specificity, excellent biocompatibility, and ability to closely mimic or modulate natural biological processes make them highly effective for addressing the root causes of musculoskeletal conditions that chiropractors manage daily.
Î Peptides are short chains of amino acids (typically 2–50) linked by peptide bonds. This intermediate size offers distinct advantages over traditional small-molecule drugs and larger biologics.
They bind with high affinity to specific receptors in targeted tissues and organs, delivering faster clinical results while maintaining a strong safety profile. Advanced modifications such as D-amino acids, cyclization, and N-methylation enhance stability against enzymatic degradation and improve pharmacokinetics.
Innovative delivery methods, including intranasal nebulization, lipid nanoparticles, and improved oral formulations, are making peptides more practical for everyday chiropractic practice.
A notable success is the GLP-1 receptor agonists semaglutide and liraglutide, originally derived from natural compounds in human digestive secretions. These peptides have transformed the management of type 2 diabetes and obesity by promoting significant weight loss, better glycemic control, and cardiovascular benefits.
Their impact has fueled rapid market growth, with the global peptide therapeutics sector currently valued in tens of billions of dollars and projected to grow at compound annual growth rates of 6.8–10% or higher through the 2030s, driven by aging populations and chronic disease prevalence.
For chiropractic care, natural amino acids and peptides are especially promising in treating chronic and traumatic musculoskeletal disorders.
They support tissue repair, accelerate wound healing, promote regeneration of tendons, ligaments, and cartilage, and deliver natural anti-inflammatory effects — providing non-pharmaceutical solutions for back pain, joint degeneration, and soft tissue injuries.
Additional applications include neuroprotective peptides for nerve conditions and antimicrobial peptides effective against resistant bacteria like MRSA.
Î Advances in peptide synthesis, genomic-guided personalization, and novel delivery systems are expanding therapeutic possibilities while minimizing side effects.
Market forecasts suggest the sector could reach $160–300 billion by the mid-2030s, with subcutaneous and nasal delivery routes leading adoption.
In summary, natural amino acids and peptides represent a powerful return to biology-inspired regenerative medicine.

What better way to acknowledge a mentor, colleague or staff member than by recognizing their leadership. The WCA has four distinguished awards for outstanding service and dedication for which nominations are accepted.
Lifetime Achievement Award
Recognizes chiropractors that have made continuous contributions to the chiropractic profession throughout their careers. Because the award reflects a lifetime of involvement, it honors chiropractors with at least 25 years of professional experience.
Chiropractor of the Year
Recognizes recent contributions to the chiropractic profession on the local, state or national levels, as well as leadership in the community, church and civic functions. The nomination will highlight achievements generally accomplished over the last several years.
Nels Bakke Award
Honors chiropractors that have been in practice for less than five years.
It recognizes the contributions of younger practitioners who have equally dedicated themselves to serving the profession and their community.
Chiropractic Technician of the Year
This award is given to an individual whose dedication to chiropractic patients is reflected in the quality of their professional work as well as volunteer work in their community.

More often now, they are judged the way one might judge any other organization, by alignment, preference, and agreement. Do I like their positions? Do they reflect how I think? Do I feel represented? That way of thinking feels natural in a profession built on independence. It is also the wrong frame.
A state association is not a vendor, not a coaching group, not a philosophical community. It is not designed to mirror individual viewpoints or function as an extension of personal identity. It is an institution, and institutions exist to carry out functions that do not depend on personal agreement. At the state level, the defining function is the ongoing negotiation of the license itself.
Scope, access, reimbursement, regulatory interpretation, and enforcement are not settled within the profession. They are shaped through sustained interaction with legislators, regulators, and agencies over time. The relevant question is not who has opinions about that process. It is who is positioned to operate inside it.
The Constraints That Narrow the Field
Four constraints determine who can do this work.
Continuity is the first. Legislative and regulatory environments run on memory. Relationships with legislators, staff, and boards develop over years and often outlast the issues that first created them. Most organizations in the profession are not built to maintain that kind of presence. Leadership changes, priorities shift, and attention moves on. A state association persists. It carries those relationships forward across time, independent of any one leader.
Legitimacy is the second. When policy is shaped, the question is not who is most active in the moment. It is who is recognized as speaking for the profession. That recognition is not declared. It is conferred through repeated engagement and consistency. Vendors, coalitions, and informal groups can advocate, sometimes effectively, but they do not carry the same standing when policymakers ask who represents chiropractors in a given state. The association does, because it has occupied that role long enough to be treated as such.
Proximity is the third. Policy rarely takes form in public view. It develops in drafts, committee discussions, and conversations with staff well before a vote. To influence those moments, presence has to be consistent. Not occasional, not reactive, but continuous. State associations operate inside that proximity. They are part of the process as it unfolds, not just participants when something becomes urgent.
Incentives are the fourth. Every organization acts according to what it is built to sustain. Vendors are aligned to products and services. Coaching groups to business models. National organizations to broader agendas. None of that is inherently problematic, but it is not the same as being aligned to a single responsibility, protecting and advancing the license within a state. The association is built around that responsibility. Its survival is tied to the conditions that allow chiropractors to practice.
These constraints are structural, not philosophical. When they are taken together, the number of entities capable of meeting them narrows quickly.
The Elimination Test
Remove the association for a moment and ask what remains.
• Who maintains the long-term relationships with legislators and regulators.
• Who is recognized, without explanation, as speaking for the profession.
• Who is present in the rooms where policy is shaped before it becomes visible.
• Who operates with incentives tied to the license itself rather than to a particular business model.
There are groups that can participate in parts
of that work. There are individuals who can influence moments. But no other entity at the state level satisfies all four conditions at once. That is not a statement about effort or intention. It is a function of design.
What Opting Out Actually Means
This is the part that tends to get missed. When a chiropractor decides the association does not reflect their views and chooses to step away, it feels like a personal decision. A matter of alignment. A simple opting out. But the system they are stepping away from does not pause or fragment in response.
The conversations continue. The relationships continue. The decisions continue to be shaped by whoever is still in the room, still present, still recognized as representing the profession. In that sense, opting out does not remove the association’s role. It removes the chiropractor from having any influence on how that role is carried out.
And over time, that absence accumulates. It narrows the range of perspectives inside the very institution that is tasked with representing the profession to the outside world. It weakens the internal alignment that gives that representation weight. It makes it easier for external decision-makers to interpret the profession through a smaller, less complete lens.
None of that happens all at once. It happens gradually, and largely out of view. But it shapes the conditions under which every chiropractor practices, regardless of whether they are paying attention to it.
Agreement Is Not the Measure
None of this requires agreement with every position an association takes. Disagreement is inevitable in a profession as diverse as chiropractic, and in some cases, it is warranted. But disagreement does not change the role the association occupies. The mistake is assuming that alignment is the primary measure by which that role should be judged.
It is not. The association operates inside a system most chiropractors will never directly see, but one that defines the boundaries of their practice every day. Its function is not to reflect individual preference. It is to carry the structure that makes the practice possible.
At the state level, there is no substitute for that role. Everything else is adjacent.

Whole Food Nutrition for the Developing Microbiome

As practitioners, we often see children who seem to need support in more than one area at once.
A child with irregular digestion may also struggle with eczema or focus at school. Another may have a sensitive stomach after antibiotics or seem to catch every illness circulating at school. Over time, these patterns have encouraged a broader conversation around one of the most important foundations of pediatric health: the gut microbiome.
The developing microbiome is shaped early in life by factors including diet, stress, illness, medications, and environmental exposures. Among these, everyday nutrition may be one of the most powerful influences of all.
The foods children eat help shape the microbial communities that live within the digestive tract, communities that play an important role in digestion, immune function, gut barrier integrity, and neurodevelopment.
Î Yet many children today are missing one of the most essential ingredients for a healthy microbiome: diverse plant fiber.
Modern diets are often heavy in ultra-processed foods and low in the variety of fibers that beneficial microbes rely on for nourishment.¹
While fiber is often associated with bowel regularity, thefy do much more. They help nourish beneficial organisms in the colon, supporting the production of beneficial microbial metabolites that influence digestive and immune health from the inside out.²
Different fibers nourish different microbial communities, which is why fiber diversity matters so much.
Whole food ingredients such as oats, apples, beetroot, green banana, and chicory root each provide unique forms of prebiotic nourishment. Beta-glucans from oats, pectin-rich fruit fibers, resistant starch from green banana, and inulin from chicory root all contribute to microbial balance and digestive wellness by nourishing beneficial microbes, including Bifidobacteria and Lactobacilli, organisms commonly associated with the healthy developing gut microbiome in children.

When beneficial microbes are paired with complementary prebiotic fibers, they create what is known as a synbiotic approach, one that reflects the interconnected nature of the microbiome itself.
Rather than focusing on isolated ingredients alone, this approach centers on nourishing the entire microbial ecosystem through whole food nutrition and targeted microbial support to help support digestion, immune health, and healthy development in growing children.

WCA Advocacy Efforts Pay Off with Addition of Medicaid Extraspinal Manipulation
The WCA continued its successful efforts to expand access to chiropractic within Medicaid and increase reimbursement with the recent announcement from the Wisconsin Medicaid program that chiropractors can now use extraspinal manipulation (98943).
According to the May 2026 Forward Health Bulletin:
Addition of extraspinal adjustment code 98943. Code 98943 may be used once per date of service and follows the same prior authorization rules as these procedure codes:
Providers may bill code 98943 with codes 98940, 98941, and 98942, but only once per date of service. The addition of code 98943 does not affect coverage of existing procedure codes.
This expanded access to chiropractic care (and additional reimbursement) was the result of WCA advocacy efforts in the Wisconsin state budget last year. The WCA worked with pro chiropractic legislators to include a provision in the 20252027 Wisconsin State budget appropriating the 40% state funding for this expansion.
The state budget provision also directed the Wisconsin Medicaid Office to submit a Medicaid Plan amendment to the federal Centers for Medicare and Medicaid (CMS) to secure the 60% federal funding match. According to Wisconsin Medicaid, the announcement of the extra spinal manipulation code in the Forward Health Bulletin reflects CMS approval of this expansion.
“The expansion of services to include extra spinal manipulation and the increase in reimbursement due to Medicaid payment parity is the result of years of work by the WCA Policy Steering Committee and our chiropractic allies in the Wisconsin Legislature,” said John Murray, WCA Executive Director. “The WCA will continue to work on behalf of Wisconsin chiropractors to expand access to chiropractic care and increase reimbursement."

Medicaid Agrees to Pause Proposed
Spell of Illness Change
Through strong WCA advocacy efforts, the Wisconsin Medicaid Office has agreed to hold off on a planned policy change that would limit access to chiropractic care by changing the current 20 visit per spell of illness policy to a 20 visit per calendar year. Instead, Medicaid is looking to make this change through the administrative rules process which will allow for more transparency and stakeholder input.
“The WCA is still opposed to this change which will limit access to care for Medicaid patients, but we are committed to having meaningful dialogue with Medicaid to address the issue of over utilization by some providers,” said John Murray, WCA Executive Director.

WCA representatives met with the Wisconsin Medicaid office on 6/4/26 to review changes to the prior authorization process that will take effect on July 13, 2026. These changes do not affect the current 20 visits per spell of illness policy.
The major changes are as follows:
ForwardHealth will no longer accept prior authorization requests by fax or mail.
• Effective July 13, 2026, providers must submit them electronically through the ForwardHealth Portal.
• Providers without a Portal account can review the ForwardHealth Provider Portal Account User Guide, P-00952.
• Providers are required to use the revised Prior Authorization / Chiropractic Attachment (PA/CA), F-11029. Found here

Understanding NCCI Denials
NCCI denials occur when a medical claim is rejected because the services billed do not comply with CMS coding guidelines.
These denials are intended to prevent improper payments by ensuring that procedures are not bundled or billed incorrectly.
There are two types of NCCI edits:
1. Procedure-to-Procedure (PTP)
These are bundling edits that identify code pairs that generally should not be billed together for the same patient on the same date of service.
2. Medically Unlikely Edits (MUEs)
These are units-of-service edits that define the maximum number of units typically billable for a service for one patient on a single date of service.
If you are coding correctly, using the appropriate modifiers, and still receiving denials, you are not alone.
Many of these denials are tied to what insurers describe as NCCI edits.
However, insurers are not always applying the standard Medicare NCCI edits available for download. In many cases, they are using their own adapted version instead.
Appealing Denials
Because these denials are often inconsistent, it is important to appeal them. While the first appeal is frequently denied, continued appeals can still be worthwhile.
• Appeals create a documented record of the denial and your response.
• Ignoring the issue will not make it go away and may allow it to become a larger problem over time.
• Persistence is often necessary when challenging these denials.

Online Comment Costs DC $25,000

A single negative review can feel personal — especially when you’ve invested years building a respected practice. But as one provider learned, how you respond in that moment matters far more than the review itself.
Dr. Harris Pollard, a solo practitioner in a suburban Indianapolis community, had worked hard to establish a strong reputation. With more than 140 positive online reviews, he actively engaged with patients by thanking them publicly and using feedback to promote his services.
Like many providers, he saw online reviews as a valuable, low-cost marketing tool that helped grow his patient base.
That changed in early 2024.
A series of negative reviews began appearing online, including one that directly criticized Dr. Pollard’s clinical judgment and suggested his treatment recommendations delayed a patient’s recovery.
Frustrated and feeling his professional credibility was under attack, Dr. Pollard responded publicly. In doing so, he crossed a critical line.
In his reply, Dr. Pollard posted a screenshot from the patient’s chart and made statements questioning the patient’s honesty and motives for seeking care.
What began as an attempt to defend his reputation quickly escalated into a serious breach of professionalism and patient privacy.
The situation didn't end online.
The patient filed a complaint with the state board, citing both the disclosure of protected health information and the tone of Dr. Pollard’s response.
Rather than stepping back, Dr. Pollard continued engaging publicly, even threatening legal action against the patient for filing the complaint.
The consequences were significant:
A $25,000 fine, 25 hours of mandated continuing education in areas including HIPAA and professional standards, and a formal public reprimand on his record.
This case underscores a critical lesson for healthcare providers:
Even when a patient publicly shares their experience, providers cannot confirm the patient relationship or disclose any details of care.
Posting chart notes, referencing treatment, or even acknowledging that someone is a patient can constitute a violation of federal privacy laws.
Equally important is maintaining professionalism.
Publicly criticizing a patient, questioning their motives, or escalating conflict can quickly shift the focus away from the original complaint and onto the provider’s conduct.
So what's the better approach?
Preparation matters. Many practices benefit from creating standard response templates for both positive and negative reviews.
A simple, professional response that thanks the reviewer, reinforces a commitment to patient care, and invites offline communication can go a long way — without risking a privacy violation.
It’s also worth remembering that a few negative reviews won’t define your reputation. In fact, how you respond often speaks louder than the review itself. A calm, measured reply can reinforce trust and demonstrate professionalism to prospective patients.
In the end, this case wasn’t about clinical care — it was about judgment, restraint, and understanding the boundaries of patient privacy in a public forum.
For providers, the takeaway is clear: protect patient information at all costs and never let a moment of frustration turn into a lasting consequence."

As technology advances, the use of AI scribing is becoming more common in healthcare. Medical providers should understand not only how AI scribing works, but also the legal and regulatory considerations involved.
AI scribing typically captures audio from a patient-provider encounter and uses artificial intelligence to generate clinical documentation.
Î Under Wis. Stat. § 968.31, recording a conversation is generally lawful when at least one party to the conversation consents.
In practice, this means a provider’s consent may satisfy Wisconsin’s one-party consent rule. However, healthcare providers must also consider additional legal, regulatory, and ethical obligations when using AI scribing tools.
HIPAA Requirements
✔ Maintain appropriate administrative, technical, and physical safeguards.
✔ Have a signed Business Associate Agreement (BAA) in place when required.
✔ Ensure the secure transmission and storage of audio data.
✔ Establish clear policies and procedures for data retention and destruction.
Best Practices
✔ Educate patients about the use of AI in your clinic and how it supports documentation.
✔ Use signage in office to notify patients of recording practices.
✔ Include information about AI-assisted documentation in patient intake forms and privacy materials.
✔ When reasonable, consider accommodating patient preferences or concerns regarding recording and AI-assisted documentation.
✔ Consult legal counsel and organizational compliance policies before implementing AI scribing workflows.
