West HQ Member Survey Survey Completion Reward – Terms and Conditions

The Promoter is West HQ Limited (ABN: 54 000 842 375), 33 Railway Street, Rooty Hill NSW 2766.

Offer Period: This offer is available exclusively to members who receive this survey directly from West HQ and complete it during the outlined period. The survey will close once the required number of responses has been reached. The food and beverage credit will expire 31 December 2026, 11.59pm.

Eligibility: Open to current West HQ members aged 18 and over who receive the survey invitation. Limit of one credit per member.

How to claim: To receive the food and beverage credit, eligible members must complete the full West HQ member survey. Your membership number will be automatically captured via the survey link, no need to enter it manually. The food and beverage credit will be loaded onto your West HQ Membership card on 01 August 2026.

Reward Details: Each eligible member who completes the survey in full will receive food and beverage credit applied to their West HQ membership card, please see amounts below. The food and beverage credit can only be redeemed at the following participant outlets at West HQ: • • • • • • • • •

The food and beverage credit must be redeemed in a single transaction. Any unused portion will be forfeited and cannot be exchanged for cash, credit, or change. The food and beverage credit expires 31 December 2026, 11.59pm.Verification: West HQ may verify membership status and survey completion before applying the credit. Incomplete responses, duplicate entries, or entries that cannot be matched to a valid membership will not be eligible.
