Prepare for Fall Sports with Magnus Forms If your child plans to participate in a fall sport, now's the time to get ahead by completing the necessary health forms. Remember, athletes must have these forms filled out before they can start practicing. And mark your calendars for June 19th, when a physician will be available on campus to assist with physical exams. Click here to fill out forms.
