Upcoming Assignment of Benefit Changes
Please be advised that on 1 July there are changes regarding the following patient forms that are to be used going forward.
From 1 July 2026, the Australian Government is implementing significant reforms to the Assignment of Benefit (AoB) process. These changes modernise how Medicare benefits are assigned, strengthen compliance, and support more integrated billing across public hospitals, clinicians, and private health insurers.

Accompanying the PEF for private billing purposes:
1. Private Hospital Claim Form - SW1340 - Private Patient Hospital Claim Form (previously HC21)
2. Informed Financial consent – private patients - SW1341 – Informed Financial consent –private patients
Early adoption of these forms is encouraged prior to 1 July. All relevant staff are required to use these new forms in place of any previous versions for compliance purposes. Please ensure you familiarise yourself with the forms and incorporate them into your workflows.
Please see your administration team leaders if you are unsure how this change impacts you.
For more information on AoB, please refer to QHEPS Assignment of Benefit Changes Effective 1 July 2026 or contact tsv-tth-revenue@health.qld.gov.au
