Whatistransition?

Whenyoureachtheageof14or15,you'restartingtostepintothe shoesofayoungadult.Thisiswhenwestartplanningtogether your‘transition’:afancywayofsayinggraduallymovingfrom children’sservicestoadultservices.

OurhopeatAlexanderDevineistomakesureeveryyoungperson heregetsthesupportandcaretheyneedwhereandwhenthey needit,makingthetransitionassmoothaspossible.It'sallabout makingsureyourneedsaretakencareofasyougrowup. Whathappensduringthetransitionprocess?

Age 14/15

You will meet with our Transition Team and we will start talking to you and your family to understand your needs, wishes and hopes for future support.

Age 16/17

We start taking small steps to make sure you have the right care and support in place to transition safely and smoothly.

Age 18/19

You are ready and supported to transition successfully into adult services and get the best care for your needs.

Wewanttoensurethefirststepstotransitionoccuratthebesttimefor you and your family. This is just a guide and it will be tailored to your specificneeds.

We want to make the transition process as easy as possible. When a young person reaches age 14 or 15, you and your family will be contacted by our Transition Lead, who will:

Helpyouwithanyquestionsyoumayhave.

Provideyouandyourfamilywithsupportondifficult legaltopicsincludingconsenttotreatment,powerof attorneyanddeputyship.

Workwiththeprofessionalsalreadyinvolvedwith yourcareandthosewhichmightstarttoworkwith you,tohelpco-ordinateservices.

ReferyouandyourfamilytoourFamilySupport Team,tohelpeffectivelymanageanyanxieties relatingtotransition.
