另外一個很有印象的是AF progression。20202年荷 蘭學者在 Europace 所發表的,使用 loop recorder 在過去 有paroxysmal AF診斷的病人上,發現經過一年的觀察, 62% 的 AF episode 維持穩定,甚至 16% 會變少, 22% 會 progress 變成較頻繁的 AF ,但只有 3% 會變成 persistent AF 。 Cormobidity 較多的病人會有比較高機率 AF 會 progression,我覺得也就是這族群的病人是需要非常積 極 rhythm control ,但是那些維持穩定甚至是會 regress 的,是不是要積極電燒介入值得商榷。這也呼應了 EAST-AFNET 4 trial,其實要CHADs-VASc>4分才能得 到 MACE 的好處。除此之外, AF progression 的主題也 講到不少減重對於AF regress的效果,從動物實驗到臨 床,所以這群族群,除了 rhythm control 外,減重也是 相當重要。
這次我自己也有投稿一篇abstract,講關於Ensite X system的peak frequency algorithm。我們發現在AF的當 下, voltage 不管是 bipolar 或 omnipolar 可能對於真正的 voltage偵測不是很準確(AF的voltage與SR時的voltage會 差很多 ) ,但是假如是 peak frequency , AF 時的 peak frequency 跟 SR 差別就不大,這個有助於用 PF 就可以 在 AF 的時候準確地找到 PV gap 或甚至是有問題的 substrate。
導管直接增添了 PFA 的功能,這意味著能同時擁有現 有的 contact force 偵測功能,透過電腦計算,執行 PFA 也能產生 Ablation Index ,成為引導電燒時間的指標, 這是一個創新且實用的設計。未來,將 PFA 結合 contact force 偵測與專用的 3D 系統,預計將成為業界 趨勢,有望解決現有設備對於 X 光透視的依賴問題。
