IN SEARCH OF A SOLUTION First of all, thank you for an amazing magazine! We enjoy it every week. I am wondering whether dealing with pinworms was ever discussed in an article or whether one of your readers can help. I’ve been battling this issue for a while now as several of my children are very prone to the condition. We’ve tried prescription and over-the-counter medications, natural supplements, garlic and what-not. It never treated the root of the problem, and the issue keeps recurring. I have been told that sugar and junk food are big no-no’s, but there’s only so much of what goes into a child’s mouth that one can control. If there’s anyone out there who has had a successful solution they can share, I’d be really grateful. Thanks in advance! A Monsey Resident

VOICE A BABYSITTER’S TAKE Did your boss ever tell you, “Starting tomorrow, don’t come in for a week or two, and I’ll deduct it from your paycheck”? For me, a babysitter, a similar scenario keeps happening time and again. Let me explain: Teachers get paid vacation on Chanukah, midwinter vacation and snow days. Office employees get sick days off, legal holidays off, and some even get paid vacation. I, as a babysitter, however, never get a day off, even if one of my clients, who may be an office employee or teacher, has a day off. But what does happen when a working mother has paid leave is that the babysitter babysits less babies, without notice, and earns a lot less per day. The same thing happens during summer months or other quiet seasons, when mothers keep their babies home, thus resulting in our much smaller paycheck. Some mothers who work from home, either temporarily or around the year, tend to take off more often, as do women who work for family. These mothers also leave me with a lighter wallet. And in addition to the above examples, there are also the times when a baby is sick, or when a working mother is on maternity leave, and so the list goes on. Then bein hazmanim comes around, and the kollel fathers drop off their babies for a scant hour or two so they can go daven or learn. Bear in mind that this occurs twice or three times each year, for three or four weeks each time. And then there are families with parents or in-laws living overseas, and for a week or two before and/or after yom tov, the babysitter again stays with a smaller paycheck. It is also important to note that babysitters usually can’t take off even when a mother does take off, because of the remaining mothers who are working. THE CHOFETZ CHAIM ONCE went to a doctor for a medical exam. The doctor asked the Chofetz Chaim how he, being a physician and not a talmid chacham, could earn a share in Olam Haba. The Chofetz Chaim replied as follows. Hashem placed Adam Harishon in Gan Eden, about which it is written, “…v’Eitz Hachaim b’soch hagan… ­— … and the Tree of Life was in the midst of the garden…” (Bereishis 1:9). Onkelos states that the Eitz Chaim was in the center of the garden, not closer to one side or another. The Chofetz Chaim explained that it was placed there so that it could be approached from any side; and from this we see that there is more than one way to approach the Eitz Chaim. The Chofetz Chaim went on to explain that the Gemara (Taanis 21b) recounts the righteousness of Abba, who was a doctor in the practice of bloodletting. In fact, the Yeshivah Shel Maalah would send Abba a greeting each day. What made Abba so righteous? He would have separate hours for men and for women; he had special tznius’dige gowns for the women; payments were put into a box in a private location to preserve the dignity of those who could not afford to pay; and he would bestow gifts upon talmidei chachamim. This was Abba’s path. Likewise, on another occasion, the Chofetz Chaim pointed out that in the Gemara (Taanis 31a) it says that in Olam Haba Hashem will form a circle of tzaddikim. Hashem will be at the center, and the tzaddikim will point to Hashem and declare, “This is our G-d!” The Chofetz Chaim adds that they will form a circle because then it is possible to approach Hashem from every side. This, too, shows that there is more than one correct path in life. How do we find our path? Parshas Bereishis, and consequently the entire Torah, opens with a clue. Dovid Hamelech declares, “Rosh d’varcha emes… — The begin-

ning of your word is truth…” (Tehillim 119:160). The Ba’al Haturim points out that the final letters of the words “bereishis bara Elokim” spell out emes. R’ Simcha Bunim of Peshischa adds that the author of a sefer hints to his name in the title of their seforim. By beginning the Torah with a hint to the word emes, this alludes to the fact that Hashem’s Essence is emes. Indeed, Chazal say that the seal of Hashem is emes (Shabbos 55a). Furthermore, the letters aleph, mem and saf, which spell out the word emes, are the first, middle, and last letters in the alephbeis. We now have a significant signpost pointing the way for us on our path: We must cling to emes and live by emes. This is where the challenge develops. So many people claim to have the truth. How can we know? Rambam, in Yesodei HaTorah 8, explains that the ultimate authority is the Torah we received at Har Sinai, which was transmitted from Hashem to Moshe Rabbeinu. It is detailed in Pirkei Avos how this transmission continued: “Moshe received the Torah from Sinai and gave it over to Yehoshua. Yehoshua gave it over to the Zekeinim, the Zekeinim to the Nevi’im, and the Nevi’im gave it over to the Anshei Knesses Hagedolah.” We have a direct chain of transmission of the Torah from Har Sinai through the gedolim, but we also know that the Torah has seventy faces. Like the many paths leading to the Etz Hachaim in the center, how do we know which way to go? The Sefer Hamitzvos tells us in Mitzvah 6 that we fulfill our obligation to cleave to Hashem by striving to be close to talmidei chachamim. This mitzvah is incumbent upon both men and women. Each and every one of us is obligated to follow a rav or rebbe. In that way, we can be zoche to finding a true path. May we accept our mesorah the way it is supposed to be received and live the way Hashem wants us to live, and by doing so, be zoche to Moshiach tzidkeinu bimheirah b’yameinu.

there is anyone in the Monsey community who has seen firsthand the value of giving kids an opportunity to develop their talents as a vehicle for improving their academic productivity, it is Rabbi Zev Freundlich, dean of Yeshiva Shaarei Arazim. With a track record that extends nearly 20 years, Shaarei Arazim has made a name for itself as a place that brings out the best in every talmid and helps each one maximize his potential in the beis midrash by providing opportunities to hone their innate abilities. This is accomplished through numerous extracurricular activities, including the yeshivah’s renowned marching band and acrobatic troupes. Seeing the academic success that his Shaarei Arazim boys had achieved during their high school careers and beyond sparked another question in Rabbi Freundlich’s mind: Why were there no afterschool activities for elementary school–aged boys? Buoyed by his positive attitude and never-ending enthusiasm, Rabbi Freundlich launched an after-school program at Shaarei Arazim that drew

8- to 12-year-old boys from every Monsey yeshivah and cheder and has grown steadily over the past three years. By last spring, the 10-week program saw an enrollment of 450 boys, which included 13- and 14-year-olds who participated in a special Motzei Shabbos program to accommodate their weeknight night seder schedules. Participants enjoyed a wide variety of classes, including music, gymnastics, basketball, woodworking, drama, choir, keyboarding, string and wind instruments, and simcha shtick, all taught by experienced professionals. A gala year-end performance that drew an audience of 700 featured drama, choir, acrobatics and the first-ever frum children’s 16-piece marching band, with every member smartly dressed in naval officer uniforms. “To see the excitement on the children’s faces during the performance as they each had their turn in the spotlight was truly a magical experience,” remarked Rabbi Freundlich. “The parents were wowed when they saw what their kids were able to do, and every child walked away proud of what they had done and inspired to keep on reaching for higher heights.” While Rabbi Freundlich continued to receive positive feedback from happy parents who

saw their children thriving both in and out of the classroom, there was one question he kept hearing over and over again. “People would say ‘What about the girls?’” Rabbi Freundlich told The Monsey View. “There were some after-school programs for girls, but they were basically limited to music, dancing and acting, and most were very expensive. Parents wanted an affordable program with high-quality classes and plenty of options that would let them discover and develop their talents.” Stepping in once again to fill the need, Shaarei Arazim’s after-school program expanded last spring to include Shaarei Shoshanim Talent Center, a sister program held in a separate location for 8-to 12-yearold girls. One hundred and fifty girls took part, enjoying a wide variety of classes that included simcha hairstyling, keyboarding, basketball and cake decorating, just to name a few. “The results we have seen have been fantastic,” said Rabbi Freundlich. “We have boys and girls who are already doing well academically, and having an opportunity to take part in a program like this has made them do even better in school. We have solid, happy children coming to us, and they leave here every week even more solid and even more happy.” Ensuring a wholesome environment with a strong emphasis on ruchniyus is priority numberone, and the program has a strict no-electronics policy and full adult supervision at all times. And while an after-school program often has a more relaxed atmosphere than a classroom, participants are still expected to take the sessions seriously. “We take attendance every week, and parents are advised immediately if there are any behavior issues,” said Rabbi Freundlich. “Kids who are taking classes that require practice like juggling and simcha shtick are asked every week if they are practicing, because there is no point in them continuing if they don’t. And because they know they are being monitored, partici-

pants give 110 percent, with the results speaking for themselves.” The upcoming fall semester starts on October 14 and continues through January 20. Prices average $20 per class, with discounts available for those who enroll in multiple classes. Rabbi Freundlich is looking forward to once again seeing students from practically every mosed in Monsey, both boys and girls. Classes for boys will be held at Yeshiva Shaarei Arazim at 201 Route 306 on Sunday afternoons from 4:00 to 6:00 p.m., Monday through Thursday nights from 7:00 to 8:00 p.m., and on Motzei Shabbos for night seder boys. Among the 20 classes being offered this semester are several new courses, including junior contracting, magic tricks and balloon sculpting, chess, and simcha band/orchestra. The program for girls will take place at Bais Shifra Miriam, 70 Highview Road, on Sundays from 1:30 to 2:30 p.m. and from 2:40 to 3:40 p.m. This year’s new classes include jewelry making, voice/recording studio, simcha dancing and calligraphy, but additional choices are often opened at parents’ request, providing the minimum number of students can be met. Several Lakewood schools will also be offering Shaarei Arazim after-school programs this fall, and Rabbi Freundlich looks forward to the project’s continued growth. Whether it is in Monsey or Lakewood, for boys or for girls, the concept remains the same. “It’s all about letting our children shine,” said Rabbi Freundlich. “Each one is good at something, and when you can tap into that, and you can give them the chance to feel at the top of the world, the difference in that child is unbelievable. Every boy, every girl, is special. You know that and I know that, but by giving a kid an opportunity to excel, it gives them the chance to see it as well, and that sense of self-assurance overflows into every aspect of their lives.”

Cashless Tolling Fines at Governor Mario M. Cuomo Bridge Almost Equal Toll by Mail Revenues Cashless tolling on the Governor Mario M. Cuomo Bridge, advertised as a measure that would be a boon to motorists, has actually turned out to be a cash cow for the New York State Thruway, according to a report released by The Journal News. The replacement of live toll booth operators in April 2016 with the cashless system should have had drivers without E-ZPass tags paying for their rides across the bridge via a Tolls by Mail program. But the system was plagued by glitches, leaving some facing hefty fines and even suspensions of their licenses for unpaid tolls, with multiple motorists saying they never received any bills or notices of overdue fines. Elected officials and media outlets called the Thruway Authority to task for the system’s failures, resulting in reduced fines and waived penalties this past spring, but even with

those measures in place, cashless tolling fines are nearly equal to the Thruway’s Tolls by Mail collections. Between August 2016 and April 2018, the Thruway Authority collected $22 million in fees and fines, just over 7 percent of the revenue from the Hudson River crossing. During the same time period, the Tolls by Mail system brought in $25 million in revenue, just over 8 percent of the bridge’s total take. E-ZPass fines and fees brought in another $28 million over the same time period. Senator David Carlucci blasted the Thruway Authority over the bridge’s tolling woes. “It’s absolutely wrong,” he said. “The system shouldn’t make more money from fines than it does on tolls.” Carlucci has been advocating for legal measures to protect drivers, cosponsoring the Toll Payer Protection

Act with Assemblyman Tom Abinanti of Greenburgh. That bill faced minimal opposition in the legislature, but has yet to be signed by the governor. A Thruway official said that the statistics collected by The Journal News are misleading, noting that the agency’s accounting system portray actual tolls collected in a single month, while data on fines can include a backlog of penalties that can span several months. Similar woes have been reported in Buffalo at the Grand Island Bridge. Senator Chris Jacobs called for both an amnesty program and legislation to protect motorists after drivers have been hit with fines that have been doubled multiple times for violations at the crossing, which has just a $1 toll. Jacobs called the fines “just another unfair Albany money grab.”

Goodbye Bear Mountain, Hello Purple Heart Veterans Memorial Still getting heat over his official renaming of the Tappan Zee Bridge as the Governor Mario M. Cuomo Bridge, Governor Andrew Cuomo has made another name change in our area, giving the Bear Mountain Bridge a new name with the stroke of his gubernatorial pen. The bridge was renamed the Purple Heart Veterans Memorial Bridge as of this past Tuesday. This name, which honors veterans who were wounded in combat, was originally intended to go on the new Tappan Zee Bridge, a measure that had been passed by the State Senate in May 2017. A month later, Governor Cuomo led an eleventh-hour

push to get his father’s name on the new twin spans connecting Rockland and Westchester counties, reported The Journal News. The name change at the Bear Mountain Bridge was sponsored by Orange County Senator Bill Larkin, himself an army veteran, and Assemblywoman Sandra Galef of Ossining. “As motorists and pedestrians cross the Hudson River over this bridge, they will be remind-

ed of the sacrifices that have been made by our military,” said Galef.

Raizel suffered from debilitating migraines that developed in her sinuses and pretty soon ballooned into a cloud of throbbing that took over her head, her vision, her life. All she could do when a migraine decided to visit was lock herself in her room, blinds drawn, and just pray for the time to pass. It was agony. She was completely incapacitated. Life went on, with her best years wedged between sharp pain and a dark cloud. The over-the-counter medicine she was dependent on wasn’t even helping, and she knew, she just knew, there had to be another way to live. Our bodies are always the most physical expression of who we are. Think about facial features, which arrange themselves according to what we feel. Our posture, which sags or soars, runs or drags, based on how we feel. So too our internal organs work along with our emotional state. Thyroid issues very often indicate an inability to speak out, or be heard. The same goes for other issues that lodge in the throat. Weight issues are often a storage or protection for things we want to hold onto. Our bodies are a wonderful manifestation of the soul. For Raizel, recovery happened when she finally allowed herself to talk. I referred her to a competent therapist. We applied the HK techniques to release some of her early traumas. I created this recipe for those of you out there who agree with me that creamy, cheesy pasta is really good, but not quite worth a sink full of dirty pots. It is hard to believe a dish this rich and creamy can come out of just one disposable pan. Enjoy your empty sink!

INGREDIENTS 10 oz. frozen broccoli

½ tsp. onion powder

8 oz. ziti or pasta of any other shape

½ tsp. garlic powder

2 cups boiling water 2½ cups milk, divided 1½ tsp. salt

¼ tsp. black pepper 2 T. flour 10 oz. shredded mozzarella cheese, divided

DIRECTIONS 1. Place broccoli and pasta in parchment-lined disposable 9x13 pan. Pour boiling water and 2 cups milk into the pan, and sprinkle with salt and spices. Cover and bake at 350° for 45 minutes. 2. Dissolve flour in remaining ½ cup milk. Pour into pan, along with 6 ounces of cheese, and mix well to distribute evenly. Cover and bake for 30 minutes. 3. Sprinkle the remaining 4 ounces of cheese on top, and bake, uncovered, for about 20 minutes, until cheese is melted and lightly browned. Allow to sit for several minutes before serving. Optional: Omit the broccoli if you wish. You can also try sauteing other vegetables — like onions, peppers or mushrooms — in butter or oil for several minutes in a pan in the oven before proceeding with the above directions.

TO THE BEST OF MY KNOWLEDGE, NO prospective mother was ever offered an application form to fill out before her child was born. Can you imagine a questionnaire asking: What gender child do you prefer? Color of hair? Eyes? Height and build? As for myself, I didn’t ask to be tall and skinny. And certainly, my mother, a”h, did not submit such a request. Which Yiddishe mother looks forward to being told, year after year, that her daughter is x number of pounds underweight for her age, and even more severely underweight for her height, because she’s considerably taller than her peers? While my mother may have cringed at the pediatrician’s annual reports, I was quite proud of being one of the tallest — if not the tallest — girls in the class. It was fun to be above it all. I enjoyed being different and distinguished. What was more, I had access to a lot of things that others had to climb onto a chair to reach. All in all, as a child, and later, as a teen, I enjoyed my status as high up on the ladder. All that changed when I left the U.S. to attend a seminary abroad. Suddenly, everyone around me was gaping at my extra inches. (Looking back, I realize that the average height in Europe must have been significantly less than in America, perhaps because

relief when she first caught sight of me. My height came into good service there in sem, especially when there was a burned-out lightbulb to be screwed into place. “Where’s Chayah?” the cry would go out. There were prickly quips here and there about “the atmosphere up there,” but I managed to brush them off without any major traumas. One Purim, I donned an oversize carton and presented myself as the Empire State Building, pairing off with another tallish girl from France, who masqueraded as the Eiffel Tower. Before I first set out for my first stint at student teaching, the menahel called me to his office and cautioned me not to stand too close to the children’s desks when I was teaching. My brow furrowed. What difference did it make where I stood? My mentor, who was particularly short, sensed that I was perplexed and was quick to explain. “When I started first grade,” he recalled with a smile, “we had a very tall rebbi. He stood right next to the front row of desks, where I was seated, because I was one of the shortest in the class. “When I came home, what did I tell my mother about my first day of school? That my neck hurt from having to look straight up all the time to see the teacher!” Thus I learned something new about being a tall teacher. Baruch Hashem, I soon got married — to a man just a tad shorter than myself. Now my height became a real boon. It was easier to get things off the top shelves, both at home and at the supermarket.


When it came to attending school plays and the like, I didn’t have to worry about my view being blocked by a mother who towered over me. But inevitably, I began to feel the pressure when my own daughters, all of them thin and tall, neared the age of shidduchim. In fact, when the neighbor’s daughter, a petite 5’4” or so, became engaged to a near six-footer, I not only wished her mazel tov, but also chided her about “not having left the tall ones for the girls who really need them!” Then a wonderful proposal came our way for one of our daughters. The young man sounded like just the right combination of middos and lomdus. “How tall is he?” I asked, holding my breath as I awaited the answer. There was a long pause, and then a surprising reply: “I don’t really know. Every time I see him, he’s bent over the shtender. But I’ll try to find out and get back to you,” the caller assured me. And he did. As it turned out, the boy was indeed a little bit shorter than our daughter, so I asked her whether that would disturb her. Her answer still makes me smile: “That’s what the hat’s for!” she quipped. We also asked our rav just how important it was to find davka a tall chasan for each of our girls. His answer made sense, and I’ll pass it on: “It doesn’t matter if the chasan is a bit shorter than the kallah, but if the difference between them is so great that people will turn around in the street to have a second look, I would weigh it carefully before going into it.” Baruch Hashem, they were a perfect match. As the mechutan put it, “Hand in glove!” Eventually, each of our daughters found her bashert. Ironically, three of the four are slightly taller than their husbands, but not noticeably so. It’s only our tallest — to whom I’ve had to look up since she was fourteen and a half — whose husband gets to look down to her. As happens so many times, I needn’t have worried about shidduchim. Hashem had it all worked out long before I even began to think about it. And our grandchildren? A number of our grandsons have reached such heights that I ask them to sit down so I can pinch their cheeks. So far, one granddaughter is taller than both her bubbies, and her son, who is only a year and a half old right now — is obviously on his way to great heights. But I no longer worry. Hashem has it all worked out, well in advance. Of course, we know that spiritually, there is yeridas hadoros, an inevitable decline in ruchnius, since each successive generation is that much farther away in time from Ma’amad Har Sinai. But so far as gashmiyus goes, our family is experiencing an “aliyos hadoros” – with each new generation rising to greater heights. On the average, our adult children are taller than their parents and grandparents were at their age. This, too, is from the Ribono Shel Olam, and, as such, is only for the good.

BEING SHORT HAS ITS ADVANTAGES (and I’ll let you know what they are when I think of them), but mostly it makes me feel invisible and somewhat inadequate. I go to the pizza shop and tell my children to sit at the table while I order the pizza. I am still waiting in line twenty minutes later. The pizza guy can’t see me over the counter so he keeps skipping my turn. He doesn’t hear me mumbling, “Excuse me” either, until finally, the kind customer behind me says, “I think she was in front of me.” “Who?” the pizza man asks. He gazes downward. “Oh, sorry.” The butcher shop, fish store and bakery present similar challenges. I’m often left peering under the glass display cases or standing on tippy-toes, clearing my throat loudly and trying to get the attention of the woman behind the counter. Perhaps these scenarios explain why I’ve always harbored a suspicion that my lack of self-confidence is connected to my short stature. One summer I visit a friend in Eretz Yisroel — a tall friend, along with her tall mother and tall sister. We have the most fabulous evening, laughing non-stop. At the end of the night we decide to snap a selfie. Looking at the picture afterward, all we can make out are three heads and an empty space in be-

tween, where I am standing. Being short makes me feel different. Yes, I’ve heard all the platitudes, like “Good things come in small packages” and “Big things come in small packages” and “Hashem only lets things grow until they’re perfect. Some of us didn’t take as long as others.” Nevertheless, it’s deflating when at a store, a saleswoman hands me sweaters to try on, and a few minutes later, while I’m in the dressing room, I hear her call out, “Where did that little girl go?” My stepstool is an integral part of my kitchen; without it, I can be seen jumping up and angling a long-handled utensil to coax down my cocoa from the second shelf. When clothes shopping with my daughter, the outfit she must have is inevitably on the rack closest to the ceiling. With no salesperson in sight, I step on the lowest shelf, hoping that no one will notice, while my daughter whispers frantically, “Ma, they’re gonna see you! Forget it, I don’t like it anyway.” Too late, I crook my arm and jump, attempting to knock the dress down. Got it! And that’s when a saleswoman hurries over. “We don’t allow people to stand on the shelves,” she says reprovingly, as I stammer, blushing, that I was just trying to get the outfit down. “Next time, please ask a salesperson,” she says stiffly as she stalks off, shaking her head. Tall people, I find, don’t understand the miseries of short people. They’re too busy feeling they have to hunch to fit in with the averagesize individual. But what is the air like up there? I often wonder. For some inexplicable reason, most of my friends are tall. I don’t know if I’m attracted to them because they compensate for some missing psychological thing, or if I happen to like them and they just happen to be tall. I once had a walking partner with whom I exercised regularly. Then she told me she couldn’t continue any-

more. I’ve always suspected that she was too self-conscious walking beside someone who was ten inches shorter than her. Let’s face it: It looks funny. Dancing in the middle of a circle at my children’s weddings with a tall partner is a nightmare as all eyes, I imagine, focus on the odd-looking couple in the center. height. Then I began experimenting with my own self-deprecating humor. Unsurprisingly, most of my children are short. But they wear their shortness differently. They can assert themselves when standing behind a high glass counter at the sushi bar (look, if you grow up in a world of sushi, it’s got to boost your self-image), and I too have grown through my height challenge. Nowadays, I take practical steps to avoid discomfit. For example, I make sure to position myself and stand on a step if I’m having a conversation with 5’10” Sarah in front of my house. Talking to someone’s stomach isn’t much fun, and craning my head for longer than five minutes gives me an awful crick in the neck. But most of all, I’ve realized that in the scheme of things, the challenges of being short is small potatoes.

Back in the days when my husband was a head counselor in a camp, I received a nightly knock on my bungalow door. On the other side stood a young man whoâ&#x20AC;&#x2122;d quickly slip inside while I went out to shmooze. Minutes later heâ&#x20AC;&#x2122;d emerge, thank me, and return to his bunk, leaving behind his refrigerated medication: an injection of recombinant human growth hormone.

Why is it that unlike other natural characteristics such as hair or eye color, height is a frequent bump in one’s relationship with self?

For a person even slightly below average height, the possibility of low self-esteem rises, especially in the case of males. The answer seems to be that on a very basic level, height and respect are intrinsically woven together. Even Yiddishkeit recognizes that height conveys power and commands respect, despite the absolute belief that middos and yiras Shamayim are more important than any physical feature. In fact, throughout Tanach there are multiple mentions of height and the power associated with it. There were the “men of stature” feared by the meraglim, whom Rashi calls “gedolim v’giborim”; Dovid Hamelech defeated the towering Golias who drove fear into the hearts of Yidden; and Og Melech Habashan, the last of the Rephaim, was so large and fearful that many places in the Gemara speak of his gigantic proportions. On a more positive note, the Kohen Gadol is described as “hakohen hagadol me’achav,” of which the Midrash Tanchuma says to mean that the Kohen Gadol must be greater than other kohanim in five areas in order for the masses to have natural respect for him, one of which is physical build. In additional to that, Chazal state that upon being anointed, the Kohen Gadol would actually grow even taller than his natural height! Perhaps it is that instinctual association between respect and height that drives some young people (or their parents) to seek medicinal methods to increase their inches. Whatever the cause, height is a hot enough topic that an entire field of study is devoted to it...

WHAT MAKES HUMANS GROW? A glimpse at height trends around the world is rather fascinating. Over in Indonesia, for example, the average female height is only 4’10”, while in Sudan, the Dinka women average 5’11” and the men come in at a whopping 6’3”. What is it that determines how tall a person will grow, and what makes various areas of the world prone to differing averages? While genetics certainly play a role, with nearly 700 known gene variants that determine potential growth, environment and “nurture” impact growth as well. Take for example, the studies of anthropologist Barry Bogin who followed the Mayan tribe of Guatemala over many years, starting in the 1970s. At that time, he noted that the Mayan men averaged only 5’2” and the women only 4’2”, but that changed after the Guatemalan Civil War caused many of the Mayans to flee. By the year 2000, Mayan refugees in America were, on av-

erage, 4 inches taller than their counterparts back in Guatemala. Better nutrition and health care were clearly dominant factors in Mayan children’s reaching their full potential. This was also notably seen in a 2011 study on 18-year-old North Korean males who defected to China. Compared to their counterparts in better-off South Korea, North Korean males who’d suffered from a severe lack of basic foodstuffs in the harsh environment of their homeland were a notable 5 inches shorter, on average. Both North and South Koreans over 40, however, had comparable heights, as the economic conditions of their youths had been similar. To understand this better, think of growth as a map. On it is point A and point B, and a clear path that one may travel upon to reach their desired destination. This is the genetic structure that contains a person’s potential height, given the right circumstances. But as on any trip, the end-point can only be reached if there is enough fuel to get there. With growth, that fuel is food. General malnutrition is one of the leading causes of growth retardation in children, but the specific type of diet makes a difference, as well. It is interesting to note that in countries considered equally poor, those whose diets consist more heavily of proteins have greater average heights than those whose diets consist of mostly cereals. Because Mama was right (as she always is): You gotta eat your chicken to grow strong! There is yet a third factor that is vital to human growth: hormones.

HORMONES AND GROWTH HORMONE TREATMENT

Those concerns pale in comparison to what we do yet not know.

The main hormones that impact growth are thyroid-stimulating hormone (TSH), gender-specific hormones, and an uber-important hormone produced by the pituitary gland: somatotropin. If any of these hormones are not secreted by the body in the correct amounts, then abnormal growth may be noted. Somatotropin (also known as growth hormone or GH), especially, has an impact on childhood growth, with abnormal levels often resulting in either growth retardation or gigantism. Injections of a synthetic form of the somatotropin, generically called somatropin, can help children with conditions that are caused by low/no production or poor usage of their own natural growth hormones to reach average height. For these children, this medication may give them a quality of life they wouldn’t otherwise have. But are there risks?

THE HISTORY AND FUTURE OF GH TREATMENT In the late 1950s, Dr. Maurice Raben successfully extracted GH from a cadaver and used it to treat a GH-deficient child, causing a storm of excitement in the medical world. Finally doctors were able to treat at least some of the causes of short stature! There was difficulty in obtaining the hormone, however, because it had to be sourced from the pituitary glands of the deceased. This led to a limited supply for many years, in which only the most severely affected children were treated. In the end, production of GH from humans was halted when

TOO MUCH SOMATOTROPIN All hormones have a delicate balance that must be maintained in order for the body to function properly. When there’s too much somatotropin being secreted — which is often, but not always, caused by a tumor in the pituitary gland — children grow at an abnormal rate. This condition is called gigantism, and carries in its wake many health disorders. With gigantism, the body must work harder to function, and lifespan is generally shorter than average. When a child is diagnosed with gigantism, the goal is to stop the body from over-producing growth hormones. Current treatments include surgery and medications. Robert Pershing Wadlow was the tallest man to have lived, measuring an incredible 8’11” at the time of his death, at age 22. His body had never stopped growing due to high secretion of somatotropin. In adults, a condition called acromegaly can develop, caused by an oversecretion of growth hormones. Symptoms include growth of hands and feet, broadening of the nose and thickening of the brow. Changes happen gradually, over time. It is important to seek medical counsel if symptoms are noted.

it was discovered that some of the extracted hormones infected its users with a disease that causes rapid brain degeneration. Twenty-six people died from the contaminated hGH (GH extracted from humans). Within a short time, pharmaceutical companies learned how to synthetically manufacture somatotropin, resulting in injectable hormones that carried low risk of contamination. This is called “recombinant human growth hormone,” or rhGH, and is what is used today. Though risk of contamination is currently of little fear, there is the possibility of common side effects such as rash, headache, swelling, etc., but these concerns pale in comparison to what we do yet not know. “We found large gaps in our knowledge of growth-hormone treatment,” reported Adda Grimberg, a pediatric endocrinologist at the Children’s Hospital of Philadelphia (CHOP). “We know little about the long-term risks of nightly hormone injections given for years.”

THE BOTTOM LINE

THE ZZZ’S THAT MAKE YOU GROW Proper sleep is important for childhood growth, as the human growth hormone, somatotropin, is secreted in bursts throughout the day, most notably at night. Experts say that an assumed 75 percent of the growth hormone is released during the early parts of the sleep cycle, several times a night. Skimping on sleep means cutting down on those growth-boosting hormones.

For the average child receiving rhGH, the process spans nearly their entire childhood, causing real concern as to what else the drug is doing in the body, beyond what is hoped for. Dr. Chris Feudtner, Director of the Department of Medical Ethics at CHOP, stated that “administering growth hormone treatment may help very short children gain a few inches in height, but it also exposes them to a powerful hormone when we do not fully know the long-term implications.” To complicate matters, some parents push for rhGH treatments when their children are diagnosed with idiopathic short stature (ISS), which translates as an unknown cause of shortness. “Growth hormone treatment for patients with growth hormone deficiency offers health benefits beyond height, but growth hormone treatment for ISS is solely about height,” said Adda Grimberg. “Another important difference is that, unlike patients with growth hormone deficiency, not all patients with ISS increase their height in response to growth hormone treatment.” Worse yet are those who push for these drugs when their child has familial short stature — shortness due simply to genetics, or for those who fall under the threshold of ISS diagnosis. With boys, this has been documented to occur at three times the rate of girls. And sadly, there are doctors who succumb to pressure and prescribe these powerful hormones for conditions not known to even be effectively treated with rhGH, leaving the dispensation of these medications rather controversial indeed. Perhaps, to relieve the pressure to inject those without a medical need, we as a society should just stop looking down on short people. ...You know what I mean!

Though children with certain conditions may benefit drastically from growthhormone therapy, caution must be exercised when considering if it is an option for your child. As of today, there are still many unknowns surrounding these drugs. If this is something you are considering for your child, research further, talk to others who have gone through it, and of course, find an expert endocrinologist before taking the leap. Expect your pediatrician and/or endocrinologist to do an extensive work-up, which typically includes blood work tested over a span of hours and possible X-rays or other tests to determine if delayed or stunted growth is actually due to a deficiency in growth hormone. The physician’s decision to treat with somatropin will depend on his or her training and previous experiences. Ask questions. Discuss with the doctor the reasons behind his or her decision to treat or to not treat. This is your child; arm yourself with as much knowledge as possible before the final decision is made. And finally, bear in mind that there is one therapy that comes with no ill side effect: tefillah. Let us, as parents, learn from Tefillas Chana to daven for children who are “just” average. Because average in build is certainly a blessing. Note: Though not mentioned in the article, growth can also be severely affected by various non-hormonal health conditions and as side effects of various medications.

Flexible ladders, usually rope ladders (also known as Jacob’s ladders), are easier to transport, store and carry, but they are way harder to climb, as they flex as weight is added. They are great for emergencies, though, especially on helicopters and boats, as well as for curved surfaces, like in mountain climbing.

Rigid ladders are more secure and easier to use, but transporting them poses a problem. They are available in many forms, from pull-down attic ladders, to a firefighter’s hook ladder, to roof ladders; in fact, there are 21 different types of rigid ladders!

Rigid ladders were originally made of wood, but upon the entrance of aluminum to the 20th century scene, ladders were manufactured with this lighterweight and more durable material. When working near overhead electrical wires, the use of fiberglass ladders is more common, since the material is an insulator.

In Southwestern China, the cliff-top village of Atule’er built a steel ladder to gain access to the rest of the world. It is this half-mile-long ladder that schoolchildren as young as six years old descend once in two weeks to attend school. Until recently, this treacherous journey was made on a chain of 17 much flimsier bamboo ladders. After pictures of young school children dangerously scaling the cliff were publicized, the local government raised 1 million yuan, (about $145,000) for a safer, modern steel ladder. Improved accessibility benefits not only the schoolchildren, but all of the members of the 75 families who use the ladder for anything they need from outside their village, including groceries.

LADDERS IN FIRE SAFETY

The National Fire Protection Association strongly recommends that all families inhabiting a second or third floor should keep a lifesaving escape ladder for each room above the main level of a home. These flame-resistant, anti-slip ladders are available from approximately $30 to $50 and are compact enough to be stored in the closet of each room. They attach quickly to most windows and stretch out to reach the ground.


As buildings erected in the late 19th century were getting taller and taller, firefighters were faced with a challenge: How would they scale these structures in the event of an emergency? The pompier ladder, more commonly known as the hook ladder, proved the perfect solution. In the shape of a question mark, it has a hook at the top, which firefighters on the ground would drive into a window of a higher floor and then scale the building until the window. From that higher point, the process was repeated for even higher floors.

These days, upper floors are reached via the hook and ladder fire trucks that have turntable ladders mounted on top of fire apparatuses. They are used both for access or egress of both fighters and victims at heights, as well as to provide a high-level water point for firefighting.

North America’s tallest aerial ladder is 137 feet tall and was unveiled in Ohio in 2010.

The world’s tallest firefighting device rises 367 feet in the air, and can even bend its crane arm at a 90-degree angle.

LADDER ACCIDENTS Each year, more than 900,000 people in the U.S. receive emergency room treatment for ladderrelated injuries! According to the Bureau of Labor Statistics, at least 50 percent of ladderrelated accidents are due to individuals carrying items while they climbed.

The four main reasons for ladder accidents are using the wrong type of ladder, using worn or damaged ladders, incorrectly using ladders, or placing the ladder on an unsafe surface.

Elevated falls account for 15 percent of occupational deaths annually — that means almost 700 deaths per year.
