Arthritis prevention and care tips Swollen, stiff and painful joints are common symptoms of arthritis or joint disease. There are more than 100 types of arthritis and related conditions that also can affect connective tissues and organs, including the eyes, heart, lungs and skin. While thick, knobby fingers may reveal surface signs of arthritis, deeper down in the joints where bones connect, and even in soft tissue, is where arthritis can wield debilitating damage. The Centers for Disease Control and Prevention reports that 54.4 million American adults, or about one in four, have arthritis, and that by 2040, an estimated 78 million U.S. adults age 18 and older will have arthritis — a leading cause of work disability in the country. Anyone of any age can get arthritis — the Arthritis Foundation says that about 300,000 babies and children have arthritis or a rheumatic condition — but the joint disease becomes more common with age. The CDC states that from 2013 to 2015, almost half of people age 65 and older in the U.S. reported doctordiagnosed arthritis, and 26 percent of women and 19 percent of men in America reported doctor-diagnosed arthritis. Common Types of Arthritis and Risk Factors Arthritis symptoms can come and go, or be steadily persistent with varying degrees of pain severity. Common types of arthritis include: • Osteoarthritis. The most com-

mon type of arthritis, osteoarthritis occurs when cartilage, the protective connective tissue on the ends of bones, thins and deteriorates, causing friction in joint movement. • Rheumatoid arthritis. An autoimmune disorder, rheumatoid arthritis occurs when a person’s immune system attacks the body’s healthy tissues, eroding the lining of joints (synovium) and triggering painful inflammation. • Psoriatic arthritis. A type of inflammatory arthritis, psoriatic arthritis is typically associated with psoriasis, a skin disease that presents with a red, scaly rash. People with psoriatic arthritis often experience swelling in their hands, knees, ankles and feet, with fingers and toes sometimes swelling to a sausage shape. Other familiar forms of arthritis include ankylosing spondylitis, mainly affecting the spine; gout, caused by a buildup of uric acid crystals in a joint; and lupus, a systemic autoimmune disorder. “While the causes of many types of arthritis are unknown, a number of risk factors increase the likelihood of getting arthritis,” said Gregg Balbera, president of Right at Home Nassau Suffolk. “Age is a factor because the older you get, the more wear and tear your joints endure. We work with a number of older clients who injured themselves when they were younger, and the accumulated stress on their joints has accelerated the occur-

rence of arthritis.” The increased risk for arthritis is also higher among individuals who have chronic health conditions such as obesity or diabetes. The Arthritis Foundation reports that 31 percent of adults who are obese have arthritis and 47 percent of diabetics are arthritic. Family genetics also influence who develops certain types of arthritis such as rheumatoid arthritis. The CDC notes that inheriting specific genes, the HLA (human leukocyte antigen) class II genotypes, elevates the risk for arthritis and can intensify arthritic conditions. How to Prevent Arthritis While some forms of arthritis cannot be prevented, many types can be minimized or slowed by making certain lifestyle changes. Some of the best ways to keep joints healthy and prevent degenerative arthritis include the following: • Maintain a healthy weight. Excess weight adds stress to joints, especially weight-bearing joints like the knees and hips. Being overweight or obese can flare osteoarthritis in the body’s most active joints. • Avoid injuries to joints, ligaments and cartilage. Athletes in high-impact sports such as basketball and running are particularly susceptible to damage to knees, ankles and hands that can lead to osteoarthritis. • Ease up on repetitive movements. Certain activities and occupations with repetitive motions such as running, jumping, lifting, bending and kneeling can wear down cartilage that cushions joints in the body. Being careful to limit continual repetitive movements can help lessen problems with osteoarthritis. • Boost and protect your immune system. A healthy immune system helps the body fight infection and prevent disease. A stronger immune system can help counter inflammatory arthritis, such as psoriatic arthritis, and autoimmune arthritis, such as rheumatoid arthritis, which results when the body’s immune system mistakenly attacks the joints and they become inflamed. A healthy immune system can also lessen symptoms of infectious arthritis caused by a virus, bacterium or fungus. • Do not smoke. Research finds that

smoking elevates a person’s risk for developing rheumatoid arthritis and can heighten the disease symptoms. Smoking also lowers the body’s immunity levels and hinders a person from staying physically active. Care Tips for Arthritis A thorough evaluation, early diagnosis and on-target treatment from a rheumatologist (an arthritis doctor) will help lessen joint changes and chronic pain. In treating and managing arthritis, a central goal is to reduce symptoms and improve a person’s mobility and function. For mild to moderate arthritis symptoms, the doctor may advise a combination of the following at-home care tips: • Taking over-the-counter pain relievers or anti-inflammatory medicines. • Taking prescription corticosteroids or anti-rheumatic drugs. • Applying heat and cold therapies to soothe pain. • Staying active, yet getting sufficient rest. • Strengthening the muscles around the affected joint. • Allowing the joint to rest and protecting it from overuse or strain. • Eating a nutritious diet to maintain a healthy weight, and boosting the body with anti-inflammatory foods such as green, leafy vegetables and fatty fish. “An arthritis diagnosis can be a lot to handle, especially for older adults and adults with disabilities,” said Balbera. “This is where a strong support system of family, friends and professional at-home caregivers proves invaluable. Liquid biopsies aid cancer treatment BY B E RT R A M DRACHTMAN In medicine, a biomarker is a measure or indicator of the severity or presence of some disease states. More generally a biomarker is anything that can be used as an indicator of a particular disease state or some other physiological state of an organism. Cancer is data starved. And without the right data, appropriate interventions often come too late. To select the best treatment, doctors and patients must have access to detailed genomic information about the disease. But this information is not easy to get. Traditional tissue biopsies require physical access to tumor tissue and come with serious risks for some patients. And for many patients, they aren’t even an option. In addition, they do little to help detect cancer early, when patients have the best chance at successful treatment. Liquid biopsy, a process that gathers all its information from a simple blood test, is a test performed on blood to look for cancer cells or DNA from a tumor circulating in the blood. Developing blood tests for early detection in high-risk populations and recurrence monitoring in cancer survivors is a goal of so many of the researchers today. Commercially available tests provide informa-

tion that helps advanced cancer patients get the right treatment and helps drug companies get new therapies to market faster. Targeted therapy is a cancer treatment that uses drugs. But it is different from traditional chemotherapy, which also uses drugs to treat cancer. Targeted therapy works by targeting the cancer-specific genes, proteins, or the tissue environment that contributes to cancer growth and survival The National Institute of Health Writing in 2016, in a release from the National Cancer Institute wrote that “tumor biopsies are an essential part of cancer care. They can confirm a cancer diagnosis, identify the cell type of the tumor, and, increasingly are used to determine whether a patient’s tumor contains particular genetic alterations that may make them candidates for targeted therapies” Going back to 2016, a study used blood samples from more than 15,000 patients with more than 50 types of tumors , compared a blood biopsy which profiles a type of mutation and chromosomal rearrangements in 70 cancer related genes, and detected alterations in these genes at the same distribution and frequency observed in previously published “genomic profiling studies” that used tumor biopsy samples. The above study, was, by

and large, highly consistent with gene alteration patterns reported in traditional tumor tissue testing In another component of the study, the researchers found the overall accuracy of the liquid biopsy in comparison with results from the tumor biopsy results was 87 percent. This accuracy increased to 98 percent when the blood and tumor samples were collected with six months of each other, Today’s blood tests are enabling timely therapy selection for patients with cancer while also advancing programs for recurrence detection and early cancer detection The key to conquering cancer is unprecedented access to its molecular information throughout all stages of the disease. Companies are developing a solution through tests that require only a blood sample. Cancer has traditionally been described by where it found in the body, breast cancer, lung cancer, etc. Today, we want to know what are the genetics of a specific cancer, its subtype, what mutations it may have, and as one report asks, what’s feeding it? Just a bit more than two short years ago, the first liquid biopsy test was approved by the FDA for use in cancer. This was a test that uses key mutations in a gene that makes patients with advanced non-small cell lung cancer candidates for treatment

with what is now known as aa “targeted therapy drugs” The FDA approved the test previously for this indication using tumor specimens. The new use is for the detection of these mutations in circulating DNA from blood samples. Cory Renauer writes In a National Institutes of Health Magazine, “Thanks to the Affordable Care Act, insurers are required to pay for recommended preventative services”. Last year, a study by the American Medical Association showed liquid biopsies outperformed actual tissue biopsies when it came to identifying targetable mutations. In 2018, the writer talks of a blood test that could detect cancer earlier. The study called Cancer SEEK, is a unique noninvasive multi analytical test that simultaneously evaluates levels of either cancer proteins and the presence of cancer gene mutations from circulating DNA in the blood. Biomarkers or biological markers are in our blood, tissues, and other fluids The use of a combination of selected biomarkers for early detection has the potential to change the way we screen for cancer, and it is based on the same rationale for using combinations of drugs to treat cancers. This blood test was able to detect most cases of eight different kinds of cancer including

some highly lethal forms- such as pancreatic, ovarian and liverthat currently lack screening tests. Just weeks ago, a study called the NILE study, a head to head comparison of blood assay to standard of care tissue testing for the identification of guideline-recommended biomarkers in first line advanced non-small cell lung cancer was published in “Clinical Cancer Research” The study added to a growing body of literature that demonstrates the diagnostic tool for a genomic biomarker detection in advanced NSCLC can increase the number of patients tested for guideline-recommended biomarkers. The guidelines resulted in testing for three times as many patients as a standard of care tissue testing. Also, when both the blood biopsy and the tissue testing where both available for a given patient, they were in agreement in more than 90% of the cases. The median time to results for the blood biopsy was 9 days versus 15 days for tissue testing. Knowing the status of the guideline-recommended biomarkers is important before beginning treatment for several reasons. Up to 30 percent of patients can be treated with targeted therapies that often have a higher response rate than chemotherapy or immunotherapy.

enopause is a time of change in a woman’s life. Much as the start of menstruation is accompanied by great fluctuations in hormones that can cause different symptoms, so, too, is the end of menstruation. During menopause, many women experience weight gain and have trouble shedding pounds. Researchers are not quite sure why women gain weight during menopause. However, the health and wellness site Healthline advises that both elevated and low levels of estrogen can lead to increased fat storage. This is compounded by a loss of muscle mass that occurs due to age, hormonal changes and decreased physical activity. According to JoAnn Pinkerton, MD, executive director of the North American Menopause Society, most women will gain about five pounds during the menopause transition. Women who do not gain weight may notice that fat is being

repositioned around their midsections. Other factors also may contribute to this weight gain..

Family cycling is an enjoyable escape G oing for a bike ride is one of the best ways for people to exercise in fresh air. Cycling is not only earth-conscious and convenient, but it also is a fun and popular activity that anyone can enjoy. Around 1.6 million residents of New York City ride a bike at least once a month, and in Portland, bike riding rose by 3 percent each year since 2012, according to Bikemunk, a website dedicated to helping people understand bikes, especially in the context of making a purchase. The Alliance for Biking & Walking discovered that 0.6 percent of all commuters in the United States biked to work in 2013, up from 0.5 percent in 2009 and 0.4 percent in 2005. But many people are embracing cycling as an entertaining form of recreation that the entire family can enjoy.

kin is the largest organ in the human body. As a result, when skin is compromised, the results can be painful and noticeable. Rosacea is a common skin condition that affects a large number of people. The American Academy of Dermatology says rosacea begins with a tendency to blush or flush more easily than other people. Rosacea may first appear on the nose and cheeks or the forehead and chin before spreading to other areas like the ears, chest and back. If simple blushing were the only symptom, people may be content to let rosacea go unaddressed. However, the AADA says rosacea has four subtypes that can cause more pronounced symptoms:

¡ Erythematotelangiectatic rosacea: This causes redness, flushing and visible blood vessels. ¡ Papulopustular rosacea: Redness, swelling and acne-like breakouts are hallmarks of this type of rosacea. ¡ Phymatous rosacea: When this occurs, skin thickens and has a bumpy texture.

¡ Keep a log of what triggers redness and avoid those triggers. Specific foods, alcohol and certain cosmetics and other skin products may be triggers. ¡ Use makeup to reduce the signs of redness. Green-tinted foundations and powders can offset red undertones. ¡ Some people have found alternative treatments like gentle facial massage can reduce swelling and inflammation. Dermatologists can work with patients to develop treatment plans for their specific symptoms. Although rosacea cannot be cured, symptoms can be managed effectively.
