Document for Certification • U.S. Passport / U.S. Passport Card • Social Security Card OR Birth Certificate WITH a VALID Driver’s License or Texas ID Resident of the City of San Antonio • Current Lease • Mortgage Statement • Utility Bill *** ”Live With” Attestation is for those whose address on their ID’s do not match the following documents.*** ***“Live With” Attestation*** Proof of Household Income • Recent pay stubs of you and people ages 18 and older living with you. o Monthly Pay: 1 Pay Stub o Weekly Pay: 5 Pay Stubs o Biweekly: 3 Pay Stubs o Bimonthly: 3 Pay Stubs • Recent evidence of receipt of the following benefits (if applicable): o San Antonio Housing Authority Voucher Program o Public Housing o SNAP o TANF o WIOA o WIC o Medicaid One of the criteria requirements is that your household income is less than 250% of federal poverty guidelines. (The table shows household/family size in coordination to the 250% income threshold.)
