SweetTree’s dedicated, award-winning team is formed by more than 200 support workers, managers, clinicians, and specialists providing community care for people who have sustained a brain injury or a long term neurological condition. Our Vision

SweetTree’s Brain Injury and Neurological Conditions Service are taking a leadership role in driving evidence-based practice into community care. We are training our support managers in the application of Goal Attainment Scaling (GAS), equipping them to plan care packages that use achievement of client-centred goals as a measure of our success.

Sonja is a specialist in neuropsychology and rehabilitation and is the Clinical Director and Consultant Clinical Psychologist at Neurolinks Psychology.

We believe that goal-setting and attainment not only empowers the people who use our service and helps them focus on the purposes of their support, but allows for a whole-system approach to an individual’s care. By defining objectives at the outset of a care package, we ensure a coordinated and efficient approach.

What are the SweetTree Support Steps?

You can contact the team to discuss client support options, signposting, pre-referral advice or just an informal chat. We also offer a 24 hours a day, 365 days a year emergency service.

A manager will arrange a time that works for you to complete the initial needs assessment, which includes a full risk assessment to ensure the safety and wellbeing of all.

On completion, a support plan will be created to incorporate the needs and preferences noted. This plan ensures the right team member/s are recruited for each client.

As personal needs and circumstances change, support packages are continually reviewed to adapt accordingly. Clients can change or leave free of charge with 7 days’ notice.

We care for adults and young people with a diversity of needs and lifestyles, including people living with a brain injury, stroke, Hungtinton’s Disease, Parkinson’s Disease, Motor Neurone Disease, and Multiple Sclerosis. What Support Do We Provide?

Our fee structure is informed by the outcome of our initial needs assessment. We consider the complexity of the care required, whether the package is over a long period of time or a shorter intervention and consider factors such as location, number of hours and type of shifts. The prices below are based on our lowest care level - rehabilitation and wellbeing supportthrough to highly complex care, which may include health complexity or complex specialised techniques.

Clients in this group are engaged in long term neuro rehabilitation, enhancing quality of life, wellbeing and disability management. Support is focused on activities of daily living such as community access, shopping or personal administration. Requires less intensive office-based support with minimal coordination of support schedules.

Clients in this group require a significant level of office-based support due to complex coordination, complex famiy situations or medico-legal care packages with a multi-disciplinary team and case manager.

Rehabilitation and Wellbeing Specialist Rehabilitation Complex Rehabilitation

Clients who present with significant cognitive dysfunction requiring highly specialist office management and community support. Typical presentations or factors include but are not limited to:

• Behavioural challenges

• Primary neuro psychological needs

• Dual diagnosis

• Adjustment challenges

• Complex rehab goals

Highly Complex Care and Support

• Specialist equipment needs

• Active neuro rehabilitation

• 2:1 staffing

• Complex 1:1 support

• Complex support networks

This stream includes clients with a brain injury or long-term neurological condition who have neuro rehabilitation input requiring a multi-disciplinary team, multi-weekly visits and intensive rehab support. Clients in this group may also require multiple support worker sign off for competency to health task complex specialist techniques or significant behavioural challenges.

All fees are inclusive of VAT and administration costs. You will not be charged if care is cancelled with a minimum of 48 hours.

Fees below show prices for our Specialist band level of care. We have four bands depending on client's needs: Specialist, Specialist Enhanced, Specialist Complex and Custom for larger care plans.

How Are our Care Packages Designed?

Every client referred to the SweetTree Brain Injury and Neurological Conditions Services participates in an Initial Needs Assessment to determine whether the team and the support workers at SweetTree are best positioned to help meet their desired rehabilitation and quality-of-life goals. If our team feels that it does not have the skills or capacity to help an individual reach their desired outcomes, we may recommend alternative care providers.

We offer an individualised client-centred support plan and risk management plan that is developed systemically with all involved. We are mindful of who clients were before their accident or condition to help us inform how best to set up their care package and which support workers will be best matched to work with them.

All our support workers have experience in rehabilitation, all receive extensive and ongoing SweetTree Care Training Academy development, and are carefully matched to our clients according to need, age, experience, and personality. Many of our team members are graduate psychologists, physiotherapists, social workers and occupational therapists.
