The Community Team The St Richard’s Community Team care for patients at home and in their local GP surgeries.
The team is made up of specialist nurses who work with doctors and other healthcare professionals to provide expert care
They also support the people who are important to the patient
The Community Team give lots of types of help. They help patients manage and control the symptoms of their illness.
