Laboratory Testing and Diagnostic Studies There is not a definitive laboratory test for osteoarthritis, so clinical presentation and diagnostic imaging is primarily used for diagnosis. However, laboratory testing may be done to rule out other conditions because joint pain is common among other types of arthritis or with infection. An erythrocyte sedimentation rate (ESR) test can be done on a blood sample to show if inflammation is present. Fluid obtained from an arthrocentesis can be analyzed and used to rule out RA, an infection, or crystal arthropathy in the joint. In addition, negative Gram stains and cultures can be performed, or the fluid may be viewed under a polarized microscope to look for crystallization. Blood may be tested to rule out other causes of pain and dysfunction. However, the Creactive protein (CRP), which is an inflammation biomarker, is usually higher in clients who have RA; studies also have shown it to be elevated in clients who have osteoarthritis.
