The Great Debate:

Enteral vs TPN

You have to study? But where are our treats? NPO since breakfast! Know the risk of Refeeding Syndrome and hypoglycemia!

ICU but do UCI am hungry?

In this issue the viral recipe for ICU’s alphabet soup: ABC.ABG.ECG.GCS.ICP.PAWP. CO.CVP.DKA.HB.SAS.SVT.ST.VF.VT. MAP.STEMI.NSTEMI. HIT.ACLS. SVO2. PH.DI. CPP.SIADH.

This

is my cat, Bob. But Bob is sleeping. Bob would say ”Hi.”

Bob is always so calm, so unbothered, so blessed. His heart rate is likely

Maybe you should rub his tummy to wake him up?

Or else his heart rate might get too low and require…

PACING

Those pacer pads will be tricky to remove off this furball.

His heart rate is probably TACHYCARDIC!

And look cranial nerve XII is intact

If he gets too excited (Increase HR) and becomes symptomatic we will need to

How does ACLS define symptomatic?

Here’s Bob sleeping, again.

Bob eventually passed in old age from heart failure. His vet said they heard a lot of pulmonary congestion. He is probably dreaming about food. His favourites.

Is that left or right sided heart failure?

LATERAL cheese from the Left Circumflex

INFERIOR worm fresh from the Right Coronary

Are you able to match up Bob’s favourite foods with what Bob’s food (victims) likes to eat?

Hint: Mice like cheese

Charlie would have been good friends with Bob.

Charlie also loves to eat everything (even stinky garbage)

On the LEFT side of town, the residents only throw out tasteless, scentless paper garbage.

Charlie will leave this garbage alone.

This garbage bags end up HOLDING ON to the garbage. Left-Shift

Right-Shift

Now, on the RIGHT side of the town. Oh BOY! There lives the most delicious and aromatic garbage. EVER.

Good ole Charlie can’t help but RELEASE the delicious garbage from their bags.

Good boy Charlie! Food this delicious and aromatic needs to be shared with the world.

Charlie is SHOCKED that you took away his delicious meal (aka garbage) that he was just about to eat.

QUICK! Cheer Charlie up by connecting the definitions to the correct profile to reveal the Shining STAR for our good boy!

Loss of sympathetic vascular tone. Everything is DOWN (rest, relax)

Massive vasodilation leading to WARM phase in the early stage

Vasoconstriction but cannot bring up the pressure! Something is blocking me.

Decrease pressures. Hypotension and ↓ CO

Tachycardia and Vasoconstriction to compensate

Trying to pump but contractility is compromised.

Goodbye CO and BP

This is Happy, he is my newest kitten. What’s an ABG? A Baby Gato (Cat) like me? Match the variables with the correct parameters to teach Happy about ABGs.

Oh no! You have woken Charlie from his slumber!

He’s judging you for constantly pressing SNOOZE as your iPhone alarms every 5 mins. How annoying!

To silence the alarm, clear a path to your phone by putting things back in the correct place.

Charlie is a distinguished gentleman.

Having walked this Earth for half a dozen years. But he is a forever puppy, even when life demands he put on his finest bowtie to earn his keep. It’s a dog-eat-dog world. Or is that…doggy-dog world?

When Charlie’s Left Over Energy is LESS THAN 60%

Charlie has been WORKING TOO HARD (High metabolic demand) Or, Nadia hasn’t given Charlie enough treats (Low O2 Supply). Tsk tsk Nadia

Charlie’s Left Over Energy (SvO2) after work is normally 60-80%

When Charlie’s Left Over Energy is MORE THAN 80% Charlie has been RELAXINGAT WORK (Low metabolic demand) Or, Charlie has been getting a lot of treats! (High O2 Supply). Yay!

FRONTAL PARIETAL OCCIPITAL CEREBELLUM BRAINSTEM TEMPORAL

This is Max.

Max is the sweetest cuddle bug.

Rumor is that all orange cats in the universe take turns sharing 1 brain cell. Not sure Max has got his turn yet.

Can you match the lobes and function so Max can learn about all the wonderful functions of the brain.

He needs to optimize his opportunity when the 1 brain cell makes its way to him.

Oh yeah, what about those 12 Cranial Nerves?

The brain is a box? Am I sitting in a brain?

The box (brain) is a fixed volume! I want to sit in the box. So, you have to come out to make room!

This is Min. Max’s older brother.

Meet Tom.

Tom was the most terrific cat. You enter the room. Tom’s eyes are already open and judging you. As he should. You ask, “Who are you to judge me?!” He defiantly tells you“ I am the KING OF THE RACCOONS!!!”

When you ask him for a High 5 he gives you one!

What is Tom’s GCS?

Always

Coca Cola. Or

is it

Always AKI in the ICU?

Can you please teach Max about AKI ?

I don’t want him to end up on Cat-Dialysis or with a kidney transplant (yes, they have these treatments for cats). Hint: Refer to the Renal/AKI Case Study

Causes

Phases

What do you mean I’ll be fluid overloaded if I drink all that?

Can’t you give me something to make me pee? What about dialysis, when do I need that?

This is Max as a kitten when we first got him. To scale! So small!

This is Bob at home from the animal ICU. Tom is comforting him. Bob cost me over $6K for his ICU stay. Tom should be comforting me. Bob was hospitalized for DKA, IBS, Pancreatitis, and Cholecystitis.

Bob’s DKA presented like in humans (e.g. polyuria) and was treated similarly. He got a triple lumen inserted and received IVF, Insulin, his K+ was corrected and eventually his blood sugar as well.

He also received antibiotics and was on bowel rest for pancreatitis. What is the patho, sign & symptoms and treatment for pancreatitis in humans?

The vet eventually inserted the EJ-Tube, and I had to bolus feed him a puree diet BID. Thankfully, it wasn’t TPN, that would have costed me more $$$$$.

Why should we not keep animals or humans NPO for too long?

What is the benefits of Enteral Feeding over TPN?

IMPOSTER!

That is not Santa Claus!

That’s SANTA CHARLIE PAWS

When a pathogen invades, what are the signs and symptoms of INFECTION?

Charlie has been eyeing that 1-hour Sepsis Bundle for Christmas ever since he saw a viral Nursing TikTok video raving about it. Tell Charlie what’s included in the bundle, so you don’t waste your money.

This is small, sweet Min.

Min is very polite. Min also had kitten lymphoma and did not make it past 10 months.

Min is politely asking you to please study hard so you can be a good ICU nurse.

Please study hard. Remember to start with major concepts.

First focus with the class slides and case studies / PPAs. Do practice quizzes and questions. Modules only if you have time.

Seriously. Go study. You aren’t a house cat.

So unbothered. So relaxed. So blessed.
