The Discharge Planner is designed specifically for you to advise patients who may need additional care after discharge: home health, non-medical help at home, skilled nursing, hospice, or senior housing options. The Seniors Blue Book and the Discharge Planning Resource Notebook are meant to work together. What is the difference? The Seniors Blue Book is for your clients, and the DPRN is for the professional – not to be handed out to clients. The Seniors Blue Book will make you compliant with the Conditions of Participation.
