ALZHEIMER’S IMPACT MOVEMENT (AIM) ADVANCES CAUSE THROUGH POLITICAL PROCESS
Alzheimer’s disease is not just an aging issue—it’s a public health crisis. More than 7 million Americans of all ages have Alzheimer’s, and in 2025, Alzheimer’s will cost the nation more than $384 billion.
Alzheimer’s affects not only individuals, but families, businesses and communities. And while there is good news—with new diagnostic tests making it easier to identify Alzheimer’s and new medications coming on line to help delay the progression of the disease—this new era of treatment is already facing hurdles when it comes to providing Alzheimer’s patients with access to the care they need.
AIM (Alzheimer’s Impact Movement) is working to change this. As the advocacy arm of the Alzheimer’s Association, AIM fights to support Alzheimer’s and dementia research funding and to advance bipartisan legislation that will have a positive and meaningful impact on people living with Alzheimer’s and their caregivers today and in the future.
“AIM works to ensure that Alzheimer’s remains a leading priority on Capitol Hill by participating in the political process,” said Michael Galvan, Director of State Affairs-Pennsylvania, Alzheimer’s Association. “We work to educate candidates of both parties to make sure that Alzheimer’s and dementia are part of the national plan. Our voices make sure that Alzheimer’s remains a national priority.”
Working collaboratively, AIM members target numerous issues on both a state and national level that affect those with the disease. These include the creation of Alzheimer’s-related public health infrastructure, pushing for more federal funding for research, and fighting the Centers for Medicare & Medicaid Services (CMS) for its decision to block access to FDA-approved Alzheimer’s treatments.
“When we talk about the need to move our mission forward and to change the trajectory of the disease, work in policy and advocacy is instrumental in that,” said Galvan. “The impact of what we do is big, because changing the laws changes the lives of so many people.”

24 HOUR CARE – Refers to “round the clock treatment” in a home environment. The care may be shared by multiple caregivers or by a single caregiver.
CARDIAC CARE – Refers to services including patient assessment specific diet information and management of the disease processmedication management and education. Cardiac care is a program designed to help a person with heart disease return to health. Cardiac care may consist of exercise modifying risk factors to prevent further heart damageor psychological rehabilitation.
DIABETIC CARE – Refers to services including patient assessment specific diet information management of the disease process medication management and education on all the above. Services are provided by Registered Nurse under the supervision of a M.D. Some agencies may have certified diabetic nurse instructors or cardiac care specialty RN’s in their employ.
ENTERAL FEEDING – Enteral feeding or tube feeding is a way to deliver nutrients through a tube if you cannot take food or drink through your mouth.
HOUR MINIMUM – The least number of hours a Non-Medical Home Care Agency will be available
for hire. Medicare certified Home Health Agencies are not on a specified time frame.
HOSPICE – Refers to a comprehensive program of care to patients and families facing a life threatening illness. Hospice emphasizes palliative rather than curative treatment. The dying are comforted. Professional medical care is givenand sophisticated symptom relief provided.
INTRAVENOUS THERAPY (IV)
–
Refers to any medical treatments/ medications that need to be introduced into the body via a patient’s veins. Numerous IV Therapies can be provided in a patient’s home under supervision of a MD. Some of these include: inter-infective/antibiotic therapies hydration therapy pain management chemotherapy and central line care and maintenance.
NON-MEDICAL HOME CARE
– Assistance with Activities of Daily Living (ADL’s)homemaking companionship and other nonmedical in home care. These services are available for hire after or in conjunction with Medicare certified services. Private payment Medicaid and Veterans Benefits are a few of the ways personal care can be paid for.
THERAPY SERVICES (ST PT OT RT)
Speech Therapy– Refers to services to assess and treat speech and swallowing disorders. Speech therapy can treat speech language cognitive skills or swallowing.
Physical Therapy – Refers to services for the treatment and the rehabilitation process where a physical therapist will often stretch strengthen facilitate muscles challenge balance test coordination abilities teach home exercise programs and enhance basic mobility skills.
Occupational Therapy – Refers to services to assess and treat patients to improve their ability to perform tasks in their daily living and working environments. Patients may have physical, cognitive, developmental or behavioral disabilities. Therapists also help them to develop, recover, or maintain daily living and work skills.
Respiratory Therapy – Refers to services for care of patients with deficiencies and abnormalities associated with the cardiopulmonary system. It may include the therapeutic use of medical gases
and their equipment environmental control systems humidification aerosols ventilator support bronchi pulmonary drainage and exercise respiratory rehabilitation assistance with cardiopulmonary resuscitationand maintenance of natural artificial and mechanical airways.
TRACH CARE – Refers to care administered to patients who have undergone a tracheotomy. A tracheotomy is a surgical procedure in which a cut or opening is made in the windpipe (trachea). The surgeon inserts a tube into the opening to bypass an obstructionallow air to get to the lungsor remove secretions.
WOUND CARE – Refers to services for providing diagnosis and treatment for ulcerspressure sores and other wounds. Treatments may include but is not limited to irrigationsdressingand wound vacs. Some agencies may employ specialty nurses to provide and instruct in care specifically for patients with more complicated wounds.

HOSPICE - Hospices provide individuals facing a terminal illness with care in a private residence or in a healthcare facility. The goal is to provide quality of life and to assist patients to live the last stages of their lives with dignity. The focus is on comfort rather than curative treatment of the terminal illness.
SERVICES PROVIDED:
RN-REGISTERED NURSE – Assesses patient’s physical needs, develops and coordinates the plan of care, ensures symptom control and management, provides patient/family education as needed
SW-SOCIAL WORKER – Assesses patient and family emotional and social needs, develops a plan of care, and provides counseling and support to meet identified needs, and makes referrals to other providers based on family and patient’s needs
CHAPLAIN – Assesses patient and family spiritual needs, develops a plan of care, provides counseling, assists with memorial preparations.
VOLUNTEERS – Provides companionship and needed nonmedical services for patients and their families, o ers support at time of death and during bereavement.
LEVELS OF CARE:
RO-ROUTINE CARE – Occurs when regularly scheduled visits are adequate in managing the individual’s disease symptoms. Care is provided at the individual’s private residence or in a healthcare facility.
RE-RESPITE CARE – Provided when the family needs time to rest or go out of town for a short time. Care may be provided in a skilled nursing facility. Hospice is responsible for financial arrangements with the facility.
GIP – GENERAL IN-PATIENT CARE – Provided in a skilled nursing facility when the symptoms cannot be managed elsewhere. The skilled nursing facility must have 24-hour R.N. coverage as deemed by Medicare guidelines. General in-patient care is usually short in duration. This higher level of care is due to symptoms which are out of control and more intensive nursing services are required. The room and board are covered by hospice during this period.
CC-CONTINUOUS CARE – Provided to prevent the individual from going to the hospital when a crisis occurs. It also is given when more help is needed to assist the individual and family in coping with the crisis. This
level of care is usually provided to the patient at their private residence or in a personal care home or assisted living facility. The criteria for continuous care may include: Severe pain or symptoms which are out of control, psychotic episodes, breakdown of caregiver, imminent death and family unable to cope. Continuous care is typically reserved for crises that require a constant presence by a nurse or care team.
THERAPY SERVICES PROVIDED:
AR-AROMA THERAPY – Occurs when regularly scheduled visits are adequate in maintaining the individual’s symptom management. Care is provided at the individual’s private residence, a retirement
community, an assisted living, or a skilled nursing facility.
MA-MASSAGE THERAPY – Used as a method for relaxation to alleviate stress, anxiety and pain with a light hands-on approach, as well as providing an alternative to those who can’t take medication or prefer not to. Massage therapy has become essential in hospice care. This is not reimbursed by Medicare.
MU-MUSIC THERAPY – Music
Therapy is an established healthcare profession that uses music to address physical, emotional, cognitive, and social needs of individuals.

experiences (listening, composition, improvisation, or playing/singing a familiar song) in order to achieve those goals. This is not reimbursed by Medicare.
P-PET THERAPY – There’s nothing like a visit from a four-legged creature to brighten the day of a terminally ill patient. Pet therapy teams visit with patients wherever they reside – at home or in professional care settings. Studies have shown many benefits of pet companionship, ranging from reduced blood pressure and anxiety to relief from boredom, pain and loneliness. This service is not reimbursed by Medicare.
24/7 ADMISSIONS – If a patient qualifies for hospice care, they should be admitted as early as the family requests. Many agencies admit patients to hospice, 24/7, evenings weekends, and holidays.
GRIEF SUPPORT GROUPS – Grief is the normal and universal reaction humans have to the loss of those they love. When your loved one passes, you may go through a period of bereavement and experience grief. Many hospice companies provide Grief Groups that meet and address issues of grief in a supportive group setting.
HOME HEALTH – Indicates whether the hospice agency has a sister
home health agency that will provide assistance with medically based home health needs such as nursing services, home health aide services, physical therapy, occupational therapy, respiratory therapy or speech therapy.
INDIGENT PATIENTS – Does the hospice accept non funded or indigent patients? This is usually on a case by case basis and at the determination of the hospice company. Many have a non-profit organization that can help support the care of non-covered patients.
PAIN PUMPS AVAILABLE
– Many patients have their symptoms controlled through oral medications. Some situations require a pain pump where medications are administered through an IV pump. If this is the situation, inquire with the hospice RN case manager to see if pain pumps are available through their agency.
PALLIATIVE
CARE
– Specialized medical care for people with serious illness. It is focused on providing patients with relief from the symptoms, pain and stress of a serious illness – whatever the prognosis. It is appropriate at any age and at any stage in a serious illness and can be provided along with curative treatment.

ACTIVITIES OF DAILY LIVING
(ADLs) – Activities involving personal care and mobility such as bathing, dressing, toileting, walking and meal preparation.
AGE RESTRICTIONS – Some communities have a restriction on the youngest age they are allowed to accept. These restrictions vary by community.
ASSISTED LIVING RESIDENCES
(AL) – Typically apartment-style with kitchenettes and private baths. Assisted living services and personal assistance with ADLs are provided. In PA there is a licensure distinction between Assisted Living Residences and Personal Care Homes. An Assisted Living Residence in PA is “a significant long-term care alternative to allow individuals to age in place… and [to] develop and maintain maximum independence, exercise decision-making and personal choice.” according to PA Code, Ch 2800.
ADULT DAY
SERVICES – Most providers can elect to provide day/ night services on a short or long term basis. Rates typically include a place to rest, one or two meals and snacks and activities for the day.
CONTINUING CARE RETIREMENT
COMMUNITY (“CCRC”) – There are many di erent types of CCRCs. There are Pay for Fee Communities (Type
C), Life Plan Communities (Type A) and even some rental models (Type D) inside CCRCs, but the unifying factor Is that they provide all levels of care - Independent Living, Assisted Living/Personal Care, Memory Care and Skilled Nursing. Many o er many additional amenities, such as Rehabilitation Facilities, meal plans, transportation, housekeeping, laundry and maintenance. Amenities are usually abundant; formal libraries, computer stations, swimming pools, gardens, and other social gathering spaces.
CCRC DEFINITIONS:
BENEVOLENT PROGRAM – A program created to maintain a resident’s lifestyle when they run out of resources through no fault of their own.
FEE FOR SERVICE COMMUNITIES
(TYPE C) – Includes housing, residential services and o ers di erent levels of healthcare, but not at the same monthly cost. Where you are on the continuum of care dictates what you pay for the monthly fee. There is typically an up-front fee that varies based on the type residential amenities and other factors.
LIFE CARE COMMUNITIES
(TYPE A) – Includes housing, residential services and unlimited use of healthcare services with little or no increase in monthly fees. Your monthly fees are not dependent on the level of care that you need, which
is why a Life Care Community may charge a larger up-front fee, as the Life Care Community is absorbing the risk on the resident’s level of care needed.
RENTAL AGREEMENT (TYPE D)
- Typically a small amount of rental properties are available which are not subsidized. Does not participate in the buy-in fee, nor has access to the continuum of care benefits.
INDEPENDENT LIVING COMMUNITIES – Apartments and communities for adults 55+.
INSTRUMENTAL ACTIVITIES OF DAILY LIVING (IADLs) – Activities involving more complex functioning such as grocery shopping, writing
checks, housework, cooking, traveling, and managing money.
INTERMEDIATE CARE – 24-hour inpatient care for residents who need licensed nursing supervision and supportive care, but do not require continuous nursing care. Residents may receive full assistance with ADL’s and transfers. Residents receive medications from a nurse following a doctor’s orders and may receive outpatient rehab services.
MEMORY CARE RESIDENCES (MC) – Licensed Assisted Living or Personal Care Homes that specialize in delivering care to patients with Alzheimer’s and other dementias. Communities are typically bedroomContinued on page 124

TRACH CARE – Refers to care administered to patients who have undergone a tracheotomy, a surgical procedure in which a cut or opening is made in the windpipe (trachea), to bypass an obstruction, allow air to get to the lungs, or remove secretions.
VENTILATOR – Moves breathable air into and out of the lungs, to provide the mechanism of breathing for a patient who is physically unable.
WANDERGUARD – O ered at some facilities. A resident with memory loss or confusion wears a small discrete pendant. Each external door in the care home is fitted with a radio detector which detects and alarms when a pendant moves near an exit.

ORIGINAL MEDICARE
Original Medicare is comprised of Parts A and B. Covers 80% of medical costs; you are responsible for the remaining 20% and deductibles.
PART A
Part A is Hospital Insurance. Part A is for: Inpatient hospital stays, Short-term nursing home care after 3 days of approved inpatient hospitalization, Hospice care, Limited home health care services, and Inpatient mental health care.
PART C
Part C is also known as Medicare Advantage. Must be enrolled in Medicare Parts A & B. The insurance company manages your Medicare benefits and sets medical guidelines for approval of services. Includes networks, pre-approvals, and co-pays.
PART B
Part B is coverage for medically necessary services such as doctors’ visits, labs, outpatient procedures, testing, vaccines, durable medical equipment. Part B can help pay for ambulance services, home health and mental health services.
PART D
Part D is a prescription drug plan. You must have Part A or B to enroll and must have this coverage or creditable coverage to avoid penalties. This coverage is available through private insurance companies.
