Why Guardianship and Care Management Matter
Life can become complicated when a loved one faces aging, illness, disability, injury, or cognitive decline. Families, attorneys, healthcare providers, and community members are often left asking the same question: “Who will ensure those I care about remain safe, healthy, and supported?”
That is where professional care management and guardianship services become invaluable.
Care management helps individuals and families navigate healthcare systems, housing options, community resources, and daily support needs. A professional care manager provides crisis management and assessment, coordinates services, advocates for the individual, and helps ensure that important needs do not fall through the cracks.
Guardianship provides an additional layer of legal protection when an individual is unable to make informed decisions regarding their personal care, health, or safety. Contrary to common belief, guardianships are not limited to older adults. Individuals of all ages may require a guardian due to intellectual or developmental disabilities, traumatic injuries, serious mental illness, or other conditions that impair decision-making.

Mom was struggling for nearly a decade and Dad was her caretaker. Dad was a proud Army veteran and a doting husband who didn’t like to ask (or even accept) help as he thought he could fully care for Mom on his own. But, like many caretakers before him, he didn’t truly understand it was to his detriment to try to do it ‘all’. After MANY hospital visits for them both, months of rehabilitation between them, and legal/estate matters, I was reminded of the inherent

WHO WE ARE
Area Agencies on Aging (AAAs) are local organizations that help older adults, caregivers, and people with disabilities to thrive in the places they call home.
INFORMATION & REFERRAL - Provides older adults and their caregivers with timely information and referrals for area resources.
MEDICARE COUNSELING - Your local State Health Insurance Assistance Program (SHIP) provides free, unbiased, individual Medicare counseling. Call with questions/concerns about Medicare, Medicare benefits, Open Enrollment or to see if you are eligible for the Medicare Savings Program.
LONG-TERM CARE RESIDENT ADVOCATES - People living in long-term care facilities, like nursing homes and assisted living facilities, sometimes request support and advocacy. Ombudsmen are advocates and ensure the rights of those living in facilities are respected. They provide information and resources to residents and their families and are trained in problem resolution.
CAREGIVER RESOURCES - AAAs provide services to help family caregivers care for their loved ones at home. Services, education, and financial support may be available to caregivers who provide in-home care for people 60+, grandparents raising grandchildren, and individuals caring for loved ones with dementia.
DENTAL & VISION FINANCIAL ASSISTANCE - Medicare and Medicaid often do not pay for dental and vision needs. The AAA offers financial assistance to help eligible older adults receive the dental and vision care they need.
TRANSPORTATION - Through local transportation providers, volunteer programs, and financial assistance, the AAA can get you where you need to be. like medical appointments, your local senior center, and shopping.
CARE NAVIGATION - Feeling overwhelmed with paperwork or your current/future needs? A care navigator can help you create a plan to map out your future goals that align with your preferences, strengths, and values.

Will Do: Don’t Procrastinate When It
Many of us procrastinate when it comes to writing a will.
Many of us procrastinate when it comes to writing a will. Perhaps we don’t want to be reminded of our mortality. It may be a fear of the anticipated expense or a loss of control over property. But having a will extinguishes those fears. Want a few other reasons? A will contains your selection of who will receive what, when and how upon your death. You decide who will take care of your minor children. You facilitate the probate process (making it less expensive and quicker). You can disinherit individuals, make gifts or charitable donations and provide for your pets. You decide who administers your estate to see that everything
goes as you have directed. If no will exists, the state will determine how your property is divided.
This can be easily avoided with proper planning. By creating your will now, you can always add to the provisions or alter the document as your life evolves. It’s advisable to review your current will every five years to be sure that it’s up to date and still reflective of your future wishes.~

Why Would I Consider A Trust For My Estate Planning In 2026?
In 2026, everyone needs an estate plan, regardless of your socio-economic circumstances.
A simple estate plan will typically contain at least three documents; a durable medical power of attorney and a durable financial power of attorney. These documents give the authority to a person of your choice, to speak on your behalf on your medical and financial issues if you’re not able to make necessary decisions due to health issues. The third document, a will, is a way to pass your property upon death to whom you wish.
A trust might be an option in your estate planning instead of or in addition to a will. Trusts are useful estate planning tools that can accomplish a variety of goals. They can help avoid probate, minimize taxes, and be used to give property to minor or disabled loved ones. Trusts can be created during a person’s lifetime (Living Trusts) or at the person’s death (Testamentary Trusts). Some different types of Trusts from both categories are:
Living Trust
A person can transfer their assets to a Living Trust and, as trustee, continue using their assets as needed during their lifetime.
Tax Planning Trusts
Several different types of Living Trusts provide flexible alternatives for minimizing capital gains and
estate taxes, including the Charitable Remainder Trust, Irrevocable Life Insurance Trust, Qualified Personal Residence Trust, Grantor Retained Annuity Trust, and Grantor Retained Unitrust.
Testamentary Trusts
A person can create a Trust under a Will, called a Testamentary Trust, which does not take effect until they are deceased.
Disability Trusts (also known as Special Needs Trusts)
A Disability Trust is a type of Living Trust that allows a disabled person under the age of 65 to use his/her own assets, other than for food and shelter, for their special needs and keep public benefits, such as Medicaid and Supplemental Security Income (SSI).

Rehabilitation Hospitals, sometimes referred to as Inpatient Rehabilitation Hospitals, provide intensive rehab to patients who are generally medically stable but need help to recover from injury or other medical conditions. They can be found within a hospital or in a free-standing building. Rehabilitation hospitals were created to provide services less costly on a per diem basis than general hospitals but provide a high level of professional therapies such as physical, occupational and speech therapies. Rehabilitation Hospitals must offer a minimum number of physician visits, increased daily rehab hours (3 hours/day, 5 days/week of PT, OT, and/or Speech), and interdisciplinary services including increased RN oversight and nursing hours per day.

All Points Transit’s Dial-A-Ride service offers seniors in the Montrose area a lifeline to independence and mobility.
Rather than relying on family members or navigating fixed bus routes, older adults can schedule door-todoor transportation that fits their personal needs and timing, a critical advantage for those with limited mobility or health appointments that can’t be missed.
All Points Transit removes one of the biggest barriers seniors face: getting to medical appointments, grocery stores, pharmacies, and community events safely and reliably. For those who can no longer drive, this isn’t a luxury, it’s essential to maintaining quality of life and avoiding isolation.

Many families delay the discussion because they worry about upsetting someone or facing resistance. However, following these tips can help make the situation rewarding and beneficial to both the older adult and family members.
The best time to talk about senior living is before there is a crisis. Waiting until a fall, hospitalization, or sudden health change can create pressure and limit options. When families begin the discussion early, everyone has more time to explore preferences, finances, and future needs together. Choose a comfortable setting and approach the conversation with empathy. Instead of focusing on what your loved one can no longer do, talk about goals such as safety, convenience, social connection, and maintaining independence. Ask open-ended questions like, “How are you feeling about managing the house?” or “What would make daily life easier for you?”
Listening is just as important as speaking. Your loved one may have fears about losing independence, leaving home, or facing change. Acknowledge those feelings and avoid arguing. Provide reassurance that they will still have control over choices and lifestyle.
It can also help to introduce senior
