Pueblo Area Agency on Aging – Since I began working with the Meeting Needs for Pueblo Area Agency on Aging, I Healthier Living have learned older adults have critical needs to remain in their homes and live a healthy life.
The Pueblo Area Agency on Aging or PAAA has great partnerships throughout Pueblo County who provide different services to those in the community. The importance of providing services that are helpful is listening to the community’s needs. One program the PAAA takes great pride in is providing Material Aid assistance to the community. This program provides assistance to those who reside in Pueblo County, age 60 and older with vouchers toward Dental, Denture, Eyeglasses and Hearing Aids. These categories are particularly important to live a well-balanced life. During my time with PAAA, the material aid program is clearly a need. I have learned many older adults have not been to a dentist for basic dental hygiene in 10 years or more in some cases. Many have dentures that they received 10+ years ago which do not fit well as they age creating sores on their gums. Some have shared that their glasses are so scratched that they are not sure if their vision has changed or the condition of their lenses are the reason. One extremely costly need for some

are hearing aids. Many times, they purchase discount hearing aids in effort to save money, however, within a year they are no longer wearing them because they have not helped overall. The material aid vouchers are payer of last resort, with insurances, Medicaid and Medicare taking the lead in covering the service. Many of the plans do not cover these needed, therefore, the voucher helps with their needs. At times, the voucher is the only way an individual can find some comfort and the ability to have a conversation and hear clearly what others are saying. AAA’s are a way for older adults to find assistance, comfort and pleasure in their daily lives. My day is complete when I hear a call thanking the PAAA because they were able to finally eat comfortably, read the paper or just hear the television clearly while enjoying their day. Consider a Discount Dental Plan Everyone wants to sell you dental for Savings insurance; but a discount plan can save you hundreds and sometimes even thousands of dollars each year.
When shopping for Dental Coverage make sure you are asking at least four questions. What are my annual premium costs? What are my maximum benefit pay outs? What is the waiting period before I can use those benefits? And does this plan cover specialty procedures? Traditional Dental Insurance typically have low annual limits on benefits; averaging around $1200 per year. Basic insurance plans with a $1000 maximum benefit can cost $478 per year and a slightly better
plan with a $1350 maximum benefit can cost you upwards of $664 per year. Are your exams and cleanings really “free” when you are paying those types of premiums? And on top of that you may have to wait 12 or even up to 24 months to be eligible for those benefits to be paid out. With a Discount Dental Plan the annual fees are very affordable ($107.40/yr), there are no maximum benefits, there are no waiting periods, and specialty services are covered. Meet Charlie and Fran Charlie and Fran have 3 children, 9 grandchildren, and 10 greatgrandchildren. They have worked hard and have enjoyed 50+ years of life on their family farm. This place is important to their entire family. Knowing that over 70% of people over 65 will need some type of long-term care (LTC), Charlie and Fran needed guidance and accurate information to prepare for the uncertain future. Whether they suddenly needed LTC, or if LTC was years away, Charlie and Fran wanted to ensure future generations can enjoy the things they worked hard for their entire life including the farm. Colorado has many resources for financial help like Health First Colorado, and Colorado’s Medicaid Program. Medicaid helps with in-home care, assisted living, and nursing facility care. Most Colorado Seniors who need help paying for LTC feel they will not qualify for Medicaid because they have too much income or too many assets. In reality, many could qualify if they had the information from a knowledgeable professional, holding

This simply is not true and can lead to an unnecessary spend down and/or liquidation of the limited assets Seniors have worked a lifetime to obtain. There are legitimate means of preserving valuable or sentimental assets and income. Here in Colorado, the Medicaid guidelines afford numerous opportunities to protect income and assets. Choosing Your Medicare Plan I often hear comments from Medicare beneficiaries about how they considered themselves to be reasonably intelligent people until they started trying to figure out Medicare. Don’t worry. That is a normal feeling. Medicare can be confusing. The first step is to choose the type of Medicare plan that fits you. There are two types of Medicare plans. You need to know the difference between Medicare Supplement and Medicare Advantage. They are not the same – in fact, they work very differently. Once you understand how each option works then you can move on to figuring out what plan suits you. It is important to have someone on your side when choosing a Medicare plan. You need to answer a few questions to make sure you are choosing a plan that meets your individual needs. Do your doctors participate in the plan? What insurance is accepted by your preferred hospitals? Do you travel out of the area frequently? Do you want an HMO, PPO or Medigap? Do you prefer to pay for medical expenses as they occur or in predictable payments? Will you incur penalties? How do the

private plans compare to employer or state plans? Once you’ve decided which route works best for you, then you can move on to actually choosing the plan. Transportation Programs Help Reclaim Older Adults’ Older adults yearn to retain their independence to move about Independence freely. It is important to remember that many older adults’ live active lives, are safe drivers and are able to use public transit.
There is no universally accepted age at which people are no longer safe drivers, even though chronic conditions and disability, which occur more frequently in old age, certainly impact that skill. It’s no wonder that the impact of having to “give up the keys” is a major, often negative, life event for many older adults. But the impact can be lessened considerably if alternatives
to driving are readily available and accessible. The vast majority of older adults – nearly 90%, according to AARP – choose to age in place in their homes and communities. Successful community living requires access to medical and other essential services. While the health impact of reduced access to needed medical services is obvious (missed appointments, ER visits, lack of continual care), social isolation due to lack of transportation can have an equally

SRDA TRANSPORTATION Through a donation-based system, SRDA transportation program provides older adults with rides to: n n n n

Monday through Friday from 8:00 a.m. to 5:00 p.m. With the strong support from Pueblo County Commissioners and the Pueblo Area Agency on Aging, SRDA is able to offer transportation that includes all areas of the county. ted with bicycle racks for recreational trips. Reservations are required at least 48 to 72 hours in advance. SRDA now offers intergenerational transportation in Rural and Small Urbanized areas of Pueblo County as a part of the Healthy living Through Active Transportation Campaign. This intergenerational service is available in Blende, Avondale, Vineland, Colorado City, Pueblo West, and Beulah. Group recreational trips are available for all ages within normal operating hours. (outside city limits) Senior Transportation programs help increase mobility and independence, in addition to decreasing isolation. 24 HOUR CARE – Refers to ‘around the clock treatment’ in a home environment. The care may be shared by multiple caregivers or by a single caregiver. ALZHEIMER’S/DEMENTIA CARE – Non-skilled care provided for clients with memory issues includes but not limited to establishing a routine, reminders for activities for daily living, rerouting if necessary and assistance with sleep issues or ‘sun downing’. By using creativity and caregiving skills, you can adapt routines and activities as needs change. BONDED – Being bonded means that a bonding company has secured money that is available to the consumer in the event they file a claim against the company. HOMEMAKER/COMPANION – Non-medical assistance with meal preparation, laundry, light housekeeping, errands, activities, outings, companion-ship and other household duties. These agencies are licensed by the state. INSURED – This refers to what happens if someone gets hurt on the job. If the company isn’t insured, then the claim gets filed against the homeowner’s insurance. If the company is insured, then the claim gets filed against the company’s insurance. LICENSURE – A home care agency, whether medical or non-medical, shall be issued a license consistent
with the type and extent of services provided. CLASS A – A home care agency that provides any skilled healthcare service. Agencies with a Class A license may also provide personal care services. CLASS B – A home care agency that provides only personal care services. An agency with a Class B license shall not provide any skilled healthcare service. MINIMUM HOURS PER VISIT – This refers to the minimum number of hours that an agency provides service. STATE REGISTRATION – The State of Colorado does not license homecare placement agencies. Placement agencies are registered with the state. Registration ensures that placement agencies are adhering to certain minimum standards such as background checks for caregivers and insurance requirements for agencies and caregivers.

CARDIAC CARE – Refers to services including patient assessment, specific diet information, and management of the disease process, medication management and education. Cardiac care is a program designed to help a person with heart disease return to health. Cardiac care may consist of exercise, modifying risk factors to prevent further heart damage, or psychological rehabilitation. DIABETIC CARE – Refers to services including patient assessment specific diet information, management of the disease process, medication management and education on all the above. Services are provided by Registered Nurse under the supervision of a M.D. Some agencies may have certified diabetic nurse instructors or cardiac care specialty RN’s. ENTERAL FEEDING – Enteral feeding, or tube feeding, is a way to deliver nutrients through a tube if you cannot take food or drink through your mouth.
INTRAVENOUS THERAPY (IV) – Refers to any medical treatments/ medications that need to be introduced into the body via a patient’s veins. Numerous IV Therapies can be provided in a patient’s home under supervision of a MD. Some of these include: inter-infective/antibiotic therapies, hydration therapy, pain management, chemotherapy and central line care and maintenance. MED-ALERT – Company offers a pendent or home alert system that notifies staff that user is in distress and needs immediate assistance.

NON-MEDICAL HOME CARE – Assistance with Activities of Daily Living (ADL’s), homemaking, companionship and other nonmedical in home care. SOCIAL WORKER – A clinical social worker whose purpose in health care is to enhance, promote, maintain, and restore the best possible social abilities of the patient or family. Services provided may be preventive, developmental, or remedial in nature. THERAPY SERVICES (ST, PT, OT, RT) – Speech Therapy – Refers to services to assess and treat speech and swallowing disorders. Speech therapy can treat speech, language, cognitive skills or swallowing. Physical Therapy – Refers to services for the treatment and the rehabilitation process where a physical therapist will often stretch, strengthen, facilitate muscles, challenge balance, test coordination abilities, teach home exercise programs and enhance basic mobility skills. Occupational Therapy – Refers to services to assess and treat patients to improve their ability to perform tasks in their daily living and working environments. Patients may have mental, physical, developmental, or emotional disabilities. Therapists also help them to develop, recover, or maintain daily living and work skills. Occupational therapists help clients not only to improve their basic motor
functions and reasoning abilities, but also to compensate for permanent loss of function. Respiratory Therapy – Refers to services for care of patients with deficiencies and abnormalities associated with the cardiopulmonary system. It may include the therapeutic use of medical gases and their equipment, environmental control systems, humidification, aerosols, ventilator support, bronchi pulmonary drainage and exercise, respiratory rehabilitation, assistance with cardiopulmonary resuscitation, and maintenance of natural, artificial, and mechanical airways. TRACH CARE – Refers to care administered to patients who have undergone a tracheotomy. A tracheotomy is a surgical procedure in which a cut or opening is made in the windpipe (trachea). The surgeon inserts a tube into the opening to bypass an obstruction, allowing air to get to the lungs, or remove secretions.

VOLUNTEER – Provides respite and needed non-medical services for patients and their families, offers support at time of death and during bereavement. Levels of Care: RO-ROUTINE CARE – Occurs when regularly scheduled visits are adequate in maintaining the individual’s symptom management. Care is

provided at the individual’s private residence, a retirement community, an assisted living, or a skilled nursing facility. RE-RESPITE CARE – Provided when the family needs time to rest or go out of town for a short time. Care may be provided in a long-term care facility depending on the condition of the individual. Hospice is responsible for financial arrangements with the Longterm care facility. GIP – General In-Patient Care – Provided in a skilled nursing facility when the symptoms cannot be managed elsewhere. The skilled nursing facility must have 24-hour R.N. coverage as deemed by Medicare guidelines. General in-patient care is usually short in duration. This level of care is usually due to symptoms which are out of control and more intensive nursing services are required. If an individual is at a care facility, room and board are covered by hospice during this period. CC-CONTINUOUS CARE – Provided to prevent the individual from going to the hospital when a crisis occurs. It also is given when more help is needed to assist the individual and family in coping with the crisis. This level of care is usually provided at the individual’s residence no matter where that may be. The criteria for continuous care may include: Severe pain or symptoms which are out of control, psychotic episodes, breakdown of caregiver, imminent death and family

24/7 ADMISSIONS – If a patient qualifies for hospice care, they should be admitted as early as the family requests. Many agencies admit patients to hospice,24/7, evenings weekends, and holidays. AROMATHERAPY – Uses plant materials and aromatic plant oils, including essential oils, and other aromatic compounds for the purpose of altering one’s mood, cognitive, psychological or physical well-being. This is not reimbursed by Medicare. GRIEF SUPPORT GROUPS – Grief is the normal and universal reaction humans have to the loss of those they love. When your loved one passes, you may go through a period of bereavement and experience grief. Many provide Grief Groups that meet and address issues of grief in a supportive group setting.

INDIGENT PATIENTS – Does the hospice accept non funded or indigent patients? This is usually on a case by case basis and at the determination of the hospice company. Many have a non-profit organization that can help support the care of non-covered patients.
PAIN PUMPS AVAILABLE – Many patients have their symptoms controlled through oral medications. Some situations require a pain pump where medications are administered through an IV pump. If this is the situation, inquire with the hospice RN case manager to see if pain pumps are available through their agency.

demic were unimaginably difficult for everyone to navigate; none more than for the senior population.
Family members far-and-wide were advised to stay away for the safety of their loved ones. Precious moments and lifelong stories were tragically missed out on as holidays and social gatherings were spent behind cameras and broadcast on computer screens. When the world suddenly slowed down and each passing day featured new guidelines and increased separation, those who thrive on person-toperson contact and social interaction struggled most. As frontline healthcare workers trudged through uncertainty on a daily basis to ensure the safety of entire communities afflicted by the virus, an incomparable obstacle faced many others. Care team members who provide hospice, palliative, and home health care services across Southern Colorado were tasked with the arduous responsibility of providing care that is predicated on human-touch, without a human touch. Strict social distancing expectations and revised practices to include an abundance of caution inhibited caregivers from connecting with their patients with
the same level of compassion that has come to be expected. In spite of the countless challenges and continuous changes, our dedicated team of miracle-workers tirelessly continued to care for patients, allowing family members to spend their loved ones’ final moments surrounded by those closest to them in the safest setting imaginable: home. PALLIATIVE CARE – Specialized medical care for people with serious illness. It is focused on providing patients with relief from the symptoms, pain and stress of a serious illness – whatever the prognosis. It is appropriate at any age and at any stage in a serious illness and can be provided along with curative treatment. PCP-OVERSIGHT – Can the hospice patient retain their own primary care physician in addition to the hospice medical director? All hospice organizations require a medical director, a physician to oversee their care. Many times the hospice medical director will work in conjunction with the patient’s own physician. The patient, family and hospice team can determine which physicians will oversee the care and certify the hospice eligibility criteria.
PET THERAPY – There’s nothing like a visit from a four-legged creature to brighten the day of a terminally ill patient. Pet therapy teams visit with patients wherever they reside – at home or in professional care settings. Studies have shown many benefits of pet companionship, ranging from reduced blood pressure and anxiety to relief from boredom, pain and loneliness. This service is not reimbursed by Medicare. VA CONTRACT – If the patient is not a Medicare Recipient and is covered by the VA, the VA will make payments directly to their contracted agencies. If a patient is a Veteran, but is a Medicare Recipient, they may choose any Medicare approved hospice agency for their care. ~

ACTIVE ADULT COMMUNITIES are traditionally geared towards the physically active senior, these communities can be apartment style, cottage/patio home, or Condo/ Townhome style living. There are often community gatherings and many offer enhanced amenities like concierge, golf courses and recreation centers. They usually do not offer meal or meal option community dining (but may have a restaurant on site). May be a buy-in or a rental property. AGE RESTRICTIONS – Some communities have a restriction on the youngest age they are allowed to accept. These restrictions vary by community. ALZHEIMER’S / DEMENTIA ASSISTED LIVING COMMUNITY – Licensed assisted living facility or skilled nursing facility that specialize in delivering care to victims of Alzheimer’s and dementia. Communities are typically bedroom-style rental units with common areas for dining and activities. Many are locked for residents’ safety. ASSISTED LIVING COMMUNITY – Typically apartment-style with kitchenettes and private baths. Some offer companion accommodations. Personal assistance with Activities of Daily Living (ADLs) bathing, grooming, dressing, toileting, eating and ambulation.
DAY CARE – Any provider can elect to provide day/night services on a short or long term basis. Rates typically include a place to rest, one or two meals and snacks and activities for the day. Licensed communities are prohibited by law to promote these services without a specific license. DEPARTMENT OF HOUSING & URBAN DEVELOPMENT - (HUD) helps apartment owners offer reduced rents to low-income tenants.

INDEPENDENT HOUSING – Apartments and communities for adults 55+ with little or no senior support services (e.g. no meals, etc...) INDEPENDENT LIVING is usually apartment style communities for adults 55+ with little or no senior support services. Many but not all are wheelchair accessible. INTERMEDIATE CARE – 24 hour inpatient care residents who need licensed nursing supervision and supportive care, but do not require continuous nursing care. Residents may receive full assistance with ADL’s and transfers. Residents receive meds from a nurse following a doctor’s orders and may receive outpatient rehab services. LONG-TERM CARE – Chronic, custodial care provided on an on-going basis MEDICAID WAIVER – An assisted living waiver may be used to transition individuals out of nursing facilities or to provide persons at risk of institutionalization with options for remaining in their communities. Assisted living waivers are designed to provide an alternative to more costly nursing home care. PRICE DEPENDENT ON LEVELS OF CARE – Flexible levels of service geared to accommodate different needs over time. Rates may vary depending on the changing levels of care while one maintains the same physical residence. PUBLIC HOUSING (PH) – Affordable apartments for lowincome families, the elderly and persons with disabilities. REHABILITATION AND THERAPY – Many skilled nursing facilities offer restorative and rehabilitative physical and occupational therapy, and speech-language pathology services. Short-term rehabilitation services may be provided to individuals with diagnoses such as joint replacements, stroke, etc. Long-term rehab may also be provided for permanent residents

of the facility who have chronic or progressive illnesses. Occupational therapy services in this setting are generally reimbursed by Medicare Part A for short-term, acute care clients; Medicare Part B for long-term residents who require skilled services and have medically necessary need for rehab; and often by private insurance, Medicaid, or private pay for younger clients. RESIDENTIAL HOME – Assisted living homes are private residential homes that typically have a small number of residents who live together and receive care from livein caretakers. Adult family homes offer room and board for seniors who want a more private, homelike community. Assistance with activities of daily living such as bathing and dressing are provided. Amenities and nursing services vary greatly between homes. Ask about their services when you tour. They are also referred to as personal board and care homes. Most accept private pay; Medicaid in some instances. RESPITE CARE / SHORT-TERM STAY – Otherwise known as a short-term stay. Any provider of IL, AL, ALZ or SNF care can elect to provide fully furnished accommodations to a person for a short period of time. Rates usually quoted on a

RETIREMENT COMMUNITY – Housing designed specifically for mature adults. Typically Studio, 1 BR or 2 BR rental apartments with full kitchens, and private baths are available. Services include meal plans, transportation, housekeeping, laundry and maintenance. Amenities are usually abundant; formal libraries, computer stations, swimming pools, gardens, and other social gathering spaces. Rates are monthly, and are typically based on size/ location of apartment. SECURED UNIT – A secured unit dedicated to Alzheimer’s/Dementia residents to prevent wandering outside the community without supervision. SHORT-TERM CARE – Acute care provided on a short term basis. SKILLED CARE – 24 hour licensed nursing services, eight hours of which are RN covered. Patient may receive rehab services either inpatient or outpatient and may be fully dependent for ADL’s. Patients receive medications by an RN according to a licensed physicians orders. SKILLED NURSING FACILITY – Facilities provide 24/7 care for residents and offer full assistance
with ADL’s. Daily rates typically include a furnished bedroom and a private or shared bath, meals, activities, transportation, phone service, cable television and all personal/ nursing care. Payor sources usually accepted are Medicare, Medicaid, VA, some insurance plans and private sources

TRACH CARE – Refers to care administered to patients who have undergone a tracheotomy. A tracheotomy is a surgical procedure in which a cut or opening is made in the windpipe (trachea). The surgeon inserts a tube into the opening to bypass an obstruction, allow air to get to the lungs, or remove secretions. VENTILATOR - Moves breatheable air into and out of the lungs, to provide the mechanism of breathing for a patient who is physically unable to breathe, or is breathing insufficiently. WANDERGUARD - Offered at some facilities. A resident with memory loss or confusion wears a small discrete pendant. Each external door in the care home is fitted with a radio detector which detects when a pendant moves near an exit. The WanderGuard will sound an audible alarm, alerting the staff of a patient wandering. ~

What to Look For When Finding the best place for Mom or Visiting a Facility Dad is one of the most important decisions you can make. isted below are some helpful hints on what to look for when you tour a senior housing or skilled nursing community. CLEANLINESS Look past the furnishings and into corners, baseboards, and windows. Ask how often housekeeping is provided. What services does maintenance provide? What is their response time?

Look for small and large group activities, trips or outings. WHAT IS THE STAFF’S ATTITUDE? Staff attitude and friendliness are of the utmost importance. Are the bathrooms accessible and do they have grab bars in convenient locations? How does a resident contact staff if they have an emergency

Medicare offers a free annual wellness visit for those 65 and older where patients can request a brain health assessment. But if you don’t see your family physician frequently, he or she may not know you well enough to determine if you are experiencing cognitive changes. To help, the Alzheimer’s Association has developed a list of five questions that individuals or family members can ask the doctor to help ensure that the visit is productive and patients get proper direction.

Do I (or the patient) have any medical conditions that could increase my risk for developing Alzheimer’s disease or another form of dementia?

Am I taking any medications that could be causing memory or cognitive-related symptoms?

“This is a good starting point for having a productive conversation with your family doctor,” said Danelle Hubbard, director of Health Systems and Family Services for the Alzheimer’s Association of Colorado. “I would suggest bringing these questions in writing along with any other specific concerns or examples you may have. There can be a number of reasons behind memory loss and personality change, so it’s important to get guidance from your doctor as early as possible.” For individuals who have immediate questions, the Alzheimer’s Association has a free, 24/7 Helpline staffed by trained professionals who can provide guidance and support: 800-272-3900. Finding Your Home Away From Home The lives of our family, and in particular our mother (Josephine Giarratano), have been touched and impacted by the health care industry and the people that tirelessly toil in this profession. That being said, as we reflect on the experiences of the past two years, we would like to express our gratitude to The Center at Park West. My mother has had the opportunity to literally rehabilitate 6-7 times over the course of that time period. She considers this facility and the people that have worked with her a “home away from home”, and we agree. The doctors, nurses, therapy staff, and support personnel individually and collectively have had a direct impact on returning my mother to a quality of life and living each time. We have been impressed by the genuine commitment to services, the day to day care, and the passion and professionalism that encompasses this care. When our family, including my mother, began the process of investigating care/rehab facilities, our number one concern and priority was safety and quality of care, in order to facilitate and enhance my mother’s quality of life. TEAMWORK is extremely important to our family, and we can attest that the philosophy of the Center champions and embodies TEAMWORK. The environment is SAFE-EFFECTIVE-EFFICIENT and

has a number of care providers in multiple areas of the facility. Patient engagement and satisfaction foster a trust and the ability to speak freely and openly to staff (a BIG plus for us), education during the pandemic and access was unfailing, and empathy was paramount for both immediate family and friends. Facility and Staff will be the biggest plus when doing your research. I empower those individuals and family members seeking a “home away from home” to reach out if you so choose and I will gladly address anything you may want to know or questions you may carry – I will leave my contact information with the facility. Josephine’s comments from her stays: “I was treated with care and respect by nurses, CNAs, Physical and Occupational Therapy members”, “enjoyed having my own room and that is was always clean”, helped me when I needed help”, “when I had appointments they took me to and from promptly“, “breakfast, lunch and dinner were healthy and served timely” and “OT and PT staff were wonderful, helped me get better and out of the hospital and home each time”. ~ Editor’s Note: This article was submitted by the staff at The Center at Park West.
