Illinois Medicaid program faces looming funding crisis due to federal changes

New federal law will restrict a key revenue stream used to pay state’s share of Medicaid costs

SPRINGFIELD — A multibillion-dollar budget crisis will hit the state’s Medicaid program in the next few years unless state lawmakers and Gov. JB Pritzker act to prevent it, budget analysts both inside and outside state government warn.

The crisis comes from changes in federal Medicaid policy that were enacted last year as part of President Donald Trump’s sweeping domestic policy agenda, known officially as H.R. 1, or the “One Big Beautiful Bill Act,” which, among many other things, slashes one of the main funding tools many states have used for four decades to fund their share of the cost of Medicaid.

“By the time you’re a few years in, you’re looking at a $4 - $5 - $6 billion a year hit, and that’s material,” Paula Worthington, a researcher with the University of Illinois’ Institute of Government and Public Affairs, said during a recent interview.

Worthington is a coauthor of a recent IGPA report that describes the upcoming changes in Medicaid funding as one of the most serious long-term fiscal challenges facing Illinois.

The funding tools at issue are known as provider taxes. Those are special taxes that states levy on hospitals, nursing homes, private insurance plans known as “managed care organizations,” or MCOs, and other kinds of health care providers.

States levy those taxes — usually on a per-patient or per-bed basis — and put the money into a fund that is used to draw down federal matching funds. The combination of state and federal funds is then used either to enhance Medicaid reimbursement rates or make direct payments to hospitals and nursing homes that serve large numbers of Medicaid patients to help sustain those facilities financially.

Capping provider taxes

When Medicaid was first established in 1965, it was a relatively modest program

that provided health care benefits to people who already qualified for other kinds of public assistance, with costs split between the federal government and the states.

As both the size and cost of the program grew, however, states began looking for new ways to pay for their share of the cost outside of their general revenues. Provider taxes emerged in the mid-1980s as one such mechanism.

By the mid-1990s, Congress began putting controls on state-levied provider taxes. Those included rules that they be broad-based and uniform, meaning they had to apply evenly across an entire class of health care providers and not just those that served Medicaid patients.

The idea was that states were prohibited from holding providers harmless by assuring them they would get all their money back, and then some, once the federal matching funds were drawn down.

But the federal rules also included a “safe harbor” provision. States could avoid having to comply with the hold-harmless prohibition as long as their taxes amounted to no more than 6% of net patient revenue.

That rule has effectively served as a cap on what states can levy in the form of a provider tax. But that cap is about to be cut nearly in half.

Under a provision of H.R. 1 – a provision that only applies to states like Illinois that expanded Medicaid eligibility under the Affordable Care Act – that 6% cap will gradually be cut starting in FY 2028 until it reaches 3.5% in FY 2032.

Impact in Illinois

In FY 2025, according to the Department of Healthcare and Family Services, Illinois spent a total of $33.7 billion through its Medicaid program, making it one of the single largest categories of expenditures in state government.

Of that total, $20.9 billion, or 62%, was federal money while the remaining $12.8 billion came from state funds, including both general revenue and provider taxes.

IGPA reported that provider taxes in FY 2025 amounted to $4.7 billion, or about 37% of all the state funds that were spent on Medicaid. The bulk of that

money, according to DHFS, came from two provider taxes, those on hospitals and MCOs.

A recent report by the nonpartisan health policy research organization KFF points out that Illinois stands to lose more than any other state when the reductions take effect because it is the only state whose hospital and MCO assessments are both above the 3.5% threshold.

The report by IGPA estimates that when the first reduction takes effect in FY 2028, revenues from those two assessments alone will fall $239 million. And depending on how much health care prices and Medicaid usage grows over the next five years, total reductions from those two sources could range from $1.25 billion to $2 billion by FY 2033.

“But remember, that’s just the state money,” Worthington said. “That is not taking into account the matching funds.”

Assuming the state continues to receive an average 62% federal match rate, the total impact to Illinois by FY 2033 would be between $3.3 and $5.3 billion a year.

That estimate aligns with a separate estimate from the Governor’s Office of Management and Budget, which reported in October that if the state does not make up for the reduced provider taxes through some other appropriation, “the gross amount of lost funding to the state and its Medicaid program will total over $4.5 billion annually by fiscal year 2031 when factoring the lost federal match.”

Issue for lawmakers

The impending limits on Medicaid provider taxes will pose a significant challenge for lawmakers who will either have to find some other way to pay the state’s share of the cost or adjust the program to fit within the new fiscal constraints.

But because the federal changes do not take effect for another year, it is possible lawmakers will not feel pressure to take immediate action in the upcoming legislative session.

Recent postal service changes could disrupt mail-in voting, county clerks warn

EAST PEORIA — New postal service changes to postmarking and transportation rules could disrupt mail-in voting in the 2026 election, Illinois county clerks warn.

Ahead of the March primary election, county clerks are telling voters not to rely on past processes and to mail their ballots as early as possible, no later than one week before Election Day.

Clerks from around the state discussed how to educate voters at the annual Illinois Association of County Clerks and Recorders Conference on Thursday in East Peoria.

One new rule, which took effect Dec. 24, states that the date a postmark is applied to a piece of mail may not reflect the day it was received by the Postal Service. Currently, 14 states, including Illinois, will accept mail-in ballots if they are received within a certain period after Election Day if they are postmarked on or before Election Day.

Clerks warned that uncertainty about when ballots will now be postmarked necessitates voters to act earlier. The way postmarks are applied is not changing, the USPS says. Mail is only postmarked at distribution centers, and that will remain the same.

But in an effort to improve mail delivery efficiency, some localities’ mail may be sent to a different distribution center than in the past, which can increase the amount of time it takes for the USPS to receive and then postmark mail at these centers.

“It’s a question mark of when will it actually get through a distribution center. So I’m advising my voters to make sure that they get their vote-by-mail ballot in the mail no less than a week before Election Day,” John Ackerman, the Tazewell County clerk, said.

In the city of Peoria, for example, the distribution center is in Peoria. Tazewell County’s center, Ackerman said, is in Champaign, although Peoria is geographically closer.

To educate voters about the change, he said, his office will include yellow index cards with the recommendation in the vote-by-mail packets sent to voters.

Voters can also request a manual postmark from their local post office or drop off their ballot at the election official’s office.

But if election officials can’t rely on

postmarks to reflect accurate dates, a number of mail-in ballots that were mailed prior to Election Day may not be counted at all clerks said.

Ballots also will no longer be automatically considered priority mail, increasing the amount of time it will take to deliver the mail.

“We’ve utilized that successfully over the years to make sure that all those ballots not only are delivered, but they are received in time. Changing that status dramatically changes how that will impact us as well,” Ackerman said.

These changes will be most felt during the recount process, when candidates are “scrambling,” Ackerman said.

The clerks generally agreed this could change the outcome of a race.

Clerks also said they are concerned about changes impacting voter trust in elections, especially because voter guidance is now going to be different depending on the county’s location and distribution center.

“We’ve told all of our voters, get your vote-by-mail ballot in your box by Election Day to be postmarked for Election Day, we will process it now. We’re ripping the rug out from underneath that,” Ackerman said. “That leads to distrust. When you can’t give a solid date, you can’t give a solid answer, when you can’t reply back to them with facts, when it’s left for interpretation. That erodes the trust that we’ve been trying to rebuild.”

In the past, when this potential change was proposed, the clerks said they were able to speak to federal lawmakers to advocate for its reversal.

In late September 2025, a delegation of clerks traveled to Washington, D.C.,

to meet with lawmakers and staff about changes to federal funding for election infrastructure and learned lawmakers didn’t know about the potential changes, Ackerman said.

“They were all unaware that this change was going to take effect. They were unaware, in my opinion, of the impacts it would have on vote by mail,” he said.

This time, however, clerks didn’t know the changes were proposed until days before they were implemented.

“It is our goal to get to the public being 100% trusting of our system, but that’s something we’ve strived diligently to do to repair that relationship, and I think this hurts that,” Ackerman said.

To be sure your vote has been counted, voters can call their county clerk’s office, or some counties offer ballot tracking online.

County clerks emphasized that voting by mail is still safe and secure, but it might take more work and planning.

Kathy Michael, the McLean County clerk, said voters can request to have their ballot manually postmarked at the post office when they return it.

“Just mail it early or go into the post office. I don’t want to say it’s that simple. I don’t want to make it simple, but that’s all you have to do,” she said. “Don’t be discouraged and not do it and not vote. … Get it postmarked, and you can go off on your vacation and not worry about it.”

But if your idea of January is long dark nights, dead batteries, slush, and getting stuck in a snowdrift, maybe you’re ready for a little reprieve. If a trip to Florida, Buenos Aires, or Tahiti isn’t possible, may we suggest a local option: Visit a conservatory. There are several of them in the area. Nichols in Rockford, Olbrich in Madison, the Mitchell Domes in Milwaukee, and Garfield and Lincoln Parks in Chicago all come to mind.

Today we are visiting a small conservatory located in Oak Park, just off the I-290 Eisenhower Expressway. If you are enroute to Chicago or your travels take you to the vicinity of Oak Park, it is a small, yet delightful place to stop and take a break.

While not as large as its aforementioned peers, the Oak Park Conservatory is the third largest conservatory in the

Chicago Area, with over 8,000 square feet containing over 3,000 plants, some of which date back to the conservatory’s founding in 1914.

The conservatory has three rooms consisting of tropical, desert, and Mediterranean display gardens. There are also working greenhouses reserved for the staff.

Outdoors, the conservatory has two more gardens: The Rubenstein Memorial Garden and the Elsie Jacobsen Discovery Garden. The Rubenstein Garden is designed to be a place to relax and refresh. The Jacobsen Garden is designed to draw from the four essential elements: light, water, air, and earth. This garden features species native to Illinois. Musical instruments and a bee skep clubhouse offer young visitors a place to explore.

The Conservatory has two parrots, Sara and Skipper, who live in the tropical room. Sara speaks and might well say “hello” to you.

The Oak Park Conservatory started

out as a community effort to house the exotic plants that Oak Parkers collected while on vacation and brought back to the village. The existing glass structure was built in 1929.

Over the years, the conservatory sadly fell into disrepair. In 1970, plans were made to demolish it. It was described as a place that would make “a perfect parking lot.” Oak Park – a community known for its civic pride and activism –rallied, and the Citizens Committee for the Conservatory saved the day.There is a plaque in the conservatory in their honor.

Today, as part of the Park District of Oak Park, beyond its educational and cultural mission, the conservatory supplies plants for other parks in the village. The conservatory is well-maintained and has a professional staff. The Friends of the Oak Park Conservatory actively support and fundraise for the garden. The conservatory was expanded in 2000 to include staff areas and a meeting room. In 2005, it was added to the National Register of

Historic Places. In 2025, the Park District of Oak Park was awarded the gold medal by the American Academy of Park and Recreation Administration.

The Oak Park Conservatory is very conveniently located at 615 Garfield Street near East Avenue. It is equidistant (3/4 mile) from both Harlem Avenue and Austin Boulevard, both of which have exits from the Eisenhower Expressway. The CTA Blue Line Oak Park Station is in the expressway trench in front of the conservatory. Generally, you can find street parking nearby.

Allow 30 minutes. Admission is free and the conservatory is open daily, 10-4. It is closed on Monday and holidays. Donations are appreciated, with $5 suggested. Reservations are requested for groups of 10 or more. Site: oakparkconservatory.org. Wouldn’t a place like this be a great use for the Belvidere Oasis? Imagine breaking up a long interstate drive with a cappuccino, banh mi, or empanada while relaxing in a tropical forest!

Wade Vance, 89 (almost 90) of Rockford, IL passed away January 5, 2026 at home surrounded by his loving family. He was born February 1, 1936 to Sim & Roberta Vance in Des Arc, AK. Wade was a lifelong resident of Rockford and graduated from East High School. He also served a short time in the US Navy.

Wade was the owner and operator of Creative Plastering for over 60 Years in the Stateline area. Creative Plastering was family owned and included 2 of his 3 sons (William & Dennis). He taught the art of the trade to his boys at a very young age and they worked with him until they established their own businesses in Rockford.

He was a lifetime member of the Eagles club, Harmony Singing Society, 3’Ls club, Lyran club and the VFW. He had a profound love for music, singing, dance and cars especially his 1977 & 1979 Lincolns. Wade met his wife Sandra and they were married February 29, 2008. Together they enjoyed Karaoke, Ballroom dancing and traveling as much as possible. Their most notable trip was to the cruise to the Bahamas.

He is survived by his wife Sandra, sons William & Dennis Vance, stepdaughters Kai Grimm & Kari (Greg) Ebens, sister in law Dixie Vance, Nieces, Nephews & Grandchildren

Wade was the oldest living member of his family and was preceded in death by his youngest son Joey Vance, his parents Sim & Roberta, his siblings Vernon, Roscoe, Betty & Lester Vance.

The family would like to give a special thank you to the Mercy Health Hospice Care and especially to Nurse Suzie and CNA Kathy.

Seed starting with winter sowing

Along with adding beets to my vegetable garden this year, I’m also trying a different approach to seed starting. In past seasons, I would pull out my seed-starting equipment in late February, setting up grow lights and trays for six to eight weeks before planting. This year, I’m switching to winter sowing. This simple seed-starting method uses recycled milk/water jugs to grow seedlings outdoors during the cold months. Popular with both native plant and vegetable gardeners, winter sowing relies on natural temperature cycles rather than traditional indoor seed starting. Winter sowing works by mimicking how seeds behave in nature. Many

native plants and cool-season vegetables are adapted to experience cold, wet conditions before they germinate. By placing seeds in seed starting mix filled containers outdoors, they receive the moisture and cold exposure they need, then start actively growing when spring conditions arrive. If you know of anyone that winter sows, you’ve probably heard enthusiastic response to this concept with many finding that their transplants are stockier, more resilient, and better adapted to outdoor conditions than those started indoors.

You don’t need much to winter sow. Clean plastic milk/water jugs work well because they allow light in while protecting seedlings from wind and extreme cold. You’ll also need a light,

well-draining seed starting mix, seeds, a cutting tool, something to make drainage holes, tape, and labels. As you’ll only be growing these 6-8 weeks before planting, a seed starting mix should be fine.

To begin, containers are cut most of the way around, leaving a small hinge so they can open and close. Drainage holes are added to the bottom and sides, then several inches of moist seed starting mix are placed inside. Seeds are sown according to packet directions, the container is closed and taped shut, and labels are added to withstand winter weather. The containers are then placed outdoors in a sunny area. Because these act as mini greenhouses, check them periodically to ensure they stay moist and open them during warm spells to

prevent overheating.

In northern Illinois, winter sowing typically begins anytime from January through early March for say native perennials and cold-hardy vegetables like lettuce, kale, and broccoli. You could also do this for your warm season vegetables like tomatoes but would want to start April. This timeline is not too different from when I would normally start my seeds.

As with any new gardening project, expect to trouble shoot. You may find that this becomes a yearly habit for you or it makes you return back to seed starting like you always have.

What does the future hold?

Following WW II, the US. dominated world markets and industrial production because the war destroyed the industrial capacity of Britain, Germany, Japan and Russia. The American economy boomed as it was the only nation left with sufficient industrial capacity to rebuild the war ravaged global economy. The US chose to help rebuild the industrial base of Germany and Japan which returned them to major industrial competitors. Russia redeveloped its industrial base and China became a major industrial competitor. Following WWII the United States also became the dominant global military and economic power. As the global economy grew the global influence of the US weakened. Under the slogan of making America Great Again Trump regained the Presidency and chose to change the role of the US in the global economy.

The new model is based on the principal of America First and is focused on restoring its power in the Western hemisphere and making it an attractive place for commerce. The US National

Security Strategy provides an overview of the potential global geopolitical future.

The vision has Russia having a major role in Europe as it has energy and mineral resources. China is seen as controlling Asia as it has expanded its green industry and has established trade relationships in the area. The US is intent on controlling the Western hemisphere for commercial dominance. The Trump prefers that conservative political leaders willing to cooperate with him will be in power in the Western Hemisphere.

The vision helps to explain why Trump intervened in Venezuela and his call for taking over Greenland, making Canada a 51st state and regaining control over the Panama Canal. Other countries like Mexico and Columbia have also been mentioned as targets as well.

The tactics used in gaining control over Venezuela included controlling shipping lanes, tariffs and military intimidation could be used to secure US. commercial dominance.

If melting of the ice sheet of Greenland is expected to reveal a treasure of resources needed in our economy it seems reasonable to assume will the melting

of ice covering the Arctic will also reveal a treasure of useful resources a climate change accelerates. As a participate in a National Science workshop on the Arctic some of the resources were discussed along with the challenges of bringing them into the marketplace.

A new organization is protesting government policies they deem are undermining the rights of citizens guaranteed by the Constitution and the Bill of Rights. It is known as the Indivisible Project. It was created by co-Executive Director Leah Greenberg and her husband and co-Executive Director Ezra Levin. They both hold Bachelor’s degrees from Carleton College, in Minnesota. She was active in politics in Virginia and he was active in Texas.

Their organization is a grassroots movement of local Indivisible groups with a mission to elect progressive leaders, rebuild our democracy and defeat the Trump agenda. They are sponsoring actions such as Stopping Trump’s War in Venezuela, and Reining in Ice Now. The Indivisible Project intends to continue taking action in support of their goals.

There are other activists raising

concerns about the policies being implemented by the Trump Administration including Ralph Nadar. He uses Trump’s views expressed in the New York Times on January 9, 2026 to illustrate concerns about President Trump. “My own morality. My own mind.” restrains him. Not the Constitution, not federal laws and regulations, not treaties signed under Republican and Democratic Presidents. While he took an oath to obey the Constitution his expressed views suggests he does not see it as a reason for limiting his actions.

There are other individuals and groups raising concerns over the current policies of the US government. The public radio network at NIU is providing good coverage of the political situation. The existing differences in political perspectives within our country are likely to intensify as we approach the midterm elections. How and if the conflicts are resolved will impact life in America.

During the turmoil of the 1960s we took a two week vacation and left the turmoil of the world behind us. Sometime it feels like a good idea again.
