BEFORE THE MOVE

Schedule your move Monday - Thursday between 8:30 AM - 5:00 PM

It is important to schedule your move when staff is fully available to assist with communication or logistics.

LOCAL PRIMARY CARE

Please let us know if you are new to the area and looking to partner with a local primary care physician. There can be a delay with getting a new patient appointment. Our team can provide a list of local providers if needed.

MAKE AN APPOINTMENT WITH XFINITY

Please call Xfinity directly at 1-855-638-2855 to schedule an on-site, in-person appointment. Your package includes basic cable (2 boxes) and broadband internet (1 modem). Xfinity will charge you a one-time, $100 installation fee and for any upgrades. Please be available to meet the technician and have a television available to confirm the connection.

Create a work order in WorxHub to hang your television.

MOVE-IN INFORMATION

SCHEDULE YOUR MOVE IN ORIENTATION

Within THREE DAYS of your arrival, please schedule your move-in orientation. Contact Kim Mansfield at KMansfield@RiverMead.org or call 603-371-8746. This valuable meeting is required regardless of actual move-in date

PROVIDE CONTACT INFORMATION

RiverMead Resident Service and Healthcare systems are connected. If you plan to use a landline or a different email than the one provided in your move-in forms please provide that information to Marketing or Resident Services.

VOTER REGISTRATION & TOWN CLERK

Voter registration requires proof of citizenship and must be completed in person at the Town Clerk’s office. For the list of required documents and office hours visit

DOG REGISTRATION

All dogs must be registered in the State of New Hampshire on a yearly basis. You may do this at the Town Clerk’s Office, which is located in the Town Hall building on Grove Street in downtown Peterborough. They will need a copy of your dog’s current rabies certificate.

DRIVERS LICENSE INFORMATION

Within 60 days please update your driver’s license. The NH DMV is now fully appointment based To make an appointment visitl www.dmv.gov/appointments-and-services.

FIRE SAFETY

All lamps, chandeliers, etc. must be UL rated for safety purposes.

A staff member will meet with you within the first weeks of occupancy to discuss fire safety guidelines and to show you your fire escape route

Gas Grills are not permitted on porches, balconies, or directly outside your apartment, villa, or cottage. Electric Grills are permitted on your Cottage outdoor patio Community grills are located at The Mead along the river near the raised garden beds and River Deck

EXTRA CAR KEY

RiverMead requests residents to provide a duplicate car key to Resident Services, so that we may move your car for snow removal and other emergencies. Place it in an envelope with your name on it and turn it in to the front desk of your campus (The Mead or The Village).

Apartment and Villa mailboxes are located in the front lobby in the mailroom. Your mail is sorted by the Peterborough Post Office and placed in your mailbox.

THE MEAD CAMPUS

The Mead Cottages: your address will be designated by your cottage number, on RiverMead Road.

What Health Services are offered here at RiverMead to Independent Living residents?

Tembo- Tembo is a telehealth urgent care available to all RiverMead residents. They are available 24 hours per day/ 7 days per week. When you use Tembo, you speak with a Board-Certified physician within a few minutes. They can diagnose, provide advice or guidance, and order non-narcotic prescriptions. They can order blood work and other testing, some of which can be done in the comfort of your own home. This service is billed to Medicare.

Nurse Practitioners- The Nurse Practitioners can provide the same services as your Primary Care Physician, except the service is on the RiverMead campus. We offer 2 different options. Lauren Stacy, APRN is affiliated with Monadnock Community Hospital and can see you if your Primary Care Physician is at Monadnock Community Hospital. She is here weekly on Tuesdays. Michelle Humphrey, APRN is associated with AlignMed and can see you regardless of where your Primary Care Physician is located. Michelle can also serve as your Primary Care Physician, if you choose. She is here weekly on Thursdays. These services are billed to Medicare. All appointments are managed through Home Health.

Home Health- RiverMead Home Health is a part of the Health Center and primarily supports Independent Living residents. They provide a variety of services including a foot care clinic, assistance with medications, assistance with personal care, pet care, wound care, escorting to medical appointments, educational opportunities, and other tasks that assist Independent Living residents be successful in their own homes. 603-924-0033 x 247. This service is billed monthly on your RiverMead statement.

Nurse Care Coordinator- The Nurse Care Coordinator assists RiverMead residents and families with transitions within the various levels of care offered at RiverMead. This nurse is the coordinator that connects all the different teams involved in the resident's RiverMead experience. She is also a resource for residents and families for complex decisions and conversations. There is not a fee for this service. 603-924-0033 x361

Health Center- The Health Center encompasses three neighborhoods of residents and provides assisted living and memory support services with 24-hour nursing staff. The Health Center also provides respite services (temporary stays) to Independent Living residents that are recuperating from an illness or surgery. 603-924-0033 X 396

Heritage Health Pro- Heritage Health Pro is our on-site physical and occupational therapy team. This team offers a variety of rehabilitation services that can assist residents in maintaining their desired quality of life. This service is billed to Medicare. 603-924-0033 X 364
