â—ŹVisiting hours are every day from 8 am-8 pm (1 person may spend the night)
â—ŹChange of shift begins at 7 am/pm (Our nurses and PCTs will come in the room and do a bedside shift report; Please inform them of any questions or concerns you may have.)
â—ŹOur doctors rotate out on Tuesdays @ 9:30 am
â—ŹBedside rounds (rounds w/ the doctor) begin at 10 am on Monday and 9:30 Tuesday-Friday. Your nurse as well as the case manager will be present during this time. (If you would like to speak to a physician about any questions or concerns regarding yourself or a loved one, please ask/call during this time.)
â—ŹQuiet Time is every day from 1 pm-3 pm
â—ŹBreakfast trays are passed starting at 7 am; you will have to call in your tray at 6:30 am. (You have the option to order all meals at one time) To order your meals, please call 77744.

MANAGING DIABETES AT HOME

Take medication as directed by your healthcare team
Keep a current list of medications with you. Let your doctor know if you have problems with your medications
Check your blood sugar as directed
You will need a prescription for the glucose meter, testing strips and lancets for insurance coverage However, a store-brand meter and testing supplies usually cost less
Check your blood sugar as directed and write it down Take your blood sugar records and meter to every doctor’s visit
Make healthy food choices
Eat a minimum of 3 meals a day and include protein snacks if necessary, so that you eat consistently every 4 to 5 hours while you are awake. Don’t skip meals Eat consistent amounts of healthy carbohydrates throughout the day.
Limit sugar, sweets, fats and fried foods
Be active for at least 30 minutes most days
Ask your healthcare team if you need to check your blood sugar before and after you exercise Drink plenty of water
Go to all follow-up medical appointments

Keep a diabetes identification card or jewelry with you
Keep your blood glucose meter and extra supplies with you (Such as “easy to chew” candy in case of low blood sugar )

Discharge: Preparations for Care Transitions

At Methodist Charlton Medical Center, your safety and comfort are our priorities. Discharge planning begins early in your stay, so please reach out to your nurse with any questions. A social worker can also be involved if needed.

Your doctor will place the discharge order, which starts the discharge process. After this, your nurse will review your medications both home and new prescriptions to ensure they match your doctor’s instructions. Educational materials will be provided to support your recovery at home.
When you ’ re ready to leave, your nurse will review your discharge instructions, including activity, diet, medications, follow-up appointments, and when to call your doctor Let your nurse know if you need help arranging support at home
The discharge process is designed for your safety and may take up to an hour or longer You may wait for your ride in the Discharge Lounge, open daily from 9 a.m. to 5 p.m. Please remember to gather your personal belongings, including your phone and medications, before leaving. If you need a work or school excuse, inform your nurse.
