Clinician’s Corner by the Residents PICO: Influence of Exercise on TMD

A RENEWED & REFRESHED COMMITMENT TO PTSMC’S MISSION The first ever Organization Inservice Roadshow is complete! If you were able to make one of the four sessions you know I spent some time talking about PTSMC’s mission. Having recently read Simon Sinek’s book, Start With Why, I was inspired to look at our mission statement more closely to reflect upon the role it’s played in our success over these past 19 years.

I also found myself inclined to review the “why” of our mission from an internal perspective. With the addition of one word, my focus sharpens to our most valuable asset: OUR PEOPLE

In his book, Sinek profiles a number of successful companies and writes of doing business in “the what, the how, and the why.” One of the points that struck me was the idea that great employees and great companies should know what and how they do what they do; this could mean building motorcycles, flying planes, securing the best prices for costumers, etc.

My deliverable is to focus on the internal reflection of improving the quality of PTSMC’s people’s lives. By doing this successfully, PTSMC employees can and will want to focus on

To me, the most powerful point Sinek made was that truly exceptional companies and their exceptional people perform to the best of their ability when they understand and believe that the “why” of what they do matters. The first line of our mission statement brings PTSMC’s “why” into clear focus:

I am confident that maintaining our commitment to delivering on our mission both externally and internally assures continued growth and success. Growth creates opportunity for all our people and allows us to provide a “PT for Life” experience to the expanding number of people and communities we serve. What you do is extremely important. CLINICIAN’S CORNER Special Article PICO: Influence of Exercise on TMD

P: Temporomandibular joint (TMJ) disorder (TMD) is a broad term encompassing disorders of the TMJ including but not limited to pain, locking, limited opening, and associated anatomical structures. Some of the characteristics of TMD are orofacial pain, restricted range of TMJ motion, joint clicking, and headaches.1 Currently, there is not a gold standard for a physical therapy approach. I: Exercise-based interventions were examined for their effectiveness in improving symptoms with TMD. C: Exercise-based interventions were compared to exercise plus manual therapy in the treatment of TMD. O: Three articles were found searching PT Now via the APTA discussing the effects of performing exercise alone for the treatment of TMD. In a randomized controlled trial in patients with TMJ pain and limited mobility, Bae2 found that both active exercises and relaxation exercises of the masticatory muscles were effective interventions for TMD, but relaxation exercises were found to be more effective.2 Hoglund3 conducted a case study in which a single patient was experiencing an open-locked TMD which he hypothesized was the result of deficits in neuromuscular control. He focused on having his patient perform long incisional biting on a wedge as a form of automobilization, followed by Rocabado exercise protocol. The patient had full mouth closure and was pain free without locking episodes at 2 weeks.3 Oh et al. conducted a study with 12 patients with a lateral shift with opening activities. Each patient was instructed to place the tongue on the anterior palate and open their mouth while maintaining contact to avoid anterior condylar translation 6 sets of 6 reps per day for 4 weeks. Lateral translation was significantly reduced and the lateral shift, mouth opening ratio was significantly improved.1 Cleland conducted a case study on a patient who had a 19-month history of TMD. After eight treatments including manual therapy, exercise, and education, the patient improved their reported pain, and their mouth opening ROM to within normal limits.4 A single blind randomized controlled trial by Craane et al. compared an education group (relaxation exercises, positions to avoid) versus an education plus exercise and manual stretching group in patients that had masticatory muscle pain. Individuals with disc disorders and TMJ arthritis were excluded from this study. Both groups improved, with no statistical difference in terms of A/PROM, pain, and reported functioning.5

Calixtre et al. performed a pre-post test protocol on 12 young women. The intervention consisted of cervical joint mobilization, deep neck flexor stabilization, and a stretching program. They found statistically significant changes in pain, ROM, and reported function in patients with orofacial symptoms.6 In the limited research available on treatment of TMD, there is some evidence that supports the use of exercise intervention and multimodal intervention including exercise and manual therapy. It is difficult to determine whether exercise alone, or in conjunction with manual therapy is superior for TMD treatment based on available research. There are also multiple types and etiology associated with TMD that may benefit greater from different approaches to treatment. Compared to control/no treatment, there is evidence to support use of manual therapy plus exercise and exercise alone for TMD treatment. Evidence was insufficient to conclude that one treatment type is more effective. There is some compelling evidence for inclusion of relaxation exercises in TMD treatment and relaxation exercises should be included in future studies. The researched techniques are cost effective but require some training. For example, controlled TMJ rotation on opening was cited multiple times in this literature review, and technique is critical. Steps that could improve understanding of TMD treatment include finding a common language for subgrouping different types of TMD, high quality RCTs comparing different treatments including active exercise, relaxation exercise, and Manual therapy, along with cohort studies to compare multimodal treatments for subgroups in order to determine best practice. HUMAN RESOURCES Timesheet Completion All employees are compensated Wednesday biweekly for time worked during the previous two weeks from Sunday through Saturday. Each employee must complete a time sheet that reflects all hours worked as well as time off taken. Bi-weekly time sheets must be completed by the end of the work day on the last Friday of the pay period for theprevious two weeks to be approved by your Director or Assistant Director. Employees not completing their timesheet will wait until the following pay period to be compensated.

Lunch Break Reminder

All non exempt hourly employees working 7.5 hours or more in a workday must take a 30-minute unpaid lunch break according to the law. It is not the intent of the policy for employees to either consistently shorten the meal break period in order to shorten the work schedule or to work additional hours.

Southington PSC Michael Sousa Michael has a way of making patients feel special on an individual basis. He gets to know patients and will always check in with them the next time they come in to see how that test was that they were so worried about or how that wedding went over the weekend that they were looking so forward to. As with the nature of our job, most times patients come in who are in pain or discomfort, but he can always get them to crack a smile or laugh because of his outgoing and funny personality. He is the first person to make sure people feel welcome to our “family.” That same funny personality makes our office an enjoyable work place. The crew in Southington wouldn’t be the same without him. I appreciate all his efforts as a co-worker and know our patients are given special care from that first phone call to their discharge date. Bragger: Anonymous Avon PSA Nancy Chilson I think everyone in the Avon office would agree that Nancy is truly the glue that holds our entire office together. She is extremely organized, efficient and just plain amazing at what she does every day. Nancy is able to multitask and communicate with all types of people in a way that not only enhances patients’ experiences but keeps the entire staff on track. Her willingness to go above and beyond is also not limited to our Avon office. Whenever help is needed in other PTSMC offices, Nancy will fill in at the front desk and spend time training new hires. She is extremely witty, friendly and positive on a daily basis, even when it’s a hectic day. It’s a pleasure to have Nancy as a coworker and she is more than deserving of a BRAG award. Bragger: Kristen Ludwig and Liz Melvin Avon PSA Nancy Chilson The best way to describe Nancy is that she is the oil that keeps our clinic working like a machine. She constantly makes sure to keep the therapists informed and up to date on what we need to do, strives to help patients to the best of her abilities and often is at work before and after her shift ends. She makes sure to assist the other front desk ladies in any way possible as well. She does this all with a smile and in a nice way. Trust me, we would all be lost and confused if it was not for Nancy. She is the true meaning of PT for Life. Bragger: Anonymous Avon Front Office Staff: Nancy Chilson, Chrys Graboski, Liz Melvin We would like to nominate the entire front desk staff for a BRAG Award. They, as a team, keep the Avon office from steering into the rocks. They always greet the patients with a smile, and so many of the patients often acknowledge and compliment them for their friendly demeanor and assistance in scheduling their appointments. Paperwork, flow sheets and new patient registrations are organized and complete. They work diligently scheduling patients, trying their hardest to accommodate both the patient and the therapist. They keep the therapists in line with documentation, which is extremely helpful for authorizations and meeting end of month deadlines. A special shout out to Nancy on this one-for keeping us on task with outstanding notes and ASHLinks! We don’t say it enough, but you are greatly appreciated!! Bragger: Avon Clinical Staff Avon PT Aides: Peter Inserillo, Conner Gavin, Cody Filingeri, Shelby Howe, Leigh Holahan, Ally Condo To the aides of Avon. For maintaining controlled chaos! We truly could not function without you. The staff is grateful for your hard work and the patients are lucky to have you. Thanks for working your tails off-it doesn’t go unnoticed! Bragger: Avon Clinical Staff Middletown PSC Jacky Severance Jacky is such a positive and enthusiastic PSC as well as a pleasant and kind person in general. Middletown was already a nice place to work, and she has made it even more so. She is awesome. She is always willing to jump in and learn new skills including some non-administrative tasks like laundry and taking ice off patients if the PTs and Aides are super busy. She has made an effort to engage patients in giving us google reviews, which is so great for our clinic!! She has gone out in her personal time and bought some decorative things to put around the clinic. She has had to deal with difficult and awkward situations already and has done so with complete professionalism. She is a great addition to Middletown!! I am so impressed at what a difference she is making in such a short amount of time. Bragger: Danielle Butsch Waterbury PT Aide Carol Corbo Carol is a staple in the Waterbury office. She comes in everyday with tons of energy and a smile on her face. Returning patients always remember her – even the ones from a long time ago that went to the Vernon Street office. It amazes me that she can remember everyone’s name, their family member’s names, the street they grew up on, where their kids went to school, etc. I have seen Carol keep an eye out for patients she knows are coming in that need help with the door. They know she will be there to assist them in and out if they have a wheel chair or just need a hand walking. She is always helping the community with her coat drives, book drives, and food bank drives. She keeps in contact with former patients that work in the community and always asks them what their needs are so she can continue relationships with them. Carol is a HUGE help to me at the front desk when I am flying solo. She can be taking care of 5 patients yet still manages to answer the phone if it rings more than twice and will help me schedule patients. I do not know what we would do in the Waterbury office without her! Bragger: Tracy Caligiuri


Wallingford PT Ashley Tighe Ashley has been an amazing addition to the clinic. As a new grad, her zest for learning and being at the top of her game is wonderful to see every day with her patients. The relationship that she establishes individually with her patients is unique to each person and her patients respect, trust and care about Ashley. That is truly PT for Life. Walking into a clinic as a new professional is never easy and can often be intimidating--Ashley never lets that show. She is truly an asset to us in Wallingford and I am looking forward to continue working side by side with her and watching her grow professionally. Bragger: Danielle Dunn Guilford/Wallingford PT Bailey McPherson Bailey has been an amazing addition to the clinic. She has hit the ground running and provides top-notch care for all of her patients in busy clinic environments. It is never easy to work between two offices due to different flows and co-workers; however, Bailey handles it with grace and a smile. She is intuitive with all aspects of care that can affect her patients and tailors her care for each patient in front of her. Bailey is always striving to learn and give the best possible care for her patients. Her patients trust and respect her which embodies PT for Life with the positive relationship Bailey establishes with all of her patients. I look forward to continuing to watch Bailey grow as a PT and continue to provide top-notch customer service and clinical excellence. Bragger: Danielle Dunn Customer Experience Coordinator Alison Pearce (covering as PSA in Guilford) Alison’s PT for Life actions are improving the quality of people’s lives. I am dedicating this brag award to Alison Pearce because she has gone above and beyond for our clinic in Guilford. Our Full Time PSA is out on maternity leave and Alison has stepped up (as always) to help us maintain a stable and organized system up front while we are trying to catch up and adapt to a new system. Our entire clinic is extremely thankful for the extra hours she has contributed here, all while maintaining several other administrative tasks. Alison is a true team player at PTSMC and is always a helping hand! Bragger: Amanda Bozzuto Guilford PSC Amanda Bozzuto Over the past number of months, the Guilford front office has gone through A LOT of changes in an already very fast paced environment. Amanda has dealt with a million things coming at her from every which direction: patients, staff, phone calls, etc. -and she still maintains a smile and is happy to help the next person with whatever they may need. A situation that may have negatively affected many others, Amanda has handled with grace and efficiency. She has kept this place afloat in a time of transition. Bragger: Anonymous Guilford Assistant Director Sue Ginsburg Sue has been an extreme help this year by extending her clinical time. It has been a rough year with the growing office, but Sue has continued to assist the clinic in a heart beat. She doesn’t always get recognized for what she does, due to her focus to get through her “list”. Although she deserves more, I want to show how much she is appreciated in front of others. Bragger: Anonymous Guilford PT Aide Danielle Spinella Danielle has been the sunshine in our office that we have needed. With her walking into the office on Fridays after testing or a stressful day at school then braving RT I95, she will always greet you smiling “HAPPY FRIDAY”. It has become one of the many things she does that brings joy to the office. Patients love her endearing personality and the staff loves how versatile she is, from “aiding” to assisting the front desk to helping out behind the scenes. We lost her for a little during clinical rotation and are excited she is back. Hoping one day she will join the PTSMC family, post graduate school. Bragger: Guilford PTs Guilford PT Aide Kyle McCormick Having a patient set up correctly and on time for treatment, answering phone calls to facilitate front desk efficiency, eloquent explanation of exercise technique to patients, a compassionate ear for patients or therapists to vent, a positive attitude in every situation, and genuine friendliness. These are all things Kyle brings to our Guilford office every day. He understands how each therapist ideally operates and goes above and beyond to help our day run smoothly. He is not only a model for new/aspiring PT Aides, but for everyone in our office. His patience, kindness, and upbeat mood are infectious. Any PT school will be lucky to have him as a student. He will be a great asset to our profession. Bragger: Guilford PTs Groton PT Lauren Peladeau It’s hard to believe Lauren is only a year out of school, because she provides such exceptional care to all of her patients. She works very closely with each and every one of her patients and consistently puts her patients’ needs before her own. She is supportive, compassionate, and always brings positive energy to the Groton clinic. Lauren is not only a great therapist within the clinic, but she is always the first to volunteer for opportunities in the local community. Whether it be through journal club, mentorship meetings, additional courses, or national meetings such as the APTA’s Combined Sections Meeting, Lauren is eager to learn and refine her skills as a practicing physical therapist. Lauren and I both joined the PTSMC community about a year ago and during that time she has truly become a great coworker, role model, mentor, and friend. This past year has brought about many changes in my life, both good and bad, and Lauren is always there to support me. Whether it be graduating from college or being accepted into DPT programs, Lauren is always the first to congratulate me when I walk into the clinic! As a PT Aide, she makes me feel included in the PTSMC community and always thanks me for my help at the end of each day.
