Welcome to The Mount Since the 1924, Providence Mount St. Vincent has been providing compassionate care for older adults and those needing transitional care between hospital and home. Today The Mount is home to nearly 300 residents and over 1,000 patients are treated annually in the Transitional Care Unit. Our services and programs include:

As expressions of God’s healing love, witnessed through the ministry of Jesus, we are steadfast in serving all, especially those who are poor and vulnerable.

• Transitional Care Unit—Short stay rehabilitation, for recently discharged hospital patients who need skilled nursing care and/or rehabilitation services. • Assisted Living Apartments offer a program that empowers each resident to make her or his choices for daily living and emphasizing dignity and respect for all. The Mount has 109 apartments, with Independent Living, Respite and Short Stay also available. • Long Term Care in specially-designed neighborhoods for 149 residents, offers 24/7 nursing care based on person-directed care, individual choice and compassionate care. • Intergenerational learning center, a licensed child care center for 125 infants and children. Internationally known for excellence, innovation and intergenerational programming. • Adult day health program through PACE (Program for All Inclusive Care for the Elderly). Monday – Friday enhanced day time care offers nursing, rehabilitation, case management and activities. From Our Administrator Since its founding, the mission and purpose of Providence Health & Services, and Providence Mount St. Vincent, “The Mount”, has been to improve the health and wellbeing of each person we serve. As The Mount has evolved over the past 100 years, our mission has remained the same.

We have created this booklet as a guide to help ease your way and answer many questions that may arise. Make Yourself at Home at The Mount We want your stay at The Mount to be comfortable and successful. Here is some information that will help you get to know The Mount better. FOOD At PMSV, we believe in “real food”, not processed food, as a way to improve health and quality of life. Dietitians are available to assist residents with choosing foods that will provide enjoyment as well as to heal and build strength as needed. Food is always available free of charge for Mount residents and patients. • Café/Bistro on the 2nd floor for tray line service and a separate Dining Room for waited service. Food is free for residents and patients; guests are welcome to dine at regular prices. Menus are posted on the weekly schedule, at the entrances to the Bistro and Dining Room, and by telephone, Ext. 28740. Guests are welcome to purchase food in the Café/Bistro, the Dining Room or the Gift Shop. SHOPPING AND MORE • The Gift & Coffee Shop is located on the first floor. Food as well as coffees, snacks and beverages are available for purchase. The Gift Shop also features a lovely selection of gifts, gourmet candy, and toiletries. • Emilie’s Treasures Thrift Shop is a volunteer-run destination at The Mount to find unique treasures. • The Sewing Shop is also a volunteer-run initiative for those needing minor clothing repairs and alterations, with many handmade and other items for sale. ACTIVITIES AND OUTINGS Throughout most days, there are opportunities for you, your family and friends to attend events, classes and presentations and to participate in activities and outings. Each week there is an in-house calendar of activities and events published. Your recreational coordinator or therapist will provide you with a copy. Classes and activities may include yoga, exercise, volleyball, baking group, creative writing groups, art classes, shopping, trivia and much more. Your recreational coordinator or therapist will let you know about outings. Family and friends are welcome to attend in-house events. If you would like to take a friend or family member along on an outing, please make arrangements in advance with your recreational coordinator or therapist. 4

CHAPEL/SPIRITUAL CARE The Pigott Chapel is located on the 3rd floor. The Chapel hosts regularly scheduled Catholic Masses and Protestant services as well as other events, visiting choirs and musicians. Always open and accessible to residents, family and friends, the chapel is a quiet place for meditation, reflection, prayer and worship. Chapel services can also be viewed on TV Channel 3. To request a chaplain to visit or other personal or family support, dial internal ext. 28335. INTERGENERATIONAL ACTIVITIES AND CLASSES The Intergenerational Learning Center (ILC) brings the children and residents together. Art and music classes, sing-alongs, service projects and visits from the babies and children to the neighborhood are just a few of the offerings. ILC activities and classes are sponsored by the Providence Mount St. Vincent Foundation. The ILC is a licensed childcare center, owned and operated by The Mount and located within Providence Mount St. Vincent. The ILC serves children ages six weeks to 5 year or pre-kindergarten. Open five days a week, six bustling classrooms serve as home base and the children engage in many visits and activities with residents throughout each day. MORE AMENITIES • Moe’s Place: During your stay with us, you may either use your own personal device, or the computers in Moe’s Place, on the 1st floor across the Gift Shop, or in the Long Term Care Library, on the 4th floor. • Internet: The Mount has an open wireless network, “Prov Guest”. • Mail: Apartment residents will have their mail placed daily in their designated mailbox. In the neighborhoods, your mail will be delivered to your room daily, Monday through Friday. TCU patients will have their mail delivered on Monday, Wednesday and Friday. If you have asked someone to assume responsibility for your mail, we will forward mail to this designated person at your request. • Telephone: To make outside calls from your room phone, dial 9 plus the area code (even if local) followed by the telephone number. For internal calls, just dial the extension or dial “0” for an operator between 7:00 a.m. and 11:00 p.m. For incoming calls, your friends and family may call (206) 937-3701 to reach our automated answering system. At the prompt, they will need to enter 18 + (your apartment number) to be connected directly to you. If calling in for a nursing center room, enter 3 + (room number) + (bed number). To reach SJR residents or for other inquiries, press 0 to be connected to the front desk. Continued on next page 5

• TV/Cable: Cable service (Dish Network) provides 75 channels which includes in-house Channel 6 and two nature/relaxation Channels 8 and 10. In-house Channel 3 shows Mass and special events being held in our 3rd floor chapel. • Housekeeping: Housekeeping and cleaning are done on an ongoing basis to keep the building and your room clean, safe and wellmaintained. Staff is also available for special requests or emergent needs. Please feel free to request help and discuss concerns with the leader or any other caregiver in your area. • The Mount’s Clinic: The Mount’s Clinic offers dental and vision/ optometrist appointments by scheduling. The Clinic is located on the first floor down the hall from the Gift Shop. IMPORTANT INFORMATION • Visiting Hours: Families and friends are encouraged to visit. Visiting hours are not limited. We ask you to be considerate of the privacy and comfort of others at all times, especially during quiet hours (8:00 p.m. to 7:00 a.m.). • Pets: Pets (dogs and cats) are allowed only for Assisted and Independent Living residents upon facility approval. Visitors may not bring pets to Providence Mount St. Vincent, unless they have been approved as a Therapy Dog (cats not allowed). Please check with the front desk for the further information and required forms. • Interpretive Services: Please let staff know if you require interpretive services. • Safety: Please do not use extension cords or multiplex outlets. At your request, we can provide you with an approved powerstrip which will provide additional electrical outlets. To comply with Washington State fire code, when storing items on the top of the armoire, leave at least 18" of clearance to the ceiling. Fire code also prohibits any open flame (no candles allowed) and items such as heating blankets and space heaters. Items cannot interfere with opening, closing and sealing doors (ie: door hangers over the tops of doors). Unannounced fire drills are conducted each month to maintain readiness in the event of a real fire. Please follow staff instructions during these drills. During a drill, elevators are not available and movement between areas in the building is restricted. • Infection Prevention: We kindly ask guests to wash your hands before visiting and to postpone your visit if experiencing any illness in recent days. These simple actions are vital in minimizing health risks to the patients and residents you are visiting, ensuring a safer environment for everyone. 6

• Resident Council: Resident Council meets to discuss ideas, issues, and concerns within the community. Meetings generally take place in Assisted Living quarterly and monthly in Long Term Care neighborhoods. Ask your social worker for specific meeting dates and times. • Sign In/Sign Out: Please let us know when you will be out of the building with family or friends, or for appointments not arranged by staff. Please tell staff about your plans. • Parking: Free parking is available for visiting friends and family members on campus and surrounding streets. Please observe all signs and do not park in fire zones and other prohibited areas. Do not park in areas marked for Saint Joseph Residence (SJR) which is strictly for their residents and visitors or the lot marked for PMSV residents only. • Smoking, Alcohol, Marijuana: We are a smoke and drug-free facility which means smoking (any forms of tobacco products) is not allowed in the building or surrounding area. Assisted living residents are not restricted in their alcohol use. Alcohol consumption is not permitted during a short-term rehab stay; any alcoholic beverages must be locked in a safe area by a Neighborhood Nurse Manager. For long-term care residents, we will collaborate closely with healthcare providers to develop safe and appropriate alcohol consumption plans, if deemed safe. This ensures both safety and compliance with health guidelines. As a recipient of federal Medicare and Medicaid funds, The Mount follows all federal laws. Since marijuana is still illegal under federal law, we will not be able to honor requests for medical marijuana. • Caregiver Recognition and Tips: If you would like to recognize a caregiver or work group for doing a good job or providing a special service, please make a donation to the Providence Mount St. Vincent Foundation. Call (206) 938-6194 or donate online at www.providence. org/themountdonate. The caregiver or team will be notified that a gift has been given in their honor. See page 12 for more information. Under Providence guidelines, employees (including volunteers) are not allowed to accept gifts of cash or cash equivalent such as a gift certificate or gift card. Non-cash gifts (i.e. fruit basket or candy) maybe given to an individual employee to be shared by department staff. • Volunteer Services: The Mount is supported by a dedicated group of over volunteers, whose ages range from late teens to those in their nineties. These volunteers generously contribute their friendship, talents, and services to enhance the lives of our residents and patients, fostering a warm and supportive community environment. If you or a family member are interested in volunteering, call (206) 937-3700 (internal ext. 28170). 7

Assisted Living Apartments YOUR MOUNT TEAM Providence Mount St. Vincent uses a team approach to help the Assisted Living residents achieve their goals. The Assisted Living interdisciplinary team consists of the following members: • You and Your Family: You are at the center of, and will direct, how we care for you and/or your loved one. Your participation in your care plan is vital to your experience and satisfaction. • Director: Leads the nursing and caregiving staff. He or she serves as a guide and primary point person responsible for all the people and services that support you throughout your stay. Dedicated to your safety, well-being, health and satisfaction, the director will get to know you personally and work with you to assure that you are receiving appropriate and individualized care provided with dignity and respect. • Nurses and Resident Assistants: Our nurse supervisor, staff nurses and resident assistants are available daily to assist in management of your medical needs. • Social Worker: Your social worker provides support to you and your family during your stay. • Recreational Coordinator: Through recreational therapy, throughout most days there are opportunities for you, your family and friends to attend events, classes and presentations and to participate in activities and outings. Classes and activities may include exercise, baking group, creative writing groups, art classes, bingo, trivia and much more. Ask your recreational coordinator about the weekly activities calendar. • Spiritual Care Team: A chaplain is available to provide emotional and spiritual support to you and your family within the context of your faith tradition and preferences. Weekly Mass, rosary and protestant services are held in our chapel on the 3rd floor.

Not sure if our Senior Living Apartments are for you? Try the Respite & Short Stay options! The Mount offers fully furnished apartments for respite and short stays, available to rent by the week or by the month. Whether you or your loved one are considering a permanent move or are in need of respite care, a short stay at The Mount will give you a chance to find out why we say “The View Is Different Here.” Independent Living at The Mount is also available. As an independent resident, you can enjoy The Mount community independently, with the peace of mind of knowing that extra assistance will be available if needed in the future, without needing to move. 8

Skilled Nursing/Long Term Care (LTC) YOUR MOUNT TEAM Welcome to the Neighborhoods! Here at The Mount, we use a team approach to help you achieve your goals. The interdisciplinary team consists of the following members: • You and Your Family: You are at the center of, and will direct, how we care for you and/or your loved ones and the services that are provided. Your participation in your care plan is vital to your experience and satisfaction. • Dietitian: The dietitian helps plan your diet and will educate and instruct you and your family regarding any special needs you may have. • Neighborhood Nurse Leader: The Neighborhood Nurse Leader leads your neighborhood and caregiving staff. He or she serves as a guide and primary point person responsible for all the people and services that support you and your neighborhood throughout your stay. Dedicated to your safety, well-being, health and satisfaction, the Neighborhood Nurse Leader will get to know you personally and work with you to assure that you are receiving appropriate and individualized care provided with dignity and respect. • Nurses: Staff nurses are available 24-hours a day to assist in management of your medical needs. These include, but are not limited to, pain, wound and medication management. • Provider: The physician and/or nurse practitioner will meet with you and your care team to evaluate your progress, direct your treatment, and prescribe medication, therapies and services. • Social Worker: Your social worker provides support to you and your family and is also available to help you with life events and planning including preparing an advance healthcare directive, establishing surrogate decision-makers and applying for financial assistance, if eligible. • Recreational Therapist: Through recreational therapy, throughout most days there are opportunities for you, your family and friends to attend events, classes and presentations and to participate in activities and outings. Classes and activities may include exercise, baking group, creative writing groups, art classes, Bingo, trivia and much more. Ask your area’s recreational therapist about the activities calendar. • Spiritual Care Team: A chaplain is available to provide emotional and spiritual support to and your family within the context of your faith tradition and preferences. Catholic Mass, rosary and protestant services are held in our chapel on the 3rd floor. Ask your care team if you would like a visit from a chaplain or Eucharistic minister, want communion and for a schedule of Mass and religious services. Continued on next page

PROGRAMS & SERVICES • Care Planning: You will be kept informed about your health condition and have the opportunity to actively participate in the preparation and updating of your care plan. If you wish, or if you’re unable to make care decisions independently, you can choose someone to assist you. Your care team will review your personal care plan at least quarterly to ensure it aligns with your needs and goals. Family members are encouraged to join your annual care conference, or more frequently if there is a significant change in your condition or upon your request, to ensure a comprehensive and collaborative approach to your care.

•Y our Belongings: You are encouraged to bring favorite items (photos, pillows, lamps, a twin-size blanket, etc.) to help make your room familiar and comfortable. Please avoid bringing in expensive items. For valuables, feel free to use The Mount’s safe. Make sure to check in with the neighborhood nurse leader or social worker when bringing in personal items so they can log. Please discuss with your caregiving team any personal furniture you would like to bring to ensure that it will fit.

• Transitional Care/Short Stay Rehabilitation: If you are hospitalized and are discharged with skilled nursing or therapy needs, you can receive these services on your neighborhood.

• L aundry: Our caregivers will label your clothing and do personal laundry at no extra charge. If you prefer, your family may do your laundry on the neighborhood or at home. Minor clothing repairs and alterations may be requested through The Mount’s “Sewing Shop,” served by a group of volunteers located on the first floor.

• Bed Hold in Long Term Care: If you leave the facility, you have the option to pay for a bed hold to secure your return to the same bed. If you choose not to hold a bed, you will be admitted to the first available bed upon your return, which may not be the same one you previously occupied. Your social worker or Neighborhood Nurse Manager will provide more detailed information and discuss this option with you and your family should the situation arise.

•T V: A group television is located in your neighborhood dining/gathering area. If you would like to have a TV in your room, we recommend a 19" screen to fit in your personal space. You will need to provide a TV stand or mounting frame. Engineering/Plant Operations will install the mounting devices. Direct TV provides cable television service at The Mount. A current TV guide is available in each resident room and available on request. •R epairs: Engineering staff are available to assist you with repairs or special projects such as hanging pictures. To place a request for services, please ask a member of your care team to call the Engineering Work Request Line at Ext. 28699.

TRA N SITION A L CARE UNI T ( TCU)

Transitional Care Unit (TCU) YOUR MOUNT TEAM Providence Mount St. Vincent uses a team approach to help with your recovery while your stay with us. It is critical that you actively participate in your rehabilitation and recovery and that The Mount fulfills its obligation of meeting your every need during your stay with us. The interdisciplinary team consists of the following members: • You and Your Family: You are at the center of, and will direct, how we care for you and/or your loved one and the services that are provided. Your participation in your care plan is vital to your experience and satisfaction. • Provider: The attending physician and/or nurse practitioner will meet with you and your care team to evaluate your progress, direct your treatment, and prescribe medication, therapies and services. • Nurses: Staff nurses are available 24 hours a day to assist in management of your medical and therapeutic needs. These include but are not limited to pain management, wound care and medical oversight. A nurse will also review with you comprehensive discharge instructions prior to your leaving The Mount. • Social Worker: Your social worker provides emotional support to you and your family during your stay and will also help you with discharge planning needs. If your stay is longer than expected, your social worker is available to help you with life events and planning including preparing an advance healthcare directive, establishing surrogate decision-makers, and applying for financial assistance, if eligible. • Recreational Therapist: The recreational specialist uses activities and interests to promote your independent functioning, ease community reintegration and enhance the skills you will need after your discharge. • Physical Therapist: The physical therapist (P.T.) works with you to improve your muscle strength, balance, walking and other physical abilities. Your P.T. may also treat any muscle or joint-related pain you may have. • Spiritual Care Team: A chaplain is available to provide emotional and spiritual support to you and your family within the context of your faith tradition and preferences. Catholic Mass, rosary and protestant services are held in our chapel on the 3rd floor. Ask your care team if you would like a visit from a chaplain or Eucharistic minister, want communion and for a schedule of Mass and religious services. Continued on next page 11


T RANSI TI O NAL C A R E U N IT (TC U)

• Occupational Therapist: The occupational therapist (O.T.) will assist you with activities of daily living such as dressing, bathing, grooming and eating. The O.T. will also assist you with upper body function, balance and adaptive equipment and may also provide a home safety evaluation prior to your discharge. • Speech Language Pathologist: For individuals experiencing difficulties with communication or swallowing, the Speech-Language Pathologist (SLP) provides specialized therapy to enhance speech, language, voice, and fluency. The SLP also addresses cognitive-communication challenges and collaborates with healthcare professionals such as registered dietitians to manage any related nutritional concerns, ensuring a comprehensive approach to improving overall communication and quality of life. • Dietitian: The dietitian is a nutrition expert who helps design personalized eating plans tailored to meet your health needs and goals. They provide guidance and education for you and your family on managing special dietary requirements, whether due to medical conditions or lifestyle choices. During your stay at The Mount, the dietitian also monitors your nutritional intake, weight, and wound healing to ensure optimal health and recovery from a nutritional standpoint. • Your Belongings: You are encouraged to bring favorite items (photos, pillows, lamps, a twin-size blanket, etc.) to help make your room familiar and comfortable. Please avoid bringing in expensive items and please check in with the neighborhood nurse leader or social worker when bringing in personal items so they can log. GOING HOME Before you leave, make sure you understand your bill. How much will you have to pay? What is the breakdown of fees? What part of the bill will be paid by insurance? For more billing information, please see page 13. As you are preparing to leave, make sure that you understand any medicines you need to take, why, and what side effects to watch for. Also make sure you understand your condition/illness and what symptoms to report, what activities are safe for you and what you can eat. Getting ready to leave should also include arranging for transportation, understanding and arranging for any equipment you may need. We can help you make sure that follow-up appointments with your doctor are made and all your questions have been answered.

Billing If you have any concerns or questions, call Providence Billing at 833-4279009 or visit the Business Office on the first floor. (Business hours may be limited so please check with the Reception Staff.) Please inform the Business Office as soon as you are able of any change in insurance or other circumstances that would affect billing. The Billing Office also offers trust accounts and safekeeping for Long Term Care and Apartment residents. Residents who have a trust account with us, can also benefit from check cashing services. Medicaid: If you meet Medicaid criteria, your social worker will assist you with the completion of the application process but you or your representative are responsible for completing and submitting the application to the Department of Social Services (DSHS). Once approved for Medicaid, you will be responsible for payment of your participation as directed by DSHS as well as charges not covered by Medicare and Medicaid. Please discuss your financial situation with your social worker. LONG TERM CARE RESIDENTS You will receive a bill on or about the 10th of each month — according to the length of your stay — which includes a summary of items and services for charges incurred in the previous month, payments posted and pre-billing for the current month’s basic daily rate. Prompt payment is expected upon receipt of the bill. The basic daily rate covers nursing care, spiritual care, food and nutritional services, social services, routine personal grooming, hygiene and hygiene items, laundry, recreational programs, housekeeping, basic utilities including telephone and cable TV, standard wheelchair, cushions and walkers. Ancillary charges for items and services not included in the basic daily rate will be charged separately to your bill. TRANSITIONAL CARE UNITS PATIENTS Medicare and Supplemental Insurance: We will ask you to provide us with photo identification, your Medicare card and a current insurance card for any supplemental insurance you have so that we can bill accordingly. We will bill Medicare for services provided during your Medicare-qualified stay and courtesy bill your supplemental insurance for coinsurance days not covered by Medicare.

Your Patient Rights We invite you to be aware of your rights as a resident or patient at The Mount. We hold your rights as highly important, and will do everything possible to make sure that your rights are respected. Privacy and dignity: You have the right to be treated with consideration, respect, and full recognition of your dignity and individuality; to have privacy during treatment and personal care; to have confidential treatment of your personal and medical records: to have privacy during visits by your spouse or significant other. Information: You have the right to be fully informed in writing, on admission, of all rights, rules and regulations which govern conduct; to be informed of the changes if policies pertaining to your rights are amended; to be fully informed of services available and related charges. Medical condition or treatment: You have the right to be fully informed of your medical condition unless medically contraindicated; to be encouraged to participate in the planning of your medical treatment; to be given the opportunity to refuse treatment; to be free to refuse to participate in experimental research or give written consent to participate. Discharge or transfer: You have the right to be transferred or discharged only for medical reasons, for nonpayment, or for your welfare or that of other residents; to be given reasonable advance notice to ensure orderly transfer or discharge. Privacy of communication: You have the right to associate and communicate privately with persons of your choice, unless this infringes upon the rights of others, or unless medically contraindicated; to send and receive personal mail unopened; to have reasonable access to the use of a telephone where calls can be made without being overheard. Activities: You have the right to participate in social, religious and community activities of your choice unless medically contraindicated. Personal Possessions: You have the right to retain and use your personal clothing and possessions as space permits, unless to do so would infringe upon the rights of others, or unless medically contraindicated. Facility policies and procedures exist to safeguard your personal property.

Delegation of rights and responsibilities: You have the right to delegate your rights to a legal guardian or other representative; to be assured that written policies and procedures exist to initiate recommendations of guardianship proceedings when a resident appears to be incapable of understanding his/her rights. Exercising your rights: You have the right to exercise your rights as a citizen or resident of the United States and a resident of this facility; to submit complaints and recommendations, and be free from restraint, interference, coercion, discrimination, or reprisal from The Mount when exercising your rights. Financial Affairs: You have the right to handle your personal financial affairs; to request in writing that Providence Mount St. Vincent handle some or all of your money for your personal use. Restraints: You have the right to be free from any physical or chemical restraints imposed for purposes of discipline or convenience, and not required to treat your medical symptoms. Work: You have the right to be free from performing services for Providence Mount St. Vincent, unless such work is part of your plan of care. Abuse and neglect: You have the right to be free from actions and practices involving the infliction of physical injury, mental abuse or neglect. To report a problem, (1) Immediately tell a staff person; (2) Also tell a Neighborhood Nurse Manager or person in charge. If you do not receive a response within 24 hours, tell them again or call the Administrator or the Director of Clinical Services (see Pg 11).

Medical Care Directives About “Living Wills” and other advanced directives Advanced directives are written instructions telling us what medical treatment you do and do not want in case you are unable to make those decisions yourself. Before creating and signing these documents, you are encouraged to talk it over with family members, friends and clergy. During admission as a patient, you will be asked if you have a Health Care Directive (sometimes called a “living will”), and if you have a Durable Power of Attorney for Healthcare (which designates someone to make medical decisions for you if you are unable to yourself). If you have these documents, please share them with your nurse or social worker who will make sure they are part of your medical record. POLST In addition to advance directives, patients may choose to include a document obtained from their provider or social worker called a POLST form. POLST stands for Physicians Order for Life-Sustaining Treatment. It is designed for individuals with health conditions, particularly in emergency, life-threatening situations. Recommended for anyone with an advanced, life-limiting illness or chronic frailty, the POLST outlines patients’ wishes as specific medical orders regarding the types of lifesustaining treatment they desire or do not desire at the end of life. Your Medical Records How we protect your personal health information: Providence handles your health information in strict accordance with federal privacy laws. We will not share this information with anyone except for other healthcare providers who require it in order to treat you.
