Community Appointment Day Invitation - Chelmsford - July

Dear Sir/Madam, You have been referred to the Provide Physiotherapy Service and we are inviting you to attend a Community Appointment Event. This has been organised with the aim of moving away from traditional healthcare boundaries and combining a comprehensive range of MSK services, including assessments, advice, health promotion, rehabilitation and community and voluntary sector support, all in a nonmedicalised environment and all on one day. This is an opportunity for us to work together to understand what is important to you and to get you access to what you need all in one place. This is something new and very different. Date – Thursday 17th July 2025 Time – Appointments available between 0900 and 1530. Location – Chelmsford City Football Club, Salerno Way, Chelmsford, CM1 2EH. (Far right hand corner of the car park as you drive into the entrance). Parking – parking onsite. What to bring – pen/pad, comfortable clothing, a bottle of water How to book – you will be sent a separate link to book an appointment. If you are unable to access this, please use the Care Co-ordination centre contact details below. What to expect? When you arrive, you will be greeted by a member of our team and checked in. During this appointment you will have opportunity to discuss your specific musculoskeletal (MSK) problem and ‘what matters to you’. There will be no individual clinic rooms, but there will be the entire experienced physiotherapy team on hand, as well as a range of non-medical services from the community. The aim of the event is to focus on what matters to you as a whole person and not just individual elements of pain or conditions. Your appointment may take up to 90 minutes depending on the services you choose to engage with. Please be aware that this will be an open environment and if you wish for a
