At Crossroads Dermatology, one of the most frequent problems brought in by seniors is very dry skin. “It’s so dry in Colorado with very low relative humidity, that dry skin is the most common problem we see,” said Sarah M. Bair, MD. “It’s a quality-of-life thing. If you are itching and your skin is bothering you, you can’t even concentrate. The first thing I recommend is changing soaps, since a lot of soaps are very drying. For example, I would switch soaps like Irish Spring and Zest for something like Dove Unscented for Sensitive Skin. I recommend putting on skin cream right after the shower when your skin is still damp. If you’re using over-the-counter creams, try a cream in a jar because you’ll get better hydration than lotion in a bottle. Again, get unscented like CereVe or Vanicream. Some dry skin could be dermatitis or eczema and then I would prescribe something stronger.” Skin cancer can sneak up on seniors because of the cumulative effect of sun exposure. will get skin cancer. Sunscreen is necessary when you’re going out, and you would check it regularly. Seek shade, avoid the peak hours of 10 a.m.-2 p.m. Wear a hat and long sleeves. People who know they need special protection should check with their primary care doctor. If you have had skin cancer, a recent study showed that niacinamide is helpful for your protection.”

Mary Baughman, senior program director for health and wellness for the YMCA of Northern Colorado, said that a pandemic pause was put on seniors and now it’s important to connect with others by doing physical activities to prevent problems and maintain health. She is most excited about the YMCAs new initiative with Natural Grocers. Their nutritional health coaches are coming into the Ys to teach a variety of classes. Every month there will be a different talk ranging from immune support to skin health and more and the classes are free to members. “One thing that stands out with the Y,” Baughman said. “The community comes together to exercise but the Y offers so much more. Applying for Medicare can be an intimidating process that boils down to four letters, A, B, C and D. Original Medicare consists of Part A and Part B. Qualified individuals can also purchase supplemental insurance which combines A and B to form Part C, and also purchase prescription coverage, which is Part D. The letters refer to different health insurance options available to seniors 65 and older and who have reached traditional retirement age. The national insurance plan is also available to adults younger than 65 who are disabled and receive other benefits or meet specific health and financial qualifications. “Generally people can save a lot of money by being on one of the supplemental plans,” said Chuck Peabody, an independent agent representing United Healthcare and owner of Charles F. Peabody Insurance, LLC, in Berthoud. “There are many, many different choices. Some choices make sense for some individuals, and some choices are better for 14 SUCCESSFUL AGING
other individuals. It depends on their financial situation and their long-term health outlook.” Medicare supplemental insurance typically is less expensive than other forms of commercial insurance, primarily employer-based plans and insurance plans purchased through the health insurance marketplace, which for Colorado is Connect for Health Colorado. The deductibles, copays, coinsurance payments and out-of-pocket maximums for Medicare plans cost less than most commercial insurance plans. “When you enroll in Original Medicare insurance, there are basically two paths somebody can follow,” Peabody said. These paths are choosing Part A or both Parts A and B. Part A covers hospital, skilled nursing, home health care and hospice care. Part B coverage consists of outpatient services including provider office visits, laboratory work, emergency room and urgent care, surgical care, radiology and ambulance services. Part A typically has no premium for those employed 10 years or more. Part B has a monthly premium, which for most people in Times-Call — TimesCall.com
2022 is $170.10. “People who are continuing to work past age 65 may choose to only sign up for Part A if they will continue to have other coverage,” Peabody said, adding that Part B enrollment can be delayed as long as a person has other coverage. “There’s no point in paying premiums to an employer and also paying the Part B premium.” Seniors who choose both Parts A and B can opt to purchase supplemental Medicare insurance and prescription drug plans, which are separate policies with separate premiums. “This is where it gets more complicated,” Peabody said. People can choose to purchase a Medicare Supplement Plan (Medigap) and a separate prescription drug plan. The premiums for Medigap insurance can range from about $42 to $155 a month and for prescription drug plans, about $30 to $105 a month. These plan premiums are in addition to the Medicare Part B premium. Seniors also can opt for a Medicare Advantage plan, also referred to as Part C, which gives them the benefits of Medicare Parts A and B rolled together with additional benefits, including a zero deductible in most cases and smaller copays. The premium for Medicare Advantage plans can range from $0 to about $45 a month on top of the premium for Part B. Part D, typically included with Part C for most Medicare Advantage plans, covers prescription drugs that are included in that plan’s formulary. Drug costs are separate from medical costs in these plans and are separated into tiers, with different copays for different tiers. Veterans with certain types of prescription coverage from the Veterans Administration can opt for Medicare Advantage plans that exclude Part D. Most medical providers accept Medicare but are not required to do so—if they do, they have to accept Medicare rates, Peabody said. If seniors opt for a Medicare Supplement (Medigap) plan, they can go to any provider that accepts Medicare, or if they choose a Medicare Advantage plan, they have to opt May 15, 2022

for in-network providers to receive the maximum benefits—the list of providers varies according to the plans chosen. “One place that people can save a lot of money is by having a plan that has no annual deductible,” Peabody said. Original Medicare, referring to Parts A and B, has annual deductibles and co-insurance costs. Generally there is a split of 80-20 with 80% coverage and 20% out of pocket. The 2022 Inpatient Hospital deductible for Part A is $1,556 per hospital benefit period and can happen up to five times annually for five separate periods, which are 60 days. For Part B, the 2022 deductible is $233 a year. “My job is to really help people to find the path that’s going to give them coverage for their current health and likely their future health and what’s affordable to them,” Peabody said. Kaiser Permanente offers an integrated approach to Medicare for its members, who have access to physicians, specialists, laboratories, X-rays and the pharmacy in one building in the integrated health care system. “It’s a high level of communication and collaboration and everything is under one roof. That saves a person a lot of time,” said Carmen Cisneros, Medicare executive team lead for Kaiser Permanente. Once members turn 65, they have the option to become Kaiser Medicare members and agree to let Kaiser manage their Medicare benefits. “Medicare is very different within our plan. Medicare Part D is included with the additional benefits,” Cisneros said. Members who sign up for Parts A and B automatically get Part D as part of their plans through the Medicare Advantage Plan. “It’s more predictable as far as what you pay out of pocket for hospital stays and every facet of care,” Cisneros said, adding that Kaiser’s OOP is at a set rate for medical services. “Most members because they love the way we deliver care automatically stay with us.” May 15, 2022

derail weekend plans during the peak of wildflower hiking season—or halfway through a camping trip. If this sounds familiar, physical therapy could help you reclaim much anticipated activities, but without the pain. Chuck Philipp owner of Alpine Physical Therapy runs two outpatient clinics in Longmont with his wife Kathy. The practice has a 53-year history serving Boulder County, he said. Over the years, Philipp has watched people walk through his doors for any number of reasons. And he himself was once considering surgery for his knee. But with regular physical therapy treatments and eliminating movements that caused the pain, he was able to avoid surgery and maintain his active lifestyle, he said. It meant Philipp retired the running shoes and opted for less jarring forms of activity. “I started doing the exercise bike and elliptical trainer.” Key point--always pay attention when certain movements cause pain, he said. Keep in mind, some movements and gym machines may seem beneficial—even though they can cause damage. Case in point: Philipp used to include the leg extension machine in his workouts—it’s a seated resistance machine that builds the quadricep muscle. Turns out, that machine had to go. “I used to do leg extensions on the weight machine. As I extended forward, I noticed the knee was hurting afterwards,” he said. “We have to realize that some of it is a little trial and error,” adding that the bottom line is to stop any movements that cause pain or damage. And it’s not just gym machines that wreak havoc on knee and shoulder joints or low back. Mother Nature is another one that re-
quires some moderation. While long distance hikes bring on the Rocky Mountain High— the full body stiffness afterward usually requires a long soak in a bathtub loaded with Epsom salt. Followed by ice or a heating pad. And for those who work from home, Philipp recommends investing in the right desk chair to encourage better posture. Prolonged sitting invites pain, he said. Take breaks and stretch periodically to help prevent the body from naturally slouching or hunching over, he added. “Get up and move around instead of typing for two-hour blocks,” Philipp said. Set a reminder on your phone. If you work from the kitchen table for a change of scenery, consider adding a pillow to support the lower back and straighten posture, he added.
PREPARE FOR THE UNEXPECTED
No doubt, getting older comes with many unanticipated surprises that fall outside our control. Learning how to make sound decisions in a short timeframe becomes a superpower. That’s why it’s important to do some planning ahead for the unthinkable. In an emergency situation, like stroke or heart attack, calling 911 is the first line of defense for lifesaving emergency care. After the patient is admitted to the hospital and is stabilized, family members may need to arrange post op care at home or elsewhere, said Joleen McGee, director of admissions and marketing, at Life Care Center, a skilled nursing and rehabilitation facility in Longmont. During the recuperation process, some patients will require a short-term skilled nursing facility with 24/7 nursing care, or services like occupational, speech or physical therapy,
she said. For example, after a stroke, speech delays may occur and require a loved one to work with a speech therapist who helps them regain their speech or ability to swallow, McGee said. Occupational therapists help people relearn fine motor skills, like tying shoes and getting dressed. Before it’s time to use a short-term rehabilitative facility, first look into Medicare coverage, including co-pays and local in-network facilities, McGee said. “Medicare offers 100 days coverage per illness for short term rehabilitation care at skilled nursing facilities,” she added. “Medicare pays the first 20 days room and board, therapy and medications, 24-hour nursing and personal hygiene supplies.” After day 20, a co-pay kicks in with a set daily rate that is determined by each Medicare plan, she added. Not all facilities are created equal. To see which Medicare facilities grab glowing reviews—or not—visit Medicare.gov to get a performance snapshot by using the compare tool. Just add the zip code and select local facilities to compare. When using the comparison tool, McGee suggests watching for the red hand icon which indicates a facility has been cited for abuse. “Go online to Medicare.gov to determine which facilities have the highest ratings before sending a loved one to an unknown facility,” McGee said. Word of mouth is another way to gauge the reputation of a facility, McGee added.

Marlo Vernon graduated from CU Boulder in 2020 then invented Care Penguin. Her father, Todd Vernon, who she calls “a serial entrepreneur,” had designed a sensor to monitor his parents’ house. Marlo just took it a step further. “The Penguin clips onto a hot water heater,” Marlo said. “Then it’s connected to the home’s WIFI with no special tools required. When someone turns on their faucet, the temperature spikes, so we know they’re using water as usual. A lot of people think older people don’t use a lot of water, but on average they turn on the water at least seven times a day. That’s basically seven times of them saying, ‘I’m active in my house.’ If there’s not the usual water activity, the device will send an alert to the phone of their loved one who is monitoring it.” Marlo’s invention is now available nationwide through her website, carepenguin.com.

The last two years have been tough. “What really stood out with COVID is that people with disabilities became even more isolated,” said Penn Street, development and outreach director. “With low vision the world seems to get a lot smaller. But learning new skills and tools can make the world bigger again.” Seniors are the largest population for the network. “They felt cut off from the world, so we do monthly check-in calls to every single listener,” Penn said. “We provide Smart Speakers to keep people connected to the world with 100 Colorado newspapers and access to weather and current updates like where to get testing and vaccinations. One of the hardest things about aging is the struggle to live independently. PACE stands for Program of All-inclusive Care for the Elderly, and it helps clear some of the roadblocks. Chad Hartman, director of Access and Palliative Care for TRU Community Care, said PACE starts with preventative care. He explained, “The interdisciplinary team of doctors, nurse practitioners, physical and occupational therapists and social workers meet every morning and go over what’s going on with all participants. They coordinate medical needs and arrange help with meals, light housekeeping, and certified nursing assistants to help with bathing and other basic needs. They arrange transportation to our clinic where participants see their primary doctors. Then we can set up appointments and transportation to specialists if needed. We even have dentistry at the clinic. There’s a day center where participants come as often as they like to do activities, like bingo, artwork, you name it.” Technology helps. “We give the participants GrandPad tablets with built-in wireless data connectivity if they want. That way they can have virtual visits to their doctors and the clinic right in their home. They can communicate with their families and schedule check-in calls or add photos that will show right on the GrandPad.” He added, “PACE is a great program that can help people on Medicare-Medicaid CMS live independently in their community and supports them in every aspect. A Day Place: Adult Day Care You can Trust By Katie Feindel for Successful Aging Renita Henson was a nurse who worked at a hospital Adult Day Care. The program shut down so in 2017, Renita started a private Adult Day Care program called A Day Place. Below is a more detailed account of Renita’s narrative on A Day Place and why she finds it so important to the community:
Why did you open A Day Place?
I opened A Day Place to provide an alternative to live in facilities. I believe it is important for elderly people to stay in their homes with their loved ones and not live in care facilities away from family. I understand that the family/caregivers need respite

and that is a big reason I started this company. We are open several hours a week for that needed respite, whether it’s for half to full time hours on various patterns. I also understand that seniors who need care need to be around other seniors too. My program provides a social outlet for these seniors with all the activities and programming we provide.”
Do you have programming for caregivers?
Not at this time. But, with our programs, caregivers can get a break on a regular basis or just for a couple days a week. Some participants come 5 days a week for the full day program so their caregivers can work, while others come part time. Participants get
to have fun and they get snacks in the morning and afternoon as well. Another perk is that all participants, whether half or full day, get a lunch from Meals on Wheels as part of the day program.
What kind of programming and activities do you have for day program participants?
We engage the mind and also provide physical activity. We find engaging the mind to be very important. We play lots of games, both engaging the mind and the body. We used to have lots of field trips before COVID but for now, we just stay in and have fun and share meals. The participants really enjoy themselves and the staff is trained and caring.

make sure that it is current, compliant with Colorado law and clearly stated in its intent, you need to pick the right people,” Germany said. “Those documents should be drafted by an attorney. Doing things yourself with an online form is like handing someone a loaded gun – they can either protect you or financially decimate you.” While her advice may sound tough, Germany spends most of her time in court nowadays dealing with situations where the proper legal steps weren’t followed, and the results can be ugly. Children who sell mom’s house without letting her know, catfishing predators who befriend widows or widowers online or at estate sales, or simply families who experience financial hardships and lose the ability to care for their older relatives – these are the unpleasantries Germany sees on a daily basis, many of which can be avoided with the right documentation. “Power of attorney is absolutely critical. I get involved when there is an abuse of that power, or people try to take advantage of seniors with disabilities,” she says. To that end, creating a medically-durable power of attorney arrangement is the first priority, which

Germany said children are usually named, but friends or other legal professionals can also take that role, as families have a challenging but thorough end-of-life discussion. In some cases, that can also involve an advanced directive or living will, which stipulates a client’s plans in case of an advanced medical condition or if they end up in a persistent vegetative state. Other important documents include a HIPAA authorization waiver, which will allow designated family, friends or profession-
als legal access to an older patient’s medical records. “I also recommend considering a financially durable power of attorney, which means that someone else can pay bills or the mortgage. Without it, if they make a mess, that can be devastating for a family’s finances.” She said it’s also equally important that any adult children helping a parent with power of attorney or estate planning paperwork understand their own legal responsibilities. Germany cautions that families consider who might really be in the best situation to handle estate issues – sometimes, the oldest sibling isn’t automatically the best choice. Picking a financial fiduciary who is bonded and recognized by the court may be a better option. A well-drafted estate plan, which includes a will and basic trusts, will cost between $2,000 and $6,000, she notes, but is infinitely less expensive than any court costs – especially trying to establish power of attorney after a loved one becomes ill or passes away.

Wondering if you’re too old to buy a home? Many baby boomers did not buy a home in their younger years – and some baby boomers lost their home in the 2007 to 2012 recession. If you are applying for a mortgage, the primary concern you need to examine is whether or not your cash flow will be able to meet the monthly payments in the long term. Aging in place is an objective for many seniors today. While a particular home may work for your level of physical mobility today, you will need to assess whether the home would still work for you and your family should a disability or other health issues arise.
Consider Space for a Live-In Caregiver
In order to age in place and forestall a move to assisted living, a space for a care giver to live in might be just the ticket. In locations where Accessory Dwelling Units (ADU) are allowed, this could be the perfect arrangement for a live-in caregiver.
Consider the Amount of Maintenance Required
At retirement age, some baby boomers love to garden and take care of minor home maintenance. Others want nothing to do with it. The trade-off is that the lowest maintenance opportunities will have the highest homeowner association fees.
Consider Access to Transportation
At some point in time, everyone will need to stop driving. Access to transportation opportunities to travel to day-to-day activities becomes more critical.
Make a Decision

STRAINING TO READ FINE PRINT? Here are three things you can do
(BPT) - It starts in your 40s: Suddenly you’re having trouble reading smaller print on the computer, electronic devices and books. You feel frustrated holding a menu farther from your face to make out blurry letters. You may even get headaches as you strain to read print that just seems smaller than it used to be. The good news is, you’re not alone. This is a normal condition called “presbyopia” that affects 128 million Americans in their 40s and beyond - and there are positive steps you can take to avoid the eyestrain and headaches that come with it. If this sounds familiar, here are things you can do to alleviate your vision problems.
Make it larger
This may sound simple, but with computers and devices, it’s easy to make the print larger. With so many working from home, enlarging what’s on the screen is a commonplace solution. There are different ways to increase font size or images on any device. With touchscreen devices, your fingers can enlarge print you’re trying to read. On computers, you can use a zoom-in feature to make fonts larger. For PCs, use the “CTRL” button with the plus sign (on a Mac, use the Command button with the plus sign). Additional options to make all images larger are found in Settings (or System Preferences on a Mac). With print, your only option is to seek out large-print publications.
Use reading glasses
Another option is getting over-the-counter reading glasses, which can help you read small print wherever you encounter it. You can find them at pharmacies, retailer shops and even online stores, but choosing just the right ones can be hard and time-consuming. If you don’t know the magnification strength you need, you’ll stand at the array of readers at the store trying on pair after pair, trying to figure out which one works. You’ll try the “self-tests” offered at these locations, 26 SUCCESSFUL AGING
which are unscientific and hard to follow. Additionally, the store conditions do not replicate what you experience while reading a book or working on the computer at home. It’s common to choose an overly strong magnification that looks clear at the store but may provide too much magnification - creating another problem. “When you wear a pair of glasses that have more magnification than you need, you’re forcing your brain to work with that correction,” says Phoebe Yu, vice president of marketing, EyeQue. “It may work for a while, but if you continue wearing the glasses, you’ll develop ocular headaches or migraines.”
