INDICATION

BRIEF SUMMARY OF PRESCRIBING INFORMATION FOR AJOVY® (fremanezumab-vfrm) injection, for subcutaneous use SEE PACKAGE INSERT FOR FULL PRESCRIBING INFORMATION 1 INDICATIONS AND USAGE AJOVY is indicated for the preventive treatment of migraine in adults. 2 DOSAGE AND ADMINISTRATION 2.1 Recommended Dosage Two subcutaneous dosing options of AJOVY are available to administer the recommended dosage: • 225 mg monthly, or • 675 mg every 3 months (quarterly), which is administered as three consecutive subcutaneous injections of 225 mg each. When switching dosage options, administer the first dose of the new regimen on the next scheduled date of administration. If a dose of AJOVY is missed, administer as soon as possible. Thereafter, AJOVY can be scheduled from the date of the last dose. 2.2 Important Administration Instructions AJOVY is for subcutaneous use only. AJOVY may be administered by healthcare professionals, patients, and/or caregivers. Prior to use, provide proper training to patients and/or caregivers on the preparation and administration of AJOVY prefilled syringe, including aseptic technique [see Instructions for Use in full Prescribing Information]: • Remove AJOVY from the refrigerator. Prior to use, allow AJOVY to sit at room temperature for 30 minutes protected from direct sunlight. Do not warm by using a heat source such as hot water or a microwave. Do not use AJOVY if it has been at room temperature for 24 hours or longer. • Follow aseptic injection technique every time AJOVY is administered. • Inspect AJOVY for particles or discoloration prior to administration. Do not use if the solution is cloudy, discolored, or contains particles. • Administer AJOVY by subcutaneous injection into areas of the abdomen, thigh, or upper arm that are not tender, bruised, red, or indurated. If a hypersensitivity reaction occurs, consider discontinuing AJOVY, and institute appropriate therapy. 6 ADVERSE REACTIONS The following clinically significant adverse reactions are discussed in greater detail in other sections of the labeling: • Hypersensitivity Reactions [see Warnings and Precautions (5.1)] 6.1 Clinical Trials Experience Because clinical trials are conducted under widely varying conditions, adverse reaction rates observed in the clinical trials of a drug cannot be directly compared to rates in the clinical trials of another drug, and may not reflect the rates observed in clinical practice. The safety of AJOVY was evaluated in 2512 patients with migraine who received at least 1 dose of AJOVY, representing 1279 patient-years of exposure. Of these, 1730 patients were exposed to AJOVY 225 mg monthly or AJOVY 675 mg quarterly for at least 6 months, 775 patients for at least 12 months, and 138 patients for at least 15 months. In placebo-controlled clinical trials (Studies 1 and 2), 662 patients received AJOVY 225 mg monthly for 12 weeks (with or without a loading dose of 675 mg), and 663 patients received AJOVY 675 mg quarterly for 12 weeks. In the controlled trials, 87% of patients were female, 80% were White, and the mean age was 41 years. The most common adverse reactions in the clinical trials for the preventive treatment of migraine (incidence at least 5% and greater than placebo) were injection site reactions. The adverse reactions that most commonly led to discontinuations were injection site reactions (1%). Table 1 summarizes adverse reactions reported in the 3-month placebo-controlled studies (Study 1 and Study 2), and the 1-month follow-up period after those studies. Adverse Reaction
AJOVY® (fremanezumab-vfrm) injection 6.2 Immunogenicity As with all therapeutic proteins, there is a potential for immunogenicity. The detection of antibody formation is highly dependent on sensitivity and specificity of the assay. Additionally, the observed incidence of antibody (including neutralizing antibody) positivity in an assay may be influenced by several factors, including assay methodology, sample handling, timing of sample collection, concomitant medications, and underlying disease. For these reasons, comparison of the incidence of antibodies to fremanezumab-vfrm in the studies described below with the incidence of antibodies in other studies to other products may be misleading. Clinical immunogenicity of AJOVY was monitored by analyzing anti-drug antibodies (ADA) and neutralizing antibodies in drug-treated patients. The data reflect the percentage of patients whose test results were positive for antibodies to AJOVY in specific assays. In 3-month placebo-controlled studies, treatment-emergent ADA responses were observed in 6 out of 1701 (0.4%) AJOVY-treated patients. One of the 6 patients developed anti-AJOVY neutralizing antibodies at Day 84. In the ongoing long-term open-label study, ADA were detected in 1.6% of patients (30 out of 1888). Out of 30 ADA-positive patients, 17 had a neutralizing activity in their post-dose samples. Although these data do not demonstrate an impact of antifremanezumab-vfrm antibody development on the efficacy or safety of AJOVY in these patients, the available data are too limited to make definitive conclusions. 8 USE IN SPECIFIC POPULATIONS 8.1 Pregnancy Risk Summary There are no adequate data on the developmental risk associated with the use of AJOVY in pregnant women. AJOVY has a long half-life. This should be taken into consideration for women who are pregnant or plan to become pregnant while using AJOVY. Administration of fremanezumab-vfrm to rats and rabbits during the period of organogenesis or to rats throughout pregnancy and lactation at doses resulting in plasma levels greater than those expected clinically did not result in adverse effects on development [see Animal Data]. In the U.S. general population, the estimated background risk of major birth defects and miscarriage in clinically recognized pregnancies is 2-4% and 15-20%, respectively. The estimated rate of major birth defects (2.2-2.9%) and miscarriage (17%) among deliveries to women with migraine are similar to rates reported in women without migraine. Clinical Considerations Disease-Associated Maternal and/or Embryo/Fetal Risk Published data have suggested that women with migraine may be at increased risk of preeclampsia during pregnancy. Data Animal Data When fremanezumab-vfrm (0, 50, 100, or 200 mg/kg) was administered to male and female rats by weekly subcutaneous injection prior to and during mating and continuing in females throughout organogenesis, no adverse embryofetal effects were observed. The highest dose tested was associated with plasma exposures (AUC) approximately 2 times that in humans at a dose of 675 mg. Administration of fremanezumab-vfrm (0, 10, 50, or 100 mg/kg) weekly by subcutaneous injection to pregnant rabbits throughout the period of organogenesis produced no adverse effects on embryofetal development. The highest dose tested was associated with plasma AUC approximately 3 times that in humans (675 mg). Administration of fremanezumab-vfrm (0, 50, 100, or 200 mg/kg) weekly by subcutaneous injection to female rats throughout pregnancy and lactation resulted in no adverse effects on pre- and postnatal development. The highest dose tested was associated with plasma AUC approximately 2 times that in humans (675 mg). 8.2 Lactation Risk Summary There are no data on the presence of fremanezumab-vfrm in human milk, the effects on the breastfed infant, or the effects on milk production. The developmental and health benefits of breastfeeding should be considered along with the mother’s clinical need for AJOVY and any potential adverse effects on the breastfed infant from AJOVY or from the underlying maternal condition. 8.4 Pediatric Use Safety and effectiveness in pediatric patients have not been established. 8.5 Geriatric Use Clinical studies of AJOVY did not include sufficient numbers of subjects aged 65 and over to determine whether they respond differently from younger subjects. “There are other places where the water is blue and clear, but there is nothing like it is in the Bahamas. It is so expansive and so absolutely beautiful, “Astronaut Scott Kelly claimed after passing over it in space for an entire year. “I think one of the reasons why we know that aliens have never come to Earth is because they would’ve all been here in the Bahamas. If you’re going to land in the most appealing place from space,” he joked, Well, if it’s that beautiful from space, it’s even more gorgeous in person. The name Bahamas comes from the Spanish term Baja Mar or shallow sea. Over 700 islands that make up the country, situated between Cuba and Florida, are all surrounded by very shallow water, which is the reason for the terrific turquoise color. The brilliant blue hue comes from its white calcium carbonate (the stuff that makes seashells) sandy bottom and the intensity of light when it’s reflected off the shallows and unabsorbed by the water - Mother Nature at its finest. A short flight from Miami, I was shuttled off the main island of New Providence, passing through the capital Nassau en route to my final destination of Paradise Island. As we crossed over the Harry Belafonte Bridge (a Bahamian native), my driver apologizes for the color of the water. “Our water is much bluer than this,” he laments, “Sorry that you do not see it as you should. You’ll just have to come back.” It was still a fantastic color, but the remnants of Hurricane Dorian had churned up the ocean enough to bring native criticism of the color. Because of Dorian, I almost didn’t visit the Bahamas. With the sad news of death and massive destruction, I almost canceled my visit 68 | PHYSICIANS OFFICE RESOURCE
to the island nation. But only two of the 30 inhabited islands were directly hit. The rest of the country was left untouched and open for business. Tourism makes up well over 50 percent of the Bahamians GDP and employees over half of its residents. The best way to help rebuild the neighboring islands effected by Hurricane Dorian is to visit the Bahamas, bring tourism back to the country. “Welcome Home,” was my greeting as I arrived at my destination. The luxury resort, The Ocean Club, a Four Seasons Resort, is tucked away like a precious secret on the north end of Paradise Island. A mile from the mass tourist attraction, the ever-popular Atlantis resort, The Ocean Club is private, pristine, and practically perfect, seemingly worlds away from its neighbor. Being one of the most exclusive resorts in all of the Caribbean, the moment you arrive at the airy whitewashed French Plantation, you exhale. As you walk up the marble steps, your eyes are instantly focused straight ahead to the wide-open doors and the azure sea and green manicured lawn on the other side of the grand house. Like the Bahamian people themselves, the vibe of the resort is warm, caring, and inviting. A chilled towel scented with Emily Rose and a glass of the famous island concoction, Sky juice, I’m perfectly content, and I’ve only just arrived. Spread out on 35 beautifully manicured acres, with just 107 super-luxe accommodations; the property never feels crowded. Choices range in sizes and modern amenities, which include guest rooms, suites, two-bedroom cottages, and 3-4 bedroom villas. Five, low-rise Caribbean-style buildings make up the Crescent Wing; each large
guest room boasts a balcony/patio facing the ocean, contemporary decor, spacious marble bathrooms with big jetted tubs, and walk-in steam showers. Beachfront suites come with more than generous space with living and dining area. The Hartford Wing is a single two-story, plantation-style building with an interior courtyard, connected to the grand house. Even though this building is part of the original 1962 property, all the rooms were part of a 25-million dollar renovation in 2015, making it even more modern than the newer Crescent Wing buildings. The Ocean Club is terrific for entire families as well; residential style 3 and 4 bedroom private villas include private infinity pools, while two-bedroom garden cottages offer plunge pools. Like all Four Seasons Resorts, they provide the complimentary Kids’ Club, welcoming guests ages 4-12 for supervised entertainment and the opportunity for the younger family members to make magical memories of their own. Whether it’s a luxury beachfront room in the Crescent wing or one of the spacious ocean view rooms in the Hartford wing, you will feel indulged. All of which have flat-screen TVs with satellite, gourmet minibars, and 24-hour concierges. Welcome amenities, including gifts of fruit and rum cake, complimentary hats, and a beach bag. Best of all, the personal butler that appears at your door daily for turndown service and happily offers complimentary champagne and chocolate-dipped strawberries. Referencing Plato; “Rhythm and harmony find their way into the inward places of the soul.” My husband and I indulged in the resort’s award-winning spa. We were escorted to one of eight Balinese-style spa villas with a private garden for our pampering. On two teak massage tables, in serene silence, we were nurtured with both relaxation and rejuvenation during a 50-minute couples massage. Afterward, we savored a Japanese Tea ceremony served to us by the spa therapists. Both then left the two of us to be secluded from the world as we experienced total tranquility amid the tropical surroundings, with the waterfall shower, the covered daybed and drawn hydrotherapy bath with two feet of bubbles and therapeutic oils. The treatment was transformative - harmonizing body and mind. The only way to match our afternoon of bliss was to dine at Dune. Michelin star chef Jean-Georges Vongerichten has brought a carefree island sophistication to Paradise Island with his waterfront restaurant positioned just above the white sand and ocean waves on the resort grounds. The excellent fare is a crossing of Asian and French cuisine with Bahamian influence. A must-try: The Black Plate for two. With the stylized design, featuring a display kitchen and open patio doors allowing the crashing waves as ambient sound, the room setting is both exciting and calm. White glove service is synonymous with all Four Seasons resorts. The Ocean Club is not the exception, but in fact, we felt the staff went above and beyond to make our visit enjoyable. One example: as my husband reminisced that the evening marked the 30th anniversary of our first date, our wait staff overheard us talking about our fond memories. We were happily surprised when they brought over a dessert for us to share with the “Happy Anniversary” scrolled on the plate. But the bigger surprise came when we walked back to our suite, opening the door to rose petals dropped on the floor leading to our bed. On the bed was a handwritten note wishing us a Happy 1st Date Anniversary, “…may you both be blessed for many years to come. With much love from your butler Ramesh and the entire Ocean Club family.” Our waiter, Silvaro, had notified our butler Ramesh, and the delightful deed was done. Service above and beyond. Our first full day, we were greeted with sunshine, swaying palm trees, and a beautiful breakfast served on our lovely patio. With our suite just a stone’s throw away from the tidewaters rolling in, we admired the view in front of us. The contrast of the two white Adirondack chairs perched on the edge of the green lawn, and the two of us sitting at our table in our Frette waffle bathrobes, it felt as though we were enjoying our *meal in our own seclusive beach house. *Side note: the BEST Brioche Mango
French Toast I will ever have the pleasure of eating. We decided our quest for relaxation would continue as we selected comfy lounge chairs at the Ocean Pool. The adults-only infinity pool was quiet and cozy. Couples, side by side, draping their arms on the outer edge and gazing out at that stunning sea, the view that incredibly was all of ours, at least for the duration of our stay. It wasn’t long, a chapter or two into my book when the wait staff walked by with some fish tacos. Immoderation, being my middle name, I told her as she walked past me, that I too needed a couple of those tacos. With great haste, she went directly to the Ocean restaurant next to the pool to honor my request. As if the exceptional french toast wasn’t enough that morning, I had to see if they tasted as yummy as they looked. They were. If I had been seriously interested in exercising any of my over the top caloric intake, The Ocean Club has many options. The six Har-Tru tennis courts are available for free for both day and night use, the resort providing racquets at no charge. Resident tennis professionals arrange tennis clinics, games, and round-robin tournaments. Private lessons are available for a fee. Fitness center has both indoor and outdoor areas, rooms for Pilates, cardio, circuit training, and stretching. Free yoga classes are available on the yoga deck overlooking the ocean. Complimentary bicycles are available to help discover the island. They even have a badminton court. Sadly, I didn’t take advantage of any of these free fitness opportunities. However, I did jump at the chance to play the 18-hole, par 72 golf course designed by golf legend Tom Weiskopf. My husband is an excellent golfer, and he enjoyed the long track, stretching over 7,100 yards on Paradise Island’s peninsula. A challenging course with water hazards, creative bunkers, and crazy crosswinds. However, I judged the course by the number of amazing water views, equally beautiful of both the Atlantic and Nassau Harbour, for my Instagram worthy images. My husband’s scorecard faired well, my golf game was somewhat frustrating, but just being on such a lovely course redeemed my several bogies. We enjoyed a meal at the Club House, and the kindness of the Bahamian staff continued. We mentioned how much we liked the herbal tea; they had served my husband for his sore throat. As we were leaving, Valencia gave me a handful of tea envelopes and insisted I take them to help my husband’s ailment. Service above and beyond. Late in the afternoon, a rain shower blew through — a perfect time to feel famous at the infamous Martini Bar and Lounge in the grand house. A black grand piano is a prominent fixture in the room because of the many black and white photos in varied size sterling silver frames that adorn the top: Sinatra, Elvis, Mandela, Hemmingway, to name a few. The Queen and the Beatles both have The Ocean Club in common, not to mention three Bonds, as in Sean Connery, Pierce Brosnan, and Daniel Craig. The resort has been featured prominently 2019, ISSUE 10 | 69
in two James Bond films, 1965’s Thunderball and the 2006 remake of Casino Royale. James Bond swims up on the beach that I walked on. He’s served a Martini at the bar, “shaken, not stirred.” I drink my share of “Switcha,” which is the Bahamian name for limeade. In the movie, Mr. 007 plays a masterful game of poker in the renowned lounge, the exact place where I beat my husband in Trivial Pursuit as we sat on the comfy couch. And for a brief time, my husband and I soaked in the nostalgia on a rainy afternoon. The history of The Ocean Club proceeds the legendary reputation of the resort. The hideaway in the Bahamas has been redefining luxury for over 50 years. One of the world’s wealthiest men, a Swedish industrialist, first developed the private estate in 1939 and called it Shangri-la. The property had a large house and the beginnings of beautiful landscaping, designed after the intricate gardens at the Chateau de Versailles in France. The property was sold twenty years later to Huntington Hartford II, a long-time winter resident of Nassau. The wealthy heir envisioned Shangri-la as a luxury hotel and a refuge for the world’s elite. He petitioned the Bahamian government to name the property Paradise Island and built a luxurious 52-room hotel, which is the Hartford Wing. Mr. Hartford invested significantly into his paradise, embellishing the 35-acres with fountains, marble sculptures, and terraced gardens. He purchased the Cloisters, remains of a 12th-century French monastery, which was initially collected by William Randolph Hearst in the 1920s, stored in a Florida warehouse for years. The Versailles Gardens today are a fascinating fixture on the small island strand. Spanning from the Nassau Harbour to the ocean on the other side, a walk on the premises or better still, a perfectly planned dinner for two, will transport you to another country and period. A memory maker if you’re a hopeless romantic like me. Fast forward to 2000, when Sal Kerzner, a South African hotel developer, acquired The Ocean Club. He restored and expanded the facilities, adding the Crescent Wing and DUNE restaurant. Beside the Ocean Pool, there are two temperature-controlled pools. The Versailles Pool is an elegant adults-only sanctuary near the Versailles Terrace restaurant. The Family Pool creates family fun with a 70 | PHYSICIANS OFFICE RESOURCE
beach-style entry and features a waterfall and tropical lagoon vibe. Of course, the beach itself stretches five miles along the north shore of Paradise Island, offering options of paddle boarding, taking a plunge, or burying your toes in the soft white sand. This luxury resort provides a respite away from the world. However, the staff also arranges many opportunities for exciting excursions during your visit. The Ocean Club guests have complimentary access to Aquaventure water park at the Atlantis, which features 141 acres of the waterscape, high-speed water slide, and a river ride. Also, providing complimentary shuttles to enjoy the nearby hotel’s casino and restaurants. But if you’re looking for something unique and tailored to your sense of adventure The Ocean Club offers: Luxury Yacht charters, Deep-Sea Fishing, Scuba Diving, and Snorkeling charters, Dolphin Interactions and Powerboat Adventures. Or perhaps you’d like to explore Old Nassau, the discovery of the islands and the colorful Pirate history, experience From Cane to Cask, a Bahamian Rum Experience. Venture on land or sea, or even both, the staff will make the arrangements for any/all exciting excursions. Resort manager, Kristen Whyly, when asked what he feels sets The Ocean Club apart, he shared that it’s the Bahamian people, the kindness and attention to the needs of the guests. He offered this narrative: “One of our resort lifeguards noticed a father wanted to bond with his sons but was competing with the boys’ handheld electronics. The lifeguard asked the guest if he and his sons liked to fish? When the father affirmed that he and his boys loved fishing, the lifeguard offered to take the three of them to his secret fishing spot the next morning and would provide the poles and tackle. The family of three spent not one but two back-to-back days fishing with our employee and newfound friend. The father was so grateful, bonding with his boys in the Bahamas,” Whyly adding, “The lifeguard saw a need and filled it.” Service above and beyond.
