Unpaid clinical rotations are scheduled during weekday hours each term. Prior to clinical placement, students are required, at the students’ expense, to complete immunizations and/or titers, a current CPR/ BLS card, drug screening, physical examination, and provide proof of personal health insurance. A background check, FBI & BCI, must also be completed at Polaris Career Center in the Adult Education Suite prior to program orientation.
To obtain licensure as a Licensed Practical Nurse (LPN), graduates must successfully complete the Practical Nursing Program, submit an application for licensure to the State Board of Nursing, and pass the National Council Licensure Examination for Practical Nurses (NCLEX-PN). The NCLEXPN is a computerized examination administered through Pearson VUE.
CAREERS
Licensed Practical Nurses work in a variety of healthcare settings, such as hospitals, nursing homes, physician offices, jails, clinics, and private homes.

CANCELED COURSES
The Adult Education Department must cancel courses that do not attain the required minimum enrollment. The Adult Education Department reserves the right to cancel, discontinue, postpone, or combine courses before or at the first class meeting. Attempts are made to contact all students whose courses have been canceled. In the event a class is canceled, a full refund of the registration fee will be given.

SENIOR CITIZENS DISCOUNT

POLARIS CAREER CENTER SCHOOL CLOSING
In the event high school classes are canceled, adult education classes may meet as scheduled. If adult education classes must be canceled because of inclement weather or other conditions, the school will post a notice on the school’s website.
WITHDRAWALS AND REFUNDS
Full refunds will be issued when a class is canceled by the Adult Education Department, or previously made payments may be applied to another class. No refunds will be issued after the first class meeting. A $10 cancellation fee will be withheld from all refunds. Books and supplies/materials are non-refundable. If a student paid by check or cash, the refund will be disbursed within 45 days in the form of a Polaris Career Center check. If the student paid using a credit card, the refund will be credited within 45 days on the credit card initially charged. Any refund that totals $1 or less will remain on the student’s account as a credit balance.

1098-T tax forms will be provided per Internal Revenue Service (IRS) Guidelines. The 1098-T tax form is an Internal Revenue Service (IRS) tuition statement which is intended to help students file their federal income tax return. Polaris Career Center provides an electronic version of the 1098-T tax form unless the student has opted out. In this case, a paper 1098-T tax form will be mailed to the student. The electronic 1098-T will be available by the end of January each year for the prior tax year within the Documents section of Achademix. Students need to keep the 1098-T tax form for their records. Students must contact the Administrative Coordinator by January 31 to receive a 1098-T form for all Continuing Education classes.
