CenteringPregnancy Group Prenatal Care in Federal Way By CHI Franciscan Health Special to the Mirror If you’re planning for a baby in 2016, CHI Franciscan Health and Franciscan Women’s Health Associates introduces a new prenatal program that will allow you to share your pregnancy with other women who know exactly what you’re going through. CenteringPregnancy is a group-visit prenatal care program available to women in Federal Way and surrounding communities. CenteringPregnancy offers the three major components of prenatal care: health checks, education and support. Eight to 12 women with similar gestational ages meet for 10 sessions through pregnancy and early postpartum, learning care skills, participating in a facilitated discussion and developing a support network with other group members. At each session, a nurse along with the certified nurse midwife or obstetrician/gynecologist leading the group, will complete standard physical health assessments within the group space. Spouses and/or partners are also encouraged to attend. These 90-minute sessions are held in the evening and are typically covered by private health insurance or Medicaid. Through this unique model of care, women are empowered to choose health-promoting behaviors that support positive outcomes for pregnancies. Centering outcomes have resulted in increased birth weight, fewer preterm deliveries, lower postpartum depression rates and higher levels of patient satisfaction. Another goal of the Centering program is to increase the number of atrisk, underserved women who can be served by timely prenatal care, resulting in increased breastfeeding rates and providing a critical social support system. “We’ve had such a positive response to the CenteringPregnancy program that we have a growing demand for more groups,” said Jennifer Riffel, CNM, ARNP, director of midwifery for CHI Franciscan. Sessions in Federal Way take place at the Franciscan Women’s Health Associates clinic located at 34709 Ninth Ave. S., Suite B-500. Staying fit and healthy in 2016 By CHI Franciscan Health

Each year, one of the top New Year’s resolutions for many people is “I want to lose weight” or “I want to get into better shape.” 2016 is no exception. Last year, a Nielsen survey reported that 37 percent of respondents listed “stay fit and healthy” as their resolution for the New Year and 32 percent hope to “lose weight.” For many of us obtaining these goals may be as simple as modifying our diet or exercising more. For others, achieving this goal is much more difficult. For some, the answer to making a lifestyle change is weight-loss surgery. That was the case with 28 year-old Amanda Emerson of Federal Way. Emerson had been very active and an athlete in high school, but has battled being overweight for many years since then. Emerson tried dieting, but it just didn’t seem to work for her. “I would lose the weight and then once I was off the diet I would gain the weight right back,” Emerson recalled. Emerson wasn’t suffering from any major health issues, other than being overweight. But, she knew eventually her weight could become a health concern. “I realized if I didn’t do something about my weight, it could become an issue,” she said. So in June 2014, Emerson had weight-loss surgery at the Franciscan Center for Weight Management in Federal Way performed by Haroon P. Anwar, MD. Today, she has gone from 340 pounds to around 210 pounds — a loss of 130 pounds. Now, Emerson has run a few 5Ks and has more energy to play with her boyfriend’s two children. For her,

having weight-loss surgery has changed her life. But, she understands people may be reluctant to have the procedure. “It look me six years to decide to have weight-loss surgery,” Amanda acknowledged. “I understand people are scared. I ask them, where do you see yourself in five years? Think about what’s best for you and how having this procedure can better your life. It has certainly improved mine.” If you are considering weight-loss surgery, here are a few factors to think about regarding your current health: • Are you 18 or older? • Are you more than 100 pounds over your ideal body weight? • Do you have a Body Mass Index (BMI) more than 40? • Do you have a BMI more than 35 and suffer from high blood pressure, type 2 diabetes or sleep apnea? • Have you tried other medical and nutritional weight-loss programs without long-term success? • Are you unable to sustain a healthy weight, even with medically supervised attempts at weight loss? If you can answer yes to most of these questions, weight-loss surgery might be the right choice for you. Having weight-loss surgery is only the beginning of your journey. As an obstetrician and gynecologist, I have an appreciation for health care needs unique to my patients. I love that medical advances over the last five to 10 years have allowed me to help many women benefit from having access to an array of treatment choices, especially related to gynecologic surgery. Many women consider gynecologic surgery for various conditions, such as abnormal bleeding, pelvic pain, ovarian cysts, fibroids or endometriosis. In the past, these surgeries were often done through large abdominal incisions requiring long hospitalizations and extended recovery time. In addition, they were sometimes accompanied by more scar tissue and increased risks of bleeding and infection. Thankfully, as with much of modern medicine, an array of minimally invasive surgery techniques now exist to provide women with more choice, convenience and benefits, depending on individual need and interest. It is usually used to treat abnormal uterine bleeding when medications have failed. It can also be used to treat small fibroids inside the uterus. The procedure does not require making an incision. Depending on the method chosen for the ablation, slender tools are inserted into the uterus through the vagina and cervix. Some methods of endometrial ablation use extreme cold, while others involve heated fluids, microwave energy, lasers or high-energy radiofrequencies to destroy the uterine lining. It can often be done using a local or spinal anesthetic. • Hysteroscopy: Hysteroscopy is a procedure for looking inside the cervix and uterus. The hysteroscope is a thin, lighted tube inserted through the vagina that can diagnose and treat uterine conditions. Other instruments can be inserted through the tube to perform minimally invasive surgery procedures. Hysteroscopy can also be used with other procedures, such as laparoscopy, in which an endoscope (a medical device with a light attached used to look inside the body) is inserted in the abdomen to view the outside of the uterus, ovaries or Fallopian tubes. • Laparoscopic surgery for endometriosis or pelvic pain: Laparoscopy is the most common procedure used to diagnose and remove mild to moderate endometriosis, a disorder where tissue that normally lines the uterus grows outside the reproductive organ. Instead of making a large abdominal incision, the surgeon inserts a lighted viewing instrument, called a laparoscope, through a small incision. If the surgeon needs better access, they make one or two more small incisions for inserting other slender surgical tools. • Laparoscopic removal of ovarian cysts or ovaries: While a woman’s health history, symptoms, examination and ultrasound images can provide a lot of informaContinued on page 17

tion, sometimes the only way to know for sure if a cyst is benign is to remove it surgically. In general, an ovary can almost always be saved if the cyst is benign and not too large. In young women, the ovary can almost always be preserved. If a woman is peri-menopausal or menopausal, it usually makes more sense to remove the entire ovary, especially if there is a concern that the cyst might be cancerous. Stay safe in the sun all year long by Dr. Molly Chan | Skin Health
If the weather in June and July were any indication of what Puget Sound-area residents can expect to enjoy this spring and summer, it is likely people should plan for more of the same. But if you’re going to be outside any time of the year, especially on a sunny day, stay safe by knowing and doing a few basic things about sunburn. The sun keeps us Dr. Molly Chan, MD, MSW warm and triggers the production of vitamin D in our skin so we can better absorb calcium into our bodies for strong bones. But when your skin has been exposed to ultraviolet rays from the sun for too long, you get sunburned. Why should I worry about getting sunburned? If you get sunburned frequently, it increases your risk of developing: • Skin cancer • Wrinkles and other age-related skin changes • Cataracts, which cause cloudy vision Why do some people get sunburned more easily than others? Your chance of getting a burn from sun exposure increases if you: • Have pale skin and light-colored hair • Are on a mountain or in a region that is closer to the sun or near a very reflective surface, such as water, sand and snow • Are taking medications that cause your skin to burn more easily How long does it take to develop a sunburn? It depends on the intensity of the ultraviolet radiation on a given day, specific to your location. The UV Index predicts the risk of sunburn in your area, based on weather conditions. The UV Index varies throughout the course of the day; generally the sun’s rays are strongest between 10 a.m. and 4 p.m. How is a sunburn different from getting burned with hot water or a hot stove? It is actually very similar. A mild sunburn causes your skin

to become red, slightly hot to the touch and a little painful; this is akin to a first-degree burn. A more severe sunburn can cause skin blisters, swelling and severe pain; this is like having a second-degree burn. What can I do to prevent getting sunburned?
Try to take frequent breaks from the sun by going indoors or moving into shade. Use sunscreen with a sun protection factor rating of 30 or higher and cover as much skin as you can. The most effective sunscreens provide protection against both UVA (long-wave) and UVB (shortwave) rays. Apply sunscreen 15 to 20 minutes before going into the sun and re-apply at least every two hours. “Water resistant” sunscreen will maintain protection through 40 minutes of water activity; “very water resistant” sunscreen is good for 80 minutes of water activity. Always reapply sunscreen after swimming. Wear a hat to protect your scalp and face. Sunglasses that block both UVA and UVB rays will protect your eyes from the intensity of the sun. Can I put sunscreen on my baby? Sunscreen is not recommended for infants younger than 6 months because their skin barrier is not fully developed; but if you are not able to protect your baby adequately with clothing, hats and shade, you can use a little bit of sunscreen on small areas, such as the face and back of the hands. For all infants and small children, choose a sunscreen for sensitive skin: the best ingredients are titanium dioxide and zinc oxide because they will protect against UVA and UVB rays, and they cause minimal irritation to the skin. What is the best treatment for a sunburn? Most sunburns will appear within three to five hours after sun exposure and resolve within three days. You can take Advil or Aleve for pain and inflammation. Cooling compresses may help soothe your skin. There are many topical products containing aloe vera or numbing medication that are made specifically for sunburn relief; frequent applications of aloe vera gel help minimize the skin peeling that occurs as you heal from a sunburn. Try to avoid sun exposure while you are recovering and stay well hydrated by drinking more water. Seek medical treatment if you develop a lot of skin blisters, severe pain, headache, fever, vomiting or dehydration. The sun doesn’t have to be your enemy if you wear sunscreen, drink water and take breaks when you start to feel too hot. And don’t forget your sunglasses. New studies link learning skills to physical activity levels from staff reports, The Courier Herald
For most children, the shorter days of fall and winter limit time and opportunity for active, physical play. This is a problem, say the child development specialists at The Little Gym in the Federal Way area, because currentresearch indicates that physical activity is much more than simply fun and games; it is an important contributor to successful learning. A recent study that set out to look at the health effects of excess body fat and inactivity in children discovered a link between physical activity and brain function. (http://www.webmd.com/ children/news/20151102/weight-exercise-may-affect-childrensthinking-skills) Researchers compared mental skills between active children and inactive children and found that, even among children of normal weight, active children did better on tests that involved skills like planning and paying attention. The Little Gym President and CEO Ruk Adams says that these results are good news, because physical activity is something that families can take steps to boost by supplementing, if necessary, the physical education that their children receive during the school day. While studies like this one show that physical exercise can enhance a child’s ability to learn, the majority of parents responding to a recentpoll that National Public Radio conducted in partnership with the Robert Wood Johnson Foundation and the Harvard School of Public Health gave their children’s schools a low grade for their lack of focus on physical activity. Fortunately, families have a range of options to choose from to encourage their children to stay physically active. School- or community-based sports programs provide children with an opportunity to grow both their physical and social skills. Some children thrive on specialized training in disciplines like gymnastics or dance and some children turn to neighborhood friends for active play. “The important thing is to choose a program that your child enjoys so that it’s something the child will want to continue to do,” Adams said. “Parents of toddlers and preschoolers should also take note: making physical activity a ‘habit’ for your children at an early age may not only enhance your child’s potential to learn; it will also help lay the foundation for a lifetime of healthy living.” For more information about The Little Gym in Federal Way, visit www.TheLittleGym.com.

Healthy Tips & Strategies Household mold and your health Are your child’s asthma attacks becoming more frequent and serious? Are you continually catching colds? These situations may be the result of exposure to mildew without you even knowing it. When a lot of mildew — or mold — develops in a home, these microscopic fungi can scatter breathable particles into the air and consequently cause a number of health problems. Depending on your state of health, age, and amount of time spent in the home, you could be more sensitive. Especially vulnerable are babies, young children, seniors, and people with depressed immune systems. How do you know if these unwelcome visitors are invading your home? Look for the following: the appearance of stains, which are often black, on walls, ceilings, or carpets; a musty or earthy smell; dark rings and buckling, which are evidence of water infiltration. Mold loves damp conditions, and those can usually be found in basements, the backs of closets, under kitchen sinks, in bathrooms, and sometimes around windows. You can prevent mold by ensuring good ventilation in your home. Open windows or switch on fans when taking baths or showers or when you’re cooking. In hot, humid weather be sure to use a dehumidifier in chronically damp places, especially in the basement. Empty and clean the reservoir regularly and add several drops of lemon essential oil to disinfect it. Repair any water leaks as soon as possible. If condensation forms on windows, especially during the winter, check the insulation. These small actions will help to prevent mold from harming your family’s health.
Chew your way to slimness Chewing our food thoroughly can help us to eat less. This fact has been demonstrated in a recent study showing the influence of oral stimulation on our feelings of satiety and the number of calories ingested. Oral stimulation means putting food into our mouths, taking the time to taste it, and chewing it slowly before swallowing. The study shows that the length of the oral stimulation is inversely proportional to calorie intake, which is sure to interest those who watch their weight — in other words the vast majority of us. The results of this research are in full agreement with the old adage, “Chew your liquids and drink your food”. This means that you should drink in small sips, rolling the liquid around the mouth, and chew your food until it is liquid before swallowing it. Chewing longer also prolongs the meal and leaves time for feelings of fullness to set in. These signals are emitted by the brain when it has received information from the stomach indicating that the body no longer needs to eat. It is essential to pay attention to these signals, especially because we live in a society that promotes fast food and quantity over quality. In fact, although we should stop eating when we no longer feel hungry, the majority of people are directly influenced by the quantity of food on their plates. For your health’s sake, start serving reasonable portions, chewing thoroughly, and listening for your stomach to tell you it has had enough.
