is the membership fee for the program?

The Mayo Clinic 365 program in Rochester has an annual membership fee of $25,000. This charge is in addition to costs for specific medical services. The fee, payable annually in a lump sum, is collected upon completion of your initial medical exam with your Mayo Clinic 365 physician. Annual billing occurs on the anniversary month of your membership.

How will Mayo Clinic 365 provide more immediate access?

Mayo Clinic 365 physicians limit the number of patients accepted into their practice. This ensures more time for your personalized care. As a member, you’ll also have 24/7/365 access to your Mayo Clinic 365 physician via cellphone or the user-friendly Mayo Clinic Patient Portal.

How

can I enroll in Mayo Clinic 365?

Members are accepted on a first-come, firstserved basis. Complete our inquiry form to learn more or contact our appointment office at 507-284-4027 to schedule a meeting with our program manager to get started.

Does Mayo Clinic participate in insurance programs and health plans?

Yes. We bill medical services through insurance. Check with your plan administrator to confirm your specific coverage. The annual membership fee is in addition to charges for specific medical services and is not billed through insurance.

When is the membership fee due?

Your membership fee is due upon completion of your first medical exam with your Mayo Clinic 365 physician. The total amount is due within 30 days of the appointment.

Can my company cover and pay for my Mayo Clinic 365 membership fee?

Yes, we have Mayo Clinic 365 corporate clients who cover this as an executive-level benefit for their CEO or C-suite. We have corporate clients who cover Mayo Clinic 365 for their highest-level business leaders and cover our Mayo Clinic Executive Health Program for their other executives. Inquire for best fits for your company.

Do I have to pay the membership fee even if I don’t use my physician’s services during that year?

Yes. The annual membership fee covers the cost of access to your physician even if you don’t use it.

What are the credentials of these physicians?

Our Mayo Clinic 365 physicians are longtime Mayo Clinic physicians who were hand-picked to care for patients within this concierge medicine program. Each is board-certified in Internal Medicine. They were selected for their excellent reputations, commitment to service, and demonstrated ability to build in-depth year-over-year relationships with patients.

Will my Mayo Clinic 365 physician be available to me while I am traveling or out of town on an extended basis, including my homes in other locations?

Yes. Your Mayo Clinic 365 physician and care team are available to you by phone or portal message no matter where you are. If your Mayo Clinic 365 physician isn’t available due to personal travel, one of their physician colleagues will be available.
