Welcome

Health and care are changing and we need to change too.

There are lots of reasons why we need to change and they include:

• More people need care now.

• People are living longer.

• Many people have long-term health problems.

• Not everyone has the same chance to be healthy.

• Some groups of people, like people with a learning disability, have worse health. This is not fair.

• Staff are working very hard.

• Hospitals are now older and need to be better.

The ways we work cannot stay the same.

We must act now.

What is RISE?

RISE is our plan for the future.

It shows what we will do.

It is honest about where we are.

We believe we can do better.

RISE puts people first.

Patients are at the centre.

Staff are important too.

We want healthcare services and buildings to be excellent.

We want healthcare that people can rely on when they need it.

We want healthcare to be fair so everyone can get help when they need it.

What will we do?

We will work with all the other health and care professionals involved in your care.

We will support different groups of people in different areas we work in.

We will help people earlier to prevent illness when we can.

We will join services together.

Teams will work together.

We will listen to patients.

We will listen to staff.

We will use what they tell us to improve.

We will keep any promises we make.

Our plan is split in 3 parts called: 1. Rising Hope 2. Rising Communities

3. Rising Standards

Rising Hope

Rising Hope is about people, including:

Patients

Staff Communities

It is about kindness and support.

We want people to feel valued.

We want people to feel safe.

We want people to feel heard and supported.

Rising Communities

Rising Communities is about local healthcare.

It is about where people live.

We will work with local services.

We will join services together.

This means you do not need to keep repeating yourself every time you see a new health professional.

Care will be closer to home.

This makes getting healthcare for people easier.

Rising Standards

Rising Standards is about giving really good care every time.

We will use information and technology to make care better and faster.

We want to give the best healthcare in England.

The 3 stages of our RISE plan

The first stage is called Recover.

Recover happens over the first 6 to 12 months of the plan.

Recover includes making sure:

• Our healthcare services are working well.

• We fix any urgent problems.

An example is that we have opened a Community Diagnostic Centre.

This means people will get tests faster and hospitals are less busy.

The second stage is called Renew.

Renew happens 1 to 3 years into the plan.

It includes making sure we improve how services work together.

An example is bringing teams together that work across hospital and community healthcare.

The third stage is called Rise. It happens 1 to 5 years into the plan.

We will make the best healthcare services we can for everyone.

An example is to provide heart surgeries locally so people do not need to travel.
