Oncologist There is a false perception that lung cancer is the result of a personal choice to smoke, so lung cancer patients are not always viewed in the same light as those with other types of cancer.
“There’s a huge stigma associated with lung cancer because the majority of people who die from it are either smokers or former smokers,” says Cleveland Clinic Florida oncologist Bruno Bastos, MD. “But, tobacco is one of the most addictive substances known to man. Addiction is a disease, and nobody deserves to get lung cancer.” Patients who have stopped smoking are still at an increased risk for lung cancer long before they receive a lung cancer diagnosis. “Additionally, the fact is that anyone can be exposed to toxins and develop lung cancer, so this is a disease that should concern everyone,” Dr. Bastos said. The stigma associated with lung cancer has had a negative effect on lung cancer research. Although statistics from the Centers for Disease Control show that more people in the United States die from lung cancer than any other type of cancer, researchers have had a difficult time obtaining funding to find a cure.

“Other types of cancers also have a lot more survivors who can advocate for funding,” said Dr. Bastos. “There are not enough lung cancer survivors to demand change and those who do survive often blame themselves, and many are not willing to tell their stories.” Despite the lack of funding for research, there has been great progress made in lung cancer diagnosis and treatments over the last several years. “There are many different types of lung cancer,” Dr. Bastos said. “Genetic testing has helped researchers to develop therapies that target specific types of cancer cells.” Immune-based therapies, in which the immune system is primed to attack tumors, also are showing potential for treating lung cancer, Dr. Bastos said. These therapies already have been approved for treating skin cancer. Development of screening tools such as CT scans help with early identification of lung cancer, too, which Dr. Bastos says “could save tens of thousands of lives.”

ome of us cannot remember what happened yesterday, much less fifty years ago. At a meeting of the Kiwanis Club of Miramar-Pembroke Pines on September 16th, Monica Severn recalled how the Kiwanis Club impacted her life. While the purpose of the meeting was to install a new set of officers and Board of Directors for the club, those in attendance were transfixed by what Severn had to say. Severn and a co-worker from CareSpot Urgent Care attended the meeting as guests and Severn introduced herself and spoke about why she was there. “Kiwanis paid for two operations on my eyes about fifty years ago,” she said. “A teacher of mine was involved in the Kiwanis Club and the club sent me to Bascom Palmer Eye Institute to get my eyes fixed. It was fantastic. I’d love to give back, I’ve never forgotten.” It was stories like that that reminded all in attendance of the important work that brings everyone together in the Kiwanis Club. Club members gathered at Bokamper’s Sports Bar and Grill in Miramar for the induction ceremony of the 2015-16 Board of Directors and Officers. Dave Lockhart was named President with Angie Font stepping in as Secretary. Ann Lee, Faye East and Caroll Neita were all named to the Board of Directors. Kiwanis is a global community service group that provides countless volunteer hours and a prodigious fundraising effort to many good causes, typically helping with children’s health issues. Lockhart has been involved with Kiwanis for nine years and is looking forward to having more community members join in. “It’s a great club, we’re not just club members, we’re friends,” he said. “Everyone pitches in. We’re looking to build the membership back up, because with more members, the more we can do for the community. Kiwanis helps so many kids, but it’s heartbreaking to see so many kids out there in unfortunate situations.” The meeting opened with Font welcoming the group and thanking everyone for their work while she served as the chapter’s President. The 2014-2015 Board and officers were congratulated before the incoming Board and officers were installed.

“We do so many great things for the community, like going to the Susan B. Anthony Recovery Center for play dates with the kids there,” said Font, a Miramar resident. “It keeps you feeling good about yourself. It’s a great complement to the hustle and bustle of everyday life.” The youth of the community was represented at the meeting as three officers from local Key Clubs, the high school affiliates of Kiwanis, were in attendance. The students talked about how the financial contributions that Key Clubs and Kiwanis Clubs have made have virtually eliminated neonatal tetanus in India. Navya Baranwal is President of the Key Club at Miramar High School and talked about why she has been involved since she was a freshman. “Key Club allows you to get more involved with leadership, service and fun,” said Baranwal. “You can never not smile when you’re in the Key Club. I want to be a doctor and perhaps volunteer at an orphanage so I can still provide service to others.” Everglades High School and Somerset Academy in Miramar also feature Key Clubs. Both the Kiwanis and the Key Clubs are gearing up for the annual Harvest Drive to collect donated groceries and household goods to donate to families in need. Community members are also being asked to donate $10 to the local Kiwanis Club for their “Adopt-a-Turkey” Thanksgiving baskets that provide holiday food to the hungry. All are welcome to Kiwanis Club meetings, which are held each month on the first, second and fourth Wednesday at 7:30 AM at Latin Flavor Cafe, 11368 Miramar Parkway and on the third Wednesday at 7:00 PM at Bokamper’s Sports Bar and Grill, 15500 SW 29th Street. Miramar Police and Fire Departments Compete in Rib Eating Challenge for a Cause BY JILL SHARPE
I
t was a battle between guns and hoses on September 8th at Texas Roadhouse as the Miramar Police Department and the Miramar Fire Department, got together for the 2nd Annual Rib Eating Challenge for charity. All in good fun, five challengers from each super hero department had to eat a rack of ribs in a relay competition which ultimately the fire department won this year. Ten percent of generated sales from the event went to the departments chosen charities, Susan G. Komen Foundation and the Miramar Foundation Youth Scholarship Fund said Jaye Wilhite, Legendary Store Marketer for Texas Roadhouse. “The Rib Eating Challenge is not only a fun event but it’s a great way to show our appreciation to people that serve us every day,” Wilhite said. Although there were only five participants from each department, both depart-
ments had cheering squads and lots of families enjoyed the competition. Family friendly activities added to the fun including a fire house bounce house and a balloon artist. Miramar Fire Department fire trucks, the Miramar Police Department SWAT team truck, K9’s and motorcycles were also present. Participating from the Fire Department were Reuben Gonzalez, Nathan Babrove, Rudy Valdes, Glenn Harrell and Jake Destajo. Participating from the Police Department were Damaso Espiritusanto, Oscar Mendoza, Cory Bogus, Seth Schaller and Adrian Spence. As the police department won the first year of the challenge and the fire department won this year, everyone is looking forward to an intense match up next year as the super hero departments meet again to see who will claim the rib eating title.

Dolphin Bay Elementary PTA is Working Hard to Raise Money for a Playground Shade Structure SCHOOL NEWS

T
he Parent Teacher Association (PTA) at Dolphin Bay Elementary School has been relentless for the past two years working hard to raise funds for a shade structure to go over the playground at the school. So far their fundraising efforts have raised $28,000 but the approximate cost of the structure is $40,000 and the PTA hopes to raise the rest of the money by the end of this school year. Katerine Alvarez, this year’s PTA President, said the project is crucially important because global warming has caused an increase in temperatures every year and the playground areas and basketball courts do not have any shade. “The playground structures get so hot these days that it is difficult for the kids to enjoy recess or P.E.,” said Alvarez. “We are also concerned about skin cancer as we want to do everything necessary to protect our children and bringing this project to fruition is an important part of that duty.” The school’s biggest fundraiser is Dolphin Fest, a full day dedicated to fun activities for the students of Dolphin Bay. During Dolphin Fest the kids enjoy bounce houses, cotton candy, pop corn, snow cones, and many other things. “We also have educational events like books fairs, and events to promote the integration of our community like dances, family field day, Spring Fest, and so on,” said Alvarez. “This year we are trying to launch Multicultural Night, an event designed to celebrate diversity in our school.” Local businesses and longtime supporters have supported this project from day one. “We want to especially thank Dr. Deborah Ferrer from Westside Orthodontics and Johanna Lopez from Pines Transportation as they gave us our first donations,” said Alvarez. “We also have to thank the families of Dolphin Bay Elementary because they believe in what we are trying to accomplish, and help with what they can. Times are tough and we understand. We cannot forget the staff of the school, who support every event that we have. Our Principal Sandra Nelson, the Vice Principal Brad Schmidt and all the teachers are very supportive of the PTA. We can’t thank them enough” Principal Nelson said the Dolphin Bay PTA has worked very hard over the past year, and is continuing to work hard to fund the PTA Playground Shade structure. “The dedication and support of Dolphin PTA and parents has been crucial for this project to move forward. The Dolphin Bay community is essential to the success of our school,” Nelson said. The PTA has been trying to get some big companies to contribute but so far has not had any luck. “Dolphin Bay Elementary is a great school,” said Alvarez. “We encourage whomever is reading this to invest in our children, not only the students at Dolphin Bay but kids everywhere. Anything you can do to help a child, do it.” Dolphin Bay Elementary offers businesses who may be interested in partnering on the Playground Shade Structure three levels of sponsorship that provide exposure to about 700 student and their families. M
y Fellow Miramar Residents: We are about to enter the season we celebrate with family, friends and colleagues for all things for which we are thankful. We are grateful for the blessings in our lives and we take time to help those who are not as fortunate. I recently presented the First State of the City Inaugural Address to an audience of close to 600 people at the City’s Cultural Center. Below is a brief summary of the great things happening in Miramar and what we - as residents, employees, business people - have to be thankful for in Miramar.
Business Climate:
The City has become a hub for the aviation sector and many others have relocated to Miramar. In fact, companies including Kellstrom Defense, Aero Accessories, 1st Choice Aerospace, Sealand and WAI Global have come to Miramar, expanded their office space and created hundreds of new high paying jobs. Since April 2015, there has been over $25 Million in capital investments to build out 240,000 square feet of office and industrial space by three International companies creating 130 jobs right here in Miramar. In addition, the City has processed 11,395 building permits at a value of $347 Million dollars since 2014.
Economic Development:
Miramar had the 15th fastest growing economy of 240 cities with 100,000 to 300,000 people. Our property values have gone up 8.07% and there are plans for over 1,800 additional residential units. This is a value of $191 million dollars. The job market continues to improve with our unemployment rate at only 4.9%, lower than that of the County (5.2%) and the State (5.6%).
Resident Quality of Life:
On the east side of Miramar, also known as Historic Miramar, we are investing $22 Million in Phase 3 infrastructure projects including drainage and sewer. Phases 1 & 2 are completed taking thousands of residents off septic and putting residents on our water and sewer system. The City’s Transit Oriented Corridor land use is underway to encourage mixed uses. We are investing $28 Million in park renovation projects city wide. The New Police Headquarters is on track to open in May 2016 and Fire Station 107 is moving forward earlier than planned and will thereby be operational at an earlier date. The Ansin Sports Complex is a treasure in our community that is helping to produce world-class athletes. Our senior community is extremely active through the programs provided by our Multi-service Complex. Miramar continues to support and receive services from our Community Partners including the Jason Taylor Foundation which recently had students from Miramar High School participate and win a national spoken word poetry competition in Chicago. Memorial Hospital Miramar is involved in a number of events and projects within our City and this will expand in the future. In advanced technology, we will soon debut the new City website followed by a new mobile application for Miramar that, when downloaded, will allow you to do so much more from your phone. We have a lot to be thankful for in Miramar. Our foundation is solid and our recent successes show signs our city is poised for even higher heights. Eye fatigue from prolonged computer use is caused by the necessity for our visual processing pathways to adapt to processing text on a computer screen in a different way than they do for letters in print. While our eyes are used to focusing on printed material that contains solid black characters with sharp edges, they are less familiar with characters on a digital screen that lack the same degree of contrast and sharpness. Words on a screen are created by combinations of tiny dots of light (pixels), which are most luminous at the center and diminish in intensity toward the edges. Therefore it is harder for our eyes to focus on these images. Instead, our eyes reduce focus to the ‘’resting point of accommodation’’ or RPA. Our eyes involuntarily adjust to the RPA and then have to make a great effort to regain focus on the text. The continuous strain on the muscles of the eyes to focus results in the fatigue and eye strain that is sometimes present during and after use of a computer or digital device. CVS isn’t a matter of concern just for those who spend a lot of time on computers. Other electronic gadgets such as smart phones or tablets can cause similar conditions and in some cases even worse. Since mobile screens are often small, in addition to pixilated, the user often strains even more to focus on images.
Treatment for Computer Vision Syndrome and Eye Strain
If you think that you might be at risk for CVS, you should make an appointment with an eye care professional sooner than later. At a computer vision exam, your eye care professional will perform tests to detect any particular vision problems that might worsen symptoms of computer eye strain. Depending on the results of these tests, your optometrist may prescribe prescription computer eyeglasses to reduce discomfort at your screen. An anti-reflective coating is highly recommended for computer glasses. An anti-reflective coating lessens glare that may interfere with your ability to focus on images on your screen.
Ergonomics for Computer Vision Syndrome
Ergonomics, or changing your work environment to reduce strains in vision or posture, can help relieve some of the discomfort of computer vision syndrome. Proper lighting and frequent breaks will cause some relief. However, since ergonomics alone cannot resolve a visual problem, using ophthalmic computer eyeglasses is also necessary. Special Needs Planning
I
f you are the parents of a child with special needs such as a mental or physical disability, you face the challenge of providing for them while you are alive, as well as seeing that they are taken care of after you are gone. The expenses of caring for a family member with special needs can be overwhelming, and ongoing medical care may not be covered by private insurance plans. State and local governments offer assistance, but it is subject to strict financial eligibility requirements. So what can a parent of a child with special needs do (or not do), to assure adequate care throughout their lifetime, yet not disqualify them from govern-
ment assistance? First, it is important that parents, grandparents, and others be discouraged from establishing custodial accounts for a minor child with special needs. Why? Because once the minor child reaches adulthood, distribution from the custodial account may disqualify them from government services. A well-meaning grandparent’s act of generosity can actually do more harm than good. The same disqualification applies if a child with special needs is left assets outright, or through a trust. What about leaving money to a trusted relative to be held on behalf of a beneficiary? While that sounds like a reasonable plan, the relative could lose the inheritance through their own divorce, bankruptcy, lawsuits or simply by mismanaging the assets. In addition, should the selected relative die, the inheritance becomes part of their own estate. Fortunately, the government has established rules allowing assets to be held for the benefit of an individual with special needs, without risking their eligibility for government assistance, in a trust, called a “special needs” or “supplemental needs” trust. How does this work? A special needs trust preserves government benefit eligibility while at the same time providing assets that can meet the additional needs of the person with a disability that goes beyond provisions for the basic needs and limited medical and long-term support and services of Medicaid or Supplemental Security Income (SSI). These life-enhancing expenditures can include: supplemental education and training; out-of-pocket medical and dental services; transportation (including purchase and maintenance of a vehicle); travel and vacations; hobbies and recreation; purchases such as computers, electronics, and furniture; personal care aide; athletic training and completion; and special dietary needs. Most importantly, a special needs trust must be designed specifically to supplement, not replace public benefits. Funds from a special needs trust cannot be distributed directly to the disabled beneficiary. Rather, funds must be disbursed to third parties who provide goods and services for the use and enjoyment by the beneficiary. Often, supplemental needs trusts are funded with life insurance proceeds, thereby providing resources exactly when they are needed. Special needs trusts are a critical component of your estate planning if you have a child or grandchild with disabilities. Estate planning is a deliberate, premeditated act of love. ow many of us have suffered from annoying and painful conditions like ingrown nails, thick nails, and bruised nails? We continue to put on our favorite shoes, our toes squirming inside them, and walk around in discomfort. We hope that the problem will go away on its own if we ignore it. Ignoring the problem, however, will not make it go away. Ingrown nails can be very painful. It occurs when a toe nail grows into the skin surrounding it. This can happen typically on either edge of the big toe. The cause of an ingrown nail may be due to improper trimming, inherited nail deformities, injuries, fungal infections or too much pressure on the nail. People commonly complain the most about ingrown nails when walking. Pain may be felt at the tip of the toe or all the way back to the base of the toe. You should not dismiss the pain as minor, because it could lead to infection, inflammation or even more serious conditions. Treatment for an ingrown nail should include an evaluation by a podiatric physician. If the nail is infected, he could prescribe an antibiotic. He may also trim the nail, remove part of the nail and/or treat the nail with a substance that will keep the ingrown part of the nail from growing back. To lesson pain, the podiatrist may give an anesthetic during the removal procedure. Bruised nails can be caused by an acute injury to the toe or a repetitive injury. Podiatrists often see these injuries in patients involved in sports like running or in patients who have had heavy objects fall on their toe. The big toe is most commonly the victim. A podiatrist may treat the bruised nail by removing it with the aid of an anesthetic, or drilling a hole in the nail to relieve pressure and promote drainage. You may also be prescribed pain medication. Soaking or icing the nail may also help. If your pain is tolerable, the nail can be left alone to fall off. A new nail commonly grows in to replace the old nail. There can be many causes of abnormally thick or crumbling nails: injury, pressure from shoes, fungal infections, diabetes, psoriasis or vascular disease. Sometimes the nail will loosen and fall off. The nail can also appear brittle, rigid, or yellow as well as thick. It is important to visit a podiatrist if you have this problem, because it may be a symptom of a serious underlying condition. If the thick nail is caused by a fungal infection, a culture may be done. If the nail is not infected, it may be thinned by trimming, filing or grinding. If there is a fungal infection, topical anti-fungals can be prescribed. For those who do not wish to take medication, ask your podiatrist if he can do a laser treatment on the fungal nail. In sum, these common toenail problems can be resolved successfully with the help of your podiatric physician. In addition, many toenail problems can be prevented by simply wearing the correct shoes and trimming your nails properly. Dr. Neal M. Dr. Velilla has achieved many academic honors and graduated top three in his class.
