CHILDREN & FAMILIES The children’s mental health system is composed of interconnected groups of organizations. Each of these organizations has a role in recognizing, addressing, and supporting children’s mental health. Families may begin or continue their journey with any organization and expect their care providers to collaborate in the following ways: • Sharing observations/positive screenings • Referring children and families to resources and support • Creating a network of support and resources for identified children and families • Maintaining network connections • Sharing a treatment plan and coordinating care with applicable systems

• Collaboratively providing quality care by following a treatment plan • Collaboratively creating a treatment plan for long‑term support with families and partners • Implementing a warm hand-off for long‑term support

Note: a release of information and other relevant consent forms are required to protect confidentiality under HIPAA, FERPA, and other privacy laws; consider creating a standardized process to ask parents for such forms.
