Working in collaboration This award will be presented to a team or individual who has successfully worked in collaboration with a range of teams, individuals or agencies to develop or improve services in the trust. This team or individual will have shown exemplary behaviour when working with internal and external partners to the project leading to proven service improvement or outcomes. The judges will be particularly looking for evidence of: • • •
Collaborative working across teams, individuals or agencies Exemplary behaviour when working with internal and/or external partners Proven service improvement or outcomes
Volunteer of the year This award will be presented to an individual whose voluntary work with the Trust has made an exceptional contribution to putting the patient at the heart of what we do. This individual or team shows dedication and commitment to the trust and improves the experience for service users and visitors. The judges will be particularly looking for evidence of: • • •
Dedication and commitment to the Trust Improved experience for service users and visitors Puts the patient first
Supporting services team of the year This award is for a non-clinical based team that consistently demonstrates flexible and effective team work with a positive, ‘can do’ approach. The team are forward thinking and proactive in their field, whilst adapting and implementing changes to improve services. The team give support to other individuals and teams across the trust without hesitation and still provide a high quality service. The judges will be particularly looking for evidence of: • • • • •
Collaboration - Flexible and effective team work Caring - Positive, ‘can do’ attitude to work Aspirant - forward thinking in their field Adapt and implement changes to improve services High quality service
Supporting services star of the year This award is for a non-clinical based individual that consistently demonstrates a CAREing attitude to work. The individual welcomes collaboration across the trust whilst maintaining respect and empathy when working with other individuals or teams, even in challenging situations. The individual comes to work with an aspirant attitude demonstrated by their understanding of the work happening in their industry and how they implement changes to suit. The judges will be particularly looking for evidence of: • • • •
Collaboration with other teams and individuals Respect and empathy for colleagues across the trust Aspirant attitude at work Implements positive changed aligned to their department
Making a difference This award will be presented to an individual who continually contributes to the positive image and reputation of the trust, an individual who has made a real difference to the personal experiences of service users, carers or colleagues by living and demonstrating the Trusts values. This individual could be an ‘unsung hero’ who deserves some recognition for their continuous positive attitude and dedication to putting the needs of others first. The judges will be particularly looking for evidence of: • • • •
Demonstrated dedication and commitment to quality by, for instance, putting others needs first Respected the needs and views of others Listened to service users, carers or colleagues and helped them make choices and contribute to decision making Show concern for the wellbeing of service users, carers or colleagues
Inspirational Leader This award will be presented to an aspirational individual who aligns their vision of the future to incorporate their employees and colleague’s values, behaviours and beliefs. Nominees do not need to be in a formal leadership role but should be someone who has a passion for innovation and improvement and has a clear focus on putting the quality of care and patient safety at the heart of service delivery. The nominee should act as a positive role model to employees, setting standards of excellence and should be inspiring and engaging. The judges will be particularly looking for evidence of: • • • •
Incorporating employees’ values, behaviours and beliefs in future vision and plans, demonstrating commitment to equality and diversity Keen focus on innovation with a dedication to quality improvement with quality of care and patient safety as the top priority Role model behaviours from the NHS People Plan, Customer Care Charter and Trust objectives in their work Supports the team/department to reach their full potential
A contribution to wellbeing This award recognises individuals or teams who have developed sustainable approaches to support the wellbeing (physical and mental health) of staff. We are looking for projects and initiatives that can demonstrate a positive impact for staff to stay well and continue to improve/prioritise their wellbeing.

This award will be gifted to an individual or team who has gone above and beyond to provide staff wellbeing. The judges will be particularly looking for evidence of: • • •
An effective wellbeing project or initiative that addresses an important issue A clear narrative that demonstrates why the project/initiative was implemented Evidence of the project/initiative’s success
A dedication to quality improvement This award will be presented to a team or individual who can demonstrate they have developed and successfully implemented a new or improved quality service or way of working. Benefits could include improved access to services, more personalised care or treatment, a more positive experience, improved outcome or patient safety in the past 12 months. It could also be a change that resulted in a financial saving or one that adds value and/ or reduction of waste. The judges will be particularly looking for evidence of: • • • •
A positive and beneficial impact on service users/ patients or staff Service user / patient / staff involvement in its development Evidence based practice Added value and/or reduction of waste and/or improved patient safety
Clinical star This award is for a clinical based individual that consistently demonstrates a CAREing attitude to work. The individual welcomes collaboration across the trust whilst maintaining respect and empathy when working with other individuals, teams and patient/service users, even in challenging situations. The individual comes to work with an aspirant attitude demonstrated by their understanding of the work and how they implement changes to suit. The judges will be particularly looking for evidence of: • • • •
Personally motivated to provide exceptionally high quality and safe care, while being responsive to the needs of individuals, helping people take control of their own health Treats everyone with dignity and respect and demonstrates commitment to equality and diversity Collaborating with individuals, teams and patient/services users to enhance the patient experience Implements positive changed aligned to their department
Clinical team This award is for a clinical based team that are motivated to providing high quality and safe care to patients. The team demonstrates effective teamwork with a positive, caring approach. The team are forward thinking and proactive in their field, whilst adapting and implementing changes to improve the patient experience. Individuals in the team support one another to give high quality care whilst maintaining patient safety to have a positive impact on the patient experience. The judges will be particularly looking for evidence of: • • • •
Personally motivated to provide exceptionally high quality and safe care, while being responsive to the needs of individuals, helping people take control of their own health Treats everyone with dignity and respect and demonstrates commitment to equality and diversity Collaboration with one another to demonstrate caring, effective teamwork. Aspirational and forward thinking in their field, implementing changes to positively impact the patient experience
