In January’s Elevate session, we launched Elevate 2026 and introduced the three engagement pathways: team/organization, individual learning, and grab-and-go resources. The session also emphasized the role of leadership in driving transformation and featured the North Carolina Community Health Center Association, who shared how NCCHCA uses the VTF Assessment to support reflection, collaboration, and action across member health centers.

In February’s Elevate session, we discussed why having an improvement strategy is essential to health center performance. We heard case examples from La Familia Health and Centerpoint Health. They shared about QI models they've used, example workflows for staff training, and tested evidence-based interventions (i.e. provider feedback and assessment, data validation, and patient engagement campaigns).

In March’s Elevate session, we explored why population health management is essential to health center performance. Participants heard case examples from Harmony Healthcare Long Island. The session highlighted strategies for empanelment and attribution, approaches to risk stratification, and care models designed to target interventions that advance the Quadruple Aim.

In May's Elevate session, we focused on evidence-based cancer screening, featuring El Centro de Corazón (Houston, TX) as a peer case study. El Centro shared how they improved screening rates through standing orders, care gap reports, and targeted patient outreach. They also contributed three clinical tools now available in the Elevate toolbox.
